Percutaneous rotational lithotripsy of urinary bladder stones. Clinical results with the Rotolith lithotriptor.

Mechanical rotational lithotripsy of urinary bladder stones, up to 20 mm in diameter, was performed in 6 male patients with the Rotolith lithotriptor. The lithotriptor was introduced through a 10 F (OD 3.3 mm) suprapubically inserted sheath, and the procedure was performed under intermittent fluoroscopy and inspection through a transurethral cystoscope. In 3 patients, the bladder stones were fragmented to pieces small enough to be evacuated through the cystoscope. The instrument, which is designed for lithotripsy of gallbladder stones, was less efficient for fragmentation of urinary bladder calculi, primarily due to the large volume of the human urinary bladder, and possibly because of the higher gravity of bladder stones. There was no substantial damage to the mucosa of the urinary bladder.

Percutaneous rotational lithotripsy of gallbladder stones. Clinical results with the Rotolith lithotriptor.

Ten patients (aged 39-94 years) with cholecystolithiasis were selected for percutaneous rotational lithotripsy with the Rotolith lithotriptor either because they were considered high-risk patients for cholecystectomy or because they had refused surgery. The procedure was completed in 7 patients. Five of these were stone-free at cholangiography 1 to 2 days after lithotripsy. Conclusive cholangiograms were not obtained in 2 patients due to gallbladder leakage, which in itself did not cause any serious sequelae. At ultrasonography after one month, one of these 2 patients had no visible gallbladder, the other one had small residual gallbladder stones. Rotational lithotripsy is an alternative to cholecystectomy in patients at high surgical risk, especially elderly patients who have undergone cholecystostomy as an emergency treatment for acute cholecystitis.

Aneurysms of the superior mesenteric artery and its branches.

Aneurysms of the superior mesenteric artery (SMA) and its branches are rare. We describe three patients with aneurysms in the SMA or its branches found at angiography in our department. The importance of mesenteric aneurysms and different procedures for treatment are discussed.

Hydatid disease of the spleen. Ultrasonography, CT and MR imaging.

Seven patients with hydatid disease of the spleen were examined by radiography, ultrasound, CT, and in one case MR imaging. The observations were confirmed by patho-anatomic findings except in 2 patients where high indirect hemagglutination tests confirmed the diagnosis. In one patient primary, and in the others secondary, echinococcosis of the spleen was assumed to be present. Secondary hydatid disease of the spleen was caused by rupture of liver cysts with abdominal and pelvic dissemination. Ultrasound and CT findings of the cysts and cystic calcifications are described. In one patient MR imaging indicated prolapse of a splenic hydatid cyst into the left hemithorax, confirmed by patho-anatomic examination.

Large vein sampling for intact parathyroid hormone in preoperative localization of enlarged parathyroid glands.

Unilateral neck exploration for primary hyperparathyroidism (pHPT) of old and high risk patients can be safely performed under local anesthesia, provided there is a correct preoperative side localization of the enlarged parathyroid gland. We performed large vein sampling and measured intact parathyroid hormones (PTH) with a new, highly sensitive immunoradiometric assay. The method was used before operation in 20 patients with pHPT. A unilateral positive gradient of serum PTH levels was obtained in 15 patients. At surgery, 13 of these (87%) proved to be correct, i.e., the adenoma was localized on the same side. Thus, the technique correctly lateralized the parathyroid adenoma in 65% of the patients. We conclude that large vein sampling with measurement of intact PTH is a potentially useful investigation for preoperative side localization of parathyroid adenomas in pHPT patients scheduled for unilateral surgery under local anesthesia. However, prior to routine bilateral neck exploration large vein sampling is not justified.

Experimental transjugular portacaval shunt. Use of a fine-needle puncture device for the implantation of Strecker stents.

A transjugular portacaval intrahepatic stented shunt was created in 16 pigs without induced portal hypertension. A fine needle (OD 0.7 mm) was used for the transjugular puncture of the portal vein. The puncture was done directly from the inferior vena cava in 5 cases and from the right liver vein in 11 cases. The puncture tract was dilated and subsequently supported by a Strecker stent. The stented shunt was patent in all cases immediately after the stent placement, but was obstructed by fibrous tissue in 7 of 8 cases reexamined by angiography and autopsy after 4 weeks. Complications seem to be related to the choice of the puncture site, with fewer complications when the puncture was done via the right hepatic vein.

Lithotripsy of urinary bladder stones with a mechanical lithotriptor inserted through a narrow introducer. An experimental study in pigs.

Mechanical lithotripsy of urinary bladder stones was performed with the RotoLith lithotriptor in 8 pigs with implanted human stones or artificial stones after open cystotomy. The effect of the treatment on the urinary bladder was investigated macro- and microscopically immediately after the lithotripsy in 4 animals and 4 weeks after the lithotripsy in the other 4 animals. The stones were fragmented into very small pieces which would have been possible to pass with the urine. The histopathologic examination immediately after the procedure showed slight mucosal edema, patches of erosions in the mucosa, and minor submucosal bleeding. Microscopic examination of the bladders removed 4 weeks after lithotripsy showed slight chronic inflammation. The experimental results have encouraged us to plan to use this lithotripsy procedure in selected patients with bladder stones.

Tumor seeding occurring after fine-needle biopsy of abdominal malignancies.

Percutaneous fine-needle aspiration biopsy is a commonly used diagnostic procedure with a high accuracy and a low complication rate. However, tumor seeding in the biopsy tracts has been recorded with a frequency of one in 20,000-40,000 biopsies. We report 5 cases of percutaneous tumor seeding recorded after 5,000 fine-needle biopsies of abdominal malignancies at our institution. The risk of implantation metastases induced by fine-needle biopsy warrants consideration in patients with abdominal malignancies since it may compromise the outcome of radical surgery. It should only be performed when the result of the procedure has a direct impact on the choice of therapy.

Sclerotherapy for ablation of the gallbladder after gallstone lithotripsy with a mechanical lithotriptor. An experimental study in pigs on the effect of absolute ethanol on edematous gallbladder wall.

Chemical ablation of the gallbladder with absolute ethanol following mechanical lithotripsy with the RotoLith lithotriptor was attempted in 10 pigs after open cholecystostomy. Two gallbladders removed immediately after the sclerotherapy demonstrated extensive ulcerations of the mucosa with preserved glands and sinuses under the ulcerations. In 8 gallbladders removed 8 weeks after sclerotherapy a gallbladder remnant with hydrops and/or mucus and more or less intact epithelium was seen in all the pigs. In order to prevent regeneration of epithelium from gallbladder sinuses and glands, more effective sclerosing agents must be found.

Ultrasonography and Doppler Duplex compared with angiography in follow-up of mesocaval shunt patency.

Thirty-four patients with portal hypertension of various etiologies were operated upon with an interposition mesocaval Goretex graft. During a period of 6 years 84 angiographic and ultrasonographic (US) examinations (53 B-mode, and 31 Doppler Duplex) were performed, with few exceptions within 3 days. Angiography served as the 'gold standard'. Shunt patency was correctly interpreted as normal in B-mode US in 38 examinations. Shunt occlusion, definite or probable, was seen in 12 B-mode examinations, where angiography demonstrated occlusion in 6 and patency in the remaining 6 shunts. Shunt occlusion was not observed sonographically in one examination. US was technically inadequate on two occasions. Doppler Duplex showed shunt patency in all 31 examinations, which was correct according to angiography. Both B-mode US, in 6 out of 6 examinations, and Doppler Duplex, in 7 out of 9 examinations failed to reveal shunt stenosis. Five patients with abundant venous collaterals (in one case with aberrant vascular anatomy) were not possible to evaluate even after the introduction of Doppler Duplex, which otherwise facilitated the evaluation. We suggest that US including Doppler should be the primary modality for follow-up in patients with interposition mesocaval Goretex grafts.

The frequency of digital and endoscopic examination of the rectum before radiological barium enema.

Doctor's delay in patients with rectal carcinoma is common with a frequency between 30% and 60%. The reason for delay has been supposed to be the neglectance to perform digital examination and rigid proctosigmoidoscopy in patients with symptoms of anorectal disease. Especially general practitioners have been blamed for a high percentage of this kind of doctor's delay. In this study 300 consecutive patients referred for radiological examination of the large bowel have been questioned if digital examination and rigid proctosigmoidoscopy have been performed by their referral doctor. The results show that in 19% of the cases neither digital nor proctosigmoidoscopic examination had been performed. Rigid proctosigmoidoscopy had been omitted in 44%. No statistical difference was observed in the frequency of digital examination between general practitioners (79%) and hospital doctors (84%). The low figures, both for digital examination and rigid proctosigmoidoscopy are probably causing many patients a delay in the diagnosis of a rectal carcinoma. Thus, a more intense evaluation in this area should be applied in the medical faculties and also at post graduate courses.

Liver necrosis after hepatic dearterialization in pigs.

In nine pigs, the livers were dearterialized by a combination of distal occlusion of the hepatic artery with Ivalon (polyvinyl alcohol foam) particles of 150-250 mu and proximal occlusion of the common hepatic artery with Gianturco coils. Necrosis of at least 90% of the liver developed in eight pigs. One pig with less extensive liver necrosis had developed an effective collateral circulation to a large part of the liver. Patency of the portal venous system did not prevent extensive infarction of dearterialized liver tissue.

Development of collateral circulation following distal embolization of hepatic artery in pigs.

The distal hepatic artery was embolized with black polystyrene microspheres (diameters of 50 +/- 10 mu and 200 +/- 25 mu) in 12 pigs. The animals were reexamined from 1 h to 2 weeks after embolization via hepatic angiography and subsequent injection of Microfil, a silicone rubber compound, into the arterial and portal vascular systems of the liver. Dissection of dehydrated and cleared liver specimens under the stereomicroscope demonstrated the presence of arterial collaterals bypassing embolized vessels within 2-3 days after embolization. Collaterals were noted to develop around occlusions in arteries with inner diameters as small as 100 mu.

Segmental hepatic arterial occlusion with absolute ethanol in domestic swine.

Four to five ml of absolute ethanol was injected into a segmental hepatic artery in eight pigs to study the occlusive effect on the hepatic arterial system. Liver cell damage was indicated by a transient moderate elevation of liver enzymes. Follow-up angiography at various time intervals showed persistent occlusion of intrahepatic arteries at different levels in all pigs and occlusion of the common hepatic artery in four. Reconstitution of the intrahepatic arterial circulation via abundant intrahepatic collaterals was, however, observed. Microscopic examinations demonstrated arterial thrombi and necroses in the vascular walls as well as a great variation in the extent of liver parenchymal damage.

Comparison between absorbable and nonabsorbable, monofilament sutures for end-to-end arterial anastomoses in growing pigs.

End-to-end anastomoses of the iliac arteries in growing pigs were constructed with interrupted 7-0 absorbable polydioxanone (PDS) or polypropylene (Prolene) sutures. Comparison between the two monofilament suture materials was made with respect to macroscopic and microscopic examinations, angiographic findings, recorded tensile breaking forces, and calculated blood flow values within the arterial walls at the level of the anastomoses. The angiographic examinations and the recorded tensile breaking forces did not reveal any difference between PDS- and Prolene-sutured anastomoses. On the other hand, the macroscopic findings, the histologic examination, and the calculated blood flow values (6.1 +/- 4.7 and 16.8 +/- 2.3 ml/min/100 gm for PDS- and Prolene-sutured anastomoses, respectively) point to a far more pronounced inflammatory tissue reaction with Prolene. The obtained differences clearly suggest the use of absorbable PDS instead of nonabsorbable Prolene sutures for arterial anastomoses.
