[Campylobacter pylori and gastritis--the density of colonization and degree of inflammation. A semiquantitative and morphometric study].

Gastric biopsies of 1000 patients were grouped according to their diagnoses, special interest was devoted to type B-gastritis. Density of Campylobacter pylori colonization was determined semiquantitatively after Warthin-Starry silver impregnation. Bacteria could never be seen in normal mucosa. In B-gastritis statistics proved a significant correlation between bacterial colonization density and degree of inflammation. In 60 selected cases this close interrelation could be objectified by morphometry. The larger the quantity of bacteria per biopsy area the greater must be its deleterious effect on the mucus layer and the superficial epithelium. Active, chronic gastritis is characteristic of Campylobacter pylori colonisation. There can be no doubt that Campylobacter pylori plays a decisive role concerning the aetiology of type B-gastritis.

[Ethylene glycol poisoning].

A 63-year-old man with arterial hypertension suffered from depression and suicidal wish after a cerebrovascular accident and transitory left hemiparesis. He was urgently admitted to hospital in severe metabolic acidosis which caused renal failure and coma, ending fatally within two days. At necropsy calcium oxalate crystals were found in the renal tubules and cerebral vessel walls with chemically induced meningoencephalitis. From these findings glycol poisoning was diagnosed. There was a lethal concentration of ethylene glycol in the urine. The toxic effects of ethylene glycol are due to its metabolites. The oxalate crystals are primarily of diagnostic importance.

Weber-Christian panniculitis with systemic cytophagic histiocytosis.

A 49-year-old woman suffered from Weber-Christian panniculitis with a typically periodic course. Subsequently, the attacks of the disease developed to a severe state: high fever, endotoxinemia, pancytopenia, and clotting disorder, in addition to the cutaneous manifestations. One and a half years after the outbreak of the disease the patient died in a septic shock. At autopsy all three stages of Weber-Christian panniculitis were found. In addition, an immense proliferation of benign cytophagic histiocytes could be observed in the bone marrow, spleen, lymph nodes and, less distinctly, in the fatty and interstitial tissues of the visceral organs. As a nosologic entity, the Weber-Christian disease is frequently questioned. Some authors consider the lobular, histiocytic, cytophagic panniculitis a unique syndrome. On the other hand, it must be emphasized that benign, cytophagic histiocytosis may exist associated with infections or may accompany different diseases. Furthermore, the Weber-Christian panniculitis is clinically and morphologically well defined.

Adrenal Size and function after irradiation with 131I-6 beta-Iodocholesterol in rabbits.

Administration of high doses of 131I-6 beta-iodocholesterol to rabbits is followed by marked size reduction of the adrenal glands with concomitant changes of the histological structure and with a decrease of serum cortisol. Stimulation of 131I-6 beta-iodocholesterol uptake into the adrenals by additional administration of ACTH had only a minor additional effect on adrenal weight. However, with further accumulation of labeled cholesterol in the adrenals the radioactivity was reduced in blood urine, and the ovaries.

[Anemia in patients with malignant testicular tumors].

Changes in erythropoiesis were studied in 94 patients with testicular germ cell tumours (24 seminoma, 70 nonseminomatous tumours). Additionally ferrokinetic studies with 59Fe were performed in 29 patients with proven metastases. Only 2 of 37 tumour-free patients sustained moderate anemia after recess of primary therapy (2 seminoma, after X-ray-irradiation). In patients with disseminated tumour disease the anemia and the reduction of 59Fe-utilisation correlated positively with the elevation of serum estradiol concentrations, whereas no correlation could be established with testosterone serum levels.

[On the origin of islands and septa of the human placenta (author's transl)].

The maternal or foetal origin of septa and islands of the human placenta was examined by counting the sex chromatin in their nuclei (Barr bodies); the origin of the septa was, in addition, determined by counts of the Y-chromatin previously stained with quinacrine fluorescence. Histological investigations were made on preparations stained after Masson-Goldner (trichrom). The results showed that in most cases the upper parts of the septa are exclusively composed of trophoblastic cells, whereas the lower third is preponderantly composed of decidual elements. Purely trophoblastic septa are rare. Islands are trophoblastic. Islands in which both cell types are present, or decidual islands appear to be very rare.

[Stimulation of prolactin secretion by drugs].

Intravenous administration of metaclopramide and cimetidine provokes an immediate rise in serum prolactin concentration. The results are compared with the response of serum prolactin to arginine, insulin and TRH.

Neurothekeoma, a rare tumour of the tongue.

Neurothekeomas are benign tumours mostly arising in the face and upper limbs predominantly in young women. Histologically they can be classified as cellular or myxoid, and complete surgical resection is the treatment of choice. In this case of a neurothekeoma located in the anterior tongue of a child, cryotherapy was used as the treatment modality, in order to avoid a greater loss of tongue function.

Correlation of telomerase activity, clinical prognosis and therapy in oral carcinogenesis.

Telomerase activity is associated with most malignant tumors. To evaluate the role of telomerase reactivation as a diagnostic and prognostic marker in oral carcinogenesis activity was investigated in mortal and immortal cell lines in eight oral leukoplakias (OL) and 46 oral squamous cell carcinomas (OSCC). Activity was also investigated in 13 histopathologically unaffected mucosas from distant sites or tumor-free margins of the same patients using a modified, highly sensitive, non-radioactive telomeric repeat amplification protocol (TRAP). This was correlated with histopathological stages and the clinical course of the disease. 50% of OL and 46% of OSCC showed activity. One patient with positive, high dysplastic OL developed an OSCC 11 month later. One of three specimens of adjacent tissue presented activity and a recurrence occurred after 6 months. Out of 10 tissues of distal normal mucosa, 2 demonstrated activity which could also be proved in the corresponding tumor. Detection of telomerase reactivation may be a novel method for early detection of immortalised cell clones and malignant cells in histopathologically normal oral squamous epithelium.

p53 mutation and detection of p53 protein expression in oral leukoplakia and oral squamous cell carcinoma.

In many studies the detection of p53 protein by immunohistochemistry (IHC) with one antibody is assumed to be consistent with a mutation of the gene. To determine the correlation of protein detection by immunohistochemistry and gene mutation, paraffin embedded specimens of 60 oral leukoplakias (OL) and 73 oral squamous cell carcinomas (OSCC) were analysed by IHC with two different antibodies (Do7; Pab 1801), PCR-SSCP and sequencing. In 98% of OLs and 94% of OSSCs p53 protein expression was detected with at least one antibody. Only 49% of all tissue specimen were positive with both antibodies. Molecular analysis of the same specimen showed mutations of the p53 gene in 13.3% of OLs and 9, 6% of OSCCs. p53 protein expression was not detected by IHC in 3 out of 7 OSCC with p53 mutations. According to these results detection of p53 expression by IHC is not always correlated with p53 gene mutation and failure to detect p53 protein by IHC does not prove the absence of mutation of the gene in the carcinogenesis in the oral mucosa.
