Immunotherapy of gynaecological high-risk human papilloma virus infection with human leukocyte ultrafiltrate.

OBJECTIVE: In this preliminary presentation the outcome of women following systemic treatment of cervical human papilloma virus (HPV) with a leukocyte ultrafiltrate is reported. METHODS: Cervical brush specimens of 819 women with low-grade CIN-1 were evaluated for HPV infection. HPV-positive patients were treated either by cone biopsy because of suspicious colposcopy or by antibiotics for symptoms of bacterial cervicitis. The remaining women were considered as asymptomatic carriers of HPV and underwent systemic therapy by a human leukocyte ultrafiltrate in a dosage of 5 IE, 6 times in the first 2 weeks, and additionally as a single dose at weeks 4, 6, 8 and 10. RESULTS: The leukocyte ultrafiltrate was administered to 97 HPV-positive women. In follow-up 88.7% were HPV negative at week 6; the remaining 11 women were tested negative after completion of a second course of therapy. CONCLUSIONS: These preliminary data on the successful treatment of HPV infection by a leukocyte ultrafiltrate are very promising but have to be supported by additional research.

[Infectious diseases in patients with solid tumors].

In malignant diseases the prognosis of tumour and the patient's quality of life are essentially influenced by the kind and severity of the immunodeficiency. The high rate of infection of up to 40% is effected by both the basic disease and the different methods of tumour therapy as well as by numerous other endogenous and exogenous factors. Especially the myelosuppressive effect of cytostatics favours the lowered defense mechanism. Hence severe infections stand in causal relation to almost 2/3 of all deaths caused by carcinoma. Resulting from these facts it is necessary to have a tight antimicrobiological regime and to carry out a specific prophylaxis of infection in oncology. Already the first symptoms of an opportunistic infection make an instant and effective, but for the moment unspecific antibiotical combination therapy necessary. If this therapy is not effective, an antimycotical treatment should be started. The most important parameters for a success are the quick reaching of normal body temperature and the subjective well-being of the patients. By means of a useful combination of measures of primary and secondary prevention we can succeed in stabilizing the defense mechanism of tumour patients and minimizing the risk of infections.

[Percutaneous transvascular embolization as a palliative measure in bleeding gynecological malignancies of the pelvis].

Between 1981 and 1983 48 patients with an average age of 54 years have had an arterial embolization of internal iliac artery. In 7 patients this was done preoperatively within a clinical study, in 41 ones because of profuse vaginal bleedings. In 6 patients there was a vital indication. In all cases survival time was 152 days. In two cases a re-embolization was necessary. In these two patients survival time was on average 338 days. The manoeuvre was done with particles of gelatin of different size suspended in contrast medium. One complication could be observed, but was not caused directly by embolization. In cases of profuse or vital bleedings which conservatively cannot be stopped embolization is method of first choice with no contraindication.

[Triapten ointment in the treatment of herpes genitalis in the female].

We report about the therapy of Herpes simplex infections. We applied triapten ointment topically in 166 women with relapse illnesses. In cases with early application we cured 80 per cent of vulvitis herpetica. In 17 per cent we observed side effects we tried to clarify the reasons.

[Cholesterol in serum and vaginal content in healthy, non-pregnant women].

We examined 97 healthy and non-pregnant women aged between 17 and 58 years, who especially did not show any symptoms of vulvitis or colpitis, for the contents of cholesterol in serum and vaginal smear. In the vaginal smear we found significantly lower concentrations of total cholesterol as in serum, whereas the part of free cholesterol on total cholesterol in vaginal smear was triple than in serum and its absolutely concentration was double than that in serum. Possible reasons are discussed.

[Sodium, potassium, magnesium and calcium in vaginal content].

Flame photometric determinations of sodium, potassium, and calcium, moreover magnesium concentrations according to a complexometric method in 197 healthy nonpregnant women aged 16-46 years were performed in serum and fluid of the vagina. The concentrations of K+, Ca++, and Mg++ in vaginal fluid showed marked increase of some 5, 6, respectively 4 times from those of serum. The sodium concentration of the vaginal fluid is half of serum. The concentration of magnesium in vaginal fluid showed a significant increase of a quarter in comparison with serum after application of hormonal contraceptiva. The origin of vaginal fluid is discussed.

[The microbial flora of the vagina in trichomoniasis].

The vaginal flora was investigated in women with trichomoniasis by means of a culture method. Trichomonas, yeast-like fungi and various types of bacteria were isolated. The applied microbiological technique allowed isolation not only of aerobic and cicrophilic bacteria but only of anaerobic bacteria. Anaerobic bacteria were nonspore-forming and were isolated very frequently in the vagina.

[Amine colpitis].

Significance of KOH test and isonitril (isocyanide) reaction tested in 104 patients was compared with our microbial culture results. In Gardnerella vaginalis as well as in no spores forming anaerobic germs the KOH test was not and the isonitril reaction was only insignificant reliable. Both tests are not able to substitute the procedures of culturing germs including those of Mycoplasma hominis and Ureaplasma urealyticum.

[Problems of vaginal mycoses].

The problem in the diagnosis and therapy of the vaginal mycosis is discussed. With help of the electron microscopic examination we found yeasts on the surface of the vagina and within the intra- and subepithelial reach. These explained with restriction the possibility for a rezidive.

[The use of Biozym as a cleaning and disinfecting agent in gynecology and obstetrics].

This paper outlines the results of investigations about the germicidal behaviour of the protease containing substance Biozym. It has been noted, that a concentration of 2.5 per cent is satisfactory for a definite effect. Biozym shows no significant effect against yeast. By this reason it is possible to clean and to desinfect bacterial contaminated matters.
