[A prospective randomized study on the fractionation of afterloading therapy of cancer of the cervix and endometrium].

A prospective randomized clinical study is made on patients with uterine cervix and endometrium carcinomas treated only by radiotherapy. The fractionation schemes of 4 times 10 Gy, and 5 times 8 Gy, and 8 times 5 Gy at the reference points A and My, respectively, of a short-term afterloading therapy combined with percutaneous telecobalt therapy with 45 Gy at the pelvic wall are investigated. A tendency is shown towards better tumor control and less radiogenic effects with an increased number of fractions. The results are discussed on the basis of the NSD and LQ models. The problems caused by the equipment type and dosage specificity hampering the applicability of the results on other afterloading therapy units are pointed out.

Epidermolysis bullosa simplex associated with muscular dystrophy with recessive inheritance.

Epidermolysis bullosa with unusually severe clinical features was associated with progressive muscular dystrophy in two siblings. Light and electron microscopic examination revealed an intraepidermal cleavage confirming that this mechanobullous disease belonged to the epidermolysis bullosa simplex group. This may represent a new disease entity inherited in an autosomal-recessive fashion.

Molecular analysis of paramutant plants of Antirrhinum majus and the involvement of transposable elements.

Paramutation is observed when the Antirrhinum majus lines 44 and 53 are crossed. These two lines both have insertions at the nivea locus, which encodes chalcone synthase (chs). The allele niv-53 carries the transposable element Tam1 in the promoter region of the chs gene; niv-44 carries the element Tam2 within the gene. The Tam1 element has previously been extensively characterised. Here the Tam2 element is further characterised, and the arrangement of the nivea locus in paramutant plants is analysed. The complete sequence of Tam2, and that of a partial cDNA complementary to it, have been determined. The cDNA is probably transcribed from a different copy of Tam2 from that present at the nivea locus, and does not encode a functional protein. Genomic Southerns of F1 plants from the 53/44 cross show that no major rearrangements are consistently associated with paramutation at the nivea locus of A. majus. The isolation from a paramutant plant arising from a 53/44 cross of an allele (niv-4432) resulting from the excision of Tam2 is reported. The excision of Tam2 resulted in a 32 bp deletion of chs gene sequences. Plants homozygous for the new niv-4432 allele have white flowers and are still paramutagenic, demonstrating that Tam2 need not be present at the nivea locus for paramutation to occur. Different interactions between Tam1 and Tam2 are discussed, and a possible model for paramutation is presented.

Long-term results of Troidl's technique of endoscopic pneumatic dilatation for achalasia of the esophagus. A prospective clinical trial.

In a prospective clinical trial, 26 consecutive patients underwent endoscopic pneumatic dilatation over a 10-year period. Dilatation was achieved by means of a balloon attached to a normal gastrointestinal fiberscope. With the endoscope in an inverse position, the device was placed in the cardia and the dilatation process was monitored macroscopically. Before dilatation, patients suffered from dysphagia (92%), reduced speed of swallowing (100%), symptom aggravation under stress (73%), weight loss (50%), aspiration, pain, regurgitation, and vomiting. After dilatation and long-term follow-up (mean of 5 years), symptoms could be markedly reduced, especially the speed of eating and symptom aggravation under stress. Excellent and good results (Visick scale) were achieved in 76%. Fair results were achieved in 20%. To date, perforation and other complications have not occurred. Mortality was zero. Our series was an uncontrolled trial, so the results are hardly comparable to other studies. Furthermore, the small number of patients in our study represents a weak point with regard to complications. We conclude that the main advantages of the procedure are its simplicity and practicability. The simple procedure may be the method of choice in elderly patients. Of course, no final decision can be made until a well-designed controlled trial has been carried out.

Functional recovery of the exocrine pancreas in rats after partial resection.

The function of the exocrine pancreas was studied in rats in which more than 50% of the glandular tissue had been resected 1 (n = 16) or 6 (n = 20) weeks before initiation of the study. Sham-operated animals served as controls (n = 15). After partial pancreatectomy, the weight and DNA content of the remaining gland increased significantly (p less than 0.001), and the digestive enzymes: DNA content ratio was temporarily elevated. The glandular tissue secreted relatively less fluid, bicarbonate and digestive enzymes than in control animals, but the outputs increased significantly (p less than 0.001) during the observation period. After 6 weeks, only the fluid and bicarbonate secretion was still reduced. It is concluded (a) that the pancreatic function recovers after partial resection; (b) that the enzyme output of the remaining glandular tissue decreases temporarily; (c) that the enzyme content of the gland thereby increases, and (d) that the function of the acinar cells recovers faster than that of the ductular system.

[Pulmonary functional disorders in ulcerative colitis and Crohn's disease].

Lung functions were tested (by whole-body plethysmography, pneumotachography, CO diffusion, blood-gas analysis) in 54 patients with Crohn's disease, 21 with ulcerative colitis and 43 healthy subjects. Abnormalities of pulmonary function were found in 38% of those with ulcerative colitis and 54% with Crohn's disease, significantly more often (p less than 0.005) than in the healthy controls (7%). Obstructive disorders were more frequent in patients with ulcerative colitis than Crohn's disease (p less than 0.01), the latter having mainly restrictive and diffusion abnormalities. The abnormal results were unrelated to previous drug treatment as well as duration, activity and site of the disease. The cause of the respiratory abnormalities remain unexplained, but they may be extra-intestinal manifestations of ulcerative colitis and Crohn's disease.

Segmental portal hypertension.

Isolated obstruction of the splenic vein leads to segmental portal hypertension, which is a rare form of extrahepatic portal hypertension, but it is important to diagnose, since it can be cured by splenectomy. In a review of the English literature, 209 patients with isolated splenic vein obstruction were found. Pancreatitis caused 65% of the cases and pancreatic neoplasms 18%, whereas the rest was caused by various other diseases. Seventy-two per cent of the patients bled from gastroesophageal varices, and most often the bleeding came from isolated gastric varices. The spleen was enlarged in 71% of the patients. A correct diagnosis in connection with the first episode of bleeding was made in only 49%; 22% were operated on because of gastrointestinal bleeding, but the cause of bleeding was not found. The diagnosis should be suspected in patients with gastroesophageal varices, but without signs of a liver disease, especially if isolated gastric varices are found. The diagnosis is confirmed by portography.

Serum angiotensin-I-converting enzyme and carboxypeptidase N in Crohn's disease and ulcerative colitis.

Angiotensin-I-converting enzyme (ACE) and carboxypeptidase N1 and N2 (CPN1, CPN2) inactivate kinins and might therefore play a role in the development of inflammatory reactions via an influence on the release of prostaglandins and inactivation of anaphylatoxic peptides of the complement system. In the present study, the serum activity of these enzymes was determined in 60 patients with Crohn's disease, 18 patients with ulcerative colitis and 70 healthy control subjects. ACE was significantly lowered in active Crohn's disease (CDAI greater than 150) and in ulcerative colitis (p less than 0.01), as long as the ileum or cecum was affected. Since ACE was detected in high concentrations in the human intestinal mucosa, decreased values may be explained by damage to the site of its production. CPN1 and CPN2 were raised in both diseases (p less than 0.005), irrespective of their activity and location. These alterations in the activity of the kininases investigated may play a role in the pathogenesis of inflammatory bowel diseases.

[Thoughts on the neurobiological aspects of Parkinson syndrome].

The Parkinson-syndrome is the most important syndrome under the extrapyramidal disease. The therapy with L-DOPA has a prominent place in the therapy of Parkinson disease since the introduction of oral effective drugs. Dopamin has an effect to the basalganglia as a neurotransmitter and perhaps an inhibition effect to specific synapses of brain. The inhibition or the optimal balance of the cholinergic systems is the effect of Dopamin and Noradrenalin. The therapy with low dosis of L-DOPA in combination with a decarboxylase-inhibitor will be prevent the side-effects of nerv-cells.

Molecular analysis of instability in flower pigmentation of Antirrhinum majus, following isolation of the pallida locus by transposon tagging.

The pal locus of Antirrhinum majus was cloned using the transposable element, Tam 3, as a probe. The pal clone was used to examine, at the molecular level, those aspects of instability previously observed phenotypically and genetically. The effects of temperature and of genetic background on excision of the element at pal are considered, and related quantitatively to the phenotype. We describe the identification of the transcript of the pal locus and show that insertion of Tam 3 blocks the production of a normal pal transcript in developing flower buds.

Output-dependent non-parallel enzyme secretion of the human pancreas.

In order to study the question as to whether the enzyme ratios in the pancreatic juice change with the secretion rate, we analysed the enzyme outputs of 20 healthy volunteers under combined stimulation with varying doses of cholecystokinin (0; 0.5; 1.0 or 1.3 IDU X kg-1 X h-1), administered in random order, plus a constant dose of secretin (0.5 CU X kg-1 X h-1). The outputs of the individual enzymes correlated significantly (p less than 0.0001) to the corresponding amylase outputs in the form of power model regressions. Mathematical transformation of these curvilinear regressions permitted a comparison of their courses. This analysis revealed that the enzyme ratios in the pancreatic secretion change continuously at increasing outputs in favour of lipase greater than chymotrypsin greater than amylase. The shift in the ratio of lipase to trypsin and to amylase and that of chymotrypsin to amylase was significant (p less than 0.01).
