Enhanced calcium-sensitivity of erythrocytes in hypertension--calcium-induced changes of erythrocyte osmotic fragility in essential hypertension.

To investigate the Ca-sensitivity of the erythrocyte membrane in hypertension, the changes of the osmotic fragility of erythrocytes by Ca-loading and the effects of Ca-channel blockers or calmodulin-antagonist were observed in patients with essential hypertension. Erythrocytes were obtained from untreated patients with essential hypertension and age-matched normotensive subjects. Treatment of erythrocytes with Ca-ionophore A23187 and Ca in bathing medium caused the reduction of the osmotic fragility of erythrocytes dose-dependently on Ca-concentration. The degree in alteration of the osmotic fragility of erythrocytes was greater in essential hypertension than that in normotensive controls. In addition, Ca-induced changes of erythrocyte osmotic fragility was inversely correlated with the plasma renin activity in essential hypertension. In the presence of Ca-antagonists (verapamil, diltiazem) or calmodulin-antagonist (trifluoperazine), the reduction of the osmotic fragility of erythrocytes by Ca-loading was inhibited, and the differences of the osmotic fragility of erythrocytes between the hypertensives and the normotensive controls were abolished by these drugs. These results suggest that the greater changes of the osmotic fragility of the erythrocytes by Ca-loading in essential hypertension might be due to the abnormality of Ca-handling of the cell membranes causing an increase in the intracellular Ca concentration, contributing at least partially to the pathogenesis of hypertension.

Thyroid hormone autoantibodies (THAA) in two cases of Graves' disease: effects of antithyroid drugs, prednisolone, and subtotal thyroidectomy.

Changes in titers of serum thyroid hormone autoantibodies (THAA) and anti-thyroglobulin (Tg) antibodies during treatment with antithyroid drugs (methimazole and propylthiouracil) were examined in two cases of Graves' disease. Effects of prednisolone and subtotal thyroidectomy were also investigated in one case (case 1). Initially both cases had only anti-T4 autoantibodies in their serum. During methimazole therapy, the titer of anti-T4 autoantibodies increased in both cases, and anti-T3 autoantibodies became detectable and their titer increased in case 2. The influence of propylthiouracil on the titer of THAA was not clear. Both prednisolone plus methimazole therapy and subtotal thyroidectomy decreased the level of anti-T4 autoantibodies in case 1. There was a significant correlation between titers of THAA and anti-Tg antibodies in both cases, although titers of anti-Tg antibodies in case 1 stayed within the normal range throughout the investigation period. These results indicate that methimazole treatment could induce and/or enhance the production of THAA and THAA are antibodies against thyroid hormone-containing Tg molecule.

A case of Sjögren's syndrome with valvular diseases.

The case of a 61 year old woman with Sjögren's syndrome with aortic and mitral stenosis is reported. She suffered from rheumatic fever at a young age. Physical and echocardiographic examinations showed findings of mitral and aortic valve stenosis. In addition, she had experienced xerostomia, a gritty sensation in the eyes and Raynaud's phenomenon. Blood examination showed hypergammaglobulinemia, positive rheumatoid factor, antinuclear and anti-Ro (SS-A) antibodies. The diagnosis of Sjögren's syndrome was confirmed by sialography and biopsy of the labial salivary gland. The combination of valvular disease and Sjögren's syndrome is rare and the etiological correlation is discussed.

Structure of HTLV and its biological function in leukemogenesis of adult T-cell leukemia.

There is a high homology of nucleotide sequence between 3' two-thirds of the X (or pX) regions of human T-cell leukemia virus (HTLV)-I, and of HTLV-II. Monoclonal antibody against p41 coded from X-IV, an open reading frame of X region of HTLV-I, was established. Two proteins coded by Xb, one of the open reading frames in X region of HTLV-II, were newly identified as p24 and p26. The expression of X protein of HTLV-II in the reconstituted mouse embryonal carcinoma cell line, which shows myoblastic morphology, reverted the morphology to that of the original embryonal carcinoma cells. This suggests that the function of X protein is to disturb the regulation of cell lineage determination. Leukemogenesis of adult T-cell leukemia (ATL) is also considered to consist of multisteps, in which HTLV-I constitutes one step, other factors also being involved. Even the role of HTLV-I factor could be similarly played by other factor(s). In agreement with this hypothesis, there are patients with ATL without associated HTLV-I.

Effects of alpha-human atrial natriuretic peptides on neurosecretion and vascular responsiveness in spontaneously hypertensive rats.

Effects of alpha-human atrial natriuretic peptides (alpha-hANP) on the norepinephrine overflow from the sympathetic nerve endings and vascular responsiveness were investigated in the perfused mesenteric intestinal loop preparations from 7- to 9-week-old spontaneously hypertensive rats (SHR) and age-matched Wistar-Kyoto rats (WKY). Pressor responses to electrical nerve stimulation or exogenous norepinephrine were inhibited by alpha-hANP (3.4 X 10(-7) to 1.0 X 10(-6) mol/l), and norepinephrine overflow during the nerve stimulation was concomitantly suppressed by alpha-hANP. The pressor responses and norepinephrine overflow by nerve stimulation were greater in SHR than the age-matched WKY, while the inhibition of these responses by alpha-hANP was significantly less in SHR than in WKY. These results demonstrate that alpha-hANP could affect both pre- and post-synaptic sites of the resistance vessels, and less reduction in the pressor responses and norepinephrine overflow by alpha-hANP in SHR suggests insufficient regulation of adrenergic neurotransmission by cardiac peptide in hypertension.

Monoclonal antibody NCC-pX-1G reactive with gene products coded from X regions of human T-cell leukemia virus.

Monoclonal antibody NCC-pX-1G (IgG1, kappa) was obtained by the hybridoma technique by immunization of mice with the C-terminal 54 amino acids of the pX protein, made from the 3'-terminal portion of the pX gene conjugated with bovine growth hormone gene transfected in E. coli. NCC-pX-1G recognized 41 kilodalton pX protein of HTLV-I, and immunocytochemically stained HUT102 and other HTLV-I-infected cell lines, but no reaction was observed with non-HTLV-I-infected cell lines or normal human tissue cells.

Inhibitory effects of polyprenoic acid (E-5166) on N-2-fluorenylacetamide-initiated hepatocarcinogenesis in rats.

The effects of the newly synthesized polyprenoic acid, 3,7,11,15-tetramethyl-2,4,6,10,14-hexadecapentaenoic acid (E-5166) on N-2-fluorenylacetamide (FAA)-initiated hepatocarcinogenesis were examined in 6 groups of male ACI rats. The numbers of altered hepatocellular foci in rats of group 1 given a basal diet containing 0.02% FAA for 13 weeks and in rats of group 2 which received E-5166 by gavage (40 mg/kg, 3 times/week) at the same time as receiving the FAA diet were almost the same, indicating that E-5166 had no effect at the stage of carcinogen exposure. However, the number of foci in group 4, in which rats were given the basal diet and E-5166 after the termination of the carcinogen exposure, and were sacrificed 16 weeks later, was significantly smaller than that in group 3 maintained on the basal diet alone (P less than 0.05). The results suggests some anticarcinogenic activity of E-5166, possibly involving the phenotypic expression of the preneoplastic foci. Furthermore, the number of altered foci in rats of group 6 (given the liver-tumor promoter phenobarbital with E-5166 for 16 weeks after the administration of carcinogen) was also significantly smaller than that in rats of group 5, which received the promoter (P less than 0.05). The incidence of neoplastic nodules of the liver in group 6 at the end of the experiment was also lower than in group 5 (P less than 0.0014). These results suggest an antipromoting effect of the polyprenoic acid E-5166 on rat chemical hepatocarcinogenesis.

[Clinical statistics on outpatients, inpatients and operations in 1984].

Statistical studies on 1,893 outpatients, 538 inpatients and 554 operative procedures at our department in 1984 revealed the following. The most frequent diseases among the outpatients were urogenital infections followed by anomalies, tumors and stones. The major diseases among the inpatients were hypospadias, vesicoureteral reflux, benign prostatic hypertrophy, bladder tumor and congenital urethral stenosis. A total of 554 operations were performed on 534 patients, and five major operations were hypospadias repair (68), ureterocystoneostomy (57), optic internal urethrotomy (50), TUR-P (42) and TUR-Bt (33).

[Investigations on the mechanism(s) of the production of anti-thyroid hormone antibodies 4: the effect of adjuvant on the immune response to human thyroglobulin].

Forty-five female DDYK mice were divided into three groups and immunized with different preparations of human thyroglobulin (HTg). Group 1 mice were immunized subcutaneously in the back with HTg emulsified with FCA. Group 2 mice were immunized subcutaneously in the back with HTg dissolved in PBS. Group 3 mice were immunized with HTg dissolved in PBS and FCA separately in the right (HTg) and left (FCA) of the back. After the fourth immunization (45 days after the first immunization), antisera as well as thyroid glands were obtained, and titers of anti-HTg, anti-T3, anti-T4 and the presence of lesions of autoimmune thyroiditis were examined. In group 1 mice, titers of anti-HTg and anti-T4 antibodies were significantly higher than those of the other two groups, whereas titers of anti-T3 antibodies were significantly lower than those of the other two groups. In group 1 mice, a significant correlation between titers of anti-T3 and anti-T4 antibodies was observed, whereas in group 2 and group 3 mice, no such correlation was observed. The incidence of lesions of autoimmune thyroiditis was significantly higher in group 2 and group 3 mice than in group 1 mice. Thus emulsification of HTg with FCA had inhibitory effects on the induction of experimental thyroiditis in mice immunized with HTg. These results indicate the possibility that FCA has a modulatory effect both quantitatively and qualitatively on the immune response against HTg in mice.

A case of renal cell carcinoma solitarily metastasized to thyroid 20 years after the resection of primary tumor.

A case of renal cell carcinoma in a 69-year-old woman is reported. The woman developed a thyroid tumor recently. She has a history of nephrectomy done 20 years ago for a renal cell carcinoma. Based on the close resemblances of the histological features and cell nuclear DNA distributions of both tumors, the thyroid tumor was diagnosed as the secondary tumor from the renal cell carcinoma. In the review of the literature, this case of renal cell carcinoma seems to be that with the longest interval of years before solitary metastasis to the thyroid.

Syncarcinogenic effects of methyl methanesulfonate with methylazoxymethanol acetate in rat small intestine and liver.

The carcinogenic potency of methyl methanesulfonate (MMS) and its combination effect with methylazoxymethanol (MAM) acetate were evaluated in rats. MMS was suggested to be weakly carcinogenic for the small intestine. Syncarcinogenic effects of MMS and MAM acetate which could be due to summation of their genotoxic influences were obtained in the small intestine and liver.

Reduced DNA repair response of cells from carcinogen-induced rat liver altered foci exposed to chemical carcinogens in culture.

Cells were cultured from rat livers containing enzyme-altered foci induced by N-2-fluorenylacetamide and the level of DNA repair synthesis in response to several carcinogens was compared in gamma-glutamyl transpeptidase-positive foci cells and in enzyme-negative hepatocytes from the same livers. The level of unscheduled DNA synthesis elicited in foci cells by either the activation-dependent carcinogens N-2-fluorenylacetamide and diethylnitrosamine or the activation-independent carcinogens N-methyl-N'-nitro-N-nitrosoguanidine and 1-methyl-1-nitrosourea was significantly lower than that in hepatocytes. The results suggest that the repair of DNA damage by these altered cells is abnormal.

Kinetic study of carboxypeptidase P-catalyzed reaction. Pressure and temperature dependence of kinetic parameters.

Detailed kinetic analyses of carboxypeptidase P-catalyzed reactions were carried out spectrophotometrically using 3-(2-furyl)acryloyl-acylated peptide substrates. The maximum kcat/Km was observed at around pH 3.5 for the synthetic peptide substrates. The kcat/Km value decreased with increasing pH, with an apparent pKa value of 4.43. However, the maximum kcat was observed at neutral pH (pH congruent to 6) and the pKa was 4.49. These apparently different pH profiles for kcat/Km and kcat of this enzyme were due to the decreasing Km value in the acid pH region. The pressure and temperature dependences of these kinetic parameters were also measured. N-Benzoylglycyl-L-phenyllactate (Bz-Gly-OPhLac) gave dependences similar to those of the peptide substrate, suggesting that there is no distinct difference in the catalytic mechanism between the peptide and the ester hydrolyses.

Classification of enlarged prostatic utricle in patients with hypospadias.

An enlarged prostatic utricle is often associated with hypospadias. To evaluate the relationship between the incidence of prostatic utricle and the degree of hypospadias, we performed retrograde urethrography and/or micturating cystourethrography on 280 patients with hypospadias. Prostatic utricles were demonstrated in 84 patients (27.5%) and the incidence increased according to the severity of hypospadias. The prostatic utricles were classified into four types from the standpoint of size and location of the opening of the utricle, and a direct relationship was demonstrated between the severity of hypospadias and increasing size of the prostatic utricle.

Effects of dobutamine on cyclic AMP content in canine upper urinary tract tissue.

There was no regional difference in tissue cAMP content throughout the upper urinary tract specimens obtained from dogs immobilized by pancuronium bromide (0.1 mg/kg, i.v.). The cAMP content was increased in all regions examined 5 min after administration of dobutamine (1 mg/kg, i.v.) and pancuronium bromide. The increase was much higher in the pelvicalyceal region than in the other upper urinary tract tissue. The responsiveness of the pelvicalyceal region where pacemaker cells for ureteral peristalsis exist is considered to be higher than that in the other upper urinary tract tissue in respect to dobutamine-induced cAMP increase.
