Inhibitory effects of chlorogenic acid, reserpine, polyprenoic acid (E-5166), or coffee on hepatocarcinogenesis in rats and hamsters.

Four different experiments were performed in order to examine the modifying effects of chlorogenic acid (CA), reserpine, polyprenoic acid (E-5166), and coffee on chemical carcinogenesis in rats or hamsters. Experiment 1: The numbers of hyperplastic liver cell foci and the incidence of colon tumors in male and female Syrian golden hamsters given a single intravenous injection of methylazoxymethanol (MAM) acetate and then fed the diet containing 0.025% CA for 24 wk were significantly lower than those of hamsters given MAM acetate alone. Experiment 2: The incidence of altered hepatocellular foci in female ACI/N rats given N-2-fluorenylacetamide (FAA, 0.02% in diet) for 10 wk and reserpine (weekly subcutaneous injections, 1 microgram/g body weight) during or after (17 wk) FAA exposure was significantly lower than that of rats given FAA alone. Experiment 3: The number of hepatocellular foci in male ACI/N rats given 0.02% FAA diet for 13 wk and E-5166 by gavage (40 mg/kg body weight, 3 times/wk) for 16 wk after the end of FAA exposure was significantly smaller than that in rats given FAA diet alone. Experiment 4: Incidences of liver tumors and hepatocellular foci of rats given concurrent dietary administration of aminopyrine (0.01%) and sodium nitrite (0.1%) and coffee solution as a drinking water for 630 da were significantly lower than those of rats given aminopyrine and sodium nitrite. Thus, the tested compounds had inhibitory effects on chemical carcinogenesis in liver or colon.

Initiation of meiosis and sporulation in Saccharomyces cerevisiae requires a novel protein kinase homologue.

SME1 was cloned due to its high copy number effect: it enabled MATa/MAT alpha diploid cells to undergo meiosis and sporulation in a vegetative medium. Disruption of SME1 resulted in a recessive Spo- phenotype. These results suggest that SME1 is a positive regulator for meiosis. DNA sequencing analysis revealed an open reading frame of 645 amino acids. An amino terminal peptide of ca 400 amino acids in the deduced protein was similar to known protein kinases. Transcription of SME1 was regulated negatively by nitrogen and glucose and positively by MATa/MAT alpha and IME1, another positive regulator gene of meiosis. By complementation analysis, SME1 was found to be identical to IME2, which had been shown to be important in meiosis. These results suggest that IME1 product stimulates meiosis by activating transcription of SME1 (IME2) and that protein phosphorylation is required for initiation of meiosis.

Combined cisplatin and radiation therapy in patients with invasive bladder cancer.

Thirty-five patients with T2-T4 invasive bladder cancer were treated with combined cisplatin and radiation therapy. In 18 patients radical cystectomy was performed after the combined therapy. In the other 17 patients radical cystectomy could not be performed for various reasons. Pathological examination of the cystectomy specimens showed down-staging in 66.7% and no residual tumour in 33.3%. These results suggest a synergistic action of cisplatin and radiation. Side effects were not severe and were well tolerated. This combined therapy of cisplatin and radiation is very effective for invasive bladder cancer.

Identification of members of the protein phosphatase 1 gene family in the rat and enhanced expression of protein phosphatase 1 alpha gene in rat hepatocellular carcinomas.

We isolated four kings of cDNA clones of isotypes of catalytic subunits of protein phosphatase 1 (PP-1) from rat liver and testis cDNA libraries. For the cloning, cDNA fragments of dis2ml and dis2m2, which encode mouse PP-1 catalytic subunit, were used as probes. Two of the four isotypes were thought to be derived from the same gene and produced by alternative splicing. Based on the comparative study of their nucleotide and deduced amino acid sequences with those reported, these cDNA clones were named rat PP-1 alpha, PP-1 gamma 1, PP-1 gamma 2 and PP-1 delta. The deduced amino acid sequences of these four cDNA clones showed about 90% identity. Their amino-terminal regions were highly conserved, and their differences were mainly in the carboxy-terminal regions. Furthermore, several amino acids located in the middle regions of the peptides were conserved in all the isotypes of the catalytic subunits of PP-1, PP-2A, PP-2B and PP-2C. These conserved regions are suggested to be the functional domains of the catalytic subunits of protein phosphatases. Rat hepatocellular carcinomas induced by a food mutagen, 2-amino-3,8-dimethylimidazo[4,5-f]quinoxaline showed increased expression of PP-1 alpha, but no increased expression of PP-1 gamma 1, PP-1 gamma 2 or PP-1 delta. Involvement of PP-1 alpha in hepatocarcinogenesis or in hepatic cell proliferation was suspected.

Postnatal growth of mouse seminal vesicle is dependent on 5 alpha-dihydrotestosterone.

The seminal vesicles (SV) develop from the lower portion of the Wolffian ducts (WD) in response to androgens, which prevent their degeneration and subsequently stimulate organogenesis of the epididymis, vas deferens, seminal vesicles, and ejaculatory ducts. Earlier studies suggest that testosterone (T) is the active androgen for WD development. By contrast, development of urogenital sinus and external genitalia is dependent upon 5 alpha-dihydrotestosterone (DHT), produced by 5 alpha-reductase within the target tissue itself. To reevaluate the possible role of DHT during SV morphogenesis, SVs from 0-day-old (day of birth) mice were grown for 3, 6, or 9 days in either serum-free or serum-containing medium in the presence or absence of T (10(-7) M) or DHT (10(-8) M). The serum-free medium consisted of Ham's F-12-Dulbecco's Modified Eagle's Medium (1:1) containing insulin, transferrin, cholera toxin, BSA, and epidermal growth factor. The serum-containing medium was Ham's F-12-Dulbecco's Modified Eagle's Medium containing 10% fetal calf serum. Epithelial branching morphogenesis of SVs occurred in serum-free or serum-containing medium supplemented with either T or DHT and was comparable to that of SVs of similar ages in vivo. In serum-containing medium the DNA content of the cultures was about 2-fold higher in T-containing vs. T-deficient medium. However, in serum-free medium the DNA content was the same in cultures grown with or without T. SVs cultured under serum-free conditions in the presence of T plus 390 MSD (17 beta-N,N-diisopropylcarbamoyl-4-aza-5 alpha-androstan-3-one, an inhibitor of 5 alpha-reductase) were completely inhibited in their development, while in the presence of DHT plus 390 MSD, branching morphogenesis was comparable to that in SVs cultured in the presence of T or DHT alone. In medium lacking either T or DHT, SV development was inhibited. In addition, it was confirmed by TLC that [1 beta,2 beta-3H]T was converted into [3H]DHT at the ratio of 15.4% for the first 2 days and at 35.3% for the subsequent 2 days of the culture of SVs in serum-free medium. These data demonstrate that T is important as a precursor of DHT and DHT is the major androgen in the postnatal development of mouse SVs from the lower WD.

Platelet noradrenaline content as an integrated measure of variations in plasma noradrenaline.

To determine whether or not platelet noradrenaline (NA) content reflects a long-term variation of plasma NA concentration, the following experiments were carried out. During a 2-hour intravenous infusion of NA in rabbits, plasma NA concentration increased rapidly to an almost consistent level 1 h later, but platelet NA content increased linearly. One hour after the infusion, the plasma NA concentration returned to normal level, while platelet NA content decreased little and only returned to the original level about 80 h later. By the 24-hour cold exposure stimulation (4 +/- 0.5 degrees C), platelet NA content and urinary NA excretion increased significantly in rats. There was a significant correlation between the urinary NA excretion and platelet NA content. In a patient with pheochromocytoma, the high plasma NA concentration returned to nearly normal levels a few hours after removal of the tumor, platelet NA content returned to normal levels 4 days after surgery. These results indicate that the measurement of platelet NA content serves as a better index for obtaining an average variation of NA concentration and sympathetic nerve activity. The measurement of platelet NA content also provides a reliable means of diagnosing pheochromocytoma.

Silent thyroiditis following rubella.

A 40-year-old housewife was referred to our hospital for evaluation of a thyrotoxic state. A month after rubella infection, she developed heat intolerance. Physical examination revealed a hard and non-tender goiter with no signs or symptoms of inflammation. Free thyroid hormones were high and TSH was undetectable. 123I-thyroidal uptake was suppressed. Antithyroglobulin and anti-microsomal antibodies were negative throughout the course. A serologic study revealed high levels of anti-rubella antibodies. After a month without any treatment, she became euthyroid. Free thyroid hormones and TSH gradually became normal and the antibodies to rubella decreased. 123I-thyroidal uptake increased. From the clinical course, the patient was diagnosed as having silent thyroiditis. We suggest that viral infection such as rubella could cause the development of silent thyroiditis.

Androgen receptor levels in patients with isolated hypospadias.

The binding of [3H]dihydrotestosterone (17 beta-hydroxy-5 alpha-androstan-3-one) to the androgen receptor was studied in 26 prepubertal boys with isolated hypospadias (no associated anomaly) using fibroblasts cultured from surgical specimens of hypospadiac penile skin and/or chordal tissue. Maximal binding capacities of androgen receptor for [3H] dihydrotestosterone in 10 patients with mild degree (2 glandular + 8 penile), 6 with severe degree (4 penoscrotal + 2 scrotal), chordal tissues of hypospadias (N = 10), and controls (10 endocrinologically normal boys) were, respectively, 11750 +/- 2410, 10720 +/- 2680, 16100 +/- 7570, and 11570 +/- 3830 (mean +/- sd) sites per cell. Dissociation constants in these subjects were 1.09 +/- 0.29, 0.93 +/- 0.22, 1.20 +/- 0.31, and 1.22 +/- 0.41 x 10(-10) mol/l respectively. There were no significant differences as to both maximal binding capacity and dissociation constant of androgen receptor among any groups. Abnormal receptor thermolability was not found in 5 patients with severe degree of hypospadias either. It is, therefore, concluded that prepubertal boys with isolated hypospadias have primarily normal androgen receptor activity.

Effects of a calcium-antagonist on vascular sympathetic nerve activity in salt-dependent hypertension.

The purpose of the present study was to investigate the effects of a Ca2(+)-antagonist on vascular sympathetic nerve activity in salt-dependent hypertension. Isolated mesenteric vasculature preparations from reduced renal mass-salt hypertensive and age-matched normotensive control rats were used to examine the effects of diltiazem on vascular responsiveness and norepinephrine release from sympathetic nerve endings. Pressor responses and endogenous norepinephrine release during electrical nerve stimulation were significantly greater in preparations from salt-dependent hypertensive rats than from normotensive control rats. Diltiazem inhibited both stimulation-evoked pressor responses and norepinephrine release in a dose-dependent manner. The attenuation of these responses was more pronounced in preparations from rats with salt-dependent hypertension than in those from control rats. These results indicate that norepinephrine release from vascular adrenergic neurons is enhanced in the mesenteric vasculatures of rats with salt-dependent hypertension. The marked reduction of stimulation-evoked pressor responses and norepinephrine release by diltiazem suggests that enhanced Ca2(+)-dependent adrenergic transmission may contribute to the exaggerated vascular sympathetic tone in salt-dependent hypertension.

[Physical activity in pregnancy].

The author reviews works dealing with the effects of physical activity during pregnancy on the maternal and fetal organism. Considering the great controversy existent in the literature comments on the tendency to recommend more vigorous activity to pregnant woman is examined.

[The effect of corticosteroids on the recovery from vecuronium induced block].

The effects of corticosteroids (hydrocortisone, methylprednisolone) on the recovery time from the vecuronium induced neuromuscular block were studied using evoked potential measurements by stimulating ulnar nerve. Plasma concentrations of vecuronium were examined after the administration of hydrocortisone and methylprednisolone using high performance liquid chromatography in 78 surgical patients. Recovery time from the block induced by vecuronium was prolonged by corticosteroids. In case of hydrocortisone, this prolongation effect was significant (P less than 0.01). In the study of plasma concentration, blood samples were taken 40 minutes after administration of vecuronium, plasma concentration of vecuronium is significantly higher in patients given hydrocortisone (124 +/- 8.32 ng.ml-1) (mean +/- SE) than control patients (68.2 +/- 4.71 ng.ml-1) (P less than 0.001). It is speculated that corticosteroids lower the clearance of vecuronium. Prolongation of recovery time from vecuronium induced block after the administration of corticosteroids may be explained not only by the direct effect of hydrocortisone on the neuromuscular junction but also by the effect of hydrocortisone to prolong the elimination of vecuronium from plasma.

[A case of a pulmonary large cell carcinoma patient who has still been surviving for four years even with triple carcinoma developed on different occasions (rectum, stomach: resected; lung: nonresected)].

The case occurs on an 86-year-old man. In May 1977, Miles' operation was performed for rectal cancer (papillary tubular adenocarcinoma), together with additional proctostomy. In March 1984, subtotal gastrectomy was performed for an early stomach cancer (moderately-differentiated tubular adenocarcinoma). In September 1985, chemotherapy with 3 courses of CDDP (50 mg x 1) plus VDS (3 mg x 3) and liniac irradiation (60 Gy in total) were performed for pulmonary large cell carcinoma. Restiform shadows and fibrosed foci were left behind, but thereafter only the former was persisted without anticancer therapy. Now in December 1989, he has still been surviving without any sign of local recurrence of remote metastasis. Neither anticancer therapy was tried for the first and second cancers nor a cancer family history was found about him.

[Metastatic lung tumor of a thyroid cancer origin detected by a periodic health examination--a case of a complete response].

Reports is the case of a 21-year-old student, who received a medical examination at our Department for an abnormal chest shadow that had been detected during a periodic health examination at his university. No special subjective symptoms such as a cough and/or sputa, pyrexia, pectoralgia, and dyspnea were noted. A thyroid tumor was palpable and a 99mTc thyroid scintigram and a 201Tl thyroid tumor scintigram gave cause to suspect thyroid cancer and a metastatic lung tumor. A total thyroidectomy and a right modified neck dissection revealed a tumor (histologically, a papillary cancer), 3.5 cm in diameter, that mainly involved the right lobe. It was found that 131I-100 mCi internal irradiation was very effective for the metastatic lung tumor, and no abnormal shadow was subsequently revealed by chest X-ray. Now, 6 years later, he is alive with no manifestations of a local relapse nor any abnormal chest X-ray findings.

[Clinical statistics on outpatients, inpatients and operations in 1988].

Statistical studies were made on 1,766 outpatients, 657 inpatients and 687 operative procedures at our department in 1988. The most frequent diseases among the outpatients were urogenital infections followed by anomalies, tumors and stones. The major diseases among the inpatients were congenital urethral stenosis, hypospadias, vesicoureteral reflux, benign prostatic hypertrophy and bladder tumor. A total of 687 operations were performed. The five major operations were optic internal urethrotomy, hypospadias repair, transurethral resection of prostate, ureterocystoneostomy and transurethral resection of bladder tumor.
