Induction of nitroblue tetrazolium reduction in mouse peritoneal macrophages by tumour promoters and inhibition of the induced nitroblue tetrazolium reduction by some inhibitors.

Two polyacetates, aplysiatoxin and debromoaplysiatoxin, as well as 12-O-tetradecanoylphorbol-13-acetate (TPA), mezerein and teleocidin enhance nitroblue tetrazolium (NBT) reduction in mouse peritoneal macrophages in vitro. The ED50 values for NBT reduction of these 5 TPA-type tumor promoters were 4.2 ng/ml for TPA, 36 ng/ml for mezerein, 0.53 ng/ml for teleocidin, 1.5 ng/ml for aplysiatoxin and 108 ng/ml for debromoaplysiatoxin. The NBT reduction induced by the 5 tumor promoters is inhibited by 2 inhibitors of tumor promotion, retinoic acid and dibromoacetophenone. The possibility that tumor promotion by TPA-type tumor promoters involves similar mechanisms such as superoxide anion radicals release in cell membranes is discussed.

Chromosome aberrations in the peripheral lymphocytes induced by brachytherapy and external cobalt teletherapy.

In the present study, the induction efficiencies of chromosome aberrations were analyzed in patients receiving various modalities of interstitial radiotherapy with small radiation sources in the oral cavity area and compared with those in patients treated with external telecobalt irradiation in the thoracic region. Further, as a local nonstochastic effect, the acute mucosal reaction was investigated. The mucosal reaction in the area treated by brachytherapy reached a maximum at two to three weeks after the implantation. The frequencies of dicentrics plus rings of peripheral lymphocytes, on the other hand, revealed rapid increases and approached plateau levels as early as two days after the implantation. The whole-body effects, evaluated on the basis of the chromosome aberration frequencies of peripheral lymphocytes in the patients who underwent brachytherapy, were compared with the effects observed in those patients treated with external radiotherapy or bleomycin injection and discussed with regard to their effectiveness in control of the tumor and side effects. The radiation doses used in the patients who received brachytherapy or a single external irradiation were found to exert the same effects on the chromosome aberration induction. However, even in the case of brachytherapy in which whole-body side effects were believed to be trivial, the peripheral lymphocyte count was temporarily reduced to one half or less of the preirradiation level. An equivalent whole-body dose of 50 cGy was obtained from the frequencies of dicentrics and rings.

Clinicopathological study of maxillary sinus carcinoma.

Autopsy was performed on 67 patients who had squamous cell carcinoma of the maxillary sinus. These patients had undergone radiotherapy and died during a 30 year period, which ended for this study in 1982. The clinicopathological picture showed several differences according to changes of the mode of therapy and to the rise in local control rate. Postmortem examinations have shown a decreased death rate from extensive carcinoma, and increased distant metastases after the introduction of megavoltage irradiation and trimodal combination therapy (radiotherapy + surgery + intraarterial infusion of antimetabolites). In addition, the introduction of aggressive chemotherapy with antimetabolites and antibiotics has given rise to several gastrointestinal complications.

Histidine increases the frequency of chromosomal aberrations induced by the xanthine oxidase-hypoxanthine system in V79 cells.

The combined effects of the xanthine oxidase (XO)-hypoxanthine (HX) system and the various kinds of amino acids in Eagle's minimum essential medium on chromosomal aberrations were studied in Chinese hamster V79 cells. Among 13 amino acids tested, only histidine significantly increased the number of aberrant chromosomes and cytotoxicity in combination with the XO-HX system. This enhancing effect of histidine on chromosomal aberrations was dose-dependent at 0.063% - 0.25%; it was not affected by superoxide dismutase, but was strongly inhibited by catalase.

Gastric small-cell undifferentiated carcinoma with adeno and squamous cell carcinoma components.

A unique gastric tumor is reported. A large portion of the tumor consisted of a diffuse sheet of undifferentiated cells reminiscent of a small cell undifferentiated carcinoma. The tumor cells showed a few dense core granules and a poorly developed attachment apparatus by electron microscopy. In addition, small portions of the tumor showed adenocarcinoma and squamous cell carcinoma. Another noteworthy finding was that some of the metastatic hepatic nodules consisted of relatively monotonous polygonal cells with a distinct cord-like pattern showing argentaffinity and argyrophilia which were indicative of an atypical carcinoid. This case is a rare example of a gastric tumor with differentiation towards endocrine as well as adeno and squamous cell carcinoma.

Multiple primary malignant fibrous histiocytoma of the stomach and small intestine.

A case of multicentric malignant fibrous histiocytoma of the stomach and small intestine is reported. The patient was a 60-year-old man who had total gastrectomy under an impression of a gastric carcinoma. The resected stomach revealed a large polypoid mass in the antral portion at the greater curvature. Three months later, he developed ileus and an 80 cm segment of the jejunum was removed. It contained two polypoid masses identical to that seen in the stomach. The tumors showed, in addition to the characteristic light microscopic appearances, strong positivity for alpha-1-antitrypsin by an immunoperoxidase technique, indicating the diagnosis of malignant fibrous histiocytoma (MFH). Electron microscopic findings were also consistent with MFH. We believe that this is the first well-documented case of MFH arising from the stomach and small intestine, to the best of our knowledge.

[A case of signet ring cell carcinoma of the urinary bladder].

A rare vesical carcinoma, arising in a 47-year-old male, is described. The tumor developed from the left ureteral orifice and posterior urethra, showing a nonpapillary appearance with a wide stalk. By light microscopy, the carcinoma was found to be mainly composed of signet ring cells and associated with small foci of transitional cell carcinoma. The previously reported cases of signet ring cell carcinoma of the urinary bladder were reviewed with a special reference to the manifold potentiality of the urothelium.

[A case of triple neoplasm with acute myelogenous leukemia, squamous cell carcinoma of the lung and adenocarcinoma of the stomach].

A 70-year-old man had undergone middle and lower lobectomy of the right lung for pulmonary squamous cell carcinoma in January 1979. He was treated with anticancer drugs including alkylating agents. In February 1983, he was diagnosed as AML (FAB, M2). Although cytoreduction effect was observed, he died of prolonged pneumonia on the 30th admission day. Autopsy revealed a well-differentiated tubular adenocarcinoma of the stomach (Borrman II type) and a poorly differentiated epidermoid carcinoma of the right hilar lymph node.

[A successful transcatheter embolization with occluding spring emboli for vertebral arteriovenous fistulas following insertion of Swan-Ganz catheter].

Vertebral arteriovenous fistulas caused by insertion of percutaneous monitoring catheter through internal jugular vein are rare complications. This is a case report of a successful treatment by transcatheter embolization using spring occluder for the fistulas. Particular attention should be paid to the following: Normal basilar and contralateral vertebral arteriograms are prerequisites to this procedure in order to avoid its potential neurological hazards. For the complete elimination of the fistulas with retrograde filling, embolic material should be placed in the artery not only proximal but also distal to the fistulas opening.

[Comparison of three pathologic classifications (Rappaport, LSG and Working Formulation) for the treatment of non-Hodgkin's lymphoma].

Comparison of three pathologic classifications (Rappaport, LSG and Working Formulation) was made for 292 cases of non-Hodgkin's lymphoma who were treated at the Department of Radiology, Tokyo Medical and Dental University, from 1952 to 1984. The Working Formulation was found to be the most convenient classification for the treatment of non-Hodgkin's lymphoma in Japan. There were two remarkable characteristics in the cases of non-Hodgkin's lymphoma classified by the Working Formulation. They were better case distribution having a strong relation to clinical prognosis and fewer high-grade cases in Waldeyer's ring lymphoma.

A rapid, simple screening method for skin-tumor promoters using mouse peritoneal macrophages in vitro.

The enhancing effects in vitro of a potent skin-tumor promoter, phorbol myristate acetate (PMA), and its derivatives on nitroblue tetrazolium (NBT) reduction, phagocytosis and cell spreading of mouse peritoneal macrophages were compared. The enhancing effects on these markers of macrophage function, especially NBT reduction, were found to correlate well with the skin-tumor promoting activities of the phorbol esters. Another potent promoter, teleocidin, strongly enhanced NBT reduction, although teleocidin is structurally different from phorbol esters. Other tumor promoters, namely saccharin, phenobarbital, cantharidin and Tween 60 did not enhance NBT reduction by macrophages. These data indicate that this test system is appropriate for screening of PMA-type tumor promoters such as phorbol esters and teleocidin.

Maxillary sinus carcinoma: result of radiation therapy.

Four hundred and sixteen patients with carcinoma of the maxillary sinus received primary therapy consisting of external beam irradiation alone or in combination with surgery and/or chemotherapy at the Department of Radiology, Tokyo Medical and Dental University Hospital, between 1953 and 1982. In our institution, methods of treating cancer of the maxillary sinus have been changed from time to time and showed different control rates and clinical courses. An actuarial 10-year survival rate of 21% has been obtained by megavoltage irradiation alone as well as 34% actuarial 10-year survival rate by megavoltage irradiation with surgery. After the introduction of conservative surgery followed by conventional trimodal combination therapy (antrotomy: intraoral partial maxillectomy + radiation + intra-arterial infusion), the local control rate has been improved. The amount of functional, cosmetic, and brain damages have been remarkably decreased by this mode of therapy. The actuarial five year survival rate was 67%. In addition, along with the improvement of the local control rate, the control of nodal and distant organ metastases have been emerging as one of the important contributions to the prognosis of this disease.

Chromosomal aberrations in V79 cells induced by superoxide radical generated by the hypoxanthine-xanthine oxidase system.

The effect of the superoxide radical, generated by the hypoxanthine-xanthine oxidase system, on chromosomal mutation was examined in Chinese hamster V79 cells. When cells were treated with this system for 1 h in Hanks' solution, the incidence of metaphases with chromosomal aberrations was increased with hypoxanthine at concentrations of 2.5 to 10 micrograms/ml. On the other hand, in Eagle's minimum essential medium (MEM) or MEM supplemented with 10% fetal bovine serum, only hypoxanthine at 5 micrograms/ml plus xanthine oxidase induced chromosomal aberrations and higher concentrations of hypoxanthine were cytotoxic to V79 cells. The increased frequency of chromosomal aberrations and the cytotoxicity of hypoxanthine plus xanthine oxidase were not affected by superoxide dismutase, but were strongly inhibited by catalase.

Primary prostatic carcinoid tumor with intracytoplasmic prostatic acid phosphatase and prostate-specific antigen.

A case of prostatic carcinoid tumor with lymph node metastases is reported. The patient was a 78-year-old male who died in ventricular fibrillation. At autopsy, a 2 X 2 cm, white, irregular tumor was found in the prostate and there were several enlarged para-aortic lymph nodes. Both specimens contained a characteristic carcinoid tumor. Argyrophil stains revealed strong positivity in the primary as well as in the metastatic tumors. Electron micrographs prepared from formalin-fixed tissue demonstrated numerous membrane-bound dense-core granules. Immunoperoxidase-labeled antibodies against both prostatic acid phosphatase and prostate-specific antigen localized in the tumor cells. The ultrastructural and immunohistochemical results support differentiation of the tumor cells toward both prostatic epithelial cells and endocrine cells. We believe that this is the first reported case of a prostatic carcinoid tumor in which specific prostatic tissue markers have been demonstrated in the tumor cells.
