[Phase II study of etoposide (VP-16) in the form of intravenous injection for malignant lymphomas and acute leukemias: Hanshin Cooperative Study Group for Hematological Disorders].

Twenty patients with malignant lymphomas and 12 with acute leukemias were treated with intravenous administration of etoposide, 60-100 mg/m2/day for five days, repeated for three to four weeks. Eighteen cases of malignant lymphoma and nine of acute leukemia were evaluable. CR was achieved in three cases (16.7%) and PR in four cases (22.2%) of malignant lymphoma. Although CR was not achieved in any patients with acute leukemia, and PR was in three (33.3%), it was found that etoposide was most effective for the patients with the M4 or M5 subtype in the FAB classification. The most serious adverse effect of the drug was leukopenia in patients with lymphoma. In three patients (30.0%), the leukocyte count was lower than 1,000/mm3. Gastrointestinal complications, such as anorexia, nausea, vomiting and diarrhea occurred in 60.7% of all patients, but were not serious. Alopecia was observed in 73.1%. Intravenous administration of etoposide was apparently effective for the patients with malignant lymphoma of the diffuse mixed type, and this efficacy found in our study was the same as that for oral administration of etoposide reported by us previously.

Granulocytic sarcoma of the intestine as a primary manifestation nine months prior to overt acute myelogenous leukemia.

A case of granulocytic sarcoma is reported. A 51-year-old male was found, upon laparotomy, to have a tumor mass in the terminal ileum. Nine months later, acute myelogenous leukemia (AML) was diagnosed upon a peripheral blood and bone marrow examination showing an increased number of abnormal myeloblasts. Scattered lesions of granulocytic sarcoma were found in the intestine, colon, testis, skin, gingiva and bone marrow. Chemotherapy, with multiple regimens for leukemia in combination, did not affect the tumor and the patient died 10 months following his development of overt leukemia. Detailed pathological findings are presented because of the difficulty in differentiating granulocytic sarcoma from non-Hodgkin's lymphoma.

[Preclinical evaluation of a new camptothecin derivative, CPT-11, on the subrenal capsule assay].

CPT-11 is a new derivative of camptothecin, a plant alkaloid developed in Japan. We investigated the antitumor activity of CPT-11 on the subrenal capsule assay by using 39 tumor samples from patients with various tumors. The overall chemosensitivity rate of CPT-11 was 44% and the mean tumor growth inhibition rate was 44% in all the tumors. In 18 ovarian cancer cases, the chemosensitivity rate was 56% and the mean tumor growth inhibition rate was 46%. The antitumor activity obtained by CPT-11 is comparable to that of adriamycin, cyclophosphamide and 5-fluorouracil in the same assay system. Based upon these results, CPT-11 is considered to be a good candidate for clinical trials.

[Subrenal capsule assay--an experimental study and clinical application to chemosensitivity tests].

We have attempted to investigate the usefulness of the subrenal capsule assay (SRCA). Initially, we compared the results of antitumor activities of 27 anticancer drugs obtained using the SRCA in BDF1 mice and the subcutaneous transplantation assay in nude mice, and it became clear that these results correlated well. It was thus apparent that the antitumor activity of drugs could be evaluated faster, more cheaply and more easily using the SRCA. In the SRCA, the overall success rate for growing tumors (with a tumor growth rate of more than 1.0) from patients was 79% (106/134) and the success rates for each tumor type were as follows: malignant lymphoma 90%, ovarian cancer 87%, sarcoma 77%, breast cancer 76%, colorectal cancer 73% and renal cancer 58%. In a retrospective analysis of correlation between the antitumor activities assayed by the SRCA and the respective clinical responses, 11 out of 25 trials were true positive (79%) and 7 trials were true negative (64%). The sensitivity and specificity levels of this test were 73% and 70%, respectively. The tumor growth inhibition rates were correlated well with the clinical responses. The advantages of the SRCA are: it has a high success rate; it allows rapid evaluation; it can be applied for masked compounds; and it has a good clinical correlation. These advantages indicate that the SRCA is of potential value as a predictive test for selection of anticancer drugs on an individual patient basis.

[Analysis of clinical responses in comparison with the results of the chemosensitivity test by subrenal capsule assay].

We attempted to examine the practicality and the usefulness of 6-day subrenal capsule assay (SRCA) as a predictive chemosensitivity test. The success rate of this assay was 81% in 150 cases of various cancers. The drug sensitivity was evaluated by the inhibition rate of tumor growth (TGIR) on day 6 after drug treatment without immunosuppression of mice. We analyzed the clinical response in comparison with the results of SRCA in 38 cases in which transplanted tumor showed over 1.0 tumor growth rate (TGR). The average TGIR in these cases attained CR, PR, MR, NC and PD clinically were 60, 60, 49, 53 and 40%, respectively. The TGIR of CR and PR cases were significantly greater than that of PD cases (p less than 0.05). These results in SRCA correspond to clinically observed responses, using a greater than or equal to 45% inhibition in TGIR to define 'sensitivity'. Of 26 patients sensitive in SRCA, 18 patients (69%) showed true positive and 75% of patients (12 cases) resistant in the assay also failed to respond clinically (true negative). In the cases with less than 1.0 in TGR, a correlation between clinical responses and TGIR was not found. Based on these results, 6-day SRCA without immunosuppresive procedures can be utilized as a predictive chemosensitivity test for cancers which can grow over 1.0 in TGR in this assay system.

[The relationship between dysplasia and squamous cell carcinoma of the esophagus and the consumption of alcohol and tobacco].

Forty-nine cases of esophagectomy and 242 autopsied cases were studied to delineate the pattern of the genesis of esophageal carcinoma. Severe dysplastic lesions found to be commonly associated with carcinoma of the esophagus, but with the exception of one case, these lesions were not associated with other carcinomas. They were more frequent in the cases of multiple primary cancer. This suggests that squamous carcinoma of the esophagus develops from severe dysplasia in a very similar manner to a carcinoma of the cervix uteri. Almost all cases of multiple primary cancers of the oropharynx and the esophagus had a history of heavy smoking and alcoholic drinking habits; so, in some manner, alcohol and tobacco consumption appear to increase the susceptibility of the squamous mucosa to a malignant transformation.

[Organ specificity and common factors in latent tumors].

Materials displaying latent tumors were obtained from autopsy cases and from surgically removed organs at our hospital during the past 20 years. Organ specificity and the common factors of these latent tumors indicated the following: a high incidence of latent tumors was found in the small organs and/or endocrine organs (prostate, thyroid, pancreas). In addition, it was found that these tumorous organs are of sufficient interest for clinicopathologic study (stomach, esophagus). Latent tumors show an increasing tendency to manifestation at an advanced age. Further, shade of difference between females and males has been noted in comparing clinically manifested tumors. Also latent tumors occasionally coexist with multiple primary neoplasms in the same and indifferent organs. The histological characteristics of latent tumors is frequently associated with thickened capsules and fibrosis. It has been speculated that the appearances of a reaction are local autoimmune mechanisms and the hosts suppressive effects against the proliferation of tumors. The biological behaviour of latent and clinical tumors should be considered different.

The technique of segmental mastectomy (lumpectomy) and axillary dissection: A syllabus from the National Surgical Adjuvant Breast Project workshops.

Surveys conducted by the American College of Surgeons indicate an increasing trend toward the treatment of primary breast cancer by segmental mastectomy and radiotherapy. This operation has never been formally taught in training programs, and scant standard descriptive material exists. The National Surgical Adjuvant Breast Project conducted a series of workshops to develop guidelines for the performance of the operation. The essential points are (1) pathologic control of margins to ensure complete surgical excision, (2) surgical principles of complete tumor excision, (3) attention to optimum cosmetic results, and (4) axillary dissection for accurate staging and regional control of the disease. This article summarizes the recommendations made at those workshops.

[Pleomorphic adenocarcinoma of the pancreas with extrapancreatic proliferation].

Two cases of pleomorphic adenocarcinoma of the pancreas are reported. The patients were both women, aged 42 and 44, respectively. Their primary lesions had proliferated extrapancreatically and revealed a clinically specific patterns. They were differed from the general findings of pancreatic cancer. In the histological appearance, tissues such as well-differentiated adenocarcinoma and a mixture of giant cell carcinoma (osteoclastic type) and spindle cell sarcomatous type were seen, and they transferred in either. The extirpation of these tumors was possible, but they recurred in the short term and had an extremely unfavorable prognosis.

[Basic evaluation of measurement of the serum level of squamous cell carcinoma-related antigen (SCC) and its value following irradiation of cancer of the uterine cervix].

The measurement of the serum level of squamous cell carcinoma-related antigen (SCC) purified from liver metastasis of cancer of the uterine cervix by an RIA kit is basically evaluated. The results of sensitivity, the recovery test, dilution test and variance test are good enough for clinical application. In gynecological disorders, the positive rate is high (62% [29/47]) in patients with cancer of the uterine cervix. Furthermore, the rate and level are related with the clinical staging. The changes of the serum SCC level in irradiated patients with cancer of the uterine cervix were a good reflection of the effectiveness of the treatment.

Preparation and properties of the water-soluble chlorophyll-bovine serum albumin complexes.

By mixing chlorophyll (Chl) a or b with a dense bovine serum albumin solution, the water-soluble Chl-bovine serum albumin complexes were prepared. These complexes, eluted near the void volume on a gel filtration, were separated well from unreacted bovine serum albumin, indicating an aggregation of such molecules in the complexes. Preparation of chlorophyllide (Chlide) a- or Chlide b-bovine serum albumin complex was unsuccessful, while the phytol-, and beta-carotene-bovine serum albumin complexes could be obtained. Chls in the Chl-bovine serum albumin complexes had the following characteristics. Main absorption peak of Chl a or b in the red region occurred at 675 nm or 652 nm, respectively. The Chl a-bovine serum albumin complex having absorption peak at 740 nm was also prepared. As compared with the stabilities of Chl a and b in Triton X-100. Both Chls in the bovine serum albumin-complexes were stable against oxidative stresses, such as photobleaching, Fenton reagent, peroxidase-H2O2 system. But they were easily hydrolyzed by chlorophyllase. These properties of Chls in the bovine serum albumin-complexes were similar to those of Chls in the isolated light-harvesting Chl a/b protein complex. A possible localization of Chls within the bovine serum albumin complexes was suggested that the porphyrin moiety of Chl was buried in bovine serum albumin; however, the hydrophilic edge of porphyrin ring, adjacent to the phytol group, occurred in the hydrophilic region of a bovine serum albumin molecule.

Fibrinolysis in patients with acute promyelocytic leukemia and disseminated intravascular coagulation during heparin therapy.

Two patients diagnosed as having acute promyelocytic leukemia (APL) and disseminated intravascular coagulation (DIC) were closely followed by serial fibrinolysis and coagulation studies from the day of admission until completion of the first course of chemotherapy. One patient was treated with intravenous heparin and Trasylol (Bayer AG, West Germany) and the other received heparin therapy without Trasylol. In Patient 1, hyperfibrinolytic activity, not observed during the administration of Trasylol, developed with its discontinuance. In Patient 2, hyperfibrinolysis was observed coincidentally with a decrease in APL cells due to chemotherapy. These results indicate that hyperfibrinolysis in APL is not associated with DIC.

Central representation of the hindlimb muscles supplied by the common peroneal nerve. A retrograde horseradish peroxidase study in the dog.

Motoneurons supplying the common peroneal nerve in the dog were identified by the retrograde horseradish peroxidase method. They were distributed within two discrete cell columns (columns 2 and 2') in the 6th and 7th lumbar segments. The extensor digitorum longus muscle was represented in the dorsolateral part of column 2; the peroneus longus muscle in the ventrolateral part and the tibialis cranialis muscle in the intermediate lateral part. The medial half of column 2 contained motoneurons supplying the superficial peroneal nerve. Column 2', which was situated dorsomedially to column 2, contained motoneurons innervating the extensor digitorum brevis muscle.

Action of TMB-8 (8-(N,N-diethylamino)octyl-3,4,5-trimethoxybenzoate) on cytoplasmic free calcium in adrenal glomerulosa cell.

To clarify the action of 8-(N,N-diethylamino)octyl-3,4,5-trimethoxybenzoate (TMB-8) on cellular calcium handling, changes in cytoplasmic free calcium concentration ([Ca2+]c) were studied in adrenal glomerulosa cell with a calcium-sensitive photoprotein, aequorin. Results of our previous study demonstrate that 100 microM TMB-8 almost completely blocks aldosterone response to angiotensin II (Biochem. J. 232 (1985) 87-92). At 50 or 100 microM, TMB-8 decreased basal [Ca2+]c significantly; however, these doses of TMB-8 had little effect on an angiotensin-induced increase in [Ca2+]c. When angiotensin-induced calcium release from an intracellular pool(s) was assessed by measuring changes in [Ca2+]c in the presence of 1 microM extracellular Ca2+, 100 microM TMB-8 had little inhibitory effect on angiotensin-induced calcium release. A higher dose of TMB-8 (250 microM) slightly inhibited calcium release. Additionally, TMB-8 did not affect exogenous arachidonic acid-induced calcium release. In contrast, 50 microM TMB-8 markedly inhibited 8 mM potassium-induced increase in [Ca2+]c. These results indicate that a major action of TMB-8 on cellular calcium is an inhibition of calcium influx but not of calcium release. We suggest that TMB-8 should not be used as an 'inhibitor of calcium release'.

Pertussis toxin blocks angiotensin II-induced calcium influx but not inositol trisphosphate production in adrenal glomerulosa cell.

Involvement of GTP-binding proteins in angiotensin II-induced mobilization of calcium has been examined in adrenal glomerulosa cells by using pertussis toxin. Pretreatment of glomerulosa cells with pertussis toxin abolishes angiotensin II-induced calcium influx without blocking inositol trisphosphate production. These results suggest a role of the pertussis toxin-sensitive GTP-binding protein in transducing angiotensin II-receptor occupancy into opening of calcium channel.
