Mechanism of action of the synthetic imidazole compound YM 534 on human promyelocytic leukemia line HL-60 cells.

The effects of a novel anti-cancer drug, YM 534 on human promyelocytic leukemia line HL-60 cells were investigated. The growth of the cells was completely inhibited with an IC50 of 2.5 X 10(-6) M. The incorporation of 3H-thymidine or 3H-uridine into acid insoluble fraction of the cells was completely inhibited at a 10(-4) M concentration of the drug. However, that of 3H-leucine was suppressed by only 32% at this drug concentration. The single strand scission of DNA of the cells as revealed by alkali sucrose density gradient centrifugation was induced by YM 534 at a 10(-4)M concentration for 60 min. When PM2 DNA in a cell-free system was examined in the presence or absence of reducing agent, no damage to DNA was observed at a 10(-4)M YM 534. The processing of preribosomal to ribosomal RNA of HL-60 cells was retarded at a 10(-5) M level of the drug.

[Biliary excretion and clinical efficacy of T-3262 (tosufloxacin tosilate) administered in the treatment of cholecystitis].

T-3262 (tosufloxacin tosilate), a new oral pyridone carboxylic acid agent, was investigated for its biliary excretion and clinical efficacy and safety to evaluate its usefulness in the treatment of cholecystitis. T-3262 was administered to a total of 4 healthy volunteers for 2 days at a dose of 150 mg every 8 hours, and A-, B- or C-bile were collected using the MELTZER-LYON method at 10-11 hours after the final administration. Bile concentrations of T-3262 in 3 cases were 0.33-2.05 micrograms/ml (A-bile), 6.13-9.50 micrograms/ml (B-bile) and 1.11-2.70 micrograms/ml (C-bile). Thus, T-3262 levels in B-bile were 15-34 times higher than serum levels (0.28-0.41 micrograms/ml). Only a trace of serum concentration of T-3262 was detected in another case with the concentration in B-bile was 0.132 micrograms/ml. A total of 10 patients with cholecystitis were treated with T-3262 at a dose level of 150 mg per dose 3 times daily for 1 to 20 days. The clinical efficacy was excellent in 1 case, good in 5 cases and fair in 2 cases and unevaluable in 2 cases, thus the clinical efficacy rate was 75%. Bacteriologically, Klebsiella pneumoniae, Enterococcus faecalis and Haemophilus parahaemolyticus were isolated from biles of 3 patients before treatment. Upon the treatment, E. faecalis was eradicated and K. pneumoniae was unchanged. The fate of H. parahaemolyticus was not known because of examination was not done after treatment. Side effects were observed in 2 cases with diarrhea in 1 case and epigastric pain in another case. But those symptoms disappeared after cessation of administration of T-3262. Abnormal laboratory test values were not observed.

[A histopathological investigation of metastatic liver tumors--the classification of their histological patterns and metastatic pathways taking into account the microscopic metastatic foci].

A histopathological study of metastatic liver cancers has been performed on 433 autopsied cases at the Shikoku Cancer Center Hospital. With respect to the intrahepatic spread of the metastatic liver carcinomas, their invasion through the portal vein and the sinusoid were separately examined. The histological patterns of the prevalence of the liver metastasis were classified by taking into account both the factors and the prevalence of the nodules. A liver metastasis which was not detected by the naked eye but by microscopy was considered to be an early stage metastasis. All histological characteristics as well as the metastatic pathways also were considered.

A direct projection from the entorhinal cortex to the mammillary nuclei in the rat.

A direct projection from the entorhinal cortex to the mammillary nuclei was found in the rat, by using retrograde and anterograde transport of wheat germ agglutinin-horseradish peroxidase. The projection originates from cells in layer III of the medical entorhinal area and terminates almost ipsilaterally within the ventrolateral region of the pars lateralis of the medial mammillary nucleus. The terminal fields consist of two distinct spots; one is located ventral to the fornix fibers entering the mammillary nuclei and the other situated more caudoventrally in the most ventrolateral region of the pars lateralis. The results suggest that cells in the medial entorhinal area are directly connected with cells in the two restricted regions of the medial mammillary nucleus.

Alpha-interferon treatment for adult T cell leukemia: low levels of circulating alpha-interferon and it's clinical effectiveness.

We describe a patient with adult T cell Leukemia to whom alpha-interferon therapy was highly effective. Although a combination chemotherapy (ACVP) first introduced was effective in reducing total leukocyte counts, the percentage of leukemic cells relative to total leukocyte counts was decreased first after the institution of alpha-interferon therapy. The patient is now under complete remission for four years. It was noted in this patient that circulating alpha-interferon, measured by a sensitive radioimmunoassay, was consistently low as compared with the value found in the age-, sex-matched healthy control (p less than 0.001). Since adult T cell leukemia is pathogenetically related to the retrovirus infection, low levels of circulating alpha-interferon of the patient may be important from both pathogenetic and therapeutic standpoints. Alpha-interferon therapy may be an useful additive for the chemotherapy of adult T cell leukemia.

A new nitrosourea derivative for the treatment of chronic myelogenous leukemia.

A new water-soluble nitrosourea derivative, methyl 6-[3-(2-chloroethyl)-3-nitrosoureido]-6-deoxy-alpha-D-glucopyranoside (MCNU), was found to be useful for the treatment of chronic myelogenous leukemia (CML) in the chronic phase. To compare the efficacy of MCNU with that of busulfan, patients were randomized. In the 40 patients administered MCNU, the median time to the achievement of a complete remission (CR) was 50 days. This value was shorter than that observed in 37 patients administered busulfan (126 days, p less than 0.05). There were no differences in the rate of CR achieved, mortality, median time to the onset of blast crisis (BC), BC rate, or survival rate during the observation period. The overall incidence of side effects was higher for MCNU (31%) than for busulfan (15%), but the symptoms were mild, transient and tolerable for most patients. These results suggest that MCNU is a safe and valuable addition to the therapeutic repertoire for the control of CML.

Prevalence of disability in instrumental activities of daily living among elderly Japanese.

The prevalence of disability in instrumental activities of daily living (IADL) was assessed for seven items of activity -- among them, using public transportation, using the telephone, and shopping -- in 7,735 elderly residents living in an urban Japanese community. The prevalence of disability generally was low (ranging from 6.1% in heating water to 15.9% in preparing meals), but increased significantly with age. The prevalence was higher, controlling by age, in females than in males, with the exception of preparing meals.

Amino acid sequence of winged bean (Psophocarpus tetragonolobus (L.) DC.) chymotrypsin inhibitor, WCI-3.

The complete amino acid sequence of winged bean chymotrypsin inhibitor 3 (WCI-3) was determined by the conventional methods. WCI-3 consisted of 183 amino acid residues, but was heterogeneous in the carboxyl terminal region owing to the loss of one to four carboxyl terminal amino acid residues. The sequence of WCI-3 was highly homologous with those of soybean trypsin inhibitor Tia, winged bean trypsin inhibitor WTI-1, and Erythrina latissima trypsin inhibitor DE-3. One of the reactive site peptide bonds of WCI-3 was identified as Leu(65)-Ser(66), which was located at the same position as those of the other Kunitz-family leguminous proteinase inhibitors.

Homogeneous liposome lysis assay for determination of anti-streptolysin O antibody titer in serum.

We developed a liposome lysis assay for determining anti-streptolysin O antibodies (ASO) in human sera involving the use of carboxyfluorescein-entrapped multilamellar liposomes. This assay system was based on the inhibition of streptolysin O-induced liposome lysis by ASO. Briefly, after incubation of a given amount of streptolysin O with ASO for 30 min at 37 degrees C, carboxyfluorescein-entrapped liposomes composed of egg yolk phosphatidylcholine and cholesterol in a molar ratio of 1:1 were added to the mixture to determine the residual streptolysin O activity. Liposome lysis, detected as carboxyfluorescein release from the liposomes, was inversely proportional to the ASO titer. The results of within-run and between-run precision studies indicated that the liposome lysis assay is accurate and gives reproducible data. Bilirubin, hemoglobin, and triglycerides did not interfere with the liposome lysis assay. The ASO titers of 100 patient sera, evaluated by our new method and the Rantz-Randall method, showed good correlation.

Effects of hypertension and antihypertensive drugs on cardiovascular complications in the elderly.

The role of hypertension and antihypertensive drugs in cardiovascular complications was evaluated in 380 elderly people living in the Tokyo Metropolitan Gerontology Center. The subjects were classified into four groups according to the presence or absence of hypertension and their antihypertensive treatment, and followed up prospectively for 5 years from 1979 to 1984. The average age of each group was 74 to 76 years. Cerebrovascular disease was observed in 19.3% of male hypertensives and 10.1% of male normotensives (p = 0.078). The drug treated group revealed no cerebral hemorrhage and less cerebral infarction. This tendency was not observed in females. Ischemic heart disease was prevalent in the drug treated group (10.9% vs 4.5%, p = 0.023) irrespective of blood pressure level. Risk factors such as body mass index, skinfold thickness, serum cholesterol, albumin, creatinine, blood urea nitrogen and uric acid at entry were elevated in the drug treated group. Diuretics were used in 92% of the drug treated group; in 53% as monotherapy and in 39% as combination therapy with other antihypertensive agents. The metabolic effect of diuretics may increase the incidence of ischemic heart disease in the elderly. We might conclude that hypertension in the aged accelerates cerebrovascular complications, and that antihypertensive treatment is effective even in this group. However, the wide use of diuretics could increase the incidence of ischemic heart disease. Careful selection of antihypertensive drugs as well as dose adjustment are needed in the treatment of elderly hypertensives.

Blood clearance of 99mTc-phytate for evaluation of hepatic dysfunction in dogs.

Organ distribution pattern, and blood clearance were examined by giving 99mTc-phytate (99mTc-P) to the normal dogs. At the same time, the relation between the severity of hepatic function and blood clearance of 99mTc-P was studied by using the dogs with acute hepatic dysfunction experimentally induced by carbon tetrachloride (CCl4) administration. Furthermore, a comparative discussion on blood clearance test of 99mTc-P was made with serum transaminase test. It appeared appropriate to administer 99mTc-P at the dose of 100 micrograms/kg in order to obtain an effective blood clearance curve. A major part of 99mTc-P intravenously administered was taken up into the liver, while the remainder of small amount into the spleen, kidneys, lung, and so on. Little was recognized in the thyroid. The disappearance rate of 99mTc-P from blood decreased with the increase in dose of CCl4 and with the passage of time after the CCl4 administration. The blood clearance test of 99mTc-P in dogs showed a sensitive reaction for the acute hepatic dysfunction induced by CCl4 equally to the serum transaminase test.

Blood clearance of 99mTc-phytate for evaluation of hepatic dysfunction in rats.

Organ distribution pattern, hepatic uptake ratio, and blood clearance were examined by giving 99mTc-phytate (99mTc-P) to the normal rats. At the same time, the relation between the severity of hepatic function and blood clearance or hepatic uptake ratio of 99mTc-P was studied by using the rats with acute hepatic dysfunction experimentally induced by carbon tetrachloride (CCl4) administration. Furthermore, a comparative discussion on blood clearance test of 99mTc-P was made with serum transaminase test and with histological test of the liver. It appeared appropriate to administer 99mTc-P at the dose of 500 micrograms/kg in order to obtain an effective blood clearance curve. A major part of 99mTc-P intravenously administered was take up into the liver, while the remainder of small amount into the spleen, kidneys, lung, and so on. Little was recognized in the thyroid gland. The hepatic uptake ratio of 99mTc-P reached the maximum 15 minutes after the administration. The disappearance rate of 99mTc-P from blood decreased with the increase in dose of CCl4 and with the passage of time after the CCl4 administration. The blood clearance test of 99mTc-P showed a sensitive reaction for the acute hepatic dysfunction induced by CCl4 equally to or higher than the serum transaminase test or histological test of the liver.
