Clinical course of acute middle cerebral artery occlusion.

Knowledge of the natural course of stroke patients has become increasingly important since new therapeutic methods have been proposed for patients with cerebral infarction in the acute stage. In order to clarify the acute stage of this disease, 188 patients admitted within 24 hours after onset of middle cerebral artery (MCA) occlusion were followed for 2 months, and data relating to mortality and changes in disturbances of consciousness and motor function were investigated. It was shown that the prognosis for MCA occlusion cases is poor, and about 80% of these patients are unable to return to their previous lifestyle. The level of consciousness in the acute stage is a good index for estimating the patients' quality and time of survival, and motor function in the acute stage is a good indicator of functional recovery. Thus, when evaluating the effectiveness of a new therapy for cerebral infarction, rapid improvement in the acute stage before and after treatment should be carefully noted.

Malignant clonal expansion of large granular lymphocytes with a Leu-11+, Leu-7- surface phenotype: in vitro responsiveness of malignant cells to recombinant human interleukin 2.

A 14-year-old Japanese female with neutropenia showed malignant proliferation of the large granular lymphocytes (LGLs). These LGLs were E rosette+ and Fc(IgG) receptor+ and therefore are referred to as T gamma lymphocytes. They were also Leu-11+ and OKT11+; however, they were clearly negative for Leu-7, OKT3, OKT8, OKM1, and HNK-1 antigens as well as for terminal deoxynucleotidyl transferase activity. Karyotype analysis revealed 47, XXX. The LGLs showed no rearrangement of T cell receptor C beta genes. The natural killer (NK) cell activity against K562 target cells was low, but was significantly augmented after stimulation by recombinant human interleukin 2 (IL 2) in contrast to minimal NK boosting by recombinant human gamma-interferon (gamma-IFN). Such a unique responsive ability to lymphokines was quite similar to that noted in fetal and cord blood cells. These LGLs also demonstrated a considerable increase in antibody-dependent cell-mediated cytotoxicity (ADCC) and lymphokine-activated killer (LAK) activity after a short incubation with IL 2. Although in a resting stage they showed no IL 2 receptor expression as examined by anti-Tac antibody, Tac antigen appeared after IL 2 treatment followed by a marked increase in 3H-thymidine incorporation and a remarkable production of gamma-IFN. To investigate the mechanism of neutropenia, in vitro IL 2-stimulated coculture studies of these cells with normal bone marrow cells were performed. Colony formation of myeloid progenitors (CFU-C) was significantly suppressed. In addition, the conditioned medium from IL 2-stimulated LGLs indicated a remarkable suppression of CFU-C. These results suggest that these LGLs with a Leu-11+, Leu-7- surface phenotype might belong to a unique subset of pre-NK cells that are functionally and phenotypically similar to those represented at any early stage of human ontogeny and that they strongly express Tac antigen under the influence of IL 2 administration, followed by remarkable cell proliferation and gamma-IFN production.

[Studies on plasma proteins in normal pregnancy and gynecological malignancy].

The purpose of this study is to know and compare the changes in plasma proteins in normal pregnancy with those in gynecological malignancies. The results obtained are as follows. Factor VIII activity continued to increase during pregnancy and reached about 200%. It also continued to increase and showed a high level (250-300%) in malignancy, in advanced stages. At III activity increased in the first trimester and showed reached a normal level after 28 weeks of gestation. It decreased on the third day of puerperium and recovered to normal 1 month after delivery. At III activity decreased only in the end stage of malignancy. Factor XIII activity continued to decrease and was 50-60% during the second and third trimester. It began to recover at the third day of puerperium and completely recovered at 1 month after delivery. Factor XIII activity also continued to decrease and was at a low level (33.8-51%) in advanced stages. Plasminogen activity increased from the first trimester and was 120-130% during the second and third trimesters and after delivery. In malignancy, it did not show any remarkable change. alpha 2-PI activity moderately increased in the first trimester, but was at a normal level during the second and the third trimester. It increased slightly on the third day of the puerperium and recovered to normal 1 month after delivery. alpha 2-PI activity showed no remarkable change in malignancy, but decreased in some advanced cases. HRG began to decrease from the second trimester and was 46.5% at the end of the third trimester. In the puerperium, HRG began to recover on the third day and reached normal at 1 month. HRG showed a low titer in advanced stages of malignancy. Sialic acid increased constantly during pregnancy, but it was within the normal range. On the third day of puerperium, it reached 72.3mg/dl. Sialic acid showed a high level with in advanced stages of malignancy. IAP did not change during pregnancy. It showed a slight increase on the third day after delivery, but it was within the normal range. Malignancy showed a high level of HRG with in advanced stages. CIG was normal during and after pregnancy. In cervical carcinoma, CIG increased at in Ib, II and cervical carcinoma. Ovarian carcinoma did not show any change.

[Fundamental study of imipenem/cilastatin sodium in obstetrics and gynecology].

Following results were obtained from drip intravenous administration of imipenem/cilastatin sodium (MK-0787/MK-0791) (500 mg/500 mg) by measuring concentration of MK-0787 in uterine arterial plasma, cubital venous plasma, oviduct, ovary and several sites in uterine tissue in cases of simple hysterectomy, and pelvic cavity fluid in cases of radical operation. Cervix uteri, portio vaginalis, myometrium showed higher concentration among various uterine tissues in any time after the end of administration. In cases of radical operation, the pelvic cavity fluid showed 6.6 approximately 7.8 micrograms/ml at 5 hours after the end of administration. In the field of obstetrics and gynecology, it was considered that MK-0787/MK-0791 has good efficacy in infections especially caused by Gram-positive aerobic bacteria.

Quantitative cerebral blood flow measurements using N-isopropyl-(iodine 123) p-iodoamphetamine and single photon emission computed tomography with rotating gamma camera.

Sixty regional cerebral blood flow measurements were performed on 4 normal volunteers, 7 epileptics, and 40 cerebrovascular disorders using intravenously injected N-isopropyl-(I-123)p-iodoamphetamine (123I-IMP) and single photon emission computed tomography with rotating gamma camera. Arterial blood sampling was combined for obtaining absolute blood flow values. The brain activity distributions of the tomographic image from 30 min after injection, when brain activity reached a plateau, were corrected to represent 5-min reference values with the use of the monitored entire brain's time-activity curve. Brain mean blood flow values ranged from 54 to 63, 34 to 59, and 20 to 60 ml/100 g/min, in normal volunteers, epileptics, and subjects with cerebrovascular disorders, respectively. Brain mean 123I-IMP uptake corrected for injection dose did not correlate with these absolute flow values. This quantitative method is especially useful for diagnosing diffuse flow reductions, which were observed in 8 (14%) of 56 studies in the patients.

The difference in sex hormone dependence for gap junction formation in the muscles of uterine leiomyoma and normal myometrium.

Gap junction is reported to be initiated by changes in levels of steroid hormones in the smooth muscle cells of myometrium. In order to clarify the difference in sex hormone dependence for gap junction formation in the muscles of uterine leiomyoma and normal myometrium, uterine leiomyoma and myometrial gap junctions, and serum estradiol and progesterone levels were determined simultaneously in 23 women who had undergone simple hysterectomy for leiomyomas of the corpus uteri. In all tissues of the myometrium from 23 women, at least three gap junctions were found between muscle cells in the series of photographs. On the other hand, there were 7 out of 23 uterine leiomyomas in which not even one gap junction was detected between muscle cells in the series of photographs. The phasic change in the number of gap junctions related to the menstrual cycle was detected in the muscles of uterine leiomyoma as well as normal myometrium. Uterine leiomyoma was characterized by less correlation between the number of gap junctions in the muscles and the concentration of serum sex steroids, compared with those of normal myometrium. Sex steroids may have a trophic effect on some uterine leiomyoma, but the sex hormone dependence of uterine leiomyoma for gap junction formation may be less than that of normal myometrium. The present study also showed that the increased gap junction formation in myometrial muscles from women with dysmenorrhea may be formed between myometrial cells in response to physiologic or pathologic stimuli, such as the production of local prostaglandins, and may result in hypercontraction of the uterus.

[Sequential changes of autoregulation in experimental focal cerebral ischemia--pathophysiology at the center, periphery and outside of the focal cerebral ischemia].

In order to elucidate the relationship between the degree of autoregulatory loss and the intensity or the duration of ischemia, and the difference of locations in and around the ischemic focus, we used the canine thalamic infarction model and studied the sequential changes of rCBF and autoregulation during 6 hours following vascular occlusion. The value of rCBF was measured by the hydrogen clearance method and autoregulation was tested by raising the blood pressure with drip infusion of angiotensin II. In the center of ischemic focus, autoregulation was impaired after 1, 3 and 5 hours following occlusion in the animals with moderate ischemia. In the animals with mild and severe ischemia, autoregulation was preserved during occlusion, but in the latter we thought it false autoregulation. In the periphery of infarctic focus with severe ischemia, autoregulation was impaired after 3 and 5 hours following occlusion. Outside of the infarctic focus with severe ischemia, autoregulation was preserved during occlusion, but in 2 of 6 animals rCBF decreased despite of raising blood pressure and it was thought to be paradoxical response.

Effects of monovalent cations and anions on ADP-induced aggregation of bovine platelets, and mechanisms thereof.

The effects of monovalent cations-inorganic alkali metal cations and organic quaternary ammonium cations-and monovalent inorganic anions on ADP-induced aggregation of bovine platelets were investigated. In the presence of K+, Rb+, Cs+, choline or tetramethylammonium, aggregation proceeded. However, aggregation was markedly restricted in media containing Li+, Na+, tetrabutylammonium or dimethyldibenzylammonium. With anions, aggregation proceeded in the order Cl- greater than Br- greater than I- greater than ClO4- greater than SCN-. The effects of cations significantly depended on Ca2+ concentration, whereas those of the anions depended little on Ca2+. Anions such as SCN- and ClO4- markedly decreased the fluorescence of the surface charge probe 2-p-toluidinyl-naphthalene-6-sulfonate, whereas cations had less pronounced effects. The relative effects of the anions on the fluorescence were consistent with their relative inhibitory effects on aggregation. These results suggest that inhibition of platelet aggregation by the anions is due to a change in the surface change of the platelet plasma membrane. On the other hand, kinetic analysis suggests that the effects of monovalent cations on platelet aggregation are due to their competition with Ca2+ during the process of aggregation.

Quantitative determination by derivative spectrophotometry of Triton X-100 in solubilized preparations of membrane proteins.

The amount of the nonionic detergent Triton X-100 in protein preparations cannot be determined spectrophotometrically from the normal spectrum because the spectral bands of the detergent and proteins overlap, but it was found that the detergent could be determined accurately by 4th-derivative spectrophotometry. The intensity of the derivative absorbance difference between the positive and negative peaks at about 277.5 and 281 nm, respectively, increased linearly with increase in the detergent concentration at more than its critical micelle concentration (about 0.03%). Proteins had little effect on these spectral bands of the detergent. This method is very simple and accurate for determination of the concentration of Triton X-100 in solubilized preparations of membrane proteins.
