[The efficacy of combined use of 1 alpha-hydroxyvitamin D3 with calcium supplements in the treatment of osteoporosis].

Synthetic analogue of active vitamin D metabolite, 1 alpha (OH) D3, has been widely used in the treatment of osteoporosis. However, the most effective method of treatment is yet to be established. Importance of calcium supplement to improve the calcium metabolism in osteoporosis is also reported by many authors. We have studied the combined effects of 1 alpha (OH)D3 with calcium supplement in preventing progressive decrease of bone mass in patients with osteoporosis. Sixty-six cases of postmenopausal and senile osteoporosis were divided into two groups: one treated with calcium alone 1,000 mg a day and the other treated with calcium 1,000 mg and 1 alpha (OH)D3 0.5 microgram a day, and both groups were followed for 24 months. Bone mass was evaluated by microdensitometry of the roentgenograms of the second metacarpal bone. The results revealed that the combined use of 1 alpha (OH)D3 with calcium has a significantly more favorable effect than the use of calcium alone in preventing bone loss. Therefore, 1 alpha (OH)D3 with calcium is useful in the treatment of osteoporosis. The conclusion would be further confirmed with longer term study and a more accurate method of measuring bone mass.

Evaluation of cerebral collateral circulation by technetium-99m HM-PAO brain SPECT during Matas test: report of three cases.

Three cases with cerebral ischemic symptoms and an intracranial aneurysm are presented. Using [99mTc]hexamethyl-propyleneamine oxime (HM-PAO) single photon emission computed tomography imaging before and during the Matas test, a quantitative measurement method was developed for evaluating brain collateral circulation. The evaluation correlated well with findings of contrast carotid angiography. This noninvasive method seems to be useful for selection of patients for appropriate surgical treatment.

Multiple brain tumours with von Recklinghausen's disease.

Over the past 10 years we have experienced 33 cases of von Recklinghausen's disease, among whom 5 were cases of multiple (3 or more) brain tumours. In the past we have extirpated multiple tumours in a one-stage operation, but the results were poor. Recently, we have done multi-stage operation in two cases thought to require surgical treatment and have obtained satisfactory results. We have discussed the autopsy results of our own case of multiple neurinomas and meningiomas and have reviewed the 44 reported cases of von Recklinghausen's disease in whom the presence of three or more brain tumours had been confirmed at surgery or autopsy.

[A study on the activity of the PGI2 producing enzyme system derived from maternal and fetal vessels of toxemia of pregnancies].

We investigated the conversion rate from arachidonic acid (A.A.) to PGI2 and the activity of PGI2 producing enzyme system in endothelial cells of umbilical veins (U.V.) and maternal omental veins (O.V.) in normal pregnancy (NOR) and toxemia of pregnancy (TOX). The conversion rate from A.A. to PGI2 for U.V. and O.V. in NOR was higher than that in TOX. There was a significant difference between NOR and TOX with the conversion rate for U.V. at a concentration of 172.4 nM/ml A.A. The apparent Vmax value for PGI2 producing enzyme system (nM/mg) in U.V. in NOR (n = 7) was 0.88 +/- 0.21 (mean +/- S.E.) and that for TOX (n = 3) was 1.63 +/- 0.18. The apparent Km value for the enzyme system (microM) in U.V. in NOR (n = 7) was 0.75 +/- 0.25, and that for TOX (n = 3) was 3.26 +/- 0.78. Significant differences (p less than 0.05) between NOR and TOX were observed with Vmax and Km values. The apparent Vmax value for the enzyme system in O.V. in NOR (n = 3) was 0.32 +/- 0.13, and that for TOX (n = 3) was 2.12 +/- 0.38. The apparent Km value for the enzyme system in O.V. of NOR (n = 3) was 0.26 +/- 0.06, and that for TOX (n = 3) was 0.97 +/- 0.09. There was a significant difference between (p less than 0.05) the Vmax and Km values for NOR and TOX. The present study showed that U.V. (fetal vessel) and O.V. (maternal vessel) in TOX had a lower activity PGI2 producing enzyme system than those in NOR due mainly to an imbalance between the substrate supply and cyclooxygenase activity.

A new noninvasive quantitative assessment of cerebral blood flow using N-isopropyl-(iodine 123)p-iodoamphetamine.

The use of a nomogram is described as a new noninvasive quantitative assessment of cerebral blood flow using N-isopropyl-(123I)p-iodoamphetamine (IMP) and single-photon emission computed tomography. The nomogram was constructed from a computer analysis of the flow value relative to varying washout rate of brain activities between early and late tomographic images, and partition coefficient between brain tissue and blood. The validity of this method was evaluated in normal volunteers and in patients with cerebrovascular disorders by comparing the cortical flow values (Y) estimated from the IMP-nomogram with initial slope indices (X) by the 133Xe inhalation method. A significant correlation (N = 42, r = 0.88, P less than .001) was demonstrated between them, and the regression line was Y = 1.6X-7. A development and confirmation of this method made it possible to measure flow values noninvasively with the use of IMP.

Interacting effects of zinc and cadmium on the cadmium distribution in the mouse.

Effects of zinc (Zn) on cadmium (Cd) distribution in the mouse body were investigated after intranasal administration. The amounts of Cd (2 x 10(-6) M/0.02 ml) reaching respiratory organs increased as the ip dose of pentobarbital increased. Administration of Zn with Cd (2 x 10(-6) M Zn + 2 x 10(-6) M Cd /0.02 ml) further increased the Cd amounts reaching respiratory organs and increased mouse mortality. A single administration of Cd increased Cd levels in blood, liver, kidney and respiratory organs. High levels of Cd were maintained for at least 90 days in these organs after single administrations of Cd. The administration of Cd plus Zn further increased the Cd content in these organs during 0-60 min after administration. However, simultaneous administration of Zn with Cd inhibited prolonged Cd accumulation in kidney and liver. These results suggest that intranasally administered Zn (2 x 10(-6) M) has dual effects on Cd movement. Zn further increases the Cd levels in the lung and mouse mortality increases. On the contrary, Zn inhibits prolonged Cd accumulation in mice and decreases the chronic toxicity in mice that survive the acute phase.

Phenotypic and functional characteristics of active suppressor cells against IFN-gamma production in PHA-stimulated cord blood lymphocytes.

Cord blood mononuclear cells (MNC) were defective in their ability to produce interferon-gamma (IFN-gamma) on stimulation with phytohemagglutinin (PHA) or recombinant interleukin 2, whereas cord MNC could induce comparable amounts of IFN-gamma with adult controls on stimulation with a streptococcal preparation, OK-432. Moreover, irradiation of cord MNC with 1,500 rad before PHA stimulation could restore the IFN-gamma production. Kinetic studies indicated that such augmentation of IFN-gamma production by irradiation was evident when cord MNC were irradiated before or by 12 hr of PHA-stimulated culture. But irradiation after 18 hr or more of PHA stimulation did not exert any significant augmentation on IFN-gamma production by cord MNC. It seemed most likely that the ability of IFN-gamma production is already mature at birth, but radiosensitive suppressor effectors on IFN-gamma production are activated within cord MNC at an early stage of PHA stimulation, resulting in poor IFN-gamma production by cord MNC. PHA-induced IFN-gamma production by OKT3+, OKT4+, and OKT8- cord cells were markedly enhanced by irradiation with 1,500 rad before the culture. Coculture experiments disclosed that cord OKT4+ cells, but not OKT4- cells, when prestimulated with PHA for 24 hr, exerted active suppression on PHA-induced IFN-gamma production by adult MNC in a dose-dependent manner. These results suggested that radiosensitive suppressor effectors on IFN-gamma production were induced within the OKT4+ T cell subset of cord MNC on PHA stimulation.

Significant increase in immunosuppressive acidic protein (IAP) in serum of patients with multiple sclerosis and other inflammatory neurological disorders.

IAP, a type of alpha 1-acid glycoprotein, is mainly produced by macrophages when stimulated in the presence of circulating immune complexes or some inflammatory substances. We assayed the serum levels of IAP by a single radial immunodiffusion method. The normal level of IAP is below 500 micrograms/ml (385 +/- 73). In multiple sclerosis patients, however, IAP increased during exacerbation (630 +/- 191) and decreased during the inactive stage (433 +/- 170). Eighty-five percent of patients with neuro-Behcet's disease also had high levels of IAP, correlating well with disease activity. In some patients with Guillain-Barré syndrome or Miller Fisher syndrome, IAP increased during the acute phase. In patients with herpes simplex encephalitis, IAP levels remained abnormally high for more than 60 days after onset. The mean value of IAP in patients with amyotrophic lateral sclerosis did not differ from that of normal controls. An increase in IAP in the serum of patients seems to reflect the activity of an inflammatory or immunopathological process.
