[Brain performance and behavior in senile dementias of the Alzheimer type].

Following a brief review of the recent literature on dementia methodological problems in the assessment of intellectual disturbances, psychopathological syndroms and modifications of behaviour in dementia are discussed. The study deals with the course of illness of untreated patients suffering from senile dementia of the Alzheimer's type. Patients were investigated five times during a period of 35 days. Scales used were the SKT-FPB test system (Erzigkeit, 1977), the Sandoz Clinical Assessment Geriatrics (SCAG, Shader et al., 1974) and the Geriatric Rating Scale (GRS, Plutchik et al., 1970). The first test system measures brain performance were the SCAG and the GRS mainly deal with psychopathological and behavioural disturbances. For resp. five factors of SCAG resp. GRS could be separated through hierarchical cluster analysis. No significant difference between the values of the SKT-FPP test system and the two observer rating scales could be found at any time of the study. This also proved right when dementias were separated clinically into mild and severe forms. These two groups could only be differentiated by the SKT-FPP test system, the factor 1 (organic impairment) of the SCAG and the factor 4 (social maladaption) of the GRS. Following a separation of clinically ameliorated, worsened or unchanged patients at the end of the study significant differences could only be found in the values of the FP-standard points and the factors organic impairment, depression and social maladaption in the observer rating scales. The impression, that prognosis of patients with a statistically unstabile course (descriptively assessed by means of the sum of variances) would be different from patients with stabile courses could not be proven. Investigation by correlation statistics of the whole patient groups at the test points showed that the FP-standard points correlated only with the factors social maladaption and organic impairment. It is concluded that according to this data best assessment of the course of illness is by psychometric methods, i.e. the SKT-FPB test system in this study. Observer rating scales, frequently mentioned in the literature seem to be more inconsistent and problematic for the valuation of the course of illness of dementias. They also seem to be in sufficient to evaluate effects of therapy, a fact being of major importance when concerning their frequent use in psychopharmacological trials in demented patients. The study could also show, that the variation in course of illness of patients suffering from senile dementia of the Alzheimer type is so big, that grouping these patients leads to the loss of statistical differences of their illness.

The sensitizing capacity of Alstroemeria cultivars in man and guinea pig. Remarks on the occurrence, quantity and irritant and sensitizing potency of their constituents tuliposide A and tulipalin A (alpha-methylene-gamma-butyrolactone).

Dermatitis in 8 female nursery workers handling Alstroemeria ligtu cultivars has been proven/proved in 6 cases to be of allergic origin. Epicutaneous tests with cut flower extracts as well as with the isolated and purified sensitizer were positive. Successful animal experiments corroborated the sensitising capacity of Alstroemeria cultivars. The responsible but unstable contact allergen, alpha-methylene-gamma-butyrolactone (tulipalin A), was found in short ether extracts of flower petals in concentrations up to 18%. Due to its instability, subsequent extractions were performed with methanol, yielding the sensitising constituent after purification in the glucosidic form (tuliposide A). This could be stored at room temperature for longer periods without loss of activity. Tuliposide A was determined in specimens of Alstroemeria ligtu cultivars of 14 different origins of various colours; its content varied between 1 and 2%. Direct testing of the plant material in human patients carries the risk of false positive reactions and active sensitisation, as the threshold for both forms of the allergen is very high. Only a concentration of 0.01% can be considered safe.

Noninvasive observations on eyes of cats after long-term maintenance of reduced intraocular pressure by topical application of prostaglandin E2.

Daily or twice daily prostaglandin E2 (PGE2) application to cat eyes was shown to maintain a reduced intraocular pressure (IOP) for several months without causing substantial flare or cellular response. We report now on detailed ophthalmic examinations performed on these cats after 5-9 months of such treatment (ie, after 150 to 250 unilateral PGE2 applications; 100 micrograms/treatment per eye). A comparison of the treated and contralateral control eyes revealed no differences in the axial length of ocular compartments, in the biomicroscopic appearance of the lens, vitreous, retina, or optic nerve head, in the rate of light-induced pupillary constriction or in the wave form of the electroretinogram. The cell density of the corneal endothelium was not decreased, but the endothelial surface did contain a few small "dark spots." A slight iridial heterochromia was generally apparent. In three of the cats PGE2 application had a sialagogic effect that became a conditioned reflex. Cats tended to keep their lids closed after each treatment; lid closure was more prolonged in the PGE2-treated eye than in the contralateral eye that received the same volume (50 microliters) of vehicle solution. It is concluded that daily treatment with PGE2, in doses sufficient to cause a maintained reduction in IOP, does have some side effects. However, none of these side effects are of sufficient importance to exclude the use of eicosanoids as potential anti-glaucoma agents.

[Bromocriptin in organic depression (author's transl)].

10 patients suffering from endogenomorphic depression on the basic of organic brain lesion were treated with 40 mg bromocriptin daily. Therapeutical efficacy was evaluated by various psychometric methods and by clinical impression. A negative correlation between performance and the depressive syndrome could be shown. Amelioration proved to be short lived, only 2 patients showed satisfactory response over a longer period of time. nausea and vomiting were the most common side effects, one patient showed a fully reversible manic syndrome. Reasons for the application of bromocriptin on the basis of a "dopamine-deficit hypothesis", and the results of the study are discussed.

[Siemens I syndrome--a case history].

Report on a patient suffering from keratosis follicularis spinulosa decalvans (Siemens I syndrome). This ia a very rare clinical picture, at least in its complete form. The patient's father and brother are suffering from an abortive Siemens I syndrome. The condition is a form of hereditary parakeratosis, a spinulose keratosis with typical localization in the eyes and on the skin. Causal therapy is not possible; attempts at conservative and surgical treatment are described. Prognostically, the condition will probably come to a standstill at puberty. The description of this case is intended to illustrate the necessity of close cooperation between dermatologist and ophthalmologist.
