The effects of high frequency jet ventilation (HFJV) were investigated in 12 mongrel dogs with normal lungs. A newly developed HFJV device with purely pneumatic regulation was used. In order to evaluate several ventilatory parameters, respiratory rate, breathing time quotient and jet emission pressure were varied. Thus, important ventilatory, hemodynamic and calculated values were recorded. Accordingly, HFJV is possible with the tested device under clinical conditions in the frequency range between 80 and 300/min, with a breathing time ratio of 1:21 to 1:1 and a driving pressure up to 0.4 MPa with an oxygen concentration of FiO2 = 0.21 to 1.0. Before clinical use, however, investigations on the damaged lung of the dog are necessary.

The efficacy of the dopaminergic substance Memantine was studied in severe cases of senile dementia of the Alzheimer type using a randomized single blind trial. Following 20 days of a basis therapy, consisting mainly of the proper therapy of medical disorders, 10 patients received 20-30mg Memantine pro die intravenously, 10 other patients were given placebo infusions. Patients were evaluated using the Syndrom Kurztest, the Funktionspsychose-Skala, the Plutchik Geriatric Rating Scale as well as the Sandoz Clinical Assessment Geriatric 5 times following the baseline period. Statistical evaluation showed no significant differences between the two groups. Mild amelioration of sleep-wakefulness cycles and impulse and drive functions could be observed clinically in both groups. Side effects (deterioration of psychopathology in two patients of the Memantine group) disappeared after withdrawal of the substance.
Twelve male patients suffering from acute paranoid schizophrenia, randomly receiving fluperlapine - a new dibenzodiazepine very similar to clozapine - or haloperidol were included in a double blind study. Plasma levels of triglyceride, cholesterol and HDL-cholesterol, prolactin, thyroxine, triiodothyronine, ACTH and cortisol were determined. There was no evidence, that fluperlapine has any relevant influence on those hypothalamic-pituitary axis mediated hormones with the possible exception of plasma lipids. These findings, together with the clinical experiences, suggest that fluperlapine might have a different mode of action than conventional neuroleptics.
This preliminary report deals with findings on the consumption of alcoholic beverages (beer, high-proof spirits, wine) and risk of tumors of the lower urinary tract from 340 male patients with urothelial cancer and an equal number of matched hospital controls (age and sex) without any malignant disease. After adjustment for smoking, beer drinkers had an overall increased odds ratio of 1.6 and showed a clear dose-response relationship (odds ratio of 1.2, 2.1 and 2.8 after daily consumption of up to 0.5, 0.5-1.0 and greater than 1 1). Drinkers of high-proof spirits also revealed an elevated odds ratio (of 1.7) compared with abstainers or occasional consumers, as well as increased risks with higher consumption (odds ratios were 1.5 and 2.7 after drinking up to 0.5 and greater than 0.51 per week). No relationship was found with wine drinking. In a multiple logistic regression analysis, only beer consumption showed a significant association.
In a matched case-control study of cancer of the lower urinary tract in northern Germany in which 340 male and 91 female case-control pairs were interviewed between 1977 and 1982, cigarette smokers were found to have an odds ratio of 2.3 for males and 2.9 for females compared with nonsmokers. A significant dose-response relation was observed for increasing cigarette consumption, and a significant decrease in risk was shown for ex-smokers. Employment in rubber, plastics, dye, textiles, and mining industries was associated with a higher risk, and increased odds ratios were also observed for exposure to spray painting, coal pitch, chromium, and zinc. Controlling for smoking, an elevated risk of 2.3 for drinking more than four cups of coffee per day and significant odds ratios of 2.1 and 2.8 for a daily consumption of 0.5-1.0 liter and above 1.0 liter of beer, respectively, were noted among men. A highly significant odds ratio of 4.0 was found for a daily fluid intake of more than 2 liters. Dietary habits such as the frequent consumption of canned food and fatty meals were associated with a higher risk, whereas a regular consumption of fruits and vegetables was associated with a lower risk. Fourteen determinants found to be important in this case-control series were analyzed by using multiple logistic regression.

Far ultraviolet light (193 nm) produced by an excimer laser has been used to produce a variety of incisions in the corneas of anaesthetised rabbits. Tissue was lost from the site of irradiation but the nature of the underlying molecular interactions responsible for such tissue erosion remain obscure. Ultrastructural analysis of the walls of the ablated areas show damage to the adjacent structures to be confined to a zone 60 to 200 nm in width. These dimensions could either be attributed to photochemical processes in which high energy photons directly break organic molecular bond, or to thermal reactions which result in limited heat flow and damage confined to the absorption depth at 193 nm of less than 1 micron. In non-penetrating incisions that reached within 40 micron of Descemet's membrane, endothelial cells were lost beneath the line of the irradiation. This spalling of cells seemed to be generated by shock or acoustic waves.

New machinable glass ceramics were tested for the application in middle ear surgery in vitro and in vivo. This ceramic consists of a glass phase and a glimmer phase. Fibroblasts and epitheloid cells seated on the test material attach to it, spread and form a confluent monolayer. From our findings on the behaviour of cell populations in vitro we can conclude that the implanted material is biocompatible. After implantation into the middle ear of rabbits, irritation-free settling was verified by the existence of a thin epithelium layer and a direct connection between bone and implant. Glass carbon did not yield equally satisfactory results.
Ten patients experiencing sudden opiate withdrawal received low doses of guanfacine. Improvement was noticeable within the first 4 days and was most evident in moderate to severe cases. The major effect was on autonomic symptoms. Tolerance to guanfacine was excellent.
