[Clinical and diagnostical aspects of acute cytomegalovirus infections in previously healthy adults (author's transl)].

The clinical symptoms of floride Cytomegalovirus infections of 18 previously healthy adults are reported. Fever was generally observed. In most cases myocarditis or hepatitis could be identified. Two of our patients died of on severe myocarditis. In 3 patients Guillain-Barré-syndrome was observed. Only in few of our cases the clinical picture was characterized by mononucleosis with systemic swelling of lymphonodes and thrombocytopenia or by pneumonia. Other symptoms were found quite seldom. The diagnosis of cytomegalovirus infection could be ascertained by: 1. detection of cytomegalovirus IgM antibody titers of greater 1:64, 2. a four fold increase in the titer of complement fixing antibodies, 3. Virus isolation from urine.

Neutralizing serum IgM antibodies in infections with Herpes simplex virus hominis.

Forty-two patients with herpes simplex virus infections were tested for neutralizing serum IgM antibodies. A technique in which serum antibody fluorescein staining was combined with sucrose gradient centrifugation facilitated the isolation of the serum IgM fraction for the use in neutralization tests. In nearly all cases with primary infection, especially those presenting heavy clinical signs (encephalitis/meningitis) the IgM tests were positive. In one case we could detect the IgM antibodies for 11 weeks after the onset of the illness, in another case in cerebrospinal fluid samples for 6 weeks. In localized herpes infections, which were mostly due to reactivations, serum IgM antibodies could only rarely be demonstrated. Among the serologic tests used in this study (NT, CFT, IFT, ACIFT), only the CFT beside the NT (with certain reservations) can be applied for subtyping HSV serum antibodies.

[Ultrasonic diagnostic measures in ambulant internal medical patients (author's transl)].

Ultrasonic investigation with the rapid B picture is a non-invasive method of examination which does not distress the patient and has no contraindications. It provides the examiner with abundant additional information, it is quickly performed without preparation and is therefore particularly to be recommended in ambulant patients. Ultrasonic investigation is no competitor to angiography or scintigraphy, but should be called on as a "forefield" diagnostic method before invasive methods. The various diagnostic possibilities are illustrated with some examples.

Antibiotics derived from a mutant of Bacillus circulans.

A mutant of Bacillus circulans, which produces butirosins only when 2-deoxystreptamine is added to the fermentation medium, was employed in the biosynthesis of antibiotics containing modified aminocyclitols. The blocked mutant converted 2,5-dideoxystreptamine into 5-deoxybutirosamine. Streptamine was incorporated into a complex differing from butirosin by an additional hydroryl at C-2.

Sensitive method to detect non-complement-fixing antibodies to Epstein-Barr virus nuclear antigne.

The anti-complement immunofluorescence (ACIF) test was modified to detect non-complement-fixing antibodies to Epstein-Barr (EB) virus nuclear antigen (IBNA). These EBNA antibodies belong to the immunoglobulin classes IgA and IgG. In our method anti-human gamma-globulin (AHG) was bound to the EBNA antibodies before the complement was added. If only non-complement-fixing antibodies are present the complement can be fixed to the AHG. Within only a few weeks of the onset of infectious mononucleosis (IM) the non-complement-fixing EBNA antibodies reach high titers while the complement-fixing antibodies (detected by the ACIF test) are not yet present. Anti-EBNA-IgM antibodies were not found in the IgM fractions of sera taken at different stages of IM.

Envelope and nucleocapsid antigens of Cytomegalovirus (CMV).

A method for the purification and concentration of Cytomegalovirus nucleocapsids and particles complete with envelope is described. Antibody to either nucleocapsid or envelope antigens can be detected in human sera. In patients with CMV disease the antibodies to the envelope antigen could not be detected before the 3rd week after the beginning of disease whereas antibodies to the nucleocapsid were detected earlier.

Clinical evaluation of patients with infectious mononucleosis and development of antibodies to the R component of the Epstein-Barr virus-induced early antigen complex.

Previous reports have emphasized that the transitory antibody responses to Epstein-Barr virus (EBV)-induced early antigens (EA) in the course of infectious mononucleosis are usually directed against the D (diffuse) component of the EA complex. In this report clinical and serologic data have been presented on 14 patients with infectious mononucleosis who responded either solely with antibodies to the R (restricted) component of the EA complex or revealed anti-R after the initial anti-D responses had subsided weeks or months after onset of the disease. Anti-R persisted usually for many months , as many as 39, but in some patients it was no longer detectable in late follow-up serum spectimens. Although many patients had an unremarkable course of illness, others, mostly those with late anti-R responses, showed unusual or protracted clinical manifestations and several complained over periods of 4 to 28 months of recurrent symptoms resembling those experienced during the acute stage of disease. The presence of anti-R and the relatively high continuous titers of antibodies to EB viral capsid antigens (VCA) may reflect a more than usual, persistent EBV activity which, in turn, may account for the recurrent symptoms.

Rapid method to detect rubella immunoglobulin M and immunoglobulin A antibodies.

Immunoglobulin (Ig) G was removed from serum specimens by precipitation with gamma chain-specific anti-human IgG of rabbit origin. The remaining rubella virus-specific IgM (and IgA) antibodies were then detected by the rubella hemagglutination-inhibition test. This procedure has proven to be as reliable as estimations carried out with IgM fractions separated on a sucrose density gradient.
