[Lung function in pregnant patients and its significance for anesthetic ventilation in cesarean section].

Spirometry was conducted preoperatively in a group of 10 women subjected to cesarean section. The measured minute ventilation served as basis for ventilation under anaesthesia. The acid-base and blood gas parameters of the newborns from this group of patients were compared to the corresponding data obtained from babies of 11 women who underwent caesarean section under epidural analgesia. It was found that part of the data were significantly better under general anaesthesia with controlled respiration than under epidural analgesia with spontaneous respiration. The maternal carbon dioxide level assumes the significance of a guiding value for the choice of the ventilation parameters. If the spirometric initial data and the actual pCO2 value under anaesthesia are not known, it is recommended to employ a minute ventilation of 11.5 l/min.

[The value of a single determination of serum uric acid concentration in the early diagnosis of hypertensive disorders in pregnancy (author's transl)].

In 200 healthy, normotensive nulliparous women, a single determination of serum uric acid concentration was done between weeks 28-32 of gestation in order to identify a possible increased risk of developing hypertensive complications. If serum concentrations higher than 3.6 mg/dl were considered as increased ("positive test"), women who developed toxaemia of late pregnancy (proteinuric hypertension), had a significantly elevated mean serum uric acid concentration already at the beginning of the third trimester (p less than 0.01). The incidence of toxaemia and hypertensive disease without proteinuria was significantly higher in the group of women with an elevated uric acid value (p less than 0.001). Only 9% of pregnant women with a "negative test" ultimately developed a mild form of a hypertensive complication. Conversely, 74% of the patients with a "positive test" remained normotensive. Thus, the predictive value of a "positive test" is low (26%) and that of a "negative test" relatively high (91%).

[Enzymatic lecithin determination in amniotic fluid for antepartal diagnosis of lung maturity - a multi-center study (author's transl)].

A new test-combination for the enzymatic determination of lecithin in amniotic fluid for the assessment of fetal lung maturity has been developed by Boehringer Mannheim. This test was evaluated by 12 hospitals and has been compared with the L/S ratio, the foam-test or the densitometric determination of lecithin. The assay is based on the hydrolysis of lecithin by phospholipase C which starts an enzymatic chain reaction in which NADH consumption if measured photometrically. The intra- and interassay precision were characterized by CV values below 10%. Average recoveries of lecithin were 95-102%. It is recommended to centrifuge the samples (10 min, 700 g) and to start the analysis as soon as possible after receipt of the specimen. The total amount of time required is 2 hours for a single determination. Batches of up to 10 samples require little extra time. An opened test-combination can be used for a maximum of 30 single determinations. Comparison of the quantitative enzymatic lecithin determination with other methods showed that the critical value for lecithin is 5.0 mg/100 ml. Above 5.1 mg/100 ml no case respiratory distress syndrome was observed. The good precision accuracy and the simple handling make the enzymatic lecithin determination suitable for routine use.

[Normal values of IgA, IgG, IgM, a1-antitrypsin, C2-complement, C4-complement, ceruloplasmin, haptoglobin and transferrin in serum of healthy, matured newborns and comparison with adults. Method: kinetic nephelometry. (author's transl)].

Concentrations of IgA, IgG, IgM, a1-antitrypsin, C3-complement, C4-complement, ceruloplasmin, haptoglobin and transferrin were determined by the kinetic nephelometric method in fasting blood serum of 88 healthy, matured newborns (male=46, female=42) and of 50 healthy adults (male=12, female=38). The age specific comparison showed that all the nine examined proteins had significantly lower levels in the group of newborns. Sex specific differences were not detected in the group of newborns.

[Semiquantitative determination of glucose in urine with a new test kit].

A new testing set for diluting urine allows to determine glucose in urine with the Diabur-test strip up to a concentration of 8.0 g/dl. The dilution by means of a pipette is absolutel;y practicable at a distinguished precision; the results show an excellent correlatiopn with the quantitative hexokinase method. Of all possible interfering substances tested only ascorbic acid influenced the reaction which, however, interfered only when minor glucosuria was present.

[Determination of glucose in hemolysed blood samples (author's transl)].

The determination of glucose in hemolysed and stabilized blood samples by the hexokinase/glucose-6-phosphate-dehydrogenase-method is described. The glucose concentration in hemolysed blood samples was stable for 7 days. Even at high glucose concentrations (linear range up to 60 mmol/l), the reaction came to completion within 5 minutes. No interference by lipemia, bilirubin and drugs was observed; the interference of fructose was slight. Compared with deproteinized samples there was a very good correlation between this method and the reference method. Precision and recovery were good. This method is also suited for the analysis of few samples and offers the possibility of blood sugar self-profiles in diabetic out-patients and gives an increasing improvement of diabetic control.

[A simple method for determining blood-glucose levels in diabetic out-patients (author's transl)].

24 hour blood-sugar profiles were obtained in 25 diabetic out-patients. The samples were obtained by the patients themselves even when at work, the glucose-level measurements in the laboratory. The relative differences between duplicate values was less than 10%. The results obtained made it possible to alter the treatment regimen so that, on average, there was a marked reduction in blood-sugar variations during the day.

[Influences of different anaesthetic techniques on serum levels of hepatic enzymes and of bilirubin (author's transl)].

The influence of four different types of anaesthesia -- halothane and enflurane inhalation anaestheis, neurolept analgesia and epidural analgesia with single shot technique -- on the serum levels of GOT, GPT, GLDH, LDH, AP, LAP, gamma-GT, total protein and bilirubin was examined in 104 comparable cases undergoing gynaecological surgery. A significant increase in GOT, GPT, GLDH, AP, and gamma-GT levels was observed between the 6th and 9th postoperative day with all types of anaesthesia. The serum levels remained elevated until the 15th postoperative day. There was no significant difference between the four types of anaesthesia as regarded their effects on the serum constituents investigated. The same applied to the serum CHE activity which reached a minimum on the 3rd postoperative day followed by a steady rise. The results indicate that the postoperative changes of the liver enzyme pattern are not related to the type of anaesthesia used. Bilirubin was elevated on the first postoperative day, then dropped rapidly and stayed below the pre-operative values until the 12th postoperative day. There was no difference between the four types of anaesthesia. Glucuronisation of bilirubin may be inhibited initially by the anaesthetic agents. The subsequent rapid fall of the bilirubin levels may be the result of enzyme induction.

[The effects of "atraumatic" surgery in halothane anaesthesia and neurolept analgesia on the liver enzyme pattern (author's transl)].

The influence of general anaesthesia for operations devoid of substantial tissue traumas on the postoperative pattern of hepatic enzymes was studied in 40 patients undergoing ophthalmologic surgery. 20 had neurolept analgesia, 20 had halothane anaesthesia. The duration of anaesthesia, age of the patients and their previous history corresponded fairly closely to those of a group of patients who had gynaecological operations and were the subject of a previous study. In contrast to the latter group total protein, cholinesterase, GOT, GPT, LDH, GLDH, AP, LAP and gamma-GT remained normal up to 12 days after the operation. The exception were changes in the total bilirubin levels which were similar to those observed in the gynaecological cases. Possible causes are discussed.

[Creatine kinase isoenzymes: idiopathic occurrence of creatine kinase BB activities in patient serum (author's transl)].

By differentiation of creatine kinase isoenzyme activities in sera using immunological methods the published data about occurrence of creatine kinase BB activities in patients with different diseases or after surgical treatment, respectively, cannot be verified in general. With a frequency in the order of magnitude of 1 : 1000 in the serum of old patients (age 57 to 85 years with one exception), however, creatine kinase BB activities can be measured. The range of activities is 15 to 234 U/1, or 19 to 94% of total creatine kinase activities, respectively. At the present time there is no possibility to correlate this phenomenon to any specific disease. These cases are detected by abnormally high results of CK-MB activity measurements with the immunoinhibition test (range 60 to 202% of total creatine kinase activities) which lead to a repeated analysis using immunoprecipitation. The results of all CK-BB patients investigated till now are presented and discussed.

[The effect of anesthesia on the postoperative behavior of liver enzymes and protein fractions].

Liver tests were performed postoperatively on 25 women with "healthy" livers. Patients were divided into 3 groups according to the type of anesthesia used: 1. halothane; 2. neuroleptic; 3. peridural. In all 3 groups the variations in serum proteins and liver enzymes were similar up to the 15th postoperative day. Variations of LDH, AP, LAP, and CHE were within normal range; slight to moderate increases above normal were observed for GPT, GOT, GLDH, and gamma-GT. All 3 groups barely differ with respect to enzyme levels during the postoperative course. We therefore interpret these changes to indicate a reaction of the liver to the stress of operation rather than to the mode of anesthesia.

[Liver function in undisturbed pregnancy].

In order to evaluate liver function during pregnancy the enzyme activities of GPT, GOT, GLDH, LDH, AP, LAP, gamma-GT and CHE were determined in 272 healthy pregnant women from the 16th week of gestation up to term. The normal range of GPT, GOT, GLDH, LDH, gamma-GT and CHE did not differ significantly from those in non-pregnant women. Increases in AP and LAP are conditioned by placental synthesis. The functional reserves of a healthy liver obviously suffice to compensate for increased demands during pregnancy. Increases in enzyme aktivities during pregnancy are not physiologic - except for increases in AP and LAP.

[A new immunochemical tube test for pregnancy using the latex-agglutination inhibition reaction. II. Clinical results (author's transl)].

In a multi-centre study 1117 morning urine samples were tested using the new immunochemical pregnancy tube test (Roche), based on the principle of indirect latex-agglutination inhibition. The test proved to be both qualitatively and semi-guantitatively suitable for demonstration of HCG in urine. There was only one false-negative result among 320 pregnancy tests (after 50th postmenstrual day). In 94 early pregnancies (test done before the 50th day) there were only two false-negative results. No false-positive result were seen. In the prognosis of threatened abortion, diagnosis of incomplete abortion, hydatid cyst, or chorionepithelioma the semiquantitative performance of the test was clinically useful and especially suitable because the result of the agglutination-inhibition reaction is unequivocal.

[A new immunochemical tube test for pregnancy, using the principle of the latex-agglutination inhibition reaction. I. Basic studies (author's transl)].

In a joint study of five laboratories the immunochemical tube test for pregnancy 'Roche', which uses the principle of the latex-agglutination inhibition test, was examined. A total of 1117 samples, complemented by a number of laboratory studies, proved that the test is specific, reproducible and low in interference with a sensitivity of 900-1000 U/l HCG. It is a simple test, the results can be easily read after 90 minutes. pH of the urine sample, protein, urea or calcium content, presence of oral contraceptives, detergents, disinfectants or organic solvents cause no or very little interference.
