[Importance of traction-wiring osteosynthesis as dynamic stabilization after acromioclavicular joint dislocation. A critical view].

Various operating procedures have been recommended for the treatment of complete acromioclavicular separation. Uncertainty regarding the way of surgical treatment leads to a distinct reduction in the number of case reports by individual authors. In consequence we established a uniform concept of surgical treatment, namely dynamic stabilisation. All patients who were operated on since 1975 were treated accordingly. Our results were in accordance with the high rates of metal breakage and loosening reported by other authors. Never the less, the functional results after surgical treatment still justify this concept of management.

[The value of the Duopleet prosthesis for the treatment of femur neck fractures].

The clinical course of 68 patients with femoral neck fractures is reported. Fracture management was chosen regarding age and accompanying health defects. Fracture management, operation time, complications, mobilization results and hospitalization time is described, compared to other authors and discussed. The results favor the implantation of a Duopleet prosthesis as the salvage operation of choice for femoral neck fractures of old patients beyond 75. Other methods are considered to remain valuable with differentiated indication.

Studies on circulatory stability during bicarbonate hemodialysis with constant dialysate sodium versus acetate hemodialysis with sequential dialysate sodium.

Eight stable patients on maintenance hemodialysis were studied while undergoing (a) acetate hemodialysis with a sequential dialysate sodium concentration from 147 to 137 mEq/L (SNa-HDA) and (b) bicarbonate hemodialysis with a constant dialysate sodium concentration of 140 mEq/L (HDB). Circulatory behavior was observed during both of these methods, and both were found to allow a high volume removal. However, as a consequence of the high sodium load during SNa-HDA, volume was shifted from the extra- to the intravascular space. This stabilizing effect on the circulation disappeared with the sequential decrease of dialysate sodium concentration (despite a constant plasma sodium concentration (despite a constant plasma sodium concentration of approximately 140 mEq/L), which was concomitant with a significant decline of the mean arterial blood pressure and an inadequate compensation of the metabolic acidosis. In contrast, a better circulatory response to comparable volume removal was found during HDB, expressed by a stable mean arterial blood pressure in the presence of well-balanced arterial acid-base values.

[Reflex dystrophy or occupational disease 2103 of the knee joint? A clinical and expert contribution to a "singular case"].

A "single case" is presented and the question is asked if a BK (occupational disease) can be applied in case of a dystrophy of the knee joint. With respect to the technical conditions according to the BK2103 and the employee, this BK had to be rejected because no active concussion had to be presumed, but a passive, only transmitted concussion. Therefore it has to be investigated if article 551, paragraph 2 of the RVO (Insurance Ordinance of the German Reich) can be applied. In opposition to the Social Court, I had recommended to apply article 551, paragraph 2 of the RVO, because there was a causal connection with respect to responsibility between the professional activity and the injury to health.

[Pregnancy after kidney transplantation. Report of 6 successful cases with multiple risk factors].

The long-term course before, during and after successful pregnancy in six women 3 to 6 years after receiving CD-kidney transplants from dead donors is described. Multiple risk factors such as hypertension, considerable proteinuria, preceding rejection crises etc. have been taken into account. The analysis includes the observation of a pregnancy which began already in the 1st month after transplantation. With the exception of a patient with hydronephrosis, whose functional data remained normal, the transplants of all patients showed better functional data after delivery than before pregnancy (maximum period of observation: 5 years 3 months post delivery). The patients mainly received alternating prednisolone therapy. Immunosuppression was generally reduced in the second trimester and was temporarily increased above the maintenance dosage after delivery. All children were eutrophic at birth and developed normally without complications. Important factors leading to the good overall clinical result were intensive out-patient care and the cooperation between nephrologists, urologists, gynecologists and neonatalogists. Despite our very good experience with 6 pregnancies successfully carried to full term by women with kidney transplants, the risk for mother and child should only cautiously be estimated, taking into account the results of future larger studies.

Electroencephalogram investigations of the disequilibrium syndrome during bicarbonate and acetate dialysis.

Continuous long-time electroencephalographic (EEG) monitoring was performed during acetate and bicarbonate dialysis in 20 patients. Persisting normal basic activity of the EEG without neurological symptoms was found only during the course of bicarbonate dialysis. However, in acetate dialysis, during the decrease of arterial CO2 tension (PaCO2), we registered EEG disturbances with moderate to severe slowing, dysrhythmic activity and high voltage discharges. The decrease in PaCO2 and the deterioration in EEG activity in the patients during acetate dialysis was concomitant with severe neurological alterations, e.g. the typical symptoms of so-called 'disequilibrium' causing a cessation of dialysis in three patients.

[Electro-encephalographic study of the disequilibrium syndrome during bicarbonate dialysis and acetate dialysis].

Continuous long-time electroencephalographic (EEG) monitoring was performed during AHD and BHD in 20 patients. Persisting normal basic activity of the EEG without neurological symptoms could be found only during the course of BHD. However, in AHD, during the decrease of arterial CO2 tension (PaCO2) we registered EEG disturbances with moderate to severe slowing down, dysrythmic activity and high voltage discharges. The decrease in PaCO2 and the deterioration in EEG activity in the patients during AHD was concomitant with severe neurological alterations, e.g. the typical symptoms of so-called "disequilibrium" causing a cessation of dialysis in 3 patients.

[Differential diagnosis of the anterior tibial syndrome].

The tibialis anterior syndrome, which is the most frequent so-called compartment syndrome of the lower extremity, is described by means of the pathophysiological processes generally applicable to the compartment syndrome. Both the clinical signs and treatment as well as the diagnosis are given in this article. To extend the differential diagnosis the authors present a woman patient referred to hospital with the pattern of a tibialis anterior syndrome as shown by anamnesis and clinical signs. However, surgery did not reveal any traumatic action as the cause of the tibialis anterior syndrome; instead, the real cause was large lipoma which in our opinion gradually resulted in a pressure increase in the anterior tibial muscle area. The accident which had been considered responsible had in our opinion merely tended to aggravate an already existing condition which would surely have resulted eventually in an intrafascial pressure increase and in the development of a tibialis anterior syndrome pattern even without any trauma. Hence, the pattern of a tibialis anterior syndrome had been caused in this particular case by a space-occupying growth in the area of the anterior tibial muscle, i.e. by a lipoma, Correspondingly, one should always consider when performing differential diagnosis of the tibialis anterior syndrome whether the underlying cause may be a space-occupying growth, such as a tumour.

[Isolated and combined abdominal traumas (author's transl)].

A group of 89 patients treated for abdominal traumas between 1975 and 1980 is analyzed in respect of type and cause of accident, concomitant traumas and intra-abdominal organ lesion patterns. Isolated abdominal traumas are compared with abdominal traumas within an overall polytrauma. Complications and causes of death among the authors' own patients are reported. The relatively high overall mortality rate of 38% is due to the above-average proportion of multiple traumas amounting to 72%. Multiple intra-abdominal injuries or organs, simultaneous involvement of the thorax and necessary massive transfusions result in particularly unfavourable prognoses.

[Migration of Kirschner's wire after osteosyntheses (author's transl)].

The article reports on four cases of migration of Kirschner's wire after osteosyntheses at the shoulder girdle and forearm. In two patients, the wire travelled into the musculature of the neck or throat, whereas in another case the dislocated wire was in transdiaphragmal position, partly intrathoracically and partly intrahepatically. In each case, removal of the implants resulted in complication-free healing. The article gives general guidelines for treatment in order to avoid wire migration in osteosyntheses using Kirschner's wire.
