The Israeli Ocular Injuries Study. A nationwide collaborative study.

A nationwide study of patients hospitalized for ocular injuries was conducted in Israel during the years 1981 through 1983, with the participation of 17 ophthalmic departments. The study comprised 2276 patients (2416 eyes), 47% of whom were children. The male-female ratio was 4.3:1. Bilateral injuries occurred in 140 patients, with children sustaining the most severe injuries. Ocular injuries occurred most frequently at home, and children at play were most vulnerable. Most patients retained good vision, 3.7% lost vision totally, and 2% of the eyes had to be enucleated.

Penetrating keratoplasty in rabbits with herpetic keratitis treated with poly I:C.

Penetrating keratoplasties were performed in which grafts were exchanged between normal rabbits and rabbits previously infected with Herpes simplex virus (HSV). Clear grafts were obtained when surgery was performed during the latent stage of the disease (3 months after HSV inoculation). Polyinosinic acid: polycytidylic acid (poly I:C) was administered before and after keratoplasty when the rabbits had active herpetic keratitis. Clear grafts were obtained when the rabbits were inoculated with HSV and treated 18 hours later with poly I:C, which was continued until two weeks before keratoplasty. This treated group had a rapid decrease in HSV titer, inhibition of the migration of HSV from the infected cornea to the trigeminal ganglia, and a decrease in antibody titer to HSV in the sera. In rabbits inoculated with HSV, subjected two weeks later to keratoplasty, and then treated with poly I:C, three quarters of the graft rejections were milder than those in the control group.

Experimental sclera-implanted keratoprosthesis.

An experimental design of keratoprosthesis anchored into the limbal sclera is presented. The surgical procedure enables a watertight junction between the keratoprosthesis and the globe. Dehiscence of the keratoprosthesis-limbal sclera junction occurs when an area of locus resistentia minoris is created in the lower limbus. An artificial drainage system is necessary for the durability of such a keratoprosthesis.

Late results of surgery for congenital esotropia.

In a retrospective study we have evaluated the surgical outcome in 46 children with congenital esotropia who were followed up for at least two years. The results support the concept of early operation for congenital esotropia. Evaluation of the effect of simultaneous weakening of the inferior obliques proved statistically that this reduced the effect of recession-resection of the horizontal recti.

Long-term follow-up of traumatic unilateral superior oblique palsy.

Twenty-four cases of traumatic unilateral superior oblique palsy were followed-up during a mean period of 5.3 years. Hypertropia in the primary position and a positive Bielschowsky test were present in 100% of the cases, compensating head posture in 71%, and excyclotorsion in 25%. Six patients needed surgical correction; 18 were cured spontaneously during a mean period of 9.2 months.

Septic retinitis.

A case of septic occlusion of the retinal vessels causing severe endophthalmitis is presented. The documentation of the retinal involvement before development of the abscess with photography and fluorescein angiography is rather exceptional. Although pars plana vitrectomy saved the eye, the vision was lost because vascular occlusion caused a retinal tear with retinal detachment.

Retinal damage after argon laser iridotomy.

We conducted a prospective clinical study to evaluate potential retinal damage after argon laser iridotomy in 25 eyes of 22 patients with primary chronic angle-closure glaucoma. Kinetic perimetry and ophthalmoscopy showed no detectable damage. Iridotomy required a mean of 11.4 +/- 10 joules to achieve a patent coloboma of 200 micron after one to three sessions. Dark-prone and mydriasis tests were performed one month after surgery. Pilocarpine was discontinued in all cases. Both static perimetry and fluorescein angiography of the midperiphery corresponding to the meridian of laser coloboma done six months after surgery showed focal damage. There was no damage in control tests of the same eyes in an opposite area of the retina.

Anterior uveitis, a presenting symptom in acute interstitial nephritis.

Two adult women with idiopathic acute interstitial nephritis (AIN) whose presenting symptom was anterior uveitis are described. On histologic examination of the renal biopsy, eosinophilic infiltrates and mononuclear cells were found. Immunofluorescence was negative. Immunological evaluation showed a slight decrease in the number of T cells with a normal subpopulation ratio. AIN responded to steroid treatment.

Oculo-palato-cerebral dwarfism: a new syndrome.

Three of four offspring of consanguineous parents presented a unique association of microcephaly, mental retardation, spasticity, connective tissue abnormalities, cleft palate, persistent hypertrophic primary vitreous, and short stature. In one patient brain atrophy was documented. All the affected individuals had severe asthma and it is thought that the asthma is associated with the syndrome complex. Genetic transmission is most likely autosomal recessive. We believe this constellation of findings to be a new genetic syndrome and have termed it the oculo-palato-cerebral dwarfism syndrome.

Intraocular pressure in uremic patients on chronic hemodialysis.

Intraocular pressure (IOP) was measured before and after hemodialysis in 30 uremic patients receiving regular treatment. Osmolality, blood pressure and weight before and after dialysis were also determined. Low IOP of 11.4 +/- 2.7 and 11.5 +/- 3.1 mm Hg was found in the left and right eye, respectively. Following hemodialysis osmolality, blood pressure and body weight decreased significantly but IOP did not rise significantly. This study suggests that there is only a remote possibility for severe IOP increase following hemodialysis in uremic patients on chronic treatment.

A rare mechanism of orbital emphysema.

Orbital emphysema is usually described in association with fractures of the paranasal sinuses involving the orbit. We describe a rare form of orbital emphysema caused by high-pressure subconjunctival air injection from an air tube used in a garage, without apparent damage to the orbital walls.
