[Clinical observation following the use of neotriozinc pasta as root canal filling pasta].

The prognosis was examined of root canal filing using Neotriozinc Pasta. Thirty teeth followed up included 40 infected root canals, aged between 14 and 67 years, and observed from 1 to 30 days. Results were evaluated from both criteria of clinical symptoms and roentgenographic findings. The results were as follows: 1. A good evaluation was recorded for 87.5% of the 40 canals treated. 2. There were differences between single-rooted teeth and multi-rooted teeth in this study. 3. A better results were obtained in molar group than incisor one.

Long-term effect of urokinase therapy in IgA nephropathy.

Effects of urokinase (UK) therapy in patients with moderate to advanced degrees of IgA nephropathy (IgAN) were examined. Twenty-seven patients were treated by "two weeks" UK administration, 14 patients were treated by "consecutive" UK administration and 16 patients were treated by antiplatelet drugs. There were marked improvements in urinary protein concentration, serum creatinine and blood urea nitrogen after UK therapy, especially in patients treated by "consecutive" UK administration which was performed by "single shot" UK injection. Clinical prognosis was favorable in patients treated by UK administration compared with those given antiplatelet treatment. It was concluded that "consecutive" UK administration might be useful for treatment of IgAN with moderate to advanced renal injuries.

[CT analysis of pulmonary emphysematous and fibrotic changes in patients with von Recklinghausen's disease].

Five cases of von Recklinghausen's disease were studied with regard to their association with chest diseases, especially pulmonary emphysematous and fibrotic changes. CT analysis revealed that four patients out of five had multiple pulmonary emphysematous lesions and that those lesions were seen in all segments. In two cases out of the four, it was difficult to detect their abnormalities by an ordinary chest roentgenogram, suggesting the possibility of higher incidence of emphysematous changes in von Recklinghausen's disease than has been expected. On the other hand, pulmonary fibrosis was observed in two cases. There were two patients who had no fibrosis but emphysematous lesions. TBLB was performed in one case, in whom no fibrotic changes were observed. Usually pulmonary fibrosis is associated with emphysematous bullae in patients with von Recklinghausen's disease. However, fibrotic and emphysematous changes may not always occur in combination in von Recklinghausen's disease.

Determination of tissue type plasminogen activator in the glomeruli of patients with IgA nephropathy.

The detection, by immunofluorescence, of tissue type plasminogen activator (t-PA) in the glomeruli of 50 patients with IgA nephropathy was described. Patients with t-PA deposits revealed a higher frequency in the glomerular deposition of fibrin and/or fibrinogen than did patients without t-PA deposits. There also was a distinct relationship between the level of serum t-PA and the degree of renal tissue injury in patients with glomerular t-PA deposits. It was concluded that the deposition of t-PA along with fibrin and/or fibrinogen might reduce the effectiveness of fibrinolysis in the glomeruli of patients with IgA nephropathy.

Effects of the tumor promoter, phorbol 12-myristate, 13-acetate, on the epidermal adenylate cyclase system: evidence for adenylate cyclase-regulation by protein kinase C.

Exposure of pig epidermal sheets to the protein kinase C (PKC) activator, phorbol 12-myristate, 13-acetate (PMA) resulted in an increase in forskolin-induced cyclic AMP accumulation in the epidermis. Cholera toxin-induced cyclic AMP accumulation was moderately increased by PMA treatment, but this was not statistically significant. On the other hand, receptor-mediated adenylate cyclase responses (beta-adrenergic-, prostaglandin E-, adenosine-, and histamine (H2)-adenylate cyclase responses) were significantly decreased. These PMA-induced effects on the epidermal adenylate cyclase system were mimicked by 1-oleoyl-2-acetyl-glycerol, a membrane-permeable synthetic diacylglycerol, and by the non-phorbol PKC activator, mezerein. 4-O-methyl PMA, a very weak PKC activator, had no effect on adenylate cyclase responses of the epidermis. The addition of the PKC inhibitor, H-7 (1-(5-isoquinoline-sulfonyl)-2-methyl piperazine dihydrochloride), to the incubation medium significantly inhibited the effect of PMA on forskolin-induced cyclic AMP accumulation. Furthermore, following H-7 treatment, the epidermal receptor-adenylate cyclase responses were significantly increased. These results indicate that PKC modulates epidermal adenylate cyclase responses resulting in an increase in forskolin-induced cyclic AMP accumulation and a decrease in receptor-adenylate cyclase responses of the epidermis.

[Estrogen and androgen receptors in hepatocellular carcinoma and in noncancerous liver tissue].

We have assayed both estrogen and androgen receptors in hepatocellular carcinomas, in the surrounding noncancerous liver tissue, and in normal liver tissue. Estrogen receptors (ERs) were detected in 5 out of 22 HCC tissues, 14 out of 22 surrounding noncancerous tissues, and in 5 out of 6 normal liver tissues. Androgen receptors (Ars) were detected in 4 out of 21 HCC tissues, in 7 out of 21 surrounding noncancerous tissues, and in 2 out of 5 normal liver tissues. These results suggest that the pathogenesis of some HCC may be dependent on estrogen or androgen. Hence, it may be beneficial to provide hormonal therapy for HCC, especially in cases that show high concentrations of ER and/or AR.

Characterization of peptide inducing cataractogenesis in lens of hereditary cataractous rat (ICR/f RAT).

A material with inhibitory action to Na+/K+ ATPase was found in the lens of the ICR/f rat, a recessive hereditary cataractous rat. The material also induced lens opacification in vitro. From the results of amino acid analysis and by secondary ion mass spectroscopy, it was suggested that the material might contain approximately equimolar amounts of four amino acids, ie, aspartic acid, serine, glutamic acid and glycine, and that the molecular weight was 444. These facts suggested that this material with Na+/K+ ATPase inhibitory action might be a peptide. However, there is not yet any corroborating evidence to show whether this peptide is only a single material or not. The peptide significantly increased with aging in the lens of the ICR/f rat until approximately 90 days, when cataract became manifest, but its content decreased thereafter. This study suggests that one of the causes of cataractogenesis in the ICR/f rat might be this peptide, which is transformed in the lens with aging, and also that the peptide might accelerate lens opacification after cataractogenesis.

Increase of IgA-specific switch T cells in patients with IgA nephropathy.

Enumeration and functional analysis of CD4+ T cells with receptors for the Fc portion of IgA (i.e. T alpha 4 cells) in the peripheral blood of patients with IgA nephropathy, their relatives and age-matched controls were performed to elucidate polyclonal activation of IgA production in this disease. Enumeration of T alpha 4 cells was performed by a fluorescence activated cell sorter, and functional analysis was carried out by separation of T alpha 4 cells, and IgM-, IgA- and IgG-bearing lymphocytes using panning methods followed by cultures of these cells for 7 days with pokeweed mitogen. There was a significant increase in the amount of peripheral blood T alpha 4 cells in patients with IgA nephropathy and their relatives. T alpha 4 cells specifically enhanced the switch of IgM-bearing cells to IgA-bearing cells, and this switch activity was inhibited by addition of human myeloma IgA. It is suggested that T alpha 4 cells may be responsible for polyclonal activation of IgA production in IgA nephropathy.

Gray platelet syndrome: relationship between morphological abnormality of the dense tubular system (DTS) and intracellular Ca++ mobilization in the platelet.

In the first familial case of gray platelet syndrome (GPS) reported in Japan in which the subjects showed platelet release abnormalities, we investigated the relationship between intracellular Ca++ mobilization in platelets and marked morphological abnormalities of the DTS. The subjects for the Aequorin assay were 12 controls and 11 GPS cases, whereas the subjects for the Fura-2 assay were 10 controls and 6 GPS cases. In order to discriminate between Ca++ influx from outside of the cells and Ca++ mobilization from DTS within the cells, the experiments were conducted under two conditions; one in the presence of 1 mM Ca++ in the external fluid, and the other with the addition of 2 mM EDTA as a Ca++ chelator. Stimulation by A-23187 in the presence of 1 mM Ca++ in the external fluid caused 2 peaks or shoulder formation; that is, normal cases showed 1 peak at all concentrations of A-23187 tested, whereas GPS showed 2 peaks or shoulder formation in 7 of the 11 cases and conspicuously good reproducibility in each case. These facts indicated that it took time for the stimulus to reach the inside of the DTS, which showed marked morphological abnormalities. During stimulation by 1.0 U/ml thrombin under the same conditions, the GPS exhibited 1 peak with a wide skirt pattern, compared with the control. In one case of GPS, which revealed one peak by thrombin and 2 peaks by A-23187 in the presence of 1 mM Ca++, 2 peaks were also noted by thrombin and the luminescence peak become lower, when Ca++ was chelated by EGTA, using the Aequorin method.(ABSTRACT TRUNCATED AT 250 WORDS)

Rapidly progressive glomerulonephritis undetected by routine school urinalysis: a case report.

A twelve year old girl with rapidly progressive glomerulonephritis (RPGN) is reported. First symptoms of renal failure developed in August 1987 just five months after a routine school urinalysis which detected no abnormalities. The patient developed end stage renal failure within 2 months of onset. Continuous ambulatory peritoneal dialysis (CAPD) was introduced to manage the renal failure, and open renal biopsy was performed simultaneously in order to elucidate the underlying renal disease in this patient. Microscopic examination of the biopsy specimen demonstrated marked formation of crescents in about 90% of the glomeruli, and some of the glomeruli were already sclerotic. Granular depositions of IgG(trace), IgA(1+), IgM(3+), properdine(3+), Clq(3+) and C3(3+) were observed by immunofluorescence staining. A definite diagnosis of rapidly progressive glomerulonephritis was made as the cause of renal failure in this patient. It was suggested that open renal biopsy is useful in diagnosing the underlying kidney disease of end stage renal failure in cases where the clinical course is relatively short.

Continuous ambulatory peritoneal dialysis in a diabetic patient with renal insufficiency and ventricular aneurysm.

A diabetic patient with renal insufficiency and a giant ventricular aneurysm was treated by continuous ambulatory peritoneal dialysis (CAPD). No apparent exacerbation of the patient's cardiovascular function was observed after starting CAPD therapy, although his ejection fraction remained low and his angina attacks persisted. It is suggested that CAPD therapy has no deleterious effects on the cardiovascular system of patients with ventricular aneurysms, and that the benefits outweigh the risks.

[The present status in prophylaxis and treatment of HIV infected patients with hemophilia in Japan].

Thirty six hemophiliacs with HIV infection were treated with high-dose glycyrrhizin, Stronger Neo-Minophagen C (SNMC). The dose was 100-200 ml of SNMC in eighteen patients and 400-800 ml in the other 18. The patients were divided into an asymptomatic carrier (AC) group and AIDS related complex (ARC)/AIDS group. SNMC was administered intravenously daily for the first 3 weeks, every second day for the following 8 weeks to 36 HIV positive hemophilia patients in accordance with the protocol proposed by the Japanese National Research Committee. The CD 4/CD 8 ratio and CD 4 positive lymphocyte counts did not change during the treatment period. However, significant improvement was noted in some cases. A slight increase in mitogenic responsiveness to phytohemagglutinin, Concanavalin A and pokeweed mitogen was noted in most patients of both groups, especially in the AC group administered over 400 ml of SNMC. Furthermore, complete improvement was noted in liver dysfunction, which has been thought to be a major problem in hemophiliacs treated with blood products. Thus prophylactic administration of high-dose SNMC to HIV positive hemophiliacs having impaired immunological ability and liver dysfunction was considered to be effective to prevent the development from AC/ARC to AIDS.

Short term effect of lactulose therapy in patients with chronic renal failure.

Effect of lactulose therapy was examined in patients with chronic renal failure (CRF) prior to hemodialysis (HD). Twenty three patients were included in this study. Ten out of the 23 patients were continuously treated with lactulose at a dosage of 18 g/day for eight weeks. However, 12 out of the 23 patients dropped out because of nausea and/or watery diarrhea during the therapy. One patient was transfered to HD one month after the beginning of lactulose therapy. Ratios of guanidinosuccinic acid (GSA) in plasma after eight weeks were significantly lower than those before treatment. Cessation of treatment was significantly related to worsening of the GSA (p less than 0.01). It seems that lactulose therapy could be useful in the treatment of CRF if compliance of the therapy is maintained after reducing some adverse effects.

Continuous flow peritoneal dialysis in pigs, using a silicone rubber double lumen catheter.

Continuous flow (CFPD) and intermittent (IPD) peritoneal dialysis were compared in pigs, using a double lumen silicone rubber peritoneal dialysis catheter. For CFPD, after instillation of 30 ml/kg into the peritoneal cavity, dialysate was continually infused and drained at approximately 25 ml/kg/h. A dwell volume of 30 ml/kg was also used for IPD. Similar length cycles of each technique were compared in Experiment 1. In Experiment 2 total solute clearance and ultrafiltration achieved over two consecutive IPD cycles were compared with CFPD of a similar duration. The catheter functioned well with few complications. In Experiment 1, CFPD produced greater ultrafiltration and significantly improved the clearance of urea, potassium and phosphate compared to IPD. No significant difference between the techniques was observed in Experiment 2. Our data suggest that for CFPD to retain its advantage over IPD intermittent complete drainage of the peritoneal cavity may be necessary.

[Studies on absorption, distribution and excretion of 14C labeled (+-)-7-(3-amino-1-pyrrolidinyl)-6-fluoro-1-(2,4-difluorophenyl)-1,4- dihydro-4-oxo-1,8-naphthyridine-3-carboxylic acid p-toluene-sulfonate hydrate (14C-T-3262) in rats and mice].

Absorption, distribution and excretion of T-3262 were studied in rats and mice after oral administration of 14C-T-3262. The obtained results are summarized as follows. 1. 14C-T-3262 was absorbed from the upper small intestine such as duodenum in rats. 2. Serum levels of radioactivity in rats reached the highest concentration at 1 hour after an oral administration, then gradually diminished. 3. Urinary excretion was 35% and 42% of the dosed radioactivity in rats and mice, respectively, and fecal excretion was about 65% and 56% of the dosed radioactivity in rats and mice, respectively. 4. Biliary excretion in rats was about 27% of the dosed radioactivity after an oral administration of 14C-T-3262, and a half amount of excreted radioactivity was reabsorbed from the intestine. 5. Radioactivity was distributed the most into the kidney and the liver among all organs other the stomach and the intestine. Radioactivity was widely distributed into other organs such as spleen, adrenal, pancreas, lung, heart and thymus. But the distribution of radioactivity into the brain was little. 6. The distribution of 14C-T-3262 was also studied with whole body autoradiography in normal male mice and pregnant mice. The radioactivity was distributed widely to whole tissues except brain, spinal cord and eye ball. In pregnant mice, radioactivity levels in the fetuses were the same as the blood level of the mother mice. 7. The binding rate of 14C-T-3262 to rats and mice serum proteins was 63-66%. 8. Urinary and fecal excretion patterns of radioactivity in mice after multiple oral administration of 14C-T-3262 for 10 days were similar to those after a single administration. This result suggests that T-3262 did not accumulate in body. 9. After oral administration of 14C-T-3262 to nursing rats, the secreted radioactivity level in the milk was higher than the blood level.
