[Histopathological findings in autopsy cases of recurrent breast cancer].

In case of breast cancer subjected to postmortem examinations, histopathological changes were observed, and the effects of treatment were analyzed. Surgically obtained specimens of 36 cases of breast cancer could be compared with their postmortem specimens: in 14 cases (38.9%), tumor cells tended to be lowly differentiated, while the cells were unchanged in 22 (61.1%). There were no cases of differentiated tumor cells. Lowly differentiated cells were common in patients resistant to treatment who died shortly. Stromal tissues were sclerotic in eight cases (22.2%), medullated in three (8.3%), and unchanged in 25 (69.4%). Long-surviving cases tended to have the involvement of sclerosis.

[Thermographic diagnosis of breast disease].

Breast thermography was applied to 372 patients (49 with breast cancer and 323 with benign disease) between June 1984 and May 1985 at this Cancer Center. The thermographic findings obtained were quantitated and subjected to multivariate analysis to establish the diagnostic criteria for breast thermography. The result of diagnosis using the criteria revealed 87.8% as the sensitivity ratio and 67.8% as the specificity ratio. Even nonpalpable breast cancer could be diagnosed correctly. These facts and the noninvasive characteristics of this method indicate its validity as a screening test.

[Intracavitary administration of recombinant interferon in cancer patients with malignant effusions].

Interferon-beta (IFN-beta), 6 X 10 units/body, was administered intraperitoneally and intrapericardially daily to five recurrent cancer patients with malignant effusion. Two of them showed complete response with a disappearance of effusion and cytological negative. However, three patients showing a decrease in effusion and cytological negative were regarded as no change because of the short duration of the response (less than four weeks). Intracavitary administrations of IFN-beta were well tolerated, and no serious side effects were observed. Autopsies showed that all intestines with disseminated metastasis maintained organic elasticity and had neither ileus-like construction nor progressive adhesions. IFN-beta showed not only anticancer effects on malignant effusion, but protection against GI tract constriction through moderate control of the growth of connective tissue.

Stimulation by ionophores of tyrosinase activity of mouse melanoma cells in culture.

The hypothesis that the melanosome is an acidic vesicle in which the tyrosinase action is suppressed under the ordinary culture conditions was examined with a variety of ionophores added in cultures of mouse melanoma cell line B16-C2M. In the presence of monensin or nigericin, which exchange H+ for Na+ or K+, respectively, through bio-membrane, the tyrosinase activity of cells in culture was more than 10 times that in the control culture. This stimulation was observed without delay after addition of the chemicals and was not inhibited by cycloheximide. The enzyme activity of sonicated cell-free extracts, in which melanosomes were disrupted, was not stimulated by these ionophores. The tyrosinase activity was stimulated to a lesser extent by a proton ionophore, p-trifluoromethoxyphenylhydrazone (FCCP). The activity was also stimulated by kryptofix 221, valinomycin (Na+ and K+ carrier, respectively), and tetraethylammonium ions (permeant cations) but only in the presence of a limited concentration of FCCP. N-Ethylmaleimide and N, N'-dicyclohexylcarbodiimide, inhibitors of lysosomal proton pump, stimulated tyrosinase activity of cells in the presence of FCCP. These facts are consistent with the hypothesis described above.

Effects of autonomic drugs on in vivo recording of electromyograms of canine renal pelvis and ureter.

The effects of the autonomic drugs (noradrenaline, isoproterenol and acetylcholine) and the urine volume change on the pacemaker of ureteral peristalsis were studied by our new method of the in vivo recording of electromyograms (EMGs) of canine renal pelvis. The EMGs of the pelvicalyceal region showed a two phasic slow-rising potential of amplitude of 20 microV and discharge interval of 5 s and was different from propagated waves and therefore considered to be the pacemaker potentials. In the diuretic state the pacemaker potentials kept a constant discharge interval. On the contrary, the discharge interval of the ureter EMG became shorter and finally corresponded to the pacemaker EMG one to one. These results suggest that the urine transport in the diuretic state is controlled by the change of efficiency of the peristaltic propagation and not by the changes of the discharge of the pacemaker itself. Noradrenaline promoted the ureteral peristalsis and isoproterenol inhibited the ureteral peristalsis. These two drugs had no effects on the pacemaker potentials. Acetylcholine had a great variety of the effects on the ureter EMG and the pacemaker EMG separately. These results suggest that the pacemaker of the ureteral peristalsis is controlled under the influence of the parasympathetic system and the ureter is controlled under the influence of both the sympathetic and parasympathetic system.

Relationship between pelviureteral peristaltic frequency and urine flow change evoked by autonomic drug administration.

Parallel recording of renal pelvic pressure, ureteral electromyogram and urine volume was carried out in dogs given intravenous injections of noradrenaline, isoproterenol or acetylcholine. With noradrenaline, an increase in urine volume was observed coincidentally with an elevation of renal pelvic pressure and an increase in frequency of ureteral peristalsis. In response to isoproterenol, the urine volume diminished in harmony with flattening of pelvic pressure waves and the disappearance of ureteral peristalsis. Conversely, ureteral peristalsis increased in frequency immediately after the administration of acetylcholine, while the urine volume diminished.

Changes in the ureteral peristaltic rate and the bolus volume in gradual and rapid urinary flow increase.

The electromyogram and bolus volume of ureteral peristalsis were measured during gradual and rapid urinary flow increase, using 16 mongrel dogs. When the urine flow was increased rapidly by administration of diuretics, the ureteral peristaltic rate always increased first, then the urine bolus volume increased and finally the ureter became a urinary column without forming the urine bolus. When the urine flow was increased gradually by intravenous drip infusion of saline solution, the ureteral peristaltic rate and urine bolus volume did not always increase in spite of urinary flow increase. The peristaltic rate and bolus volume helped each other for effective urinary transport, as both of them not only always increase but also one of them occasionally decreased.

Effects of dobutamine on cyclic AMP content in canine upper urinary tract tissue.

There was no regional difference in tissue cAMP content throughout the upper urinary tract specimens obtained from dogs immobilized by pancuronium bromide (0.1 mg/kg, i.v.). The cAMP content was increased in all regions examined 5 min after administration of dobutamine (1 mg/kg, i.v.) and pancuronium bromide. The increase was much higher in the pelvicalyceal region than in the other upper urinary tract tissue. The responsiveness of the pelvicalyceal region where pacemaker cells for ureteral peristalsis exist is considered to be higher than that in the other upper urinary tract tissue in respect to dobutamine-induced cAMP increase.

[A case of breast cancer with lipid secretion].

Lipid-secreting carcinoma of the mammary gland is a very rare disease entity. We have experienced a 59-year-old woman diagnosed as having cancer of the left breast and treated by mastectomy. The excised material was found to be afflicted with common duct carcinoma partially involved in lipid secretion. This patient had a relapse and died three and a half years after the mastectomy. The autopsy material consisted mostly of lipid-secreting carcinoma. The pathogenesis of this disease and changes of histological figures are also discussed.

Immotile cilia syndrome associated with polycystic kidney.

The immotile cilia syndrome is an inherited disorder characterized by the inability of ciliated structures to beat effectively. The urological manifestation of this syndrome is sterility. We report a case of the immotile cilia syndrome associated with polycystic kidney, which also is a hereditary disease.

Urometric evaluation of intramural ureter function with continual ureteral perfusion.

Urometry provides a valuable test of ureteral function wherein intraureteral pressure changes associated with ureteral peristaltic contraction are recorded via a fine catheter inserted through the bladder into the ureter. An attempt was made to evaluate the function of the intramural ureter by analysis of urometric wave pattern of resting pressure recorded during the phase of no ureteral peristaltic contraction with continual instillation of sterile water via the catheter at a constant rate. By this technique, the length of functional intramural ureter (FUimL) was estimated to be 10-15 mm and the resting pressure in the intramural ureter (Pr) was between 5 and 10 mmHg in patients with possibly normal intramural ureter (4 ureters). In cases of chronic dysuria with cystographic evidence of morphologic changes in the bladder and with no vesicoureteral reflux (VUR), the FUimL was 5 mm and the Pr 2-3 mmHg. The corresponding values were 5 mm and 1 mmHg with a ureter showing VUR. The data obtained indicated that the product of these parameters, FUimL X Pr, may reflect the function of intramural ureter. The method reported herein being procedurally simple requiring no particular apparatus provides a remarkably valuable test for the evaluation of function of intramural ureter which has been virtually impracticable.

Response of the renal pacemaker to beta 1-adrenoceptor agonist: difference from the cardiac pacemaker.

Electromyographic responses of the canine-pyeloureter to dobutamine, a beta 1-adrenoceptor agonist were investigated in vitro. Dobutamine produced a marked enhancement of potential in the pelvicalyceal (PC-) border, but failed to evoke noticeable responses in the renal pelvis and ureter. Dobutamine, however, made the pacemaker rhythm a little slow. This finding shows that the renal pacemaker is somewhat different form the cardiac pacemaker, pace of which is quickened by dobutamine.

[An autopsy case of metastatic thyroid tumor].

In a survey of the thyroid glands of 621 patients with terminal malignant tumors, 50 (8.1%) were found to be associated with metastatic tumors. Examination of routine autopsy materials revealed this association in 11 out of 269 cases (4.1%); observations using multiple sections showed in this association 39 of 352 (11.1%) cases. The primary cancer sites most commonly associated with metastasis to the thyroid gland were the breast (8 of 25 cases, 32.0%), the lung (13 of 65. 20.0%), and the esophagus (3 of 16, 18.8%). Morphological observations showed the formation of regional nodules in 13 cases, diffuse infiltration into the whole thyroid gland in 17 and intravascular embolism alone in 20 patients. There changes were inferred to be due to hematogenous metastases.
