[Transverse fetal position syndrome--a combination of congenital skeletal deformities in the newborn infant].

We describe a pattern of connatal postural deformities observed in approximately 0.6% of otherwise healthy, mature newborn infants. It comprises unilateral flattening of the skull (dolichocephalus), ipsalateral mandibular hypoplasia and torticollis, deviation of the septum nasi, pes calcaneovalgus on one and pes adductus supinatus on the other side. The deformities occur in four distinct combinations attributable to the prenatal lie of the fetus: dorsoanterior or dorsoposterior, left or right oblique lie. The deformities respond well to manipulative therapy.

Abnormalities, Multiple

[Normal values for free thyroxine, free triiodothyronine, reverse triiodothyronine, thyrotropin, thyroglobulin and thyroxine-binding globulin in umbilical cord blood of healthy mature newborn infants].

The venous cord blood levels of free thyroxine (fT4), free triiodothyronine (fT3), reverse triiodothyronine (rT3), thyrotropin (TSH), thyroglobulin (TG) and thyroxine binding globulin (TBG) were studied in 56 mature and healthy newborns. Newborns with a gestational age less than 37 or more than 42 weeks, a delivery by forceps or cesarian section, a birth-weight less than 2500 g, a pH-value of the cord-artery blood less than 7.15, an Apgar-value after 1 minute less than 7 were excluded from the study. All mothers were non-smokers. The values of fT4 were 18.66 +/- 4.18 pmol/L, of fT3 were 1.59 +/- 0.75 pmol/L, of fT3 were 2152 +/- 666 pg/ml, of TSH were 7.83 +/- 4.49 mU/ml, of TG were 44.61 +/- 23.84 ng/ml, and of TBG were 25.61 +/- 5.42 micrograms/ml. A weak negative correlation was found between the TG-value and the pH-value of the cord-artery blood (r = -0.27, y = 191.55 - 22.82.x, p less than 0.05), and between the fT4 values and the gestational age (r = -0.34, y = 67.53-1.22.x, p = 0.01). The rT3-values were positively correlated to the gestational age (r = 0.29, y = -4571 + 167.x, p less than 0.03).

[Routine examination of the mature newborn infant. Incidence of frequent "minor findings"].

From 1st January to 31st August 1989 2,248 healthy, mature infants were delivered at the Department of Obstetrics and Gynaecology in Graz, Austria. They were routinely examined on day 1 and before discharge from hospital on day 4. Attention was paid to the presence or absence of minor abnormalities such as skin lesions, eruptions or rashes, cephalhaematoma, tongue tie, undescended testis, increased physiological jaundice, heart murmurs, talipes calcaneo-valgus, hip click, fracture of the clavicle and other common abnormalities and limb deformities. 20.9% of the investigated babies had no such findings whatsoever. In 79.1% of the subjects with one or more abnormalities, minor lesions due to birth trauma dominated with 24.2%, followed by postural deformities of the lower extremities in 20.5% and hip clicks and/or asymmetric skin-folds in 12.9%. Dislocation of the hip, however, was found in only 17%. Elevated serum bilirubin levels were noted in 33.0%, and in many of the jaundiced infants bruising or haematoma due to birth trauma was evident. Traumatic lesions were much rarer in infants delivered by caesarean section than in infants born vaginally.

[Serological studies of cytomegalovirus in the Melut district, South Sudan].

Out of 1071 investigated patients had antibodies in 73.7% against Cytomegalovirus. 99 ambulant Kala-Azar-patients reacted positive in 33%. Inhabitants of different villages have shown 35.2 to 50% positive reactions. Patients with hepato-splenomegaly in 94% and patients with diarrhoea and fever in 81% were positive.

A contribution to the situation of antibiotic resistance in Abeokuta (Ogunstate) and Minna (Nigerstate) in Nigeria.

After testing various chemotherapeutics the following conclusions could be drawn: Pseudomonas was sensitive only to gentamicin. Gentamicin, aminopenicilline + calvulanacid and cefoxitin were 100% effective against E. coli. Gentamicin also proved effective against Enterobacter (83%). Cefoxitin, aminopenicillin + clavulanacid, gentamicin and trimethoprim + sulfonamide were effective against Klebsiella. Concerning Proteus sp., cefoxitin showed best results (100%). Acinetobacter was 100% inhibited by gentamicin. Gentamicin was most effective (93% sensitivity) against Staph. aureus. Trimethoprim + sulfonamide and erythromycine showed resistance rate of 17%, cefalosporine and isoxazolylpenicilline a rate of 21%. Aminopenicillin and aminopenicilline + calvulanacid were most suitable against Enterococcus (100%).

Acinetobacter

[Children of insulin-dependent diabetic patients. A determination of status].

We report on 143 newborns of mothers with insulin dependent diabetes during pregnancy. The mean birth weight was 3.091 (+/- 619) g at a mean gestational age of 38.4 (+/- 2) weeks. Neonatal morbidity was associated with the disease class, insulin levels in the amniotic fluid, urine and cord blood; and especially persistent fetal hyperinsulinism as indicated by repeatedly high amniotic fluid insulin levels during pregnancy. One infant died (neonatal mortality 0.69%) of a severe cardiac malformation combined with a caudal regression syndrome. This was the only major malformation in the group.

The application of PVP-iodine in developing countries.

In this report the possibilities for the application of PVP-iodine-preparations in the developing countries in the treatment of wounds, including burns and in diseases of the mucous membrane, eyes, mouth and genital region are discussed. The application proved particularly valuable where a high antibiotic resistance was found.

Serological investigation of measles (morbilli) in the Melut district, South Sudan.

In Sudan, measles are often not identified and are diagnosed and treated as "fever". Apart from this, the unimmunized population is an important factor, however, cold storage of the vaccine is not always possible. For this reason detailed instruction and introduction in diagnostic identification was given by our team of doctors. The contamination rate and antibody verification varies according to region. The highest percentage of 83% was reached in Melut among hospitalized patients. At the time of our investigation, immunization had not been carried out in this region.

Brucellosis in the upper Nile region--Melut district, Sudan (report).

In the Melut district, South Sudan human and animal blood samples were investigated for Brucella abortus and Brucella melitensis. 9.2% of the investigated cattle and 16.2% of the green long-tailed monkies were Brucella abortus-positive. School children revealed a 3.7% positivity rate, Melut's hospital patients 5.2% and village children and adults 2.2%. However, only a small number of hospital patients was investigated. A small percentage of sheep and goats was investigated for Brucella melitensis revealing 12% positive results. 8.7% of cattle were positive. 2.2% of school children reacted in high titers against Brucella melitensis, 5.2% of Melut's patients and in the Melut district 3.6% of children and adults.

[Tracheobronchial lavage--a supplementary measure in the initial management of meconium aspiration syndrome].

Pulmonary lavage - a supplementary measure of resuscitation in newborn infants with severe aspiration of meconium stained amniotic fluid. During 1984-1985 150 mature newborn infants who had aspirated heavily-stained or pea soup-like amniotic fluid were resuscitated at the University Hospital of Obstetrics and Gynaecology in Graz. In 109 of these infants (controls) routine aspiration of both the upper and lower airways was accomplished as soon as possible, whilst in the remaining 41 infants (lavaged) pulmonary lavage with saline was additionally performed via an endotracheal tube after aspiration to clear the trachea and the upper bronchi from thick meconium which could not have been removed as efficiently otherwise. The incidence of manifest meconium aspiration syndrome was significantly higher (22%) in the lavaged infants than in the controls (8%). Mechanical ventilation was performed in 36% of the lavaged infants, whereas only 5.5% of the controls had to be ventilated for more than 1 hour. The mean duration of ventilation, however, was shorter (Mh = 5.1 hrs vs Mh = 69 hrs) and the average age at discharge lower in the lavaged. Three infants died, two of whom had severe malformations. Only one death in the lavaged infants was attributable to severe meconium aspiration syndrome already manifest at birth. Three infants (two lavaged, one control) showed radiological evidence of pneumothorax, but without clinical appearance of respiratory distress. Since the lavaged group naturally contained the more severely distressed infants, with a high incidence of caesarean section on account of intrauterine asphyxia and lower Apgar scores at one and five minutes, the high incidence of radiologically confirmed meconium aspiration syndrome was not surprising.(ABSTRACT TRUNCATED AT 250 WORDS)

Investigations in the problem of diarrhoea in the Melut district, South Sudan (1981-1982).

Studies of diseases associated with diarrhoea in the Melut district of South Sudan in the years 1981-1982 showed amoebic dysentery and Rota-virus to be predominate, whereby according to our previous results Yersinia, Campylobacter, Shigella and Salmonella infections also play a role. In addition other parasitological bacterial and viral-infections are presumed.

Serological testing of human blood samples for infectious diseases in the Abeokuta and the Minna Hospitals/Nigeria.

In serological investigations undertaken in two hospitals in Nigeria a total of 188 blood samples were examined and the following positive reactions for various diseases found: malaria 100%, leishmaniasis 9.5%, biharziasis 2.1%, yersinia 16.4%, Legionella pn. 9%, gonorrhea 6%, syphilis 6.9%, measles 65.4%, rubella 84%, cytomegalic 78.2%, herpes simplex 67%, varicella 30.8%, Resp. sync. virus 34.6%, influenza A 57.4%, influenza B 73.9%, para-influenza 1, 2, 3, 20.7%, 16.5%, 52.6%, adenovirus 25%, Mycoplasma pneumoniae 33.5%.
