Serological responses of rams to a Brucella ovis-vitamin E adjuvant vaccine.

Rams which were vaccinated at 6-8 months of age with a water-in-oil Brucella ovis-vitamin E adjuvant vaccine had significantly higher serum antibody levels than rams vaccinated with a commercial B. ovis bacterin or B. melitensis Rev 1. The adjuvant vaccine did not cause abscesses at the site of injection as some water-in-oil emulsions do. Two years after vaccination, the vitamin E adjuvant-vaccinated rams had higher antibody level than the other groups. This was most likely due to a secondary response to naturally occurring infection with B. ovis.

Nephrotoxicity of high and low osmolar contrast media: case control studies following digital subtraction angiography in potential risk patients.

The urinary excretion of kidney-specific marker proteins before and 120 hours after intravenous injection of either high- or low-osmolar contrast media (CM; diatrizoate, iopamidol 370) was monitored in patients after digital vascular imaging. Inclusion criteria for the randomized clinical study in a total of 40 patients (15 women, 25 men; mean age, 64.5 years) were at least 50 years of age or diabetes mellitus with normal creatinine concentration in serum. Compared with the control period, the elimination of tubular indicator enzymes alanine aminopeptidase, gamma-glutamyltranspeptidase, alkaline phosphatase, as well as of glomerular localized angiotensinase A was significantly higher in all patients after injection of the CM. The most significant differences were observed after 48 hours. In contrast, lysosomal N-acetyl-beta-D-glucosaminidase activity in urine specimens reacted less clearly and appears to be a less sensitive parameter in assessing CM nephrotoxicity. Elimination of brush border as well as of glomerular marker proteins was significantly lower after intravenous injection of low-osmolar CM iopamidol 370 (832 mOsm/kg) than after meglumine diatrizoate 76 (2100 mOsm/kg). In all 40 patients a significant decrease in creatinine clearance was observed; however, patients receiving diatrizoate had a significant decrease in creatinine clearance (period 0 versus 24 to 48 hours after CM), whereas patients after administration of iopamidol had not. No difference was found between creatinine clearance after 48 hours of CM injection within both groups of CM. Due to noninvasive parameters of kidney damage nonionic, low-osmolar CM are less nephrotoxic in potential risk patients, and should be preferred to conventional CM.

Teat canal diameter and other cow factors with possible influence on somatic cell counts in cow milk.

A case-control study on matched pairs of cows in 2 counties of Norway indicated a strong positive association between teat canal diameter (TCD) and somatic cell count (SCC) in milk. The study indicated further a highly significant association between leakage of milk and high SCC and a significant association between teat injury and high SCC. Finally, an association between the shape of the teat end and SCC that was nearly significant (p = .067) when considered in isolation, was shown to exist through it's dependency on TCD. A flat or inverted teat end is associated with a large TCD while a pointed or round teat end tends to have a smaller TCD.

[Pulmonary manifestations of malaria].

We report on the two different types of pulmonary manifestations in acute plasmodium falciparum malaria. The more severe variant shows long standing interstitial pulmonary infiltrates, whereas in the more benign courses only short-term pulmonary edemas are visible.

[Digital subtraction angiography of patients with Marfan's syndrome].

Marfan's syndrome is a rare inborn error of metabolism. Marfan patients are prone to aneurysms of the ascending aorta and run a high risk of rupture of the aortic arch. The diameter of the aneurysm is the most important predictor of the risk and therefore the leading point for surgical interventions. IV and IA-DSA according to our experiences are simple and effective methods in pre- and postoperative evaluation of patients with the syndrome.

[Imaging of aortocoronary bypasses with intravenous digital subtraction angiography].

We examined 24 patients with 52 coronary bypass grafts, an average of 18 months after their respective operations. During the course of 1 week, a coronary angiography and a digital subtraction angiography (DSA) incorporating an intravenous injection of contrast medium were performed. Conventional coronary angiograms showed 40 bypasses as being open, ten as being occluded, and two could not be displayed at all. With the aid of digital angiography, 50 out of 52 bypasses could be classified as either open or occluded. In 44 out of 52 bypasses, DSA and coronary angiogram results were identical. Using DSA, three out of ten angiographically occluded bypasses were falsely diagnosed as being open and three out of 40 open bypasses as occluded. Two bypasses could not be interpreted due to poor picture quality. In the diagnosis "open bypass" the degree of both sensitivity and specificity subsequently amounted to 92.5%, and 70% in the diagnosis "occluded bypass". The distal part of the bypasses, as well as the proximal and distal part of the anastomoses, could not be evaluated for the most part. Furthermore, on account of the comparatively inferior quality of the pictures, detection of bypass stenosis is not reliable using digital subtraction angiography. Intravenous digital subtraction angiography may therefore serve as a screening method in the evaluation of coronary bypass grafts.

[Intra-arterial fine-needle DSA of the extremities].

We report on our initial results of 19 intraarterial digital subtraction angiographies in 18 patients. We saw diagnostic angiograms in all examinations. There were no complications. We conclude that the fine needle approach offers the possibility of outpatient angiography in selected cases.

[Spontaneous lysis of acute Leriche syndrome].

Usually, obstructions of the terminal aorta require surgery involving an aortobifemoral bypass, especially if they occur with acute signs and symptoms, the clinical pattern of these signs being very marked. The authors report on a case where an extensive obstruction of the lumbar aorta and of the pelvic vessels finally dissolved without on-target therapeutic intervention via an obviously spontaneous lysis.

[Contrast medium concentration for intra-arterial digital subtraction angiography of vessels in the extremities].

The influence of iodine concentrations and different volumes of a conventional ionic contrast media and a contrast media with low osmolarity on the quality of the results of intra-arterial digital subtraction angiography of the lower extremities in patients who underwent percutaneous transluminal angioplasty (n = 52) are reported. According to our results, intra-arterial injections of a conventional ionic contrast medium diluted with 0.9% sodium chloride solution, resulting in an iodine concentration of 190 mg/ml injectable solution, reveals the best results when injected in an amount of 6 ml per injection. The use of low-osmolarity contrast-medium showed no advantages, the quality of the resulting pictures was inferior to that revealed with the ionic compound. Probably this is because the iodine concentration in the diluted low-osmolarity compound was less.

Pulmonary digital subtraction angiography.

Intravenous digital subtraction pulmonary angiography was performed in 220 patients. Of these, 206 had suspected pulmonary embolism. Our intravenous studies yielded images of diagnostic quality in 98% of cases. Compared to conventional pulmonary angiography, intravenous digital subtraction angiography (IV DSA) is safer, faster, and easier to perform. This technique is an acceptable substitute for routine pulmonary angiography in all patients with pulmonary embolism. Intravenous DSA is currently the procedure of choice for the evaluation of patients with suspected pulmonary embolism.
