Normovolemic hemodilution in peripheral arterial disease.

In 25 patients with arterial occlusive disease in clinical stage IV (Fontaine) a normovolemic hemodilution to an average hematocrit of 0.31 +/- 0.02 was performed. Eighteen patients had peripheral ulcers of the lower limbs and three had ulcers of the upper limbs. Four patients had lesions of both hands and feet. The average age of the patients was 43. Hemodilution was achieved by repeated withdrawals of approximately 500 ml of blood, followed by reinfusion of the autologous plasma and infusion of low-molecular weight dextran solution to avoid hypovolemia. In all patients with peripheral ulcers of the upper limbs, a good to a very good clinical effect was obtained. The healing tendency of lesions on the feet and toes were less marked and depended basically on the total number of vascular impediments. The rheological and hemodynamic basis of the therapeutic effect of hemodilution and the preliminary criteria for indicating this therapy are discussed.

Fluorocarbon perfusion medium applied to the isolated rat brain.

In order to overcome the disadvantages of erythrocytes and albumin, a totally synthetic medium for the perfusion of the isolated rat brain was developed. the perfusion medium was based upon Krebs-Henseleit solution and contained in addition a fluorocarbon (FC 43) for oxygen transport and a polyol (Pluronic F 68) to furnish emulsifying and oncotic properties. The physico-chemical properties of this fluorocarbon perfusion medium allowed perfusion of an isolated rat brain to continue for more than 7 hr, while the medium could be stored as a stock emulsion for several months. The appropriate function of the isolated perfused rat brain is demonstrated by the EEG and by various substrates of the energy metabolism. The suitability of the fluorocarbon medium for perfusion of an isolated rat brain preparation is discussed.

Hemodilution (HD) in patients with ischemic skin ulcers.

In 25 patients with arterial occlusive disease in the clinical stage IV according to Fontaine a hemodilution down to an average hematocrit value of 0.31 +/- 0.02 was performed. Eighteen patients had peripheral ulcers of the lower limbs and 3 patients had ulcers of the upper ones. Four patients had tissular lesions both at the level of the feet and toes and hand and fingers. The average age of the patients was 43. Hemodilution was achieved by repeated withdrawals of approx. 500 ml blood, followed by reinfusion of the autologous plasma associated with infusion of substitutive solutions of low-molecular weight dextran to avoid hypovolemia. In all patients with peripheral ulcers of the upper limbs, with no exception, a good to a very good clinical effect was obtained ranging from extended to complete healing. The healing tendency of lesions in the feet and toe region was on the whole less marked and depended basically on the total number of vascular impediments found in one patient. The rheological and hemodynamic mechanism of the therapeutic effect of hemodilution, the preliminary criteria for indicating this therapy and its observed complications are discussed.

Effects of chlorpromazine and some of its metabolites on the EEG and on dopamine metabolism of the isolated perfused rat brain.

The study concerned the effects of chlorpromazine (CPZ), monodesmethyl-chlorpromazine (NOR1-CPZ), didesmethyl-chlorpromazine (NOR2-CPZ), and chlorpromazine-N-oxide (CPZ-NO) on the EEG and on dopamine metabolism of the isolated perfused rat brain. Isolated brains were perfused with 100 ml of a perfusion medium containing 30% bovine red cells (v/v), 2 g bovine serum albumin, 14 mM glucose as well as one of the agents in a concentration of 10 micrometers. The main dopamine metabolite homovanillic acid (HVA) was measured fluorimetrically in the striatum of the isolated brain. The EEG was recorded by two symmetrical bipolar leads from the parietal regions at various times during the 30 min perfusion period and was stored on magnetic tape. The recordings were evaluated visually and quantitatively by automatic analysis. CPZ-NO was found to be the most active agent both in changing the EEG and in elevating the HVA level in the striatum. The mean EEG amplitude and the slow wave activity increased significantly. The increase of the HVA level in the striatum was correlated with the increase of delta waves as well as excess of kurtosis and skewness calculated from the amplitude histography data. The desmethylated metabolites caused only moderate central effects.

[Recurrent iritis caused by persistent hyperplastic primary vitreous body with pseudophakia lipomatosa].

In a 27-year-old patient a microphthalmic eye with congenital cataract was enucleated because of persistent recurrent iridocyclitis with a secondary glaucoma. Histopathological examination has shown a lens shrunken up to the capsule and a retrolental typical fatty tissue (pseudophakia lipomatosa). Secondary angle closure glaucoma and chronic uveitis seem to be due to an untreated hypermature cataract. These findings and the clinical course are suggesting that the opaque lens and the persistent hyperplastic vitreous body should have been operated in order to avoid complications.

[Oneirodelirium in paroxysmal hypersomnia with periodic respiration and obstruction of the airway (author's transl)].

Case report on a patient aged 57 with periodic respiration and paroxysmal tendency to fall asleep in daytime. Between sleep and waking he had typical apnoic intervals with passing, but definite, hallucinations. The cause, in accord with the literature, is considered to have been edema of the upper respiratory tract which followed at times on irradiation of a squamous epithelium carcinoma of the roof of the nasopharynx. The psychologic and respiratory disturbances and the attacks of somnolence proved fully reversible after anti-edematous treatment.

[On eye changes associated with the pigmented spots syndrome (Peutz-Jeghers) (author's transl)].

After a survey of cases of the pigmentpolyposis (Peutz-Jeghers) in the literature reported, a woman with an abortive form of this syndrome (pigmentanomaly ) and a bilateral embryotoxon posterius (Axenfeld) is described. Both signs are heritable in the autosomal-dominant manner. The association of pigmentpolyposis and embryotoxon is not yet observed. Possibly there is present a new-mutation. The examination of the family detected no other special cases. For the future on had in cases of pigmentpolyposis possible ocular changes more in view than till now.

[Spontaneous platelet aggregation associated with administration of sex hormones in men and women (author's transl)].

Spontaneous platelet aggregation was measured by an identical rheological method in 19 women and 17 men. Eight of the women (average age 26 years, range 19-56) were at the time on oral contraceptives (group A), the other eleven (average age 26, range 19-50 years) served as controls (group B), as did 17 men (group C) of similar age (average 28, range 22-50 years). Mean maximal aggregation amplitude was significantly greater in group A than B and C (P less than 0.05 and less than 0.025, respectively), which makes it probable that there was a greater tendency to aggregation of the blood platelets under the influence of the oral contraceptive.

[Quantification of the effects of fibrinolytic therapy upon the flow behavior of blood (author's transl)].

The effect of Streptokinase-infusion upon the flow properties of blood was investigated by viscometric and aggregometric methods. Under fibrinolytic therapy, we found in all cases a significant fall in fibrinogen content, as well as a strong reduction in plasma viscosity, velocity of red cell aggregate formation and shear resistance of red cell aggregates. In addition, a drop in apparent blood viscosity at all shear rates was found. In no case, a total desaggregation of red cell was noted. The hematocrit value remained practically constant; consequently, the drop in apparent blood viscosity at high (160 s-1) shear rates and intermediate (8 s-1) shear rates can be solely accounted for by the observed drop in plasma viscosity. At low shear rates (2.3 s-1) the observed drop in apparent viscosity is partly caused by aggregation desaggregation, but mainly by a drop in plasma viscosity. The presented results again confirm that the method of rotational viscometry only incompletely records the improvement in the flow properties of blood that are caused by a therapy aimed at a reduction of the plasma fibrinogen content.

[Influence of temperature on shear induced platelet aggregation in vitro (author's transl)].

The extent of platelet aggregation (PA) in vitro was investigated in relation to the temperature. The measurements were performed with a newly developed device in which PA is induced by a well-defined viscometric flow without addition of specific aggregation promoting agents. It was found that the PA occurs in two phases as related to temperature: at 4 degrees C and 37 degrees C there is none and a minimal PA respectively, whereas at room temperature strong PA takes place. It is proposed that these results indicate that both physicochemical mechanisms and the availability of metabolic energy linked with activation of contractile processes are prerequisites for platelet aggregation. The clinical aspects are discussed.

[Long-term studies of patients with headache and pathological EEG].

In 53 patients, consulting for headache of different types, a pathological EEG had been observed several years ago; at this time, a neurological or medical origin of the bioelectric abnormalities could not be found. These patients (average age: 32.4 y.) were reexamined systematically after an average delay of 3 years. Generally, one observes a clear improvement or a normalization of the EEG and the clinical state. In 1/9 only of the population the EEG-abnormalities persisted. All these patients belonged to a subgroup, where certain complicating factors (cerebral-organic or general) could be specified. No brain tumour had developed meanwhile. The authors conclude: a pathological EEG in the context of headache does not obligatory indicate bad prognosis.
