[Morphologic studies on the occurrence of HBs antigen in liver cirrhoses, epicirrhotic and pure cancers in a large autopsy sample].

1063 hepatic cirrhoses, 138 epicirrhotic carcinomas, and 138 carcinomas without cirrhosis from the autopsy material of the Dresden region obtained from 1970 to 1979 were examined for the occurrence of HBsAg by means of reclassification and after-staining of the documented section material with orcein staining according to Shikata. The portion of the post-B-hepatic cirrhoses of the liver was 95 (8.9 per cent of the total material). In epicirrhotic carcinomas a HBsAg-containing cirrhotic portion was identified in 14 cases (10.1 per cent), whereas hepatic tissue of carcinomas without cirrhosis never showed HBsAg. A relation between the activity degree of the hepatic cirrhosis and the quantity and distribution of HBsAg could not be established. The found out numbers (about equally frequent occurrence of HBsAg in hepatic cirrhosis and epicirrhotic carcinomas, no identification in the hepatic tissue of carcinomas without cirrhosis) reveal that the thesis of a hepatocarcinogenic effect of the hepatitis B virus cannot be supported by the histologic identification of HBsAg in our investigational material. The possibility of confusion of ground-glass hepatocytes with liver oncocytes in the HE-stained section material of the autopsy material is pointed out.

A procedure to verify an amino acid sequence which has been derived from a nucleotide sequence: application to the 26S RNA of Semliki Forest virus.

We describe a peptide sequencing procedure which can be used to verify an amino acid sequence which is derived from a nucleotide sequence. One first labels the protein with a 3H- and a 14C-labelled amino acid and then cleaves the protein into a set of peptides using a cleavage reaction specific for a particular amino acid residue. Finally one performs Edman degradations on the whole mixture of peptides. The released amino acids reflect the combined aminoterminal amino acid sequences of all the peptides that have been formed by the cleavage reaction. The data can therefore be used to check a deduced sequence simultaneously at several regions of the polypeptide chain. We have applied this sequencing procedure to verify the amino acid sequence deduced from the 26S RNA of Semliki Forest virus.

Storage of the complement components C4, C3, and C 3-activator in the human liver as PAS-negative globular hyaline bodies.

Liver biopsies of a 58-year-old clinically healthy patient with a hepatomegaly and intracisternal PAS-negative globular hyaline bodies were immunofluorescent-optically examined for the content of the complement components C 1 q, C 4, C 9, C 1-inactivator, C 3-activator. Further examinations were performed for fibrinogen, IgG, IgA, IgM, IgD, IgE, L-chain (type chi and lambda), alpha 1-antitrypsin, alpha 1-fetoprotein, alpha 1- and alpha 2-glycoprotein, cholinesterase, ceruloplasmin, myoglobin, hemopexin, HBsAg and HBsAg. Th inclusion bodies reacted with antisera against the complement components C 4, C 3 and C 3-activator, as also identified by double immunofluorescence. Probably this is a disturbance of the protein metabolism of the liver cell with abnormal complement storage in the presence of normal total complement and normal complement components in the serum.

Expression of the structural proteins of Semliki Forest virus from cloned cDNA microinjected into the nucleus of baby hamster kidney cells.

The three structural proteins of Semliki Forest virus--i.e., the capsid, p62, and E1 proteins--were expressed in baby hamster kidney cells from cloned DNA transcribed from the virus-specific 4.1-kilobase mRNA. The cDNA was engineered into an expression vector developed by others [Mulligan, R. C. & Berg, P. (1980) Science 209, 1422--1427] downstream from the simian virus 40 early promoter and was introduced into cell nuclei by microneedle injection. Immunofluorescence analysis of injected cells showed that the capsid protein was located in the cell cytoplasm, whereas the membrane proteins were associated with cellular membranes. The p62 protein was shown to be transported from the rough endoplasmic reticulum to the plasma membrane, whereas the E1 protein remained in the rough endoplasmic reticulum.

Nucleotide sequence at the junction between the nonstructural and the structural genes of the semliki forest virus genome.

The nucleotide sequence at the junction between the nonstructural and the structural genes of the Semliki Forest virus 42S RNA genome has been determined from cloned cDNA. With the aid of S1-mapping, we have located the 5' end of the viral 26S RNA on this sequence. The 26S RNA is homologous to the 3' end of the 42S RNA and is used as a messenger for the structural proteins of the virus. The nucleotide sequence in the noncoding 5' region of the 26S RNA (51 bases) was thus established, completing the primary structure of the 26S RNA molecule (for earlier sequence work, see Garoff et al., Proc. Natl. Acad. Sci. U.S.A. 77:6376-6380, 1980, and Garoff et al., Nature (London) 288:236-241, 1980). An examination of the nucleotide sequences upstream from the initiator codon for the structural proteins on the 42S RNA genome shows that all reading frames are effectively blocked by stop codons, which means that the nonstructural genes in the 5' end of the 42S RNA molecule do not overlap with the structural ones at the 3' end of the molecule.

Local BCG-immunotherapy in bladder cancer patients. Immunological follow-up studies.

In a controlled clinical study 10 patients with bladder cancer were treated with an additional local BCG-therapy. For this investigations only patients with tumour stage T1-2 NO MO were elected. The local BCG-therapy was done as intralesional BCG-injections, the interval between each management was 4 weeks over a period of 4 months to 1 year. Under this treatment in all 10 patients tumor regression in different forms was seen. In 5 patients the tumor was not more found, neither in clinical nor in histological examinations. Side effects of the therapy were: influenza-like syndrome (6 patients), mild temperature (6 patients), increase of liver transaminases (5 patients), and granulomas in lung and liver (2 patients). In the immunological follow-up studies we have seen an increase of the lymphocyte activity against tumor-associated antigen from bladder cancer tissue in the electrophoretic mobility test in 6 patients. The results in skin testing, using 5 recall-antigens and DNFB, showed an increase of reactivity in only 4 patients. The total number of lymphocytes increased over a longer period. We didn't find typical humoral changes. The results of our 10 patients show that an intralesional BCG-therapy could be successful in 5 patients. The effect of BCG for immunostimulation was seen in vitro in 6 patients and in vivo in 4 patients.

[The immunologic behavior of carcinoma in situ of the bladder].

A report is given on 13 patients with c. i. s. of the bladder, which are examined in vivo and in vitro with respect to their immune reactivity. In skin tests with 5 recall antigens and the primary antigen DNFB there is no difference as compared with a control group of 29 patients. In an EM test 11 of the 13 patients with c. i. s. reacted with histospecific lymphocyte sensitisation, 10 out of 13 positive results were obtained with the antigen HEP. These findings support the current view of c. i. s. of the bladder as the first stage of a malign degeneration.

[Effect of allopurinol on adrenergically induced changes in serum lipid levels, heart rate and blood pressure in normal and ischemic rabbits (author's transl)].

In accordance with previous results obtained in healthy volunteers 1H-pyrazolo[3,4-d]pyrimidin-4-ol (allopurinol) showed an inhibiting effect on the adrenergically induced mobilization of free fatty acids in normal rabbits. The effect of allopurinol was nearly equal to that of prostaglandin E1 but less than that of pyridyl carbinol. Similarly like in patients with coronary heart disease, this effect was not found in rabbits with cardiac damage following isoprenaline pretreatment. Allopurinol at higher dosage diminished the noradrenaline-induced decrease of heart rate and increase of blood pressure.

[Significance of uric acid concentration in the serum for the early recognition of the risk of EPH gestosis].

102 patients with EPH-gestosis in different stages of severity and 29 patients with normal pregnancy were examined regularly in a longitudinal study. 54 clinical and laboratory parameters were continuously measured. The concentration of uric acid in serum has proven to be the most important parameter in the early detection of the risk of gestosis. This serum concentration correlates with blood pressure and in individual cases shown to be of additional predictive value. Significant differences, compared to normal pregnancy, exist already in the 17th to the 24th week. Until now these differences had been described for a much later period. Other laboratory parameters have not been useful at this early stage in the detection of likely development of EPH-gestosis. Hyperuricemia is primarily due to the isolated reduction of uric acid clearance by normal glomerular filtration rate. Thus it is assumed that hyperuricemia is an indication of renal cortical blood flow dysfunction which parallels the decreased perfusion of the placenta.

[Problems of assessment and occupational rehabilitation of patients with malignant urogenital neoplasm].

The expert opinion and the rehabilitation of patients with urogenital tumours often render great problems and tasks for the urologist. A schematic invalidisation of these patients is to be refused. The leading idea of an expert opinion should be the rehabilitation. This demands from the expert a judgment of the function with evaluation of the oncological prognosis quoad vitam and a judgment of the remaining functions of the treated organism as well as a recommendation for the further professional activity based on this judgment. A close cooperation with industrial institutions, the factory public health, the medical advisory commissions and the rehabilitation commissions is necessary for rehabilitation. The notion of rehabilitation--professional as well as social--is to be extended to the not unconsiderable number of patients at pensioner's age. The resumption of a suitable work effects in a patient sith successfully treated tumour disease the improvement of the general physico-psychic resistance, increases the will to recovery and activity and furthers the acclimatization and adaptation to remaining disturbances. Thus expert opinion in the sense of rehabilitation is also a therapy. The results of modern tumour therapy should not be measured only at the survival rates, but also at the degree of the social and professional rehabilitation of the patients.

Estimation of decision criteria for the uric acid concentration for the early diagnosis of gestosis.

An increase of the serum uric acid concentration of pregnant women can indicate an EPH gestosis with a high risk. Above a uric acid level of 270 mumol/l (and of 315 mumol/l after the 32nd. week of pregnancy) and gestosis has become moderate. An increased risk for mother and fetus must then be assumed. This decision point is estimated by comparing three techniques: The ROC-curve-method and the determination of the maximal efficiency and of the maximal information content.

[Finding and significance of copper protein complexes in liver cirrhoses (author's transl)].

302 liver cirrhoses obtained from the post-mortem material 1970 to 1979 have been examined for the occurrence of the cuprous protein complexes by means of the orcein-staining according to Shikata. The part of the B-posthepatic cirrhoses (89%) could be simultaneously determined with the help of this method. Cuprous protein complexes have been found in bilious (100%), alcoholic (43.9%), etiologically uncertain (34.4%), and B-posthepatic (25,9%) liver cirrhoses. They prove an existing or experienced chronic cholestasis. The demonstration of copper has no principal significance for the etiologic classification of cirrhoses. In the bioptic diagnostics the demonstration of copper has a certain significance for the differentiation of the primary destructive cholangitis from chronically active hepatites of other genesis. The investigations incidentally revealed that HBsAg-containing hepatocytes could be identified by means of orcein in sections performed on archive material after destaining independent of the age of the sections and their primary staining.

[Differential diagnosis of pyeloureteritis cystica, follicularis et granularis].

At the instance of the observations of two patients the differential-diagnostic difficulties in the pyeloureteritis cystica and the ureteritis follicularis or granularis are described. Here first of all the following other diseases must be excluded: 1. neoformations in the upper urinary tract, 2. uro-tuberculosis, 3. X-ray-negative calculi, 4. HWI with gas-producing germs. The knowledge of the X-ray diagnostic criteria of the individual diseases is often of life-important significance for making a certain diagnosis for the patient. For this reason also the performance of a retrograde ureteropyelogramme may be indicated, if, exhausting all possibilities of the excretion urography, a diagnostic ascertainment is not possible.

[Morphologic contribution to a progeria syndrome (Hutchinson-Gilford) (author's transl)].

Report of necropsy findings of a 17 years old girl with a progeria syndrome. There was a high degree of generalized atherosclerosis, involving the visceral arteries, chiefly those to the kidneys. The patient died of nephrosclerosis with uraemia. The aetiology of progeria syndrome remains obscure, single findings (lipoprotein- and amino-acid metabolism) may be understood as an inborn error of metabolism.
