Carbenoxolone patients with gastric ulcers. A double-blind trial.

Carbenoxolone was compared with placebo in a randomised double-blind study involving 40 patients with gastric ulcer (20 each in the treated and placebo groups). The evaluation was based on eight variables. The endoscopic findings on completion of 4 or 6 weeks of treatment, revealed greater improvement in response to carbenoxolone than to placebo. The difference was statistically significant at the previously fixed level of significance of p less than 0.05. The plasma potassium level fell significantly in the treated group but the actual reduction of 7% was slight. No statistically significant differences were measured in any of the other variables, i.e. plasma sodium, SGOT, SGPT, systolic and diastolic blood pressure, bodyweight, or oedema formation.

[Immunohistochemical and radioimmunological demonstration of alpha1-fetoprotein in nonmalignant changes of human gastric mucosa (author's transl)].

The occurrence of alpha1-fetoprotein in nonmalignant changes of the gastric mucosa was investigated by means of immunohistochemistry and radioimmunoassay. The investigations were performed in tissue sections, cytological imprint preparations as well as in homogenized tissue samples (obtained by gastroscopy). alpha1-fetoprotein could be demonstrated by immunohistochemistry in about 90% of the samples originating from the surroundings of gastric ulcer and the region of gastrojejunostomy after B II-resection. The RIA was positive in about 75% of the tissue samples, whereas from gastric juice only 40% of positive results could be obtained. No alpha1-fetoprotein-activity could be demonstrated in serum samples. These investigations indicate that alpha1-fetoprotein is not exclusively synthesized by embryonic or neoplastic tissues and also can be synthesized also by regenerating cell-systems. It may be supposed that this synthesis represents an unspecific answer to growth-stimulation.

[Blood group compatibility testing for blood transfusions].

Acute and chronic dangers of blood-transfusion are listed. Three serologic demands in selecting donorblood are identity in the ABO-system compatibility in the Rh-system and a negative cross-match in the mode of a three-step Coombs-test. The often claimed consideration of the Kell-system is shown to be numerical in the background of transfusion-practice. The highest responsibility is taken by the physician in transfusing patients with Coombs-positive auto-immune-diseases in which the cross-match is positive and so present antibodies could be hidden.

[How healthy are our blood donors? Result of a liver screening in voluntary blood donors of Blutspendedienst Innsbruck].

A screening of hepatic function and HBs-antigen, made in 22344 voluntary blood donors in the bloodbank of Innsbruck, gave the following results: 0,33% HBs-AG negative donors were found to have pathological liverfunctiontests. Overweight and alcohol would be established as the most important etiological factors. 0,4% of the donors are carriers of the HBs-antigen. A control examination after 2 years showed a persistence of that antigenemia. Antigen carriers are requested to have regular examinations. Since HBs-AG positive hepatitis is not only transferred by blood and blood-constituents, it would be important to use the same screening methods applied in the blood donor organisation throughout any hospital area, to reduce the incidence of this disease.

[Immunohistochemical studies on non neoplastic and neoplastic gastric mucosa. Determination of embryonic and specific antigens (author's transl)].

The distributions of acid alpha1-glycoprotein, alpha1-fetoprotein, beta-galactosidase and gastrin in gastric carcinoma and gastric ulcer as well as in the neighbourhood of these lesions were studied by means of immunohistochemical methods on imprint preparation. We could not find significant differences between gastric carcinoma and the nonneoplastic lesions, except for the acid alpha1-glycoprotein. The results of this first study indicate that the immunochemical and immunohistological assay of acid alpha1-glycoprotein might be of practical value in diagnosing malignant changes of gastric mucosa.

[Serological identification of carcinospecific antigens (and their significance as donor screening or for specific groups of diseases)].

Alpha-1-fetoprotein is an example of a circulating, measurable tumor product of diagnostic and therapeutic value. The involvement of its synthesis could be a result of a premalign cellular change in cell biochemistry. Contrary to the synthesis of trophic hormones in certain undifferentiated neoplasms, alpha-1-fetoprotein in hepatoma is specific for the organ origin of the tumor. Unlike the immunoglobulins in myeloma or the corticosteroids in adrenocortical tumors, it is a protein that normally can be synthetized in the fetus only. The purpose of the present paper is to discuss a new method for testing serum samples of blood donors being suspected of an alpha-1-protein by means of counterelectrophoresis. The diagnostic value is shown in a blood donor who could be singled out as a suspect of primary liver carcinoma only by means of serological testing.

[Balanced parenteral nutrition following total pancreatectomy. (Metabolic data)].

The replacement of components of normal metabolism getting lost during operation is an important problem for the clinician. On the basis of a case of total pancreatectomy because of carcinoma of the pancreas this essential problem for the postoperative treatment is demonstrated. After control of the predominant metabolic parameters of the patient over a relatively long postoperative period, we want to formulate our conclusion as follows: In spite of the massive surgery we found only relatively small destruction of the patient's metabolic system. The evident variations in blood sugar level were probably caused by the often changing kinds of nutrition (the patient having been some weeks at home). As the most distinctive sign of a well performed therapy we regard the results of the daily weight control. In spite of the heavy physical stress over many months we could stabilize the patient's body weight after a short postoperative loss.
