[Universal measurement site for the processing-oriented recording of cardiovascular data].

A measuring place is described for the registration of standardized, evaluable by means of the computer deviations of bioelectric signals (parameters of heart and circulation, peripheral pulse curves and rheogramms). The high demands which concern the adequate establishment measuring places with regard to quality and comfort of attendance and condition corresponding technical solutions are explained. At the instance of the rheogram the mode of function is described. A short prognosis on the trend of development in the recognition of biological measuring values is given.

Temperature and endocrine activity during sleep in man. Activation of cortisol and thyroid-stimulating hormone, inhibition of human growth hormone secretion by raised or decreased ambient and body temperatures.

1. Polygraphic night sleep recordings in eight healthy male volunteers with simultaneous measurement of rectal temperature, plasma growth hormone (HGH), cortisol, and TSH concentrations were performed during normal, raised, and lowered ambient and body temperature. 2. There was a statistically significant increase in plasma cortisol and TSH levels during cold nights with a smaller rise during high temperatures. 3. Growth hormone levels, measured as the mean highest plasma concentration in the first two NREM-REM sleep cycles, were slightly lower during hot and cold nights than corresponding baseline values. It is suggested that there may be an inverse relation between ACTH and HGH secretion by the anterior pituitary gland. 4. During the nights of high ambient temperature, decreased total duration of sleep and particularly low values of paradoxical sleep were observed. Night sleep in low ambient temperature with a significant decrease of body temperature is not different from baseline conditions. 5. The results suggest that a pronounced increase in stress hormone secretion may occur without changes in polygraphic EEG criteria.

Computer aided surgery with special focus on neuronavigation.

The term computer aided surgery (CAS) is now mainly used for an intraoperative navigation within the body combining a 3D-digitizer with preoperative CT/MR-imaging. This method has become indispensable in neurosurgery for the removal of deep-seated and/or critically located intracranial tumors and vascular malformations. Also ENT surgery within the paranasal sinuses and setting of pedicle screws in orthopedic surgery profit greatly from the high targeting precision of CAS. And still a growing number of surgical disciplines are employing this method. Today infrared-optical 3D-digitizers are state of the art, but electromagnetic spatial digitizing using novel, miniature localizers is promising, too. The results of our CAS study 1994-mid-1997 with 50 patients suffering from small intracranial lesions are presented.

[Complicated malaria tropica in an infant].

We report on a five month-old infant from Cameroon, who was admitted because of febrile gastroenteritis after living in Germany for two weeks. Since the patient was somnolent, had a parasitaemia of Plasmodium falciparum of 230,000/microliter, a haemoglobin concentration of 5.3 g/dl and a thrombocytopenia, a complicated falciparum malaria was diagnosed. Treatment was started immediately with intravenous 20 mg quinine/kg bw as a loading dose, followed by 10 mg/kg bw every 12 hours, combined with intravenous clindamycin 10 mg/kg bw bd. Red blood cells were transfused once. The parasitaemia dropped to 2000 trophozoites/microliter within 48 hours. No asexual stages were detectable from the third day of treatment on. Weekly controls for the following four weeks remained negative. The mortality rate of complicated malaria is 50% in the first year of life, which can be reduced by early treatment. We present this case to draw attention to therapeutic options in infants.
