Pseudohypoparathyroidism and hypocalcemic "myopathy". A case report.

A patient with the clinical features of pseudohypoparathyroidism and elevated concentrations of serum CK and LDH, which normalized after successful therapy, is described. Clinical signs of myopathy did not exist. The bioptical material from the m. tibialis anterior was microscopically normal. The biochemical analysis revealed a reduced phosphorylase-a-activity with the total phosphorylase-activity (a and b) being within the normal range. The significance of these findings as well as possible pathogenetic mechanisms are discussed.

[The nameless pill. The effects and side-effects of the therapeutic use of placebo preparations].

Information is provided on the frequency of therapeutic use of placebos and on the quality and quantity of the effects observed after placebo treatment. The possible causes of these effects and theoretical explanatory models are also discussed. The ethical aspects of the clinical use of placebos are considered and more scientific investigation to enhance unspecific effects of all pharmacotherapy is called for.

The influence of progesterone on the antidiuretic and oxytocic activities of the posterior pituitary lobe of female rats.

Mature, female rats received 2 mg progesterone i.m. during proestrus and estrus and 4 mg during all stages of the sexual cycle. 24 h after injection the animals were decapitated. Antidiuretic and oxytocic activities of the posterior pituitary lobe were tested by bioassay. The increased hormone content of the neurohypophysis in reaction to progesterone administration is explained by an inhibiting effect of progesterone on hormone secretion. The physiological and possible pathophysiological importance of this effect is discussed, expecially with regard to the genesis of cyclic edema and late pregnancy gestosis.

On electrolyte determinations in the pituitary and pineal gland: is there a diurnal rhythm?

At four different times during the day determinations of the electrolyte content of the anterior and posterior pituitary as well as of the pineal had been carried out by means of atomic absorption spectroscopy. There are significant differences in the sodium and potassium content between these organs. There are also distinct changes in the adenohypophysial Na+, K+ and Mg++ content between the light and the dark period. Values are low over the day and high during the night. In the light period there is also a significant higher sodium concentration in the neurohypophysis and a greater potassium concentration of the pineal tissue. Despite a similar phylogenetic development the neurohypophysis reveals a considerably less amount of univalent cations than the pineal gland. Data from the literature, however, let suggest that there are no greater differences in the water content of neurohypophysis and pineal. The possibly resulting "hypotonicity" of neurohypophysial tissue and its possible implications for the mode of the release of hormonal substances from the neurohypophysis are discussed.

Cranial sclerosis with striated bone disease (osteopathia striata).

This report concerns another case of striped bone disease (Osteopathia striata) together with sclerosis of the base of the skull. The mode of inheritance of this rare genetic disorder is still unknown. The clinical characteristics of 20 cases reported in the international literature are summarized and compared with our own findings.

[Expert assessment of work disability--do we realize the effects?].

Three inquiries on the results of medical expertises concerning incapacitation for work in cases where employers had doubted this fact, seem to establish that the Medical Service of the Social Health Insurance (MDK) is quite capable of keeping improper use of the epithet "unfit for work" in check. If the design of the studies is critically analysed, it becomes evident that most problems are still unsolved. More clarity regarding the sense or nonsense of this work-intensive topic may result from a carefully and completely planned randomised trial. Such a study appears mandatory, the more so since the economic benefit of the expertises does not seem to be very bright, taking several hypotheses into consideration.
