Systemic corticosteroid treatment in Vogt-Koyanagi-Harada disease.

Twenty patients with Vogt-Koyanagi-Harada disease who had been given systemic corticosteroids were retrospectively analyzed. Sixteen patients were successfully treated with systemic corticosteroids without recurrence of uveitis. Since the advent of corticosteroid treatment, there have been fewer ocular complications as well as extraocular sings and symptoms in most cases. Recurrence of the inflammation, however, occurred in four cases. Drip infusion of 200 mg prednisolone daily, widely employed for the treatment, did not always have good results. It is suggested that the dosage of corticosteroid should be adjusted according to the severity of uveomeningitis. Oral administration of corticosteroid (less than 100 mg prednisolone daily) may be enough for suppression of the inflammation in the Harada type, while a drip infusion of the massive dosage (more than 200 mg prednisolone daily) with gradual tapering off is possible requisite in the Vogt-Koyanagi type.

A scanning electron microscopic study of nylon degradation by ocular tissue extracts.

A scanning electron microscopic study showed that the surface of the nylon used for corneoscleral wound closure was smooth, while the nylon suture thread used to fix the intraocular lens against the iris showed cracks and roughness on its surface. The surface of the nylon thread was also roughened after treatment with bovine iris-ciliary body extracts. The nylon-degrading factor in the extract was nondialyzable, heat-labile, active at acidic pH, and inhibited in part by leupeptin, suggesting that lysosomal enzymes may be a factor in this phenomenon. Of the bovine ocular tissues, ciliary body extract degraded the surface of the nylon thread most significantly. The nylon surface was moderately digested by the retinal pigment epithelial extract, and slightly eroded by the extracts from the iris, sensory retina, and choroid. Corneal and lenticular extracts did not affect the nylon surface. Nylon suture thread may be hydrolyzed by lysosomal enzymes and should be considered a late-degrading suture.

Antigenicity of galactocerebroside in experimental allergic demyelinating diseases.

Monkeys, rabbits, guinea pigs, and rats were inoculated with GC, carrier protein and complete Freund's adjuvant. Clinical and pathological changes were obtained in monkeys and rabbits, but not, to date, in guinea pigs and rats. In rabbits, alterations were restricted to the PNS tissue, distributed in spinal roots, ganglia, and peripheral nerves. The lesions were characterized by perivenous myelin breakdown and accumulations of macrophages. The response in monkeys, also restricted to PNS, was mainly an axonal degeneration. Myelin breakdown was interpreted as secondary to the axonal damage. Axonal reactions were observed in root ganglion and anterior horn cells, being accompanied by ascending degeneration in the posterior columns of the spinal cord. Demyelinating antiserum was present in all rabbits, but absent in all monkeys. Myelination inhibiting factor was also positive in all rabbits but not in monkeys. From these findings, it can be concluded that the antigenicity of the GC is not a generalized response in all animals. The lesions produced by GC in monkeys seem to be degenerative rather than allergic.

Efficacy of anti-ulcer drugs on the recovery of gastric mucosal glycoproteins with aspirin-induced gastric damage in rat.

The efficacy of two anti-ulcer drugs, Cimetidine and Cetraxate, on the mucus glycoproteins of gastric mucosa in the aspirin-induced gastric damage was studied in rats. Simultaneous or previous oral administration of Cimetidine or Cetraxate with aspirin reduced the diminution of the mucus glycoproteins which was occurred by aspirin administration. The recovery of the content of mucus glycoprotein in drug dosed rats occurred within 3 h after aspirin dosing and was nearly 90% of control at 5 h in all cases. Single administration of Cetraxate or Cimetidine produced an increase in the mucus glycoprotein content greater than that of the untreated control. Although a macroscopical method for the measurement of gastric damage was applied to this work, neither erosions nor linear ulcers were observed in all cases except the single administration of aspirin. The biochemical method used should be able to assess the efficacy of drugs on the mucosal lesion which cannot be expressed as the ulcer index.

Changes of adenosinetriphosphatase (ATPase) activities in lymphocytes from patients with carcinomas of the gastrointestinal tract.

The activities of both total and Oligomycin-sensitive ATPase in lymphocytes from patients with gastrointestinal tract (GI) carcinomas in different Karnofsky performance status (KPS) were determined to investigate whether or not the changes of these enzyme activities were correlated to those of the immune responsiveness of the patients. In the patients whose KPS indicated more than 50%, the mean value of the total ATPase activity in lymphocytes tended to increase slightly as compared with that determined in the control subjects, but the mean value of the Oligomycin-sensitive ATPase activity in lymphocytes tended to decrease slightly as compared with that determined in the control subjects. In the patients whose KPS indicated less than 30%, the mean values of the activities of both total and Oligomycin-sensitive ATPase in lymphocytes tended to decrease remarkably as compared with those determined in the control subjects. The ATPase activities in lymphocytes were suggested to be useful measures to the immune responsiveness of the patients with GI carcinomas.
