Effect of the proton pump inhibitor, NC-1300, on gastric mucus secretion in rats.

The effects of NC-1300 on gastric acid and mucus glycoprotein secretion were investigated in pylorus-ligated (4 h) rats. NC-1300 (30 mg/kg, intraduodenally) significantly inhibited gastric acid concentration more than 90%. The agent did not affect the total mucus glycoprotein output in the gastric juice, but caused by about 1.5 times denser mucus glycoprotein secretion than that in the control group. The amount of mucus glycoprotein in the mucous gel layer increased by about 1.4 times with this agent. Thus, NC-1300 not only inhibits acid secretion but also promotes mucus secretion, and this may explain its ability to protect the stomach against necrotizing agents.

[A case of advanced gastric cancer (Borrmann's type 2) that disappeared following short-term preoperative chemotherapy with UFT].

On June 2, 1988, a 63-year-old male visited Kitasato University East Hospital because of body weight loss. After an upper GI series and endoscopic examination the patient was diagnosed as having an advanced gastric cancer (Borrmann's type 2) that occupied the anterior wall of the mid portion of the stomach. Endoscopic biopsied specimens showed a poorly differentiated adenocarcinoma, histologically. Thus, the patient was administered UFT (600 mg/day) for preoperative chemotherapy for 3 weeks, and then underwent surgery on July 7, 1988. In a resected specimen, the cancerous lesion was found to have reduced in size and, macroscopically, had changed to a more flat type. Further, on histological examination, no cancerous cell were found in the primary lesion or in the regional lymph nodes in the resected specimen. Thus we report this case as representing a complete to UFT chemotherapy.

[A case of metastatic lung tumor of the colon cancer with ova of schistosoma japonicum in the resected lung specimen].

A 69-year-old man underwent right middle lobectomy because of the tumor lesion in the right middle lung. He had ever been in Celebes and China (riverside area of the Yangtze), the infected area of schistosomiasis japonica, for military service during the World War II. Resected lung specimen was carefully examined at the department of pathology in our hospital. Several ova of Schistosoma japonicum were discovered scattered in the specimen, and the pathological feature of the lung lesion was revealed to be metastasis of the sigmoid colon cancer which was resected 4 years ago at a certain hospital. The ova of Schistosoma japonicum were also distributed near the colon lesion. Recently in Japan, it is said, acute infected case of schistosomiasis japonica has been ceased, however, in cases of chronic stage of this disease, hepatoma or lower intestinal cancers are still discovered through the careful follow up study.

[Evaluation and determination of P-type-alpha amylase activity in human serum by monoclonal antibody].

We evaluated a kinetic procedure for determining pancreatic type of alpha-amylase isoenzyme (EC 3.2.1.1.) by using monoclonal salivary amylase antibody in human serum. The principle of this method is to measure residual pancreatic amylase activity after the reaction of monoclonal anti-salivary amylase antibody which is specifically inactivated human salivary amylase. The reaction of antibody with salivary alpha-amylase completed within 2 min of incubation and residual activity of pancreatic amylase was 100%, and the residual salivary amylase activity showed less than 3% of salivary amylase activity after incubation with antibody. The reproducibility and/or precision was 1.2% to 3.4% and the linearity obtained up to 1.500 U/l. Correlation coefficient between the proposed method (y) and electrophoresis (x1) and wheat germ inhibition method (x2) was well, respectively (r = 0.99 with both method, the regression curves was y = 1.02 x1-0.7 and y = 1.04x2-0.7). The reference value from healthy adults (male 98 and female 102) showed 13.6-48.2 U/l according to statistical parametric method and sex difference was not observed. The monoclonal anti-salivary amylase antibody was not cross-reacted with the other animal serum salivary type of amylase.

[Role of protein C in endotoxin-induced release of plasminogen activator inhibitor from endothelial cell].

To elucidate the role of protein C (PC) in the release of plasminogen activator inhibitor (PAI) from endothelial cells, the effect of PC and activated protein C (APC) on plasma levels of PAI in endotoxin (ET)-treated rats was examined. When activated by snake venom, human PC significantly prolonged the activated partial thromboplastin time (APTT) of both human and rat plasma samples. Addition of APC also prolonged the APTT of both human and rat plasma samples. PAI activity in plasma from septicemic patients and ET-treated rats was neutralized by APC. A small dose of ET (0.1 microgram/kg) gradually increased plasma PAI activity, which became maximum 3h after ET-treatment. APC administered prior to ET-treatment, PC decreased PAI activity, however, no such inhibition was seen when administered after ET-treatment. A significant negative correlation between PC concentrations and PAI activities was observed in plasma from septicemic patients. These findings indicated that activation of PC on endothelial surface plays a regulatory role in releasing PAI and that endotoxin might inhibit the surface activation of PC.

[Anaerobic threshold and oxygen uptake in patients with mitral stenosis].

The exercise stress test with a bicycle ergometer was performed for 31 patients with mitral stenosis and for 10 normal subjects. The patients were categorized in two groups. Group 1 consisted of 16 patients having indication for surgical intervention and Group 2 consisted of 15 patients without such surgical indications. Oxygen uptakes at the anaerobic threshold and at peak exercise (MAX) were assessed by percent attainment of the predicted normal value from Posner's equation. Heart rates during exercise did not differ between the two groups. However, Group 1 had significantly smaller values of percent attainment of oxygen uptake both at the anerobic threshold and peak exercise than the controls and Group 2. Oxygen pulses in Group 1 were also significantly less than in the controls or Group 2. Seven cases were reassessed six months or more after surgery including open mitral commissurotomy in two and mitral valve replacements in five. The improvement of oxygen pulse showed a statistical significance. Percent oxygen uptake attainment was also significantly improved both at the anaerobic threshold and at peak exercise. Percent attainment of oxygen uptake in mitral stenosis differed significantly according to the NYHA class both at the anaerobic threshold and peak exercise. These values are considered useful for making decisions for surgical treatment in borderline cases.

[Cardiac operation in children of Jehovah's Witnesses].

The perioperative courses of 5 open heart operations in children of Jehovah's Witnesses are reviewed. The age of patients ranged from 6 to 13 years old and the body weight, from 21.5 kg to 38.5 kg. All the patients survived and are doing well now. The cardiopulmonary bypasses, primed with lactated Ringer's solution, were performed safely with the use of moderate hypothermia. Hemodilution techniques are the key for the safe exocorporeal circulation and we set the lowest limit of body weight at 20 kg. Postoperative bleeding occurred in one case and was treated by early exploration. All the cases did not receive any blood or blood products during hospitalization. Therefore we can conclude that cardiac operations over 20 kg of body weight can be safely performed without any blood or blood products. With regard to social aspects, especially in case of emergency, we should make the decision regarding transfusion under the guidance proposed by the Japanese Medical Association.

[Combined aortic and mitral valve replacement with or without preservation of posterior mitral apparatus].

Seventy patients who underwent aortic and mitral valve replacement were reviewed. 37 patients had the mitral valve replaced with preservation of posterior mitral apparatus (AVR and modified MVR; modified group), and the others without (AVR and conventional MVR; conventional group). Hospital deaths occurred in 4 patients (12.1%) in conventional group and in only 1 patient (2.7%) in modified group. Four out of these five hospital deaths were due to low output syndrome (LOS). The other one died of unknown cause. Severe LOS, necessitating intra-aortic balloon pumping, occurred in 4 (12.1%) in conventional group and in 2 (5.4%) in modified group. In the early postoperative period modified group showed better left ventricular function than conventional group in terms of cardiac index and left ventricular stroke work index. Cardiac performance was evaluated by means of echocardiography at the time of discharge. Modified group showed better fractional shortening and shorter PEP/ET (pre-ejection period/ejection time). These findings suggest that the lower mortality of modified group is due to better left ventricular function preventing LOS in the early postoperative period, and that preserving the posterior mitral apparatus has a more beneficial effect on postoperative left ventricular function even in combined valve disease.

[Treatment of congenital tracheal stenosis. Retrospective study of thirteen cases].

Thirteen cases of congenital tracheal stenosis (CTS) were retrospectively studied in terms of operative indication and treatment. The intratracheal transverse ratio of the stenotic/normal diameter (stenotic ratio, SR) was obtained by chest X-ray, tracheogram or autopsy, as one of the parameters of stenosis. SR ranged from 0.14 to 0.50 (mean 0.35 +/- 0.13 SD). Two cases (SR = 0.14) received tracheal resection and reanastomosis successfully. In three cases with pulmonary artery sling (PAS), PA translocation was performed to release tracheal compression. One (SR = 0.49) had good result but two (SR = 0.20, 0.30) died from respiratory failure (RF) and autopsy revealed PAS. Four cases underwent surgery for congenital heart disease (CHD), 3 (SR = 0.41, 0.46, 0.49) had good results but one (SR = 0.36) died from RF. In four cases medically treated, 3 (SR = 0.40, 0.44, 0.50) improved with growth but one (SR = 0.25) died before operation due to RF and autopsy revealed PAS. When SR greater than or equal to 0.40, cases without PAS improved with conservative treatment, those with PAS improved by PA translocation, and those with CHD tolerated open heart surgery. When SR less than 0.40, tracheal reconstruction is indicated. Cases with PAS would require primary or staged operation of PA translocation + tracheal reconstruction. The most appropriate therapy should be selected on the basis of symptoms, associated diseases and parameter of tracheal stenosis, for instance "SR".

Effect of limonene and related compounds on the percutaneous absorption of indomethacin.

The promoting effect of cyclic monoterpenes present in essential oils on the percutaneous absorption of indomethacin (IMC) from gel ointments was investigated in rats. As compared Azone, drug absorption was markedly enhanced by the addition of d-limonene, which is the main component of orange or lemon oils. Similar activity was observed in the cases of the l- and dl-forms of limonene, p-menthane, alpha-terpinene or terpinolene. On the other hand, no effect was obtained when the additive had hydroxyl or carbonyl groups, or ether oxygen in its chemical structure. The effect of pretreatment of skin with d-limonene on subsequent percutaneous absorption of IMC was investigated in order to estimate the influence of d-limonene on the barrier properties of the skin. No effect was observed suggesting that d-limonene might reversible alter the skin structure as the barrier for the drug transport. The serum concentration of IMC increased proportionally when increasing amounts of ethanol were present in the formulation (gel ointment) containing d-limonene. Therefore, ethanol may act as the accelerating agent for the promoting activity of d-limonene.

Mitochondrial aspartate aminotransferase by immunoprecipitation method in patients with acute myocardial infarction.

The semiautomated kinetic procedure for determining cytoplasmic aspartate aminotransferase (c-AST) and mitochondrial aspartate aminotransferase (m-AST) activities was studied by the use of the immunoprecipitation method with anti-c-AST antibody in serum samples. The measured activity for m-AST remained constant after the addition of c-AST up to 1,000 IU/l throughout the 60-min incubation period. The measurements of m-AST activity were reproducible, selective and complete as determined by a purified m-AST. The precision of this method was as good as that of the manual method (CV 2.04%). The present method and the manual method gave approximately equal results for m-AST (r = 0.987). The effects of activations on m- and c-AST activity were compared by the addition of pyridoxal 5'-phosphate to sera of various diseases. A higher activation ratio by pyridoxal 5'-phosphate was observed on both aspartate aminotransferase activities in the serum of patients with ischemic heart diseases than in the serum of patients with liver diseases.
