[A case of sitosterolemia with spastic gait due to intradural-extramedullary xanthoma].

A forty-eight-year-old woman with a 30 years history of tendinous xanthomatosis developed spastic gait and mild urinary incontinence. Myelography, metrizamide-enhanced CT and magnetic resonance image revealed multiple intradural-extramedullary tumors extending from C1 to Th7. The gait disturbance entirely disappeared by the removal of the tumors. Morphological examination of the tumors showed xanthogranulomatosis. The analysis of her sera and tumors revealed increased amount of plant sterols, especially beta-sitosterol and the diagnosis was made as beta-sitosterolemia. Tendinous xanthoma and beta-sitosterolemia were seen in 2 of 6 her brothers but none showed neurological symptoms. This is the first case of sitosterolemia with neurological complication i.e., spinal cord compression due to extramedullary sitosterolemia xanthomas.

Rotation-advancement flap method for correction of partial anomalous pulmonary venous drainage into the superior vena cava.

Three patients with partial anomalous pulmonary venous drainage into the superior vena cava underwent repair by a rotation-advancement flap method. The technique consisted of atrial partitioning, enlargement of the superior vena cava, and protection of sinus node function. Follow-up studies of all patients were done between 12 and 15 months after the operation. The superior vena cava was not stenosed and its diameter was normal, as demonstrated by cavograms. Pulmonary venous return appeared normal on angiograms, and sinus node function was normal by electrophysiologic studies.

[Hamilton's operation in Bland-White-Garland syndrome].

A 14-year-old boy, who had been treated medically for mitral regurgitation, underwent cardiac catheterization which revealed not only mitral regurgitation but also anomalous origin of the left coronary artery from the pulmonary artery. Thus he was diagnosed as Bland-White-Garland syndrome which was repaired by Hamilton's operation. Under cardiopulmonary bypass, the pulmonary trunk was opened longitudinally. After aorto-pulmonary window (A-P window) was created, a heterogenous pericardial baffle was attached to the posterior wall of the pulmonary trunk connecting A-P window and the ostium of left coronary artery. Thereafter the pulmonary trunk was enlarged with a patch of the same material. He did well postoperatively. Postoperative angiogram demonstrated satisfactory reconstruction of two-coronary-artery system.

[Role of HPLC in separation analysis in laboratory medicine].

The principle, theory and analytical application of high performance liquid chromatography (HPLC) in laboratory medicine was described. The importance of separation analysis was focussed with respect to accuracy, precision, short-time assay and easy operation. The pretreatment of samples such as deproteinization was also important for the accurate results and the analysis of plasma catecholamines and hemoglobin A1c were described as examples with use of column switching technique, step-wise separation, gradient elution method on TSK gels. We demonstrated the results of plasma free amino acids, gamma-carboxyglutamic acid and catecholamines in the aged which had been engaged in our laboratory as routine test and application with use of HPLC connected to chemiluminescent analyzer. Catecholamine analysis showed the importance of reproducibility and recovery. The reproducibility was within 2% as CV and 93-96% as recovery %. In the case of plasma free amino acid analysis, ASP, GLU, ALA showed decreases in the aged, but CYS did increase with age. Concerning about the drug monitoring of cyclospolin A in the infant whole blood after liver transplantation, the serum levels of cyclospolin A showed the lower concentration than whole blood. The extraction of cyclospolin A was required after deproteinization of whole blood as pretreatment.

Prostaglandin levels in the rat resting gastric wall and enhancement of prostaglandin E2 generation after administration of mild hyperosmotic saline solution into the gastric lumen.

The PGE2 level in the rat resting stomach, dropped into liquid nitrogen after removal, was 44.6 +/- 8.3 ng/stomach, when determined by radioimmunoassay, but cutting of the greater curvature with scissors after removal caused an approximately sixfold increase in the PGE2 level. Compression of the stomach in situ with metal tongs refrigerated in liquid nitrogen markedly reduced the PG levels, and further purification by reversed-phase high performance liquid chromatography reduced the PGE2 and 6-keto-PGF1 alpha levels to 4.7 +/- 0.6 and 4.8 +/- 0.8 ng/stomach, respectively. Pretreatment of the rats with indomethacin (10 mg/kg, i.v.) did not decrease the PGE2 level, but the 6-keto-PGF1 alpha level was reduced slightly. The lowest level of PGE2 in the resting state was significantly increased after administration of 1.0 M NaCl solution into the gastric lumen. The peak value was 70.0 +/- 21.8 ng/stomach at 30 min, after which the level decreased gradually. The level of 6-keto-PGF1 alpha was not significantly increased during the administration of 1.0 M NaCl solution.

[Effects of acid secretagogues on rat gastric mucus glycoprotein content and gastric mucosal resistance].

In our previous reports, low dose acid secretagogues caused increase in corpus mucus glycoprotein content in rats. This increase was not due to the direct action of protons. The present study was conducted to determine whether this increase has a mucosal function or not Rats were given histamine (0.8 mg/kg) intraperitoneally or tetragastrin (12 micrograms/kg) subcutaneously. One hour following the acid secretagogues administration, the animals were given 1 ml of 30, 40 or 50% ethanol orally and then sacrificed after one hour. Gastric mucus glycoproteins were isolated from the corpus region of stomach. The following results were obtained: 1) Histamine prevented the gastric mucosal damage and the decrease of corpus mucus glycoprotein content induced by 40% ethanol ingestion. 2) Tetragastrin prevented the gastric mucosal damage induced by 30 approximately 50% ethanol ingestion and the decrease of corpus mucus glycoprotein content induced by 40 approximately 50% ethanol ingestion. Based on these results, the increase of corpus mucus glycoprotein content induced by acid secretagogues has a mucosal function. Moreover, tetragastrin strengthened the mucosal defence together with acid secretion.

Clinical significance of mucin-like high molecular weight glycoprotein originated from lung cancer as tumor marker.

A high molecular weight, mucous glycoprotein (MG) from the pleural fluid of lung adenocarcinoma was purified by the DEAE-cellulose, gel-filtration and wheat germ agglutinin affinity chromatography. Protein portion of the molecule was composed of amino acids rich in serine, threonine and proline, but methionine and tyrosine concentrations were relatively low. About 65% of the weight, was composed of galactose, galactosamine, glucosamine, fucose and sialic acid. The gel-filtration pattern on Sepharose 4B revealed Mr greater than 10(6) Da. The SDS-PAGE pattern revealed a main band at the position of the Mr about 350 kDa under the reducing condition. Rabbit antibody against this molecule recognized mainly the peptide portion, and the radioimmunoassay (RIA) using the double antibody method was developed by this antibody. Serum MG level was low in healthy subjects and in benign diseases (0.8 +/- 0.7 U/ml; mean +/- SD and 1.1 +/- 2.3 U/ml, respectively). Thus, 3 U/ml was used as the cut-off value. The mean of serum MG levels and positive rates in malignant diseases were significantly high; 4.4 U/ml and 32.3% in lung cancer, 20.1 U/ml and 77.5% in pancreas cancer 11.6 U/ml and 64.3% in gastric cancer, 12.9 U/ml and 57.1% in hepatoma, 12.3 U/ml and 77.8 in colon cancer. Other malignancies such as ovarial and uterus cancer showed also high levels. Elevated values in these malignancies were observed frequently in patients with metastasis. On the other hand, the false positive cases were found in 10% of benign diseases. Determination of MG seems to be useful for the detection of several kinds of malignancies, but it is not adequately sensitive as a screening method for early cancer detection.

Homozygous variant of antithrombin III that lacks affinity for heparin, AT III Kumamoto.

Abnormal antithrombin III (AT III) was found in the plasma of a 31-year-old female who suffered from recurrent thrombotic episodes. Heparin cofactor activity was 28% of normal and undetectable when measured by inhibition of thrombin and factor Xa (F.Xa), while both progressive antithrombin and antifactor Xa activities were normal. The concentration of plasma AT III antigen was 37 mg/dl. Analysis by crossed-immunoelectrophoresis (CIE) in the presence of heparin and affinity chromatography on heparin-Sepharose revealed that the propositus' AT III did not bind to heparin. When heparin cofactor II (HC II) was removed from propositus' plasma, heparin cofactor activity of AT III was not detected. Thus, HC II seemed to account for the plasma heparin cofactor activity found in the presence of thrombin. The patient's parents and three of her brothers demonstrated qualitative abnormality of AT III; heparin cofactor activity was 30-50% of normal levels in the presence of both thrombin and F.Xa. These findings indicate that the propositus' AT III lacks affinity for heparin and the mode of its inheritance seems to be autosomal dominant and, hence, the propositus would be a homozygote. For this variant, the name of AT III Kumamoto is proposed.

A case of benign cementoblastoma.

A rare case of benign cementoblastoma is reported. Active cementoblasts adding cementoid tissue were observed histologically by means of specific staining for unmineralized matrix of hard tissue. A polarizing microscope and an x-ray diffractometer were used to clarify the cementum-like tissue of benign cementoblastoma. The collagen bundles of tumor tissue showed irregular and random arrangements under polarized light. The qualitative analysis revealed that the mineralized component was composed of low-crystalline hydroxylapatite.

Pituitary adenoma with multiple ciliated cysts: transitional cell tumor?

The case of a prolactin-secreting pituitary tumor with multiple cyst formation is described. Ultrastructurally, the cyst-lining cells were ciliated and closely resembled those of Rathke's cleft cyst but contained secretory granules, and no basal lamina formation was seen between the adenoma cells and the lining cells. Immunohistochemical study revealed that the adenoma cells consisted of both prolactin-secreting cells and growth hormone-secreting cells. The lining cells were immunoreactive with the antiserum to cytokeratin. No S-100 protein-positive cells were seen. The origin of this tumor is discussed.

Primary extranodal non-Hodgkin's lymphoma of the oral region.

Seventy cases (47M, 23F) of primary extranodal non-Hodgkin's lymphoma of the oral region were studied to determine tumor characteristics. The most frequent disease sites were the palate (21 cases), gingiva (17 cases) and parotid gland (13 cases). Each lymphoma was classified according to the criteria of the Working Formulation for Clinical Usage. Only 5.7% of cases were follicular lymphomas while diffuse lymphomas had a high incidence. Histologic subtypes included small lymphocytic (1%), small cleaved cell (7%), mixed small and large cell (20%), large cell (43%), large cell, immunoblastic (17%), lymphoblastic (9%) and small non-cleaved cell lymphoma (3%). Immunologic study utilizing the avidin-biotinylated horseradish peroxidase complex (ABC) technique demonstrated the presence of intracytoplasmic monoclonal immunoglobulin in 24 (34%) of the suggested B-cell lymphoma cases; 20 tumors (28%) were classified as T-cell lymphoma based on a positive reaction for mouse monoclonal antibody (UCHL-1) to T-cell related membrane antigen; 11 tumors (16%) contained intracytoplasmic alpha 1-antitrypsin, suggesting true histiocytic lymphoma; 15 tumors (22%) did not contain immunoglobulin, UCHL-1 or alpha 1-antitrypsin positive cells and showed no definite characteristics.

Differential lectin-bindings in normal and precancerous epithelium and squamous cell carcinoma of the oral mucosa.

In order to establish a useful and objective marker of malignancy of oral mucosa, the binding sites for Ulex europaeus agglutinin I (UEA-I). Bandeiraea simplicifolia agglutinin I (BSA-I) and peanut agglutinin (PNA) were comparatively examined in the surgical materials from the normal, dysplastic and cancerous epithelium of the oral mucosa by a novel lectin-antilectin immunoperoxidase method. Based on the staining patterns of the normal keratinized epithelium, UEA-I was regarded as the marker for the prickle cells, BSA-I for the cells in the upper prickle to the horny layers, and PNA for those in the basal layer. As the degree of dysplasia advanced, all layers of epithelium came to react with UEA-I and PNA, whereas the BSA-I binding was negative. Positive reactions for UEA-I and PNA were seen in most carcinoma cells other than the keratinizing foci stained by BSA-I. The results indicate that a UEA-I-positive reaction in the basal cells, a PNA-positive in the prickle cells and loss of receptor for BSA-I occur in the course of malignant transformation of oral mucosa, and that these lectins may be regarded as useful markers of oral epithelial cytoplasmic differentiation.

Characterization of the fibrinolytic state by measuring stable cross-linked fibrin degradation products in disseminated intravascular coagulation associated with acute promyelocytic leukemia.

Plasma samples from patients with disseminated intravascular coagulation (DIC) associated with acute promyelocytic leukemia (APL) exhibited higher levels of the D-fragment of fibrin and fibrinogen degradation products [FDP(D)], with relatively lower levels of cross-linked fibrin degradation products (XDP), than samples of DIC with non-APL. The difference between FDP(D) and XDP levels increased only when alpha 2-plasmin inhibitor (alpha 2-PI) fell below 60% of the normal level in APL patients. These findings suggest that fibrinogenolysis occurs in APL patients when the alpha 2-PI level has decreased significantly.

Arterial infusion of combination therapy using dibutyryl adenosine 3',5'-monophosphate and mitomycin C for hepatocellular carcinoma occluding main portal vein: case studies.

Intraarterial infusion therapy combining dibutyryl adenosine 3',5'-monophosphate and mitomycin C was administered to 8 patients with hepatocellular carcinoma occluding the main portal vein. Three patients survived more than nine months, including 1 who has been well for forty-eight months with technical imaging showing complete regression. A decreased level of serum alpha-fetoprotein of more than 90% was obtained in 38% of patients. Causes of death included hepatic failure, rupture of esophageal varices, acute gastric ulcer, and cerebral hemorrhage. It is concluded that the combination therapy may be useful for HCC.

[In vitro replication of plasmid pKYM].

The crude extract (fraction II) prepared from E. coli could replicate plasmid pKYM, only when the extract contained the rep protein which was produced by the plasmid and essential for its multiplication in vivo. The intermediate of replication was accumulated when a dideoxynucleotide triphosphate was added to the reaction mixture. By analyzing the intermediate, the initiation site of the deoxyribonucleic acid replication and the direction of replication could be determined. The replication initiated inside the ori region of pKYM and the direction was unidirectional. The analysis also suggested that the synthesis of the lagging strand stopped at almost the same site as the initiation site.

Immunohistochemical and ultrastructural study of hyaline bodies in odontogenic cysts.

Hyaline bodies in odontogenic cysts were examined by using histochemical and immunohistochemical techniques and transmission electron microscopy in order to study their origin. These methods revealed no evidence of keratinization, erythrocytes, and endothelial cells in the hyaline bodies. Ultrastructurally, epidermoid cells were attached to the hyaline bodies by hemidesmosomes and induced numerous vesicles in their cytoplasm. Electron probe X-ray microanalysis demonstrated a resemblance between components of the vesicles and that of the hyaline bodies. This close relationship of both structures supports the hypothesis that hyaline bodies are a secretory product of odontogenic epithelium.

[Bactericidal action of lomefloxacin a new pyridonecarboxylic acid derivative].

Lomefloxacin (NY-198) [(+/-)-1-ethyl-6,8-difluoro-1,4-dihydro-7-(3-methyl-1-piperazinyl)-4-oxo -3- quinolinecarboxylic acid hydrochloride] strongly inhibited the growths of not only Gram-negative Escherichia coli but Gram-positive Staphylococcus aureus. In vivo and in vitro experiments showed deoxyribonucleic acid (DNA) synthesis was specifically inhibited by this drug in E. coli.
