The hypertrophic uncinate process of the pancreas wrapping the superior mesenteric vein and artery was discovered during surgery on a patient with nodular chronic pancreatitis. Such a rare anomaly has never been noted in the English literature so far as we have been able to find. This malformation of the pancreatic uncinate process was considered to be due to excess fusion between the ventral and dorsal pancreatic anlages during embryonic stage.
A total number of 1105 cases with viral conjunctivitis, mainly epidemic kerato-conjunctivitis (EKC) consisting of 354 cases from Sapporo, Japan, of 628 from Kaohsiung, Taiwan and of 123 from Busan, Korea, encountered during the periods, 1980-81 and/or 1983, were studied aetiologically and epidemiologically. Patients were aged from 27 days to 88 years with a peak in the 20-29 year age group. Aetiological diagnoses were established in 610 cases (55%), consisting of 536 cases (49%) caused by adenoviruses and of 74 (7%) by EV 70. The most frequently detected agent was Ad 8 (57%), followed by EV 70 (12%), Ad 3 (9%) and Ad 19 (7%). The aetiological profiles of viral conjunctivitis were generally similar in three cities of East Asia. EKC (70%) was mainly caused by Ad 8, Ad 19 and Ad 37, but AHC (13%) by EV 70 and PCF (5%) by Ad 3 and Ad 11. Ad 19 and Ad 37 isolates in three cities were compared with the cleavage patterns with restriction endonucleases, BamHl, Smal, Sall and Hindlll. All of the Ad 19 isolates tested were identical to Ad 19a reported by Wadell et al, and all but three isolates of Ad 37 were identical to the Ad 37 prototype prevalent in Europe and the US. The three isolates of Ad 37, different from the prototype in cleavage pattern with Hindlll, designated as Ad 37A, were detected in Sapporo and Kaohsiung in 1980. From the cleavage patterns with four restriction endonucleases, Hindlll, BamHl, Sa1l and Sstl, the Ad 8 isolates in 1983 were divided into three subtypes, which were associated with the cities isolated.(ABSTRACT TRUNCATED AT 250 WORDS)
A rare instance of subperiosteal hemorrhage of the orbital roof with satisfactory operative results is described here. Two-dimensional computed tomography was of great value in the preoperative evaluation of the impaired upward gaze and diplopia.

A 29-yr-old man with neurological symptoms that included tremor, dysarthria, and loss of consciousness was diagnosed as having variegate porphyria, because of increased urinary excretion of delta-aminolevulinic acid, porphobilinogen, uroporphyrin, and coproporphyrin, and increased fecal uro-, copro-, and protoporphyrins. He showed marked retention of indocyanine green (R15 min: 53%-78%) and bromosulfophthalein (R45 min: 24%-10%) as well. The kinetic analysis revealed that both hepatic uptake and hepatic excretion were decreased for both dyes. Results of other liver function tests were unremarkable except for intermittent hyperbilirubinemia (0.7-3.3 mg/100 ml), of which the ratio of conjugated and unconjugated fraction was between 0.7 and 1.2. Binding of serum protein and indocyanine green or bromosulfophthalein was normal. An angiographic and scintigraphic study of the liver was normal. A minimal deposition of hemosiderin pigment was found in liver biopsy specimen without other abnormalities. Deposition of porphyrin was not present in the liver. Plasma exchange and erythrocytapheresis did not show any beneficial effects on the clinical and laboratory abnormalities of the patient. A possible relationship between variegate porphyria and defective hepatic anion transport is discussed.

Horizontal excision and closure is not usually recommended for lesions of the eyelids. We have used the technique for 35 benign lesions in 33 patients over the last 3 years with satisfactory results. This method is a simple one that can provide excellent cosmetic results in properly selected patients. Six representative cases are described in detail and the limitations of and indications for horizontal closure are discussed.
Blood urea nitrogen (BUN/creatinine ratio was abnormally high (24.8 +/- 0.6) in untreated hyperthyroid patients due to both increase in BUN and decrease in creatinine concentration. BUN, creatinine and BUN/creatinine ratio were all completely normalized after restoration of euthyroid status. On the other hand, BUN/creatinine ratio was slightly suppressed in hypothyroidism before treatment and it was reversed by thyroxine treatment (12.6 +/- 4.0 and 16.3 +/- 3.3, before and after treatment, respectively). An age-related increase in BUN/creatinine ratio, which was primarily due to an age-related increase in BUN, was also found in hyperthyroid subjects (21.9 +/- 2.8 vs 27.7 +/- 9.0; first vs fifth decade) and in normal controls (13.7 +/- 2.8 vs 16.0 +/- 2.9; first vs fifth decade). To elucidate reasons for abnormal increase in BUN/creatinine ratio in hyperthyroidism, measurement of cardiac output and kinetic analysis on urea nitrogen (UN) and creatinine were performed. The results indicated a marked increase in cardiac output. Serum creatine concentration was clearly increased in hyperthyroid patients. Thus, serum creatinine concentration was suppressed due to a decrease in creatinine synthesis and an increase in renal creatinine excretion. BUN was high, primarily due to an increase in UN production secondary to excessive protein catabolism together with insufficient excretion of UN.
A girl manifesting a false median cleft lip associated with hypertelorism, anophthalmos, hydranencephaly with holoprosencephaly, and osteogenesis imperfecta, among other disorders, survived and was fed mainly milk products up to the age of 11. Then, surgical closure of the false median cleft lip was undertaken to improve the face aesthetically and to make the child more acceptable to society at large. The patient's median cerebrofacial dysgenesis with other anomalies was unique, compared with disorders reported in the literature. The embryological distinction between a false and true median cleft lip is also discussed.
A case of chondroid syringoma in the palm, probably the fourth to be reported, is presented. There was a large cystic cavity in the center of the tumor. The peculiar findings of hyaline, myxoid, or chondroid degeneration of the abundant stroma were observed histologically. This tumor should be considered in the differential diagnosis of painless, slow-growing palm tumors.
In order to evaluate the pathogenesis of the systemic hyperdynamic circulation in portal hypertension, serum concentration of eight kinds of hormones including glucagon (Glu), aldosterone (Ald), renin (Ren), and epinephrine (Adr), and the hemodynamic parameters were measured in a series of 30 patients, of whom 23 were patients with liver cirrhosis, 3 were with Banti's disease, 2 with chronic active hepatitis, and 2 with pre-cirrhotic change. The average cardiac index was 4.6l/min, m2, with normal PCWP of 6.7 mmHg. CI. and SVR. showed significant inverse correlation of r = -0.767 (p less than 0.01), however, PCWP and CI did not have any significant correlation. Average serum concentrations of Glu, Ald, and Ren were 160 pg/ml, 139 pg/ml, and 5.4 ng/ml, respectively, all of which were increased up to 2.5 times above the normal values. Adr, norepinephrine, cortisol, estrone and estradiol were within normal limits. Of the eight hormones being measured, only Glu had significant correlation with both liver function tests and the cardiac index (r = 0.479, p less than 0.05). Neither Ald nor Adr had significant correlation with hemodynamic parameters.
The authors have carried out six free omental transfers for the treatment of two chronic ulcers, two progressive hemifacial atrophies, one precancerous disorder and one contused wound. The follow-up periods ranged from 3 years 9 months to 7 years 7 months. From our experience the transferred omentum maintained its volume and nature under normal circumstances. The decrease in volume or atrophy of the transferred omentum may be caused by panniculitis of the omental fatty tissue or narrowing of the anastomosed vessels.
An extensor carpi radialis longus (ECRL) myocutaneous flap was successfully used for the repair of a posterior defect of the elbow joint in a physically handicapped female with Parkinsonism. This muscle is of value for a one-stage reconstruction of an elbow lesion.
An island groin flap was used for satisfactory reconstruction of defects in the anterior thigh, lower abdomen, or perineal regions in 9 patients. This flap has several advantages: simple operation, wide range of coverage, large rotation arc, direct closure of the donor site, and no sacrifice of muscle. Comparison with tensor fascia lata and other musculocutaneous flaps is made.
