A case of early gastric cancer of the small cell type is presented. The small cell carcinoma was situated in the antrum and arose in association with well-differentiated adenocarcinoma. Well-differentiated adenocarcinomas were limited to the mucosa, whereas small cell carcinoma infiltrated into the submucosa with evident vascular invasion. Small cell carcinoma revealed argyrophilia and focal squamous cell differentiation. The patient died of massive hepatic metastasis of small cell carcinoma 6 months later. We will describe clinicopathological features of our case and review the literature.
This is a case report presenting interesting findings on two-dimensional and Doppler echocardiograms due to multiple pulmonary thromboses. A 67-year-old woman had a history of surgery for colon polyps in August, 1986. After surgery, she suffered from tachycardia and dyspnea, and was admitted to our hospital because of a loss of consciousness. Sinus tachycardia, complete right bundle branch block and T-wave inversion in leads III, aVF and V1-4 were observed on her electrocardiogram. Cardiomegaly was noted on her chest radiograph. On the third hospital day, echocardiography was performed. On two-dimensional echocardiography, marked right ventricular dilatation with hypokinesis of the wall, and a flattened interventricular septum were observed on the short-axis view, and M-mode echocardiograms showed disappearance of the E wave and delay of the opening of the tricuspid valve, resulting in a monophasic triangular pattern. Doppler echocardiography showed the tricuspid valve flow to be delayed, mainly observed during atrial systole. These findings indicate decreased right ventricular compliance. The patient died on the 6th hospital day, and multiple new and old thrombi were found in the pulmonary artery at autopsy.

Ultrastructural localization of immunoreactive corticotropin-releasing factor (CRF) was visualized for the first time in the human hypothalamus and pituitary gland with specific antibodies against human/rat CRF. In the hypothalamus most of the positive immunoreactivity to CRF was present in granules with a wide range of diameters, 50-250 nm, in the perikarya of parvocellular neurons in the paraventricular nucleus. Among these, neurosecretory type granules, 100-150 nm in diameter, were dominant, and small vesicles, 50-80 nm in diameter, were sparse. Some of surfaces of rough endoplasmic reticulum and polyribosomes were also positive in some of these cells. CRF-positive reactions were also observed in the nerve fibers of the pituitary stalk and the posterior pituitary gland revealing two types of granules: small vesicles, 50-80 nm in diameter, and neurosecretory granules, 100-150 nm in diameter. These results support the theory that the human CRF, which is identical to rat CRF, is synthesized in parvocellular neurons of paraventricular nucleus, transported in nerve fibers, and controls ACTH secretion in the human anterior lobe of pituitary gland via the portal system.
Hypoxia is suggested to increase intracellular Ca2+, thereby affecting cellular functions. If this is the case, the effect would be modified by other treatments which also elevate intracellular levels of Ca2+. To test this possibility, in mouse diaphragm preparations, the effects of hypoxia were examined on twitch potentiations after application of neural conditioning pulses which are considered to elevate intracellular Ca2+. The effects were compared with those of 2,4-dinitrophenol (DNP) and ouabain. Hypoxia increased the tension of twitches elicited directly or neurally with 0.1 or 0.5 Hz pulses at 36 degrees C but not at 24 degrees C. Paired pulses to the nerve induced a complex response profile in which the amplitude of the second twitch was enhanced (twitch facilitation). Tetanic pulses (50 Hz) to the nerve were followed by post-tetanic twitches of increased amplitude (post-tetanic twitch potentiation, PTP). Hypoxia little affected the twitch facilitation but abolished PTP at both temperatures. These effects differed from those of DNP and ouabain in some ways. Thus, present experiments indicate that hypoxia selectively influences the process which is responsible for the PTP phenomenon rather than for the twitch facilitation. It is possible that the mechanism by which hypoxia would accumulate intracellular Ca2+ may be included in the process through which PTP occurs.

Cultured hepatocytes isolated from livers of 17 alpha-ethynylestradiol-treated rats were used to investigate the change of lipid metabolism induced by administration of 17 alpha-ethynylestradiol. Treatment with 17 alpha-ethynylestradiol caused a decrease of rat plasma lipids (free cholesterol, cholesterol ester, triacylglycerol and phosphatidylcholine). No difference in the ability of urea nitrogen synthesis could be demonstrated between cultured hepatocytes isolated from livers of 17 alpha-ethynylestradiol-treated rats and propylene glycol-treated rats (control). Total cholesterol and cholesterol ester contents of cultured hepatocytes isolated from livers of 17 alpha-ethynylestradiol-treated rats were increased in comparison with those of the control. Triacylglycerol content of cultured hepatocytes was not affected by 17 alpha-ethynylestradiol treatment. There was no difference in the composition of lipid content between liver tissues and cultured hepatocytes. These results suggest that hepatocytes isolated from livers maintain the character of livers treated with 17 alpha-ethynylestradiol or livers treated with propylene glycol. Free cholesterol and cholesterol ester synthesis from [14C]acetic acid by cultured hepatocytes isolated from livers of 17 alpha-ethynylestradiol-treated rats were decreased to about 30% of the control. Triacylglycerol and polar lipid (phospholipid) synthesis from [14C]acetic acid were not affected by 17 alpha-ethynylestradiol treatment. Microsomal hydroxymethylglutaryl-CoA reductase activity of rat liver treated with 17 alpha-ethynylestradiol was decreased to about 50% of control. The secretions of free cholesterol, cholesterol ester, triacylglycerol, phosphatidylcholine, apolipoprotein BL and BS by cultured hepatocytes isolated from livers of 17 alpha-ethynylestradiol treated rats were not decreased when compared with the control. Because lipid and apolipoprotein secretions from cultured hepatocytes treated with 17 alpha-ethynylestradiol were not decreased and cholesterol contents of liver tissues and cultured hepatocytes treated with 17 alpha-ethynylestradiol were increased and hepatic microsomal hydroxymethylglutaryl-CoA reductase activity was decreased by 17 alpha-ethynylestradiol treatment, it is suggested that the liver plays an important role in hypolipidemia induced by 17 alpha-ethynylestradiol by increasing the plasma lipid uptake mediated by an increased amount of lipoprotein receptors of liver membranes.
An interesting case of right and left coronary arteries-right atrial fistulas diagnosed by Doppler echocardiography was presented. A 59-year-old woman was referred for evaluation of her continuous murmur. A thrill was palpable at the left sternal border in the third intercostal space. The proximal portions of the dilated right and left coronary arteries and distal portions of the tortuous and converging fistulas of both coronary arteries were imaged by two-dimensional echocardiography. Bidirectional continuous turbulent Doppler signals were detected in the proximal portions of the dilated right and left coronary arteries, in the distal portions of the fistulas around the crux and in the right atrium. These findings facilitated our diagnosis of right and left coronary arteries-right atrial fistulas. The flow velocity at the ostium of the right coronary artery was highest and nearly the same as the flow velocity (about 2 m/s) obtained by continuous wave Doppler from the maximum point of the thrill. The maximum pressure difference was considered located at this portion. Selective coronary angiography confirmed the right coronary artery and left main trunk-left circumflex coronary artery to be large, elongated and tortuous. These fistulas were communicating with the right atrium. This case demonstrates the usefulness of Doppler echocardiography in the noninvasive diagnosis of coronary arteriovenous fistula.

PiM subtypes were determined by isoelectric focusing of the sera of 746 Japanese at the age of 16-60 years. The gene frequencies were calculated: PiM1 = 0.7855, PiM2 = 0.1528 and PiM3 = 0.0617. The serum concentration of alpha-1-antitrypsin was measured by laser-nephelometric immunoassay of the sera of 284 individuals. A statistically significant difference (p less than 0.01) in the serum concentration was found between M1 (2.63 +/- 0.67 g/l) and M1M2 (2.39 +/- 0.59 g/l).
Four out of approximately 36,000 specimens, examined by immunoelectrophoresis, showed no precipitin line of alpha-2-macroglobulin (alpha 2M) with anti-whole human serum and antiserum to alpha 2M. Serum alpha 2M levels were below 10 mg/100 ml with single radial immunodiffusion. These cases were diagnosed as having prostate cancer based on prostate biopsy. After estrogen or anti-androgen therapy, the initially severe deficiency of alpha 2M changed toward normal limits. Phytohemagglutinin (PHA)-induced lymphocytes from patients with severe alpha 2M deficiency revealed normal response. Sera from patients with alpha 2M deficiency and healthy subjects (serum alpha 2M concentration of 8-208 mg/100 ml) did not suppress lymphocyte response to PHA, while purified alpha 2M from fetal cord and healthy sera, and serum from a patient with advanced urinary cancer (serum alpha 2M concentration above 360 mg/100 ml) to PHA-induced lymphocytes demonstrated a dose-dependent suppression. Thus, these results indicate that alpha 2M in elevated concentration results in decreased PHA-induced lymphocyte response and suggest that alpha 2M may have, principally suppressive, immunoregulatory properties in vivo.

Cases of increased OKT 8-positive T cells were found among patients with multiple myeloma, and the suppressor function of these fractions was studied. The suppressor activity of OKT 8 cells with myeloma was not increased in the Igs synthesis system. Therefore it was suggested that the increase of OKT 8 cells was not correlated to the activity of suppressor T cell function.
