[New results concerning the effectiveness of therapy of chronic glomerulonephritis. Results of cytostatics-anticoagulants-aggregation inhibitors, cystostatics-anticoagulants-aggregation inhibitors-prednisone and cystostatics-prednisone therapy, derived from a retrospective study].

The indication and effectiveness of the immunosuppressive therapy of chronic glomerulonephritis is now as ever controversial. As a result of a retrospective therapy study in an at least one-year therapy with cytostatics, thrombocyte aggregation inhibitors and anticoagulants (CAA-therapy, n = 27), additional prednisone therapy (CAAP-therapy, n = 95) and cytostatic-prednisone therapy (CP-therapy, n = 33) we find favourable results for the CAA/CAAP-therapy in the nephrotic syndrome and glomerulonephritides with distinct clinical exacerbation also in sclerosations in histology. Clinically inactive nephritides with exclusive urinary findings and sclerosations cannot be influenced therapeutically.

[Biopsy findings in kidney transplants after treatment with cyclosporin A].

51 renal transplant biopsies of 37 patients immunosuppressed with Cyclosporin A were histologically reevaluated. Lesions related as well as unrelated to Cyclosporin A treatment were found. Cyclosporin A related findings corresponding to the so-called Cyclosporin A nephropathy were divided into early postoperative (diffuse fibrosis), acute (toxic tubulopathy and others) and chronic changes (arteriolopathy, striped fibrosis with tubular atrophy). Since there is often a combination of Cyclosporin A related and unrelated lesions, it is necessary to define the main lesion in the individual case. Accordingly, the following diagnostic groups were present in the biopsies: Cyclosporin A nephropathy (19 cases), rejection nephropathy (18), rejection and Cyclosporin A nephropathy (9), acute renal failure (3) and other diagnoses (2 cases). For the further management of the patients, the exact diagnosis of Cyclosporin A related and unrelated pathology is important in renal biopsies.

[Incidence of glomerular diseases in kidney biopsy materials using WHO classification].

A total of 1,308 renal biopsies performed during the years 1978 to 1983 in Moscow (M, 852 cases) and Rostock (R, 456 cases) showed that 685 (80.4%) and 346 (75.9%), in the two cities respectively, had glomerular disorders when classified according to WHO nosology (Churg and Sobin 1982). Among the 1,031 glomerulopathies from the two cities there were 121 with minor glomerular abnormalities (M: 49 = 7.1%; R: 72 = 20.8%), 145 with focal segmental glomerulopathies (M: 76 = 11.1%; R: 69 = 20.0%), and 765 with diffuse glomerulopathies (M: 560 = 81.6%; R: 205 = 59.3%). The 765 diffuse glomerulopathies from the two series were distributed according to the WHO classification into 77 membranous (M: 63 = 9.25%; R: 14 = 4.1%), 519 mesangioproliferative (M: 380 = 55.5%; R: 139 = 40.2%), 11 endocapillary proliferative (M: 5, R: 6), 99 mesangiocapillary (M: 65 = 9.5%; R: 34 = 10.4%), 2 dense deposit (in R), 4 extracapillary (M: 1, R: 3) and 53 sclerosing glomerulonephritides (M: 46 = 6.7%; R: 7 = 2.0%). Utilization of the WHO classification of glomerular diseases proved to be a useful compromise, allowing two different schools of nephropathology to agree on classification of renal biopsy material. The group of minimal change abnormalities and the focal segmental lesions should be subclassified further according to the clinical and morphologic findings.

Tubular function disturbances in chronic glomerulonephritis and their significance for identifying tubulointerstitial lesions.

The tubular and tubulointerstitial renal functions of 237 patients with chronic glomerulonephritis and functional compensation corresponding to their plasma creatinine levels were investigated and the results were compared with the light microscopic findings obtained by examination of kidney interstices obtained by biopsy. Pronounced structural interstitial lesions (TiC) were found in 30% of the cases. Investigation of the predictive values of tubular function data in respect of the presence or exclusion of TiC showed that, although individual parameters of the renal function permit the exclusion of TiC disturbances with a high degree of certainty, the diagnostic value can be enhanced by considering pairs of such parameters. Five parameter combinations were found to have the highest predictive value regarding the diagnosis of TiC. These were disturbed concentration capacity accompanied by reduced ammonia excretion or total acid excretion, reduced water diuresis accompanied by disturbed ammonia excretion or total acid elimination; and, finally, the total acid excretion and maximum dilution capacity. The highest predictive values for the exclusion of TiC are shown by inconspicuous concentration capacity accompanied by normal ammonia excretion, total acid excretion, water diuresis, free water clearance or urine dilution capacity.

[Pathohistology of allotransplanted kidneys. A 10-year report from Santiago, Cuba].

Of altogether 201 allotransplantations in Santiago de Cuba the material was histologically examined 328 times. Transplant biopsies and explants most frequently revealed a rejection nephropathy, in which case the forms of the prognostically unfavourable rejection vasculopapty were more frequent than the types of the tubulointerstitial rejection nephropath which are better to be influenced therapeutically. A histologically diagnosed acute failure of the kidney was casually based on (reversible) lesions of the donor kidney, infections and haemorrhages after the transplantation as well as on the combination with a rejection. A transplant pyelonephritis and transplant glomerulonephritis were rarely observed. Complete necroses of a transplant biopsy cylinder allow the urgent suspicion of an obliterating rejection vasculopathy with secondary transplant necroses.

[Heart wall rupture in acute myocardial infarct. An autopsy study in comparison with infarcts without rupture].

Among 237 patients who died of acute myocardial infarction (4% of the 5,390 autopsies from 1975 to 1979) 43 cases (18%) with a rupture of the cardiac wall were found. These 43 cases of rupture were compared with 43 non-rupture cases of the same period. In difference to the non-rupture cases the average weight of the heart was smaller. Myocardial scars, additional basic diseases and a lung oedema were rarer; an extended region of the infarction and an anamnestically known chronic ischemic heart disease were observed significantly more frequently. In what respect these and other analysed factors are responsible for the rupture mechanism, cannot be clearly estimated. Apart from a continuing hypertension as well as a relatively smaller weight of the heart and the absence of myocardial scars the extension of the area of the infarction as well as the possible effect of granulocytic enzymes in the area of the infarction appear significant for the origin of a heart rupture.

[Correspondence of functional and structural findings in tubulo-interstitial disorders in chronic glomerulonephritis].

In 237 patients with chronic glomerulonephritis and according to the serum creatinine level of functional compensation tubular and tubulointerstitial renal functions, respectively, were investigated and compared with the light-microscopic findings of the renal interstice. A distinct structural tubulointerstitial lesion (tiK) was found in 30%. The diagnostic predictivity for the recognition and the exclusion of the tiK was tested for single or paired data of the renal function. As a result it is possible with a suitable combination of parameters in on an average more than 80% of the cases to establish or to exclude function-diagnostically a tubulointerstitial structural lesion even in still inconspicuous creatinine values. From the clinical point of view thus the information value of functional diagnostic investigations of the renal water treatment is confirmed (concentration power, dilution ability, water diuresis), in which case the differentiated test of the renal acidification may supplement the informations about the tiK-situation.

Morphologic and functional tubular changes in chronic glomerulonephritis.

In 135 patients with different glomerular diseases but normal glomerular filtration rates (CCr and In = 103 +/- 2ml/min) correlations between morphological tubular lesions (tic) and changes in tubular function were investigated. Between the extent of tubular lesions and the impairment of specific tubular functions (formula; see text) exists a highly significant correlation. The combined diminution in Uosmmax and UNH4 + V reflects in 72 per cent the appearance of a severe tubular lesion and should be used for its clinical recognition.

Local BCG-immunotherapy in bladder cancer patients. Immunological follow-up studies.

In a controlled clinical study 10 patients with bladder cancer were treated with an additional local BCG-therapy. For this investigations only patients with tumour stage T1-2 NO MO were elected. The local BCG-therapy was done as intralesional BCG-injections, the interval between each management was 4 weeks over a period of 4 months to 1 year. Under this treatment in all 10 patients tumor regression in different forms was seen. In 5 patients the tumor was not more found, neither in clinical nor in histological examinations. Side effects of the therapy were: influenza-like syndrome (6 patients), mild temperature (6 patients), increase of liver transaminases (5 patients), and granulomas in lung and liver (2 patients). In the immunological follow-up studies we have seen an increase of the lymphocyte activity against tumor-associated antigen from bladder cancer tissue in the electrophoretic mobility test in 6 patients. The results in skin testing, using 5 recall-antigens and DNFB, showed an increase of reactivity in only 4 patients. The total number of lymphocytes increased over a longer period. We didn't find typical humoral changes. The results of our 10 patients show that an intralesional BCG-therapy could be successful in 5 patients. The effect of BCG for immunostimulation was seen in vitro in 6 patients and in vivo in 4 patients.

[The primary malignant non-Hodgkin lymphoma of the stomach].

From 1970 to 1980, 643 patients suffering from a malignancy of the stomach were seen at the Surgical Department of Rostock University. 8 of them were identified to have a Non-Hodgkin lymphoma. Treatment should be done depending on the staging: Stage I and II extirpation of the tumour followed by adjunctive chemotherapy, Stage III and IV primary chemotherapy only.

[Lymphogranulomatosis and heart involvement].

In the early extralymphatic affection of the organs without relevant clinical and roentgenological findings the diagnosis of the lymphogranulomatosis makes difficulties. It is referred to the peculiarities of the extra-nodal manifestation. The case of a lymphogranulomatosis with early participation of the heart is described and it is gone more fully into the differential diagnosis. Disturbances of the regression of excitation in the ECG are etiologically ambiguous and may be the first recognizable findings of a penetration of a tumour into the pericardium.

[Alcoholic liver damage in rats: histological and histochemical findings of an one year trial (author's transl)].

30 female Wistar rats on the normal pellet diet received ethyl alcohol in increasing concentrations (5-40%) ad libitum as the only fluid intake and were sacrificed after 6, 9 and 12 months together with control animals. Alcohol dehydrogenase activity of liver parenchyma was consistently reduced in the central parts of the lobuli; histological findings however were inconsistent; fatty degeneration, focal hepatitis, and necrosis of single hepatic cells could be found as well as normal livers in rats receiving alcohol for longer time periods. In primates sclerosis of central lobular veins occurring early in the process of alcoholic liver damage has been described previously; these changes were supposed to have diagnostic relevance in regard to progression of the liver disease; such sclerotic changes can be observed as well after 6 to 12 months intake of alcohol in rat livers in varying degrees of severity.

Scanning electron microscopy of the rat renal glomerulus.

Glomeruli of normal rat kidneys were studied by scanning electron microscope. A three-dimensional image is presented of the outer and the inner surface of the glomerular capillaries. The findings are compared with their correlates as seen in transmission electron microscopy. The possible application of scanning electron microscopy in the examination of renal disease is briefly discussed.

[Immunostaging of patients with chronic pancreatitis. I. A study of humoral and cell-mediated immunity (author's transl)].

Immunostaging was performed in patients with chronic pancreatitis of different etiologic groups. Immune responifveness was assessed by immunoglobulin serum levels, non specific autoantibody formation, C3 levels, antistreptolysin reaction, antibody titer against Toxoplasma gondii, E. coli, Herpes simplex-, Mumps- and Cytomegalovirus and screening for soluble immune-complexes (PEG precipitation assay), number of T- and B-lymphocytes, lymphocyte responsiveness to PHA, PPD, tetatoxoid and mumps-antigen in a whole blood assay, skin testing with candidin, trichophytin, tuberculin and streptokinase and HLA-typing. In some cases we found a depressed cell-mediated immune responsiveness. Patients were divided into two groups based on this main finding. Cellular hyporesponsiveness was correlated to the severity of the disease and a high incidence of HLA-B5 but not to any etiologic factor of the disease.

Allyl alcohol-induced changes in the rat exocrine pancreas.

A histologic, histochemical and electron microscopic study was carried out on the rat exocrine pancreas 6-96 h after a single allyl alcohol intake. Histologically, acidophilia, necrosis and vacuolation of the pancreatic acinar cells were observed. Histochemically, a low but evident alcohol dehydrogenase activity could focally be demonstrated in the pancreatic acinar cell cytoplasm of the experimental and control rats. By electron microscopy, cytoplasmic lipid droplets, mitochondrial degeneration and necrosis were found in the acinar cells. Focal cytoplasmic degradation and vacuolation were seen in acinar, centroacinar and ductular cells of the pancreas. The findings are regarded as the consequence of the toxic action or acrolein that originates after allyl alcohol oxidation by alcohol dehydrogenase. Analogies between the liver and the pancreas in their response to acute allyl alcohol and chronic ethanol intake are discussed.

[The three-dimensional ultrastructure of the pancreatic acinar cell (authors transl)].

The pancreatic acinar cells of normal rats were studied by scanning electron microscope. A three-dimensional image is presented of the structure and arrangement of the zymogen granules as well as of the apical microvilli and the zymogen discharge in the pancreatic acinar cell. The study of these structures in physiologic and pathologic conditions by scanning electron microscopy can be recommended to support the understanding of findings obtained by light and transmission electron microscopy.
