[Infectious causes of death in renal transplant patients. A clinical pathological autopsy study].

The infectious consequences of immune deficiency after kidney transplantation are reflected in the spectrum of the causes of deaths in a renal transplant population. From 1976 to 1991, 650 kidney transplantations were performed in Rostock. An autopsy was made in 92 of the altogether 106 deaths; 45 (49%) showed infectious and 47 (51%) non-infectious causes of death. A significant decrease of bacterial causes of death was found in the transplant group from 1985 to 1991 (3/25 = 12%) in comparison to the group from 1976 to 1985 (30/67 = 45%). The important increase of patient survival is attributed to the progress of transplantation management in general as well as to the striking decrease of lethal bacterial infections due to increasing ciclosporin application in renal transplantation.

Chronic cyclosporine-associated nephrotoxicity in bone marrow transplant patients.

This study describes the prevalence and degree of chronic cyclosporine-associated nephropathy and its risk factors. For this purpose we reviewed all available renal histology specimens in 169 bone marrow transplant recipients treated during an eight year period with cyclosporine for prevention of graft-versus-host-disease, and determined their pattern and degree of histomorphological changes. A total of 51 specimens obtained from 49 patients by biopsy (n = 12) or autopsy (n = 39) was evaluated. The pattern of histomorphological changes was compared with diagnosis, age, sex, and potential risk factors--such as cyclosporine dose, levels, duration of therapy, changes in serum creatinine and onset of hypertension. Morphological lesions of chronic cyclosporine-associated nephropathy were found in 67% of the specimens. They were more frequent and more severe with increasing duration of cyclosporine therapy, in patients with a higher increase in serum creatinine during the first 3 months and in patients given total-body irradiation for conditioning. These latter findings suggest that additional damage sensitizes the kidney to irreversible toxic effects of cyclosporine.

DNA cytometric and histologic findings in mouse tumors (BP and S 180) with different response to treatment with tumor necrosis factor.

DNA cytometric and histopathologic investigations were performed in two tumor models (BP and S 180) which differed in their sensitivity to tumor necrosis factor (TNF). TNF induced strong necrosis in both tumors, but only the sarcoma 180 showed total regression. After TNF administration DNA cytometry revealed in the BP tumor an increase of cells in the S-phase, and in the S 180 tumor a loss of aneuploid cell populations. Histologic examination revealed a more obvious effect of TNF on tumor blood vessels in BP tumor, whereas infiltration of inflammatory cells was observed only in the S 180 tumor. We concluded that cell infiltration may be of importance for tumor regression and that aneuploid cell populations are more sensitive to TNF treatment than eudiploid cells.

Cystic fibrosis associated islet changes may provide a basis for diabetes. An immunocytochemical and morphometrical study.

The pancreases of 23 patients (mean age 10.5 years, range 5-22) years dying of cystic fibrosis (CF) were evaluated at autopsy by routine histology and immunostaining for changes in their endocrine cell compartment. The severely altered pancreatic tissues showed end stage CF, with either a fibrotic pattern (CF-FIB, n = 14) or a lipoatrophic pattern (CF-LIP, n = 9) prevailing. In all specimens, irrespective of the dominating pattern, the islet system was affected by marked periinsular and intrainsular sclerosis. Quantitatively, the volume densities (relative tissue components) of the parenchymal, fibrotic, fatty and total endocrine compartments as well as the four islet cell types (B, A, D, PP) were determined by point counting. Compared with controls, the CF patients (including two patients with overt diabetes and glucose intolerance, respectively) had a significantly decreased insulin (B)-cell ratio (from 64.4 to 34%) with a concomitant rise in non-B-cells (A-cells: 23.2 to 35%; D-cells: 10.4 to 22%; PP-cells; 2 to 9%). Comparison of endocrine cell ratios in CF-FIB pancreases with CF-LIP pancreases revealed no significant differences. The reduction of approximately 50% of insulin cells in CF patients with advanced disease supports the concept that destruction of exocrine tissue with concomitant fibrous disorganization of islets gradually changes the proportional distribution of the endocrine cells in favor of the noninsulin cells. This slowly ongoing process probably provides the basis for islet dysfunction, i.e. diabetes, increasingly observed in final stage CF.

[Megakaryocytes and thrombocytopoiesis in idiopathic thrombocytopenic purpura].

In idiopathic thrombocytopenic purpura (ITP) patients with histological signs of stronger auto-antibody synthesis (spleen follicle hyperplasia) more frequently revealed megakaryocyte changes due to antibodies and less frequently thrombocyte production compensatory increased than ITP patients with signs of weaker auto-antibody synthesis (spleen follicle normoplasia). The observation suggest that insufficient and frequently lacking compensatory increase of thrombocyte production in ITP is caused by an autoallergic disturbance of megakaryocyte maturation.

[Long-term prognosis of the results of splenectomy in idiopathic thrombocytopenic purpura].

The value of selected clinical and histological parameters for the prognosis of long-term results of splenectomies was investigated retrospectively in 50 patients with idiopathic thrombocytopenic purpura 4-14 years after surgery. Two parameters showed an association to the results of splenectomy, viz. spleen follicle size as well as site of thrombocytopenia degradation. Non-responders to splenectomy (n = 7) only turned up in spleen follicle hyperplasia, never in follicle normoplasia, however (p less than 0.01), four times more frequently in mainly extralienal thrombocyte degradation, also in predominantly lienal ones (p greater than 0.05).

[Kidney transplant rejection and the original kidney disease of the recipient].

Glomerulonephritis, pyelonephritis, diabetic nephropathy, and polycystic kidney disease were the four most frequent original renal diseases leading to altogether 500 kidney transplantations in Rostock from 1976 to 1989. A total of 457 pathomorphological specimen (255 transplant biopsies, 159 explants, 43 postmortem grafts) of 348 renal transplants was histologically studied. The most striking findings were a more frequent hyperacute rejection and an earlier loss of explants as well as a higher incidence of relevant vascular rejection in graft biopsies of glomerulonephritis versus pyelonephritis patients. The obtained results suggest a possible disposition of recipients with glomerulonephritis to a more rapid and intense graft rejection in comparison to transplant patients with pyelonephritis as underlying renal disease.

[Pathology of the transplanted kidney and basic renal disease of the recipient].

Among 359 kidney transplantations performed from 1976 to 1986 at Rostock the 4 most frequent original renal diseases were glomerulonephritis (280 x), pyelonephritis (74 x), diabetic nephropathy (22 x) and polycystic kidney disease (14 x). From 225 transplant kidneys in 284 cases pathomorphological examinations were performed (104 biopsies, 138 removed grafts, 42 postmortem transplant examinations). The most striking finding was the earlier occurrence of rejection in grafts from recipients with glomerulonephritis compared with pyelonephritis. Therefore, a more rapid and intense immune reaction could suggested in patients with glomerulonephritis as original renal disease.

Prognostic significance of splenic follicle size in splenectomized idiopathic thrombocytopenic purpura patients.

The prognostic significance of splenic follicle (B-lymphocyte compartment) size was studied in 62 patients splenectomized for idiopathic thrombocytopenic purpura (ITP). Patients with hyperplasia of splenic follicles (mean follicle diameter greater than 500 micron) were more likely to relapse or to develop additional autoimmune disorders than patients without hyperplastic splenic follicles (mean follicle diameter less than 500 micron) (p less than 0.01). The enlargement of splenic follicles had a positive predictive value of 27% and a negative predictive value of 100% for a poor outcome of splenectomy. Thus, the histological examination of spleens surgically removed for ITP seems to be an appropriate method to obtain the first indication of the possible long-term effect of splenectomy almost immediately after the operation.

[B- and T-lymphocyte compartments in the spleen in idiopathic thrombocytopenic purpura].

The exstirpated spleens of 62 patients in each case with idiopathic thrombocytopenic purpura (ITP) and traumatic rupture of the spleen were comparatively examined macroscopically, histologically and morphometrically. The mean follicle diameters of the spleen were significantly greater in ITP patients than in those of controls. With 500 microns identified as an upper limit of normal follicle diameter, ITP patients could be divided into two groups of equal size below and above this limiting value. The thrombocyte values determined at the time of splenectomy and 6 months before it were significantly lower in ITP patients with enlargement of spleen follicles than in those ITP patients with spleen follicle being in the normal range. Thus, the morphometrical determination of the spleen follicle size can provide a semi-quantitative reference to the extent of synthesis of autothrombocytic antibodies. In T-lymphocyte compartments there was no difference between ITP and controls.

Cyclosporine-associated nephropathy in patients with heart and bone marrow transplants.

The morphology of kidneys from heart (n = 55) and bone marrow (n = 112) transplant recipients treated either with cyclosporine (CSA) or conventional immunosuppression was investigated at autopsy. The major findings were: In the bone marrow transplant recipients glomerular collapse, tubular atrophy, interstitial fibrosis, striped form, CSA-associated arteriolopathy and thrombi in glomeruli and/or arterioles were more often found in the CSA group as compared to conventional immunosuppression. In the heart transplant recipients glomerular collapse and obsolescence, tubular atrophy and intimal fibrosis in arteries were more frequent in the CSA group. Vascular interstitial toxicity known to be associated with CSA treatment from renal transplant patients was found in 54% (25% severe) of the bone marrow and 19.5% (9.7% severe) of the the heart transplant recipients. The prevalence of vascular interstitial toxicity in bone marrow versus heart transplant recipients is possibly due to higher CSA dosage and pretreatment with cytostatic drugs and irradiation. Analyses of the lesions from early stages to the full picture of vascular interstitial toxicity suggests that CSA causes a form of thrombotic microangiopathy with focal glomerular and/or arteriolar thrombosis followed by typical CSA-associated arteriolopathy which results in interstitial fibrosis with tubular atrophy.

[Breast diseases (fibroadenomas, mastopathies, breast cancers) in hormonal contraception].

282 female patients with histologically confirmed fibroadenomas (71), mastopathies (187) and breast carcinomas (24) were questioned for hormonal contraception. 144 women had taken oral steroid contraceptive pills, 138 denied such treatment. Statistically significant differences could not be observed in the occurrence of the breast diseases studied. Under hormonal contraception, fibroadenomas were found in 28% (without contraception in 22%), simple mastopathies in 41% (without in 40%), proliferative mastopathies in 22% (without in 30%) and mammary carcinomas in 9% (without in 8%) of the corresponding cases. The histological finding of a so-called apocrine metaplasia was found in all the diseases (except carcinomas) more frequent in the contraceptive group. Histometrically, the breast parenchyma showed a changing lobular hyperplasia under hormonal contraception in all the disease groups studied. An influence of questioned gynecological risk factors for a breast carcinoma was not striking in the women with and without hormonal contraception.

[Histologic classification of breast cancer and its precancerous stages].

A proliferating mastopathy with severe epithelial atypia as well as with so-called radial scars, intraductal papillomatosis, and findings of so-called lobular cancerisation are regarded as possible precursors of mammary carcinoma. The histological differential diagnosis can be difficult between atypical epithelial proliferations in mastopathy and noninvasive intraductal carcinoma, between tubular formations in radial scars and tubular carcinoma, papilloma and papillary carcinoma as well as between primary and secondary lobular cancerisation. Noninvasive and invasive breast carcinomas can be diagnosed according to the WHO classification (12) with a relatively good reproduceability. Besides the histological type, other macroscopic (tumour size, number of metastatically involved lymph nodes, distant metastases), microscopic (tumour grading) as well as biochemical findings (receptor status) are also important for treatment and prognosis of breast carcinoma.

[Morphology of IgA nephritis (Berger disease). Immunohistologic, light- and electron microscopic findings].

An IgA nephritis was diagnosed in 109 (24%) out of 445 renal biopsies with glomerular disease in the years from 1977 to 1985. The mean age of the patients in question was 29 years. The male:female sex ratio was 2.6:1. Immunohistologically, the characteristic branching mesangial IgA deposits were uniformly present. In addition 96% showed mesangial C3 and 54% mesangial IgG and/or IgM deposits. Besides, immunoglobulin and/or C3 deposits could be detected at glomerular basement membranes (20%). The histological types of IgA nephritis were minor glomerular abnormalities (10%), focal/segmental glomerulonephritis (29%), diffuse mesangioproliferative (58%), membranoproliferative (1 case), and sclerosing glomerulonephritis (2 cases). Additional tubulointerstitial lesions were found in 55%. Electron microscopically, in 53 cases under study, mesangial electron dense immunodeposits were seen. At the glomerular basement membranes, deposits could be found only in 23 examined cases (43%). The comparison of clinical data with morphological findings showed relationships between the degree of proteinuria and creatinine level increase with the histological type of IgA nephritis and with the degree of glomerular sclerosis, tubulointerstitial lesions, and electron microscopically proved glomerular basement membrane deposits. Hematuria seemed not to correlate with the morphological picture.
