Experiences with the tracheoesophageal shunt method for vocal rehabilitation after total laryngectomy.

The tracheoesophageal (T-E) shunt technique reported by Komorn et al. (1973) was used for vocal rehabilitation in 8 laryngectomized patients including one case of hypopharyngeal cancer. It was proved that this technique was a simple one-stage procedure with a low incidence of complication. Four out of 7 patients, excluding one patient who died of another disease, acquired excellent speech without symptomatic aspiration. Two patients developed fairly good speech. One shunt was closed by aspiration. The shunt constructed by a flap measuring 2 x 4 cm, opened 1 cm below the distal end of the trachea, showed good results. The T-E shunt speech exceeds esophageal speech in duration and intelligibility.

Atypical subacute sclerosing panencephalitis.

An unusual case of panencephalitis in a 4-year-old Japanese boy, with onset at three months after measles infection and rapid progression to a comatose state in approximately one month, is described. A rapid rise in serum measles antibody titre after the onset of the symptoms, and the appearance of various abnormal antibodies in the serum, were noted. Pathologically, the brain showed sclerosing polio- and leucoencephalitis with diffuse gliosis and sporadic intranuclear inclusions. The process is suggested to be intermediate or transitional between acute measles encephalitis and SSPE.

Effect of prenatal treatment with ethinyl estradiol on the mouse uterus and ovary.

Oral ethinyl estradiol (0.01 or 0.02 mg. per kilogram) administered prenatally to ICR-JCL strain mice before the development of the uterine and ovarian blastemata induced cystic glandular hyperplasia with epidermization in the endometrium and excessive formation of degenerating follicles in the ovaries at 10 to 14 weeks of age. To the evidence for the production by estrogen of malignant uterine neoplasms in rodents and the clinical data showing that cystic glandular hyperplasia develops after prolonged estrogen administration in postmenopausal women and adenocarcinoma in premenopausal or menopausal women using oral contraceptives including ethinyl estradiol, we add the present results which suggest that ethinyl estradiol administered transplacentally can cause malignant transformation of the uterine endometrium in mice.

Renal responses to diuretic drugs in freshwater catfish Ictalurus punctatus.

Renal tubules of freshwater teleosts consist of proximal, intermediate, distal, and collecting segments. Diuretic drugs were injected into freshwater channel catfish to define the mode of natriuresis and diuresis. Although the catfish lack a loop of Henle, ethacrynic acid (1 mg/kg), furosemide (1 mg/kg) and, to a smaller extent, hydrochlorothiazide (2 mg/kg) produced marked chloruresis and natriuresis. The amount of sodium and chloride reabsorbed decreased, and the decrease in fractional reabsorption of chloride was greater than that of sodium. All three drugs, however, elicited only mild diuresis. This finding suggests that these drugs inhibit sodium and chloride reabsorption in tubules which are highly impermeable to water, most likely along distal segments of the nephron. Ethacrynic acid and furosemide appear to inhibit both reabsorption and secretion of potassium. Acetazolamide (50 mg/kg) induced moderate diuresis which seemed to reflect increased glomerular filtration rate. Smaller doses of acetazolamide increased urinary bicarbonate excretion without the increase in filtration rate, suggesting a possible inhibition of carbonic anhydrase in the renal tubules.

Frequent coexistence of ocular malformations in externally malformed human fetuses.

60 undamaged human fetuses with some types of external malformations were taken from the human embryo and fetus collection at the Department of Anatomy, Kyoto University. They were examined macroscopically and stereo-microscopically for ocular and other external and internal malformations. 18 cases, that is 30% of the group, showed ocular malformations of the following type: approximated eyes (2 cases), approximated eyes + microcornea (1), microphthalmia (8), microcornea (3), corneal opacity (1), transparent sclera (2), antimongoloid obliquity of the palpebral fissure (1). In addition, asymmetric insertion site of the recti muscle(s) which can be regarded as a minor deviation, was found in 10 cases (16.7%). Other types of malformations were found simultaneously in a variety of organ systems and regions. No particular maternal factors were suggested to be related to such coexisting ocular malformations.
