[Does the use of free transplanted jejunum segments in advanced laryngeal and hypopharyngeal cancer prolong survival? A matched-pair analysis].

The effect of free transplanted jejunum autografts, inserted after resection of extensive squamous cell carcinoma of the larynx and hypopharynx is evaluated in terms of survival and the development of local and regional recurrences. In a matched-pair analysis, 22 patients with jejunum transplants were compared with 44 patients with the same performance status, in whom after laryngopharyngectomy, the pharynx had been closed with local mucosa. The cumulative survival rate was calculated using the Kaplan-Meier method; the statistical comparison of the survival curves of the two groups was made with the Mantel-Haezel test. Patients of the study group had a statistically significant (p less than = 0.01) better survival rate than those of the control group, the 50% survival rate being 36 and 11 months, respectively. Local recurrences occurred in 9.1% of the patients in both the study group and in controls, and regional recurrences in 40.8 and 43.2%, respectively. This militates against the possibility that the surgery in patients in whom reconstruction with jejunum was possible might have been more radical. Further, it may be speculated that the statistically significant better survival rate is due to the jejunum itself, presumably to a tumourprotective effect of the lymphatic tissue of the transplant.

[The treatment concept and results of peri-/sub-prosthetic fractures].

The treatment of choice in ipsilateral femoral shaft fractures in patients with hip prosthesis depends on the location of the fracture lines and the observation of loosening of the prosthesis. Either a change of prosthesis, if necessary in combination with refixation of the greater trochanter or simple open reduction with internal fixation (ASIF) along with autologous and/or homologus spongiosa are the used techniques. The rate of complications in peri- or subprothetic fractures is high. Lack of motion in hip and knee joint occurs frequently.

Genetic flanking markers refine diagnostic criteria and provide insights into the genetics of Von Hippel Lindau disease.

Von Hippel Lindau disease (VHL) is a hereditary syndrome, associated with tumors and cysts in multiple organ systems, whose expression and age of onset are highly variable. The availability of a genetic test for the early and reliable detection of individuals carrying the defective gene would be beneficial for VHL patients and their relatives, since many of the manifestations of VHL can be successfully treated if detected in their early stages, while the complications of undetected disease can be devastating. We have previously shown that the VHL gene maps to chromosome 3p. To provide genetic markers for the development of a reliable diagnostic test, and to further narrow and eventually clone the VHL defect, we have generated DNA markers for chromosome 3p. With these markers, we have performed a multipoint genetic linkage analysis in 28 VHL pedigrees, comprising 470 individuals, 164 of whom were affected with VHL. Here we report the identification of tightly linked markers, including flanking markers that bracket the VHL gene to a small region on chromosome 3p25-p26. This finding has several major implications. While visceral cysts of the kidney, pancreas, and epididymis are commonly found in VHL and are considered diagnostic criteria for this disorder, they also occur in the general population. The presence of cysts, unaccompanied by other more typical lesions such as retinal and cerebellar hemangioblastoma, may therefore represent a major diagnostic problem, leading to errors in the assessment of disease status. The application of flanking markers for the VHL gene for presymptomatic diagnostic testing confirms that epididymal cysts are indeed not suitable as a diagnostic criterion in this disorder. Pheochromocytomas occur nonuniformly in VHL families and may also be associated with other hereditary tumor syndromes; our genetic studies imply that the phenotype in VHL families with and without pheochromocytomas is caused by defects within the same gene. The absence or presence of this tumor type is therefore due to the pleiotropic expression of a single gene rather than to the existence of several different genes for VHL. The region on chromosome 3p13-p14 known to contain several chromosomal translocation breakpoints in families with "pure familial renal cell carcinoma" is quite proximal to the VHL locus in 3p25-p26 we have identified. Chromosome 3p may therefore contain two loci for renal cell carcinoma: one gene (or genes) in 3p13-p14 and the VHL gene in 3p25-p26, whose aberration is also associated with other typical manifestations of VHL. Since renal cell carcinoma, pheochromocytoma, and visceral cysts can occur sporadically even in young people and may also be associated with other tumor syndromes, the availability of flanking markers for the VHL gene will be useful in identifying VHL gene carriers, particularly among those individuals at risk in whom these are the only manifestations of disease. The isolation and characterization of the VHL gene, based on the identification of flanking markers, will have important implications for diagnosis and treatment of patients with VHL, as well as for a much larger number of individuals having the sporadic counterparts of VHL-associated tumor types.

Cell Line

Perforated ventricular aneurysm in a male suffering from pneumonia.

In a 49-year-old male with fever, dyspnea, and chest pain, thoracic x-ray revealed pneumonia with enlarged heart silhouette. Antibiotics were successful, pneumonia healed and complaints disappeared. Yet, during the following 3 months, echocardiography showed mild persistent pericardial effusion while in ECG both sinus tachycardia and ST-T changes were found suggesting chronic pericarditis. Magnetic resonance imaging, however, revealed an extensive posterobasal aneurysm with pericardial effusion substantiated by ventriculography. Coronary angiography showed diffuse three-vessel disease. Surgery revealed aneurysm with distinct perforation of the left ventricle and pericardial thrombi, thus aneurysmectomy as well as bypass grafts were performed. One year postoperatively, magnetic resonance imaging confirmed the absence of aneurysm with only a small irreversible posterobasal perfusion defect remaining as shown by thallium scintigraphy.

[Long-term results after operation of Achilles tendon rupture].

From 1973 to 1986 90 patients with rupture of Achilles' tendon were operated on (4 of these had open ruptures). Different surgical approaches were employed during the period under report. The assessment schemas after Arner/Lindholm and after Trillat formed the basis of both clinical follow-up and a questionnaire operation (participants were 81 patients 10 months to 14 years after the operation). 92.6% good and very good results confirmed the authors' concept of surgical treatment around the 4th or 5th day.

Achilles Tendon

[Important laboratory parameters of deep frozen coagulation active fresh plasma and virus inactivated lyophilized pool plasma].

Fresh Frozen Plasma is the most important therapeutic agent in acquired coagulation disorders. We investigated the quality of a new virus-inactivated lyophilisated human pool-plasma (VFL) and conventional fresh frozen plasma (FFP). In VFL we found a significantly reduced coagulation activity, a significantly higher pH-value, a highly significant rise of heparin values and a significantly lower number of thrombocytes. Electrolytes, protein levels and complement activity showed no significant difference. In conclusion, this virus-inactivated lyophilisated human pool plasma does not seem to be susceptible to therapy coagulation disorders like the conventional fresh frozen plasma.

[Analysis of occupational disease number 70--expert assessment in the Dresden district--results and conclusions].

100 cases of occupational disease 70--expertises from the years 1982 through 1986--were reviewed. Partly, considerable deficiences were found in hygienic exposure analyses as well as in orthopaedic expertises. About one third of the workers with acknowledged occupational disease 70 had fitness relevant occupational independent pre-damage of the spine. From the results conclusions were drawn for the prospective processing of the occupational disease 70 as well as for the fitness evaluation of workers with heavy physical work.

[Perioperative prophylaxis in patients with head and neck tumors].

Sixty-one cancerous patients of the larynx, hipopharynx and tongue underwent surgery under perioperative prophylaxis with Cefuroxim and Metronidazole during 3 days. Alterations of the healing process could not be observed and the parameters both clinic and chemical were normal. The seric level of antibiotics was effective against most of the bacteria cultured from tracheal secretions and from the posterior wall of the oral cavity. Perioperative antibiotic prophylaxis is compulsory when major surgery is performed in head and neck.

Structural variability in the genome of the Thermoproteus tenax virus TTV1.

Six variants of the TTV1 genome, including the primary isolate, have been characterized. DNA sequence comparison of 'wild-type' virus (WT) and one of the variants (VT3) showed that differences are due to insertions and deletions that were confined to contiguous portions of two distinct ClaI fragments. Seven similar short DNA sequences (30-102 bp) were involved in the variation. The deletions and insertions of these short DNA sequences occurred in every case adjacent to the 8 bp consensus sequence 5'-ACXCCTAC-3' which formed the 5' flank of the segments involved.

[Epidemiology of malignant neoplasms of the respiratory and upper digestive tract in Austria].

Between 1981 and 1985 the incidence of malignant tumours of the buccal cavity and mesopharynx, the larynx and hypopharynx and of the oesophagus in males was higher in the mainly rural Burgenland and Lower Austria than in Vienna. In lung tumours a slight urban-rural gradient tended to level out (Tables 1-4). In women, the cancer incidence on all locations showed an urban-rural gradient, steeper in the buccal cavity and mesopharynx, in the larynx and hypopharynx and in the oesophagus than in lung tumours (Tables 1-4). Cancer incidence in the nasal and paranasal cavities, middle ear, epipharynx, salivary glands and lips was low. Sex ratios, lips excepted, were lower than in other sites studied (Tab. 6). In Eastern Austria the mortality of malignant neoplasms of the mouth and pharynx in males increased steadily from 1960 to 1987. In laryngeal and oesophageal cancer no persistent trend was discernible. The lung cancer mortality decreased from the mid-seventies onwards (Figs. 1-2). In women, the lung cancer mortality increased continuously, and for some years now also that of the mouth and pharynx. Cancer of the oesophagus decreased, and laryngeal cancer mortality showed nearly constant rates (Figs. 5-6).

On the hypothetical protein F154 of the TTV1 virus from Thermoproteus tenax. Part III: Immunological identification of the protein with anti-peptide antibodies.

To assess the expression of an hypothetical protein F154 encoded in an open reading frame of the DNA sequence of the virus TTV1 identified in the archaebacterium Thermoproteus tenax, two synthetic peptides related to characteristic repeats of the primary structure and corresponding to the sequence portions [Ser81] (73-84) and (79-101) were selectively grafted at their N-termini to the surface-exposed single cysteine residue of iso-1-cytochrome c via the maleimide thiol principle. Antisera were raised in rabbits against the two conjugates and the antipeptide-(79-101) immunoselected antibodies were found to crossreact with a strong protein band in Western blots of cell lysates of TTV1-infected bacteria, thus confirming expression of the protein F154 in Thermoproteus tenax.

The dynamics of the life expectancy is quantified using the example of some European industrial countries. It is possible to determinate the part of the mortality of infants and children at the life expectancy by performed prototype calculations. Further increase of years of ane's life is especially possible by decreasing mortality in the middle and higher age.
