Spinal dural arteriovenous malformation: report of an unusual case.

We present an unusual case of a spinal dural arteriovenous malformation (AVM) which produced compression radiculopathy of the left S1 nerve root. The nerve root was compressed by epidural vessels, draining vessels, and the nidus, without the characteristic myelographic findings of a dural AVM. No feeding vessels could be identified. It is important to consider the possibility of a spinal dural AVM in middle-aged patients who suffer lumbosacral radiculopathy, even if myelography does not demonstrate the characteristic findings.

Reaction of (R)-(-)-2-aminomethylpyrrolidine(1,1-cyclobutanedicarboxylato)platinum( II) with guanosine.

The reaction of a new antitumor platinum complex, (R)-(-)-2-aminomethylpyrrolidine(1,1-cyclobutanedicarboxylato++ +)platinum(II) (1) with guanosine at room temperature in an aqueous solution was followed by proton nuclear magnetic resonance (1H-NMR) spectroscopy and high performance liquid chromatography (HPLC) at intervals. Both techniques showed that a new compound was formed by displacement of the 1,1-cyclobutanedicarboxylate moiety of 1 with two guanosines, and its 1H-NMR spectrum and HPLC chromatogram were proved to be identical with those of [(R)-(-)-2-aminomethylpyrrolidine]bis(N7-guanosine)platinum(II) (2), which was obtained upon successive treatment of (R)-(-)-2-aminomethylpyrrolidinedichloroplatinum(II) (3) with AgNO3 and 2 mol eq of guanosine in water. The binding sites of the platinum to the two guanosine moieties in 2 were confirmed by the pH dependence of the two G-H8 signals.

Evaluation of the pathogenicity and immunogenicity of vaccinia virus to piglets.

The pathogenicity and immunogenicity of vaccinia virus were examined in order to evaluate the possibility of its application as a recombinant viral vaccine in pigs. Following scarification inoculation with vaccinia virus, a mild reddish papulation developed only on the scarified part of the skin. No symptoms of illness such as fever or stunting were noted. Vaccinia virus was recovered in titers from scarified skin 4 and 7 days after inoculation. Control piglets cohabited with inoculated animals remained normal for the whole 5 week observation period. Hemagglutination inhibition and indirect immunofluorescence tests detected antibodies against vaccinia virus in the inoculated piglets, whereas no anti-vaccinia virus antibodies were detected in the contact control animals. Antigen-induced blastogenic tests of peripheral blood lymphocytes from animals, revealed that lymphocytes obtained from inoculated donors 5 weeks after inoculation, had a higher stimulation index (P less than 0.05) than did those from uninoculated piglets. These results suggested that vaccinia virus would be useful as a recombinant viral vector for pigs.

Structural investigation of the antibiotic sporaviridin. XIV. Isolation of components of intact sporaviridin.

Six components of sporaviridin (SVD) were successfully isolated by HPLC using methano-1 M ammonium chloride as the mobile phase. Each component possesses expectedly antimicrobial activity. To ensure the structural relationship between the intact SVD and N-acetylated sporaviridins (N-Ac-SVD), they were converted to the corresponding of N-acetates by acetylation in methanol, whose structures had already been determined as glycosides consisting of a 34-membered macrocyclic lactone, viridopentaoses, D-glucose and N-acetylvancosamine.

Structural elucidation of aculeximycin. I. Further purification and glycosidic bond cleavage of aculeximycin.

A new insecticidal antibiotic, aculeximycin (ACM), was produced by an actinomycete identified as Streptosporangium albidum. ACM has been successfully isolated from culture filtrate by a combination of Diaion HP-20, Amberlite CG-50, reversed phase silica gel and Sephadex LH-20 chromatographies. It was found that ACM is a basic glycosidic antibiotic with a molecular weight of 1,672 including five monosaccharide units, three double bonds and a hemiketal ring by preliminary spectral analyses. Treatment of ACM with 1,8-diazabicyclo[5,4,0]undecene-7 caused a glycosidic bond cleavage to give aculexitriose, pseudoaglycones I and II.

Structural elucidation of aculeximycin. II. Structures of carbohydrate moieties.

Treatment of aculeximycin with 2% 1,8-diazabicyclo[5,4,0]undecene-7 (DBU)-methanol yielded three products, aculexitriose, pseudoaglycones I and II. The structural elucidation of aculexitriose was carried out by spectral analyses (MS, NMR (1H-1H 2D NMR spectroscopy, nuclear Overhauser effect] and chemical degradations of aculexitriose and its derivatives. The structure of aculexitriose was established to be a branched trisaccharide, O-6-deoxy-beta-D-glucopyranosyl-(1----2)-O-[3-amino-2,3,6-trideoxy-beta- D- arabino-hexopyranosyl-(1----3)]-6-deoxy-D-glucopyranose. On the other hand the pseudoaglycones I and II were stereoisomers with respect to a chiral center newly formed by the DBU reaction. The pseudoaglycones contain one neutral sugar and one amino sugar, which turned out to be D-mannose and L-vancosamine, respectively.

[Reverse redistribution in the stress thallium scan--correlation of coronary blood flow and myocardial damage].

In order to make clear 'reverse redistribution (RR)' phenomenon in stress thallium scan we investigated 477 consecutive patients who were consisted of 74 cases with PTCA (PTCA group), 86 cases with coronary artery bypass graft (CABG group) and 317 cases without any revascularization (N group). RR was detected in 12 cases out of PTCA group (16%), in 11 cases out of CABG group (13%) and in 10 cases out of N group (3.2%), and the incidence of RR in PTCA or CABG group was higher than that of N group (p less than 0.01). In 33 patients with RR the cases without significant coronary artery stenosis were more than those with stenosis (p less than 0.01) and the cases with myocardial damage were more than those without damage (p less than 0.01). In quantitative analysis washout rate at the RR areas showed relatively high value, but it was not significant compared with normal value (53 +/- 9.0% vs. 49 +/- 1.9%). In conclusion, RR had a tendency to appear in the condition of normal myocardium mingled with scar and of sufficient coronary blood flow. The mechanism of the phenomenon was thought to be that initial uptake of thallium in the partially scarred area was as high as in the normal myocardium, but washout of thallium from its remaining normal myocardium is normal or faster so that the defect appears at the myocardium mingled with scar. Therefore existence of reverse redistribution suspects the myocardium mingled with scar and sufficient coronary blood flow to the remaining normal myocardium.

[Influence of ischemic heart disease on early and late mortality after aortic reconstruction for abdominal aortic aneurysm and aortoiliac occlusive disease].

A retrospective study was undertaken to assess the influence of ischemic heart disease (IHD) on the early and late mortality of patients undergoing elective repair of an abdominal aortic aneurysms (AAA) or aortoiliac occlusive disease (AIOD). The patients were divided into IHD and non-IHD groups on routine clinical grounds. Among 157 patients with AAA, postoperative myocardial infarction occurred in 12.8% of the IHD group compared with 0.9% of the non-IHD group (P less than 0.05). The late mortality rate in the IHD group was higher than in the non-IHD group (p less than 0.05), and the mortality rate from myocardial infarction was 30% in the IHD group compared with 13% in the non-IHD group. Among 119 patients with AIOD, clinical evidence of IHD was found in 24 patients and extra-anatomical bypass was performed in 54% of these patients, compared with 35% of the patients in the non-IHD group. There was no occurrence of postoperative myocardial infarction. This study shows that an aggressive diagnostic approach should be taken for patients with AAA who have clinical evidence of IHD and that reevaluation of IHD should be performed in patients with AIOD after aortic reconstructive surgery.

[Prevention of spinal cord ischemia after cross-clamping of the thoracic aorta--monitoring of spinal cord perfusion pressure and somatosensory evoked potentials].

The pressure difference between the mean distal aortic pressure (MDAP) and the cerebrospinal fluid pressure (CSFP), defined as the spinal cord perfusion pressure (SCPP), as well as somatosensory evoked potentials (SEP) were monitored intraoperatively to detect and prevent intraoperative spinal cord ischemia in 24 patients who required cross-clamping of the descending thoracic aorta. A temporary axillo-femoral shunt, utilizing a 10 mm woven Dacron tube graft, was employed in 10 patients and partial cardiopulmonary bypass was employed in fourteen. Ischemic SEP changes were seen in six patients. Two patients, whose SCPPs were 32 and 35 mmHg, showed a complete loss of SEP and subsequently developed paraplegia. In the other four cases, increase of the MDAP and/or withdrawal of cerebrospinal fluid were performed to increase the SCPP to more than 60 mmHg when ischemic SEP changes occurred. The SEP gradually recovered in two of these cases. The ischemic SEP changes seen in one patient, who had the longest aortic cross-clamping time, (175 minutes) returned to normal immediately after unclamping. In another case, who had a thoracoabdominal aortic aneurysm, the intercostal arteries were reimplanted since the ischemic SEP changes did not revert. These four patients recovered without any neurological deficit. In the other 18 cases without ischemic SEP change, SCPP was kept at more than 40 mmHg during aortic cross-clamping. We conclude that the SCPP should be maintained at more than 40 mmHg during aortic occlusion, and increased to more than 60 mmHg when ischemic SEP changes occur, by increasing MDAP and/or withdrawing cerebrospinal fluid in order to prevent postoperative paraplegia.

Metal hypersensitivity in dentists: a patch test study.

Patch test of dental alloys was conducted on dentists in the Department of Prosthetic Dentistry, Hiroshima University School of Dentistry. The positive rate of the first patch test in the test group was 47.1%, and that in the control group was 10.0%, showing a large difference between the two groups. Furthermore, a second patch test was conducted on those who showed a positive response together with pustular or follicular reaction. As neither positive response nor pustular or follicular reaction was observed in the second patch test, the results were regarded as negative. Only a very slight difference in the positive rate could be observed when the results of the two tests were combined. The results of the present study suggest that the possibility is small for dentists to develop metal hypersensitivity in normal clinical practice.

[Clinical usefulness of physiologic components obtained by factor analysis--application to 99mTc-DTPA renography].

The clinical usefulness of physiological components obtained by factor analysis was assessed in 99mTc-DTPA renography. Using definite physiological components, another dynamic data could be analyzed. In this paper, the dynamic renal function after ESWL (Extracorporeal Shock Wave Lithotripsy) treatment was examined using physiological components in the kidney before ESWL and/or a normal kidney. We could easily evaluate the change of renal functions by this method. The usefulness of a new analysis using physiological components was summarized as follows: 1) The change of a dynamic function could be assessed in quantity as that of the contribution ratio. 2) The change of a sick condition could be morphologically evaluated as that of the functional image.

Immunoperoxidase evaluation of pneumonic lesions induced by Pasteurella multocida in calves.

To evaluate the relationship between pneumonic lesions and distribution of bacteria, lungs from calves inoculated with Pasteurella multocida were examined histologically by use of immunoperoxidase technique. Pneumonic lesions fundamentally consisted of broncho-pneumonia with fibrinopurulent pleuritis. The lesions were confirmed to be associated with inoculated P multocida, using the immunoperoxidase technique. The P multocida antigen was detected not only in the bacterial clusters in the lesions, but also in the cytoplasm of infiltrating neutrophils and macrophages. Further, immunoelectron microscopy indicated that the inoculated bacteria generally were phagocytosed and digested by neutrophils.

[Magnetic resonance imaging of acoustic neuroma].

Thirteen patients with acoustic neuroma were studied on a 1.5 T superconductive magnetic resonance (MR) imager. Acoustic neuromas appeared as lower signal intensity than the surrounding brain stem on T1 weighted image (W.I.), and as higher signal intensity on T2 W.I.. Axial and coronal sections of T1 W.I. were very useful in observing the tumor in the auditory canal and in investigating the anatomical relations of the tumor and the surrounding structures. MR imaging is very excellent examination to make early diagnosis of the acoustic neuroma and preoperative anatomical evaluation.

[Development of a new quantitative analysis for thallium myocardial SPECT images--quantitative STEREO-VIEW method and evaluation of its clinical usefulness].

We have developed a three-dimensional quantitative display method, Quantitative STEREO-VIEW method (STEREO-VIEW method), for analysis of thallium (Tl) myocardial SPECT images and have evaluated its clinical usefulness. SPECT data were obtained immediately and 4 hours after the exercise, and contour lines for outer edges of left ventricular (LV) myocardium were detected from 10 slices of short axial images in delayed scan. LV wall was divided into 510 segments by dividing each slice into 51 sectors and LV myocardium was three-dimensionally reconstructed. Mean Tl counts, %T1 uptake and washout rate (WR) of each segment were calculated and these values were represented such as infarcted area display, redistribution display and WR display on STEREO-VIEW. Furthermore, extent index (EI), severity index (SI) and intensity index (II) were defined to express the degree of perfusion abnormality using WR value of each segment. In the 25 patients with coronary revascularization, improving rate of these indices correlated well with that of double product significantly. STEREO-VIEW method resembled in the original shape of left ventricle of each patient, so STEREO-VIEW method has made it easier to discriminate the accurate extent of ischemic lesion. In addition, EI, SI and II using WR were good indices for coronary perfusion and these were thought to be useful on clinical study.

Analysis of thallium-201 myocardial SPECT images using fuzzy set theory.

Using the fuzzy set theory, a method was developed for analyzing the extent and severity of ischemia by comparing the exercise and delayed SPECT images with Thallium-201. For each short axial image (slice) for exercise and delayed study, a defect probability matrix (DPM) is created so that it shows the severity of ischemia of myocardium in that slice. Each slice is divided into 8 equi-angle sectors, and the left ventricle (LV) also is divided into 8 equi-angle vertical sectors from apex to base. Using DPMs, the defect probability is calculated for each slice, sectors of each slice and vertical sectors of the LV. The results are displayed on a CRT by means of images, curves and histograms. They show what percentage of the area of each slice and that of the lateral, anterior, septum and inferior portions have how much defect. They provide comprehensive and easily understood information about the condition of the LV in exercise and delayed stages. Persistent and transient ischemia can be diagnosed by visual comparison of patients' curves and histograms with their corresponding normal limits.
