Tuberculosis on regular hemodialysis--a case of pericardial tamponade.

The patient presented in this paper had been stable for 3 months after the induction of hemodialysis, when nausea, vomiting and hepatomegaly suddenly developed. A chest film revealed rush cardiomegaly, and massive pericardial effusion was demonstrated by echocardiography. One liter of hemorrhagic fluid was removed by pericardiocentesis and subsequent pericardial drainage under echocardiography. The patient received chemotherapy against pulmonary tuberculosis 30 years ago and calcification on chest film was apparent. Although sputum smear and pericardial effusion was negative for acid-fast organisms, combination therapy was initiated for suspected tuberculosis. The patient recovered completely and 2 months later it was demonstrated that cultures of sputum grew mycobacterium tuberculosis. Tuberculin skin test (PPD), which was negative 2 months previously, converted to positive. Tuberculosis must be considered as a potential cause of pericardial tamponade in patients on regular hemodialysis, and prompt therapy for both cardiac tamponade and the occult infection is warranted.

[The effect of the cementing material on the strength of the all ceramic crown].

On the assumption that the all ceramic crown was cemented on an abutment tooth, the effect of the cementing materials on the strength of the ceramic crown was analyzed using two-dimensional finite element technique. The results obtained were as follows: 1. When the compressive load was applied to the ceramics, the principal stress in the cementing material was compressive in the region of Young's modulus of the cementing materials 200-700 kg/mm2, but was in tension beyond it. 2. When the tensile load was applied to the ceramics, the principal stress in the cementing material was in tension, and decreased as Young's modulus of the cementing materials decreased. 3. When the adhesive effect of the cementing material was investigated with examining the presence of the crack between the ceramics and the cementing material, the principal stress in the ceramics increased as the crack propagated. 4. The creation and propagation of the crack, the increase of the principal stress in the ceramics and the destruction of the ceramics were suggested. 5. When the analyzed results were compared with the mechanical properties of the cementing materials from the point of view of the breaking strength, it was considered that the adhesive resin cement was favorable as the cementing material of the ceramic crown. And the Young's modulus of the adhesive resin cement was about 400 kg/mm2.

An analysis of ocular counter-rolling measured with search coils.

Ocular counter-rolling (OCR) was studied by using a scleral search coil magnetic system in normal subjects and in pathological cases. Normal ocular counter-rolling was 2.7 degrees-7 degrees when the head was tilted 10 to 30 degrees. In most cases of benign paroxysmal positional vertigo, the OCR to the ipsilateral side was reduced, while that to the contralateral side was normal or only slightly reduced. Soon after unilateral labyrinthectomy, the OCR to the ipsilateral side was reduced or was 0, whereas OCR to the contralateral side was normal or slightly reduced. Some 3-5 years after the operation, however, the OCR seemed to depend on the compensation achieved. In cases of acoustic neurinoma, OCR to both sides was reduced, that to the ipsilateral side being more strongly impaired than the OCR to the contralateral side.

Structure and expression of genes coding for xylan-degrading enzymes of Bacillus pumilus.

The complete nucleotide sequence of the beta-xylosidase gene (xynB) of Bacillus pumilus IPO and its flanking regions was established. A 1617-bp open reading frame for beta-xylosidase, a homodimer enzyme, was observed. The amino acid sequence of the N-terminal region and the molecular mass 62607 Da) of the beta-xylosidase subunit, deduced from the DNA sequence, agreed with the result obtained with the purified enzyme. The Shine-Dalgarno sequence was found 8 bp upstream of the initiation codon, ATG. The xylanase gene (xynA) of the same strain was 4.6 kbp downstream of the 3' end of xynB, and its DNA sequence was reported in our previous paper [Fukusaki, E., Panbangred, W., Shinmyo, A. & Okada, H. (1984) FEBS Lett. 171, 197-201]. The results of the Northern hybridization suggested that the mRNA of xynA and xynB were produced separately. The 5' and 3' ends of the xynA and xynB gene were mapped with nuclease S1. The '-10' regions for promoter sequences of both genes were similar to the consensus sequence for B. subtilis RNA polymerases, the '-35' regions were different from all the known promoters for B. subtilis RNA polymerases.

Crystallization and preliminary X-ray studies of Bacillus pumilus IPO xylanase.

The xylan-degrading enzyme xylanase, from Bacillus pumilus IPO, has been crystallized. The crystals are monoclinic, space group P21 with a = 40.8 A, b = 66.8 A, c = 34.7 A and beta = 103.0 degrees. The asymmetric unit contains one molecule of Mr 22,500. The crystals diffract to at least 2.5 A resolution, and they are suitable for X-ray crystal structure analysis at high resolution.

T antigen expression in benign hyperplasia and adenocarcinoma of the prostate.

The status of Thomsen-Friedenreich antigen (T antigen), in histological sections of human hyperplasia (30 cases) and adenocarcinoma (45 cases) of the prostate, was investigated by the immunohistochemical method. All benign prostatic hyperplasias were negative for T antigen but positive for cryptic T antigen after neuraminidase digestion. Of the 45 cases of adenocarcinoma of the prostate, 27 (60%) were positive for T antigen, 13 (29%) were negative for T antigen but positive for cryptic T antigen, and 5 (11%) were negative T and cryptic T antigens. The status of T antigen was found to be correlated with the histological grade and bone metastasis. However, there was statistically no definitive correlation between the status of T antigen and survival of patients with adenocarcinoma of the prostate.

New singlet oxygen source and trapping reagent for peroxide intermediates.

Application of newly-developed water-soluble singlet oxygen sources in the oxidation of biologically important compounds and the electron-transfer process involving singlet oxygen has been reviewed. Particularly, oxidation products of tryptophan by chemically generated singlet oxygen were compared to those obtained in dye-sensitized photooxygenation. The usefulness of trimethylsilyl cyanide as a trapping reagent for dipolar peroxide intermediates has been demonstrated in the photooxygenation of N-methylindoles, 2-(methoxymethylene)adamantane and adamantylideneadamantane in aprotic solvents. Based on these trapping reactions mechanism of singlet oxygen reaction of electron-rich enol ethers and enamines is discussed in light of theoretical calculation.

Bone resorption in cholesteatoma: epithelial-mesenchymal cell interaction and collagenase production.

Clinical evidence has shown that chronic otitis media with cholesteatoma causes greater bone resorption than otitis media without cholesteatoma. What is the role of the epithelium and its products on the morbidity of cholesteatoma? We have studied the mechanism of collagenase production in cultures of rat epidermal cells and skin fibroblast-like cells under various conditions. The epithelial cells significantly induced mesenchymal cells to produce collagenase. We conclude that interaction between epithelial cells and fibroblast-like cells enhances production of collagenase. This enhancement of proteolytic activity may be crucial in the series of molecular events resulting in bone resorption associated with cholesteatoma.

[Studies on the usefulness of a long-term, high-dose treatment of methylcobalamin in patients with oligozoospermia].

Methylcobalamin (CH3-B12) was administrated in a dose of 6 mg per day (group A, 16 cases) or 12 mg per day (group B, 23 cases) for 16 weeks to patients with oligozoospermia. There was no difference between group A and group B regarding vitamin B12 concentrations in the serum or seminal fluid. CH3-B12 appeared to be transported to seminal fluid very efficiently. However, results from group A and group B did not differ with respect to this parameter. The efficacy rate for group A was 37.5% and that for group B was 39.1%. From these results, it was concluded that a long-term, high dose treatment of CH3-B12 was useful for the treatment of patients with oligozoospermia. Since the efficacy rates were not different between the two groups, 6 mg per day would be the recommended dose. Clinical adverse reactions were observed in one patient in each group. Drug-related laboratory adverse reactions were observed in only one patient in group B.

[Combined administration of human chorionic gonadotropin and human menopausal gonadotropin in idiopathic male infertility].

Clinical experiences of HCG-HMG therapy for 56 cases of idiopathic male infertility were studied. Serum levels of LH, FSH and testosterone were measured before therapy. Sperm density improved in 25% of all cases. In the low FSH group, sperm density showed a high rate of improvement. The basal FSH was the best indicator to predict the prognosis of fertility. Additional measurements of LH and FSH response to GnRH gave further information on the prospect of fertility.

Determination of sodium dodecyl sulfate in hydrophilic ointments by thin-layer chromatography with flame ionization detection.

Thin-layer chromatography with flame ionization detection (TLC-FID) was compared with colorimetric and HPLC methods for measuring sodium dodecyl sulfate (SDS), an anionic surfactant, in hydrophilic ointments prepared according to the Japanese Pharmacopoeia, the United States Pharmacopoeia and British Pharmacopoeia. The TLC-FID method was found to be better than the colorimetric method in terms of simplicity and accuracy in the determination of SDS in hydrophilic ointments. Quantification of a small amount of SDS was not possible using HPLC with refractive index detection due to the low sensitivity of the detector.
