Depression of host-defence mechanisms following cardiac surgery.

The purpose of the present study was to determine the effects of open-heart surgery on host-defence mechanisms, by studying serial changes in serum immunoglobulins, circulating lymphocyte function in patients undergoing open-heart surgery. Serum proteins and immunoglobulins were significantly depressed immediately after operation and these depressions were correlated to the degree of the durations of cardiopulmonary bypass and associated hypothermia. White cell counts in peripheral blood increased significantly after operation. In contrast, total circulating lymphocytes were depressed postoperatively. The depression of circulating lymphocytes was mainly due to a marked decrease in the number of T-cells. Mitogen responses of lymphocytes stimulated by phytohemagglutinin were also depressed postoperatively. The quantitative and qualitative depression of cell-mediated immunity following open-heart surgery was observed. However, these depressions returned to preoperative levels in one week and had no clinical significance.

[A comparison with Dardik graft and Mindich graft on arterial bypass].

Arterial reconstruction using human umbilical cord vein graft as a vascular prosthesis was carried out in 24 patients, 26 legs, with arteriosclerosis obliterans. Dardik grafts were used in 13 legs and Mindich grafts in 13 legs. Twenty-six operations were divided into two groups; one with the proximal anastomosis above the femoral artery (proximal group) and the other with the proximal anastomosis below the femoral artery (distal group). Postoperative evaluation of all patients was performed at intervals of one to twenty-two months. To make the determination of graft patency, at least, it was required that the graft pulse was palpable. Cumulative patency rates were 54% and 69% in the Mindich grafts and Darkik grafts, respectively. In the proximal group, patency rates were 60% and 50% in Mindich grafts and Dardik grafts, respectively. While in the distal group, patency rates were 37.5% and 78% in Mindich grafts and Dardik grafts, respectively. The causes of the graft obstruction were as follows: 1) graft infection in 4 (2 with Mindich and 2 with Dardik grafts) 2) pseudointimal hyperplasia of the distal anastomosis in 4 with Mindich graft, 3) progression of atherosclerosis in one case with Dardik graft, and 4) substandard Dardik graft in one. It was concluded that Dardik graft should be used as first choice when compared with Mindich graft and these grafts should not be used easily, since these grafts were apt to be infected.

[Clinical evaluation of a femoral compression method for ischemic legs by transcutaneous oxygen tension measurement].

Transcutaneous oxygen tension (TcPo2) of the lower extremities was investigated in 20 patients of peripheral arterial occlusive disease. TcPo2 was monitored from bilateral calf and dorsum of the foot. Femoral compression method meant that the tourniquet was set at femoral region and the tourniquet was inflated to a suprasystolic pressure for 4 minutes, after cuff was deflated, TcPo2 returned to the initial level. T(min) was the time it took until initial TcPo2 level. T1/2(min) was the time it took until 50 percent of the initial level. The following results were obtained. T1/2 showed significant relation to Ankle Pressure Index. T1/2 of dorsum of foot showed blood flow of distal side of calf region and T1/2 of calf showed blood flow of femoral region. T1/2 was significant difference between ASO and TAO, among different sites of peripheral vascular occlusion. T1/2 measurement was a much more useful indicator of limb flow after operation than TcPo2 measurement.

Operative arteriography: an indicator for infrainguinal bypass.

Preceding infrainguinal artery bypass surgery, a complete arteriographic delineation of the recipient artery is not always feasible. For 13 out of 63 patients with occlusion of the infrainguinal arteries, distinct delineation could not be accomplished with preoperative angiography. However, prebypass operative arteriography was successful in 8 of the 13. In 5, we performed bypass surgery on vessels not visualized on the preoperative angiography. In the other 3, bypass was performed on vessels in which the status of the distal circulation had not been well defined, although the popliteal and infrapopliteal arteries were partially visualized on the preoperative angiography. Intraoperative arteriograms--a technique requiring operative exploration of the target vessel--can significantly improve the visualization of the recipient artery and the distal circulation. Therefore, prebypass operative arteriography may become another indicator for arterial bypass surgery.

Calmodulin-binding proteins in the cytosol extract of sea urchin eggs.

Calmodulin and calmodulin-binding proteins in the cytosol extract of eggs from the sea urchins Hemicentrotus pulcherrimus and Strongylocentrotus intermedius were studied in an attempt to elucidate the physiological role(s) of calmodulin in eggs. Calmodulin in the cytosol extract was found both in a free form and in complexes with other proteins, either Ca2+-dependently or Ca2+-independently. The extracts contained at least three calcium-dependent calmodulin-binding proteins. One was an NAD kinase of unknown molecular composition. The apparent molecular weights of the other two calmodulin-binding proteins were 50K and 55K + 17K daltons, as estimated by polyacrylamide gel electrophoresis in the presence of sodium dodecyl sulfate. These three proteins formed complexes with calmodulin only in the presence of calcium as demonstrated by gel filtration. The 17K-dalton protein was found to be calmodulin itself; it did not dissociate from the 55K-dalton protein regardless of the presence or absence of calcium. The native molecular weights of these protein-calmodulin complexes obtained by gel filtration through a Sephacryl S-300 column were 190K for the NAD kinase, 130K for the 50K-dalton protein and 100K daltons for the 55K + 17K-dalton protein.
