A single pericardial patch technique for repair of partial anomalous pulmonary venous drainage associated with sinus venosus atrial septal defect.

A technique is described for repair of partial anomalous pulmonary venous drainage associated with sinus venosus atrial septal defect. The procedure, using a single autologous pericardial patch, is able to facilitate both reconstruction of the pulmonary venous channel and enlargement of the superior vena cava. This technique also reduces the incidence of arrhythmias.

Assessment of left ventricular function in patients with mitral regurgitation: value of the forward ejection fraction.

This study was aimed at assessing the value of preoperative forward ejection fraction (FEF) and corrected forward ejection fraction (CFEF) as an index of postoperative myocardial function. Utilizing 34 patients with mitral regurgitation, usefulness of the preoperative FEF and CFEF was evaluated with special reference to incidence of postoperative low output syndrome (LOS) and improvement in the New York Heart Association (NYHA) classification, comparing other indices of left ventricular function such as cardiac index (CI), stroke index (SI), left ventricular end-diastolic volume index (LVEDVI) and left ventricular end-systolic volume index (LVESVI). There was a significant correlation between FEF and CI, SI, LVEDVI or LVESVI, and between CFEF and CI, SI, LVEDVI or LVESVI. The FEF and CFEF, however, were better indicators for predicting the occurrence of postoperative LOS than EF. Postoperative improvements in the NYHA classification were parallel to that of the FEF and CFEF changes, but did not coincide with that of the EF. Based on these findings, it is concluded that FEF and CFEF are more useful indicators than EF for evaluation of left ventricular myocardial performance in patients with mitral regurgitation.

Surgical closure of ventricular septal defect in the first year of life: forty-three consecutive successful cases.

Forty-three infants (less than 12 months of age) underwent VSD closure without operative mortality between June 1982 and December 1987. Average age and body weight at the operation were 6.9 months and 5.3 kg, respectively. Associated cardiac anomalies were PDA (11 patients), ASD (5) and PS (1). Preoperative pulmonary to systemic peak pressure ratio (Pp/Ps), resistance ratio and flow ratio, and pulmonary vascular resistance were 0.79 +/- 0.15, 0.29 +/- 0.16, 2.52 +/- 0.60 and 3.05 +/- 1.94 unit.m2, respectively. VSD was closed under combined surface/perfusion hypothermia with total circulatory arrest in 25 patients and standard cardiopulmonary bypass in 18. All patients were discharged from the hospital in good condition, but there was one late death due to pneumonia. Postoperative Pp/Ps decreased to 0.39 +/- 0.11. Most of the patients demonstrated satisfactory body weight gain after VSD closure. These results support our current policy of aggressive surgical intervention for refractory VSD in the early stage of life.

Acquired von Willebrand disease in patients with polycythemia rubra vera.

Platelet function and factor VIII complex were evaluated in ten patients with polycythemia rubra vera. Seven patients showed abnormal epinephrine-induced aggregation. The intracellular concentrations of adenosine diphosphate (ADP) were below normal, and the ratio of adenosine triphosphate (ATP)/ADP was greater than normal. In four of eight cases, there was a decrease in ristocetin cofactor activity and a reduction in the slowly migrating forms of vWF:Ag on crossed immunoelectrophoresis. Defect of large multimers of vWF:Ag was also observed. The ratio of vWF:Ag to ristocetin cofactor was elevated in these patients. Plasma from the patients had no effect on normal plasma except in one case, in which isolated IgG appeared to cause inactivation of ristocetin cofactor. Treatment with 1-deamino-8-arginine vasopressin caused correction of the vWF abnormalities with rapid return of ristocetin cofactor to baseline in some patients. The present study shows that the alterations of multimeric structure of vWF occur in more than 50% of patients with polycythemia rubra vera and are in some part due to the inhibitor specific for vWF.

Topographical relationship between the axonemal arrangement and the bend direction in starfish sperm flagella.

Since starfish spermatozoa have spherical heads, it is not easy to determine the topographical relationship of the axoneme to the directions of the flagellar bends, the principal, and the reverse bends as defined by Gibbons and Gibbons [J. Cell. Biol. 1972, 63:970-985]. The demembranated spermatozoa are known to take the quiescent "cane" shape with a sharp principal bend at the proximal region of the flagellum in the presence of high concentration of Ca2+. When such spermatozoa were placed on a grid for electron microscopy, fixed with osmic acid vapor, washed with distilled water, and negatively stained with uranyl acetate, the head of the spermatozoon was disrupted and dispersed disclosing the proximal centriole at the proximal end of the flagellum. The proximal centriole was always found on the concave side of the "cane"-shaped flagella. Electron microscopy of the serial thin sections of intact and demembranated spermatozoa revealed that the doublet microtubules numbers 5 and 6 were contained in the convex edge of the principal bend.

Acquired von Willebrand disease due to inhibitor of human myeloma protein specific for von Willebrand factor.

A patient with acquired von Willebrand disease associated with multiple myeloma (IgG-lambda) is described. Mixture of his plasma or IgG fraction with washed control platelets resulted in the inhibition of aggregation with ristocetin, but mixture of control plasma or IgG fraction with washed patient platelets showed no inhibition of ristocetin-induced aggregation. Although his vWF: Ag, RCo, and factor VIII coagulant activity were all normal, inactivation of RCo was induced in normal plasma by incubation with patient plasma. Crossed immunoelectrophoretic analysis showed that vWF:Ag was composed of much more anodic component. A marked increase of Factor VIII and a rapid return of RCo to the baseline after 1-deamino-8-arginine vasopressin (DDAVP) infusion were observed. A transient increase in vWF:Ag after the infusion of DDAVP showed with less anodic forms and in the relative proportion as in normal. Treatment of the underlying disease also led to a correction of the bleeding time, improvement of platelet adhesion and ristocetin-induced aggregation, and normalization of crossed immunoelectrophoresis of vWF:Ag. The present study showed that myeloma-associated IgG interacted specifically with the antigenic sites on the von Willebrand portion of the Factor VIII complex.

Acquired von Willebrand's syndrome due to an inhibitor of IgG specific for von Willebrand's factor in polycythemia rubra vera.

A patient with acquired von Willebrand's syndrome associated with polycythemia rubra vera is described. Ristocetin cofactor activity was decreased, while the levels of vWF:Ag and VIII:C were normal. Crossed immunoelectrophoretic analysis showed that vWF:Ag was composed of much more anodic component. The mixture study using pooled normal plasma and the patient IgG fractions showed the inhibition of ristocetin cofactor and the decrease of less anodic parts of vWF:Ag in normal plasma. After 1-deamino-8-arginine vasopressin (DDAVP) infusion the marked increases of vWF:Ag, VIII:C and ristocetin cofactor and a rapid return of ristocetin cofactor to the baseline were observed. Transient increase of vWF:Ag after DDAVP infusion showed less anodic forms and in the relative proportion as normal plasma. The present study showed that the patient IgG fractions had the specific inhibitory activity against the antigenic sites on the active subfractions of von Willebrand's factor.

Effects of gradient centrifugation on human sperm.

The quality of ejaculated human sperm was examined for 24 h at room temperature by Percoll continuous density gradient centrifugation. One hour after ejaculation, apparent densities of good motile sperm with normal morphology were above 1.10 g/ml, whereas those of immotile and/or immature sperm recovered from upper layers were 1.06-1.09 g/ml. As time passed, sperm became immotile and the number of sperm with higher densities was diminished, their density shifting to 1.06-1.09 g/ml. Morphological observations by transmission electron microscope showed that after 24 h the sperm with higher densities were morphologically normal, except that the plasma membrane and the outer acrosomal membrane were lost. On the other hand, the sperm with lower densities were abnormal, such as immature, without the outer and the inner acrosomal membrane, with the nuclear decondensation, and with membraneous structure in the nucleus. These results suggest that after ejaculation the apparent density of human sperm became lower, concomitant with the loss of motility.

Buoyancy and sedimentation of human X- and Y-bearing sperm.

The buoyant and sedimentation behaviors of human X- and Y-bearing sperm were examined in discontinuous density gradients of Nycodenz. The washed sperm placed at the bottom of the discontinuous density gradients of Nycodenz (23-50% in 3% steps) were centrifuged at 250 x g for various periods. The buoyant velocity of X-bearing sperm was faster than that of Y-bearing sperm. The sedimentation profiles of human sperm in the discontinuous density gradients of Nycodenz (4-20% in 2% steps) showed that X-bearing sperm sedimented faster than Y-bearing sperm. The separation of X- and Y-bearing sperm by means of centrifugation usually have been based on the assumption that smaller Y and larger X chromosomes might provide the difference in their apparent densities. The present results suggest that the separation cannot be due merely to a difference in their densities.

Sialic acid in purified human sperm.

Sialic acid levels of human sperm were estimated using newly developed sperm purification procedure (inner column method). This was used to estimate net sialic acid content of forward motile sperm of normal morphology without any contamination of the seminal plasma, other cellular components, and immature or abnormal sperm. The sialic acid content in purified sperm was approximately 40% of that in the washed (dilution-washing method) sperm. It is suggested that biochemical properties of human sperm should be reevaluated using the inner column purification procedure.

Effects of a high-voltage electrical impulse and an anticancer drug on in vivo growing tumors.

Application of a high-voltage electrical impulse (5 kV/cm, 2 msec) after bleomycin administration resulted in a significant size decrease of subcutaneously inoculated AH-109A hepatocellular carcinoma in Donryu rats. The tumor size decreased to an average of 17% of the initial mass 4 days after the treatment. Neither the high-voltage electrical impulse nor bleomycin administration alone showed an inhibitory effect on the tumor growth. It was concluded that the concomitant use of a high-voltage electrical impulse and an anticancer drug has the potential to be applicable for cancer treatment.
