[Effects of flutropium bromide, a new anti-asthma drug, alone or in combination with salbutamol, aminophylline and disodium cromoglycate on the acetylcholine induced bronchoconstriction].

Effects of flutropium bromide, a new bronchodilator with an anticholinergic action, alone or in combination with other antiasthma drugs were investigated in guinea pigs by using an index of inhibition of the acetylcholine (ACh)-induced bronchoconstriction. Single inhalation of flutropium bromide (0.0003%) into the airways of guinea pigs inhibited the ACh (i.v.)-induced bronchoconstriction without changing the fall in blood pressure induced by ACh. When salbutamol (3 micrograms/kg, i.v.), aminophylline (5 mg/kg, i.v.) or disodium cromoglycate (10 mg/kg, i.v.) was administered in combination with flutropium bromide (0.0003%), bronchodilation was enhanced as compared with single administration of the respective antiasthma drugs. From the above results, it is indicated that inhalation of flutropium bromide provides a more efficient bronchodilation in combination with other antiasthma drugs that possess different mechanisms of antiasthma effects.

Purification, properties and determinations of recognition sequence and cleavage site of restriction endonuclease from "Agrobacterium gelatinovorum" IAM 12617, a marine bacterium (AgeI).

A new restriction endonuclease, designated as AgeI, was purified from cell-free extracts of a marine bacterium, "Agrobacterium gelatinovorum" IAM 12617 by streptomycin treatment, ammonium sulfate fractionation, combined column chromatographies on heparin-Sepharose CL-6B and DEAE-Sepharose CL-6B and FPLC on Mono Q (HR 5/5) and Superose 12 (HR 10/30). The purified enzyme was homogeneous on SDS-polyacrylamide gel disc electrophoresis and free from other phosphatase and exonuclease activities on ligation-recutting test. The relative molecular mass of the enzyme was 24,000 daltons by SDS-polyacrylamide gel disc electrophoresis. The gel filtration using Superose 12 (HR 10/30) gave the same calculation (23,000 daltons). These data indicated that the enzyme is a monomer. The isoelectric point of the enzyme was 6.5. The purified enzyme cleaved lambda and Ad2 DNAs at 10 or more and 5 sites, respectively. However, the purified enzyme did not cleave SV40, phi X174 RF I, M13mp 18 RF I or pBR322 DNAs. pBR328 DNA was cleaved at 1 site by the purified enzyme. The purified enzyme worked best at 37 degrees C and pH 7.5 in a reaction mixture (50 microliters) containing 1.0 micrograms lambda DNA, 10 mM Tris-HCl, 7 mM 2-mercaptoethanol, 7 mM MgCl2 and 50 mM NaCl. The purified enzyme did not require monovalent cations necessarily for the enzyme reaction. The enzyme recognized the palindromic hexanucleotide DNA sequence 5'-ACCGGT-3' and cut between A and C, producing a 5'-cohesive tetranucleotide extension.

Purification and properties of restriction endonuclease from Deleya marina IAM 14114, a marine bacterium (DmaI).

A restriction endonuclease, designated as DmaI, was purified from cell-free extracts of Deleya marina IAM 14114 by streptomycin treatment, ammonium sulfate fractionation and two steps of chromatographies on heparin-Sepharose CL-6B and Mono Q (HR 5/5, FPLC). The purified enzyme was homogeneous on SDS-polyacrylamide gel disk electrophoresis and a ligation-recutting test. The relative molecular mass measurements of the purified enzyme gave 28,000 daltons by SDS-polyacrylamide gel disk electrophoresis and 56,000 daltons by gel filtration. These data indicated that the purified enzyme (56,000 daltons) has a dimeric structure composed of two 28,000-dalton subunits. The isoelectric point was 5.5. The purified enzyme worked best at 37 degrees C in a reaction mixture (50 microliters) containing 1.0 micrograms lambda DNA, 10 mM Tris-HCl, 7 mM 2-mercaptoethanol, 7 mM MgCl2 and 100 mM NaCl (pH 7.5). The enzyme was stable up to 55 degrees C and between pH 7.0 and 9.0. The purified enzyme recognizes the palindromic hexanucleotide DNA sequence 5'-CAGCTG-3', cuts between G and C and produces a flush end (isoschizomer of PvuII).

[Clinical evaluation of cefbuperazone in severe infections complicated with hematological disorders].

The efficacy and the safety of an antibiotic in cephamycin group, cefbuperazone (CBPZ), were investigated in 93 patients with severe infections complicated with hematological disorders. The efficacy evaluation was made in 85 cases with underlying hematological disorders including 49 cases (57.6%) of leukemia and 18 cases (21.2%) of malignant lymphoma. The overall efficacy rate was 50.6% of the 85 evaluable cases. The clinical efficacy rate for sepsis and suspected sepsis was 53.4%. The most frequently used group of antibiotics for combination therapy was aminoglycosides in 37 cases, in which an efficacy rate of 62.2%, a higher rate than the efficacy rate of 48.5% for all the combination therapy cases, was obtained. In 16 cases in which penicillins were used as combination drug, the efficacy rate obtained was low, 31.3%. Efficacy rates obtained for cases with different neutrophil counts at the start of therapies were as follows: 52.2% in 23 cases with neutrophil counts below 100/mm3, 46.2% in 13 cases with neutrophil counts between 100 and 499/mm3 and 51.3% in 39 cases with neutrophil counts equal to or above 500/mm3, thus no significant differences in efficacy rates were observed for patients with different neutrophil counts. These results appear to suggest that CBPZ, alone or in combination with other antibiotic such as aminoglycosides, may be quite useful in the treatment of severe infections in patients with hematological disorders.

[Usefulness of rest-redistribution on thallium myocardial scintigraphy in patients with acute myocardial infarction by SPECT: analysis by bull's eye and unfolded map images].

The purpose of this study was to clarify the clinical significance of rest-redistribution in myocardial scintigraphy (SPECT) in acute myocardial infarction (AMI). Thirty patients with AMI within one week after the onset of attack were studied. SPECT images were obtained 10 min and three hours after injection of 201Tl. Bull's eye images and unfolded map images were prepared. A 201Tl uptake was studied at the infarct and non-infarct sites. Exercise SPECT and radionuclide angiography (RNA) were performed in all patients one month after the onset of AMI, and the findings were compared with clinical and coronary angiographic (CAG) findings. 1. Redistribution of thallium at rest was observed at the infarct sites in nine of the 30 patients. 2. Redistribution at rest was observed at the non-infarct sites in eight patients. 3. Redistribution at rest was observed during exercise SPECT one month after the onset of AMI in patients with redistribution at rest in the acute phase. 4. In patients with redistribution at rest at the infarct site, left ventricular ejection fraction (EF) improved one month after the onset of AMI (delta EF greater than 5%), but it decreased slightly during exercise. 5. Wall motion at the infarct site was not much impaired in patients who showed redistribution at rest at the infarct site. 6. Angina pectoris and recurrence of myocardial infarction were observed more frequently in those with redistribution at rest on SPECT. 7. No characteristic findings were obtained on CAG in those with redistribution at rest.(ABSTRACT TRUNCATED AT 250 WORDS)

[A long-term study on dentofacial changes after sagittal splitting osteotomies in skeletal Class III malocclusions].

The purpose of this study is to clarify the long-term dentofacial changes after sagittal splitting ramus osteotomy in skeletal Class III patients, and to study the factors involved to find its countermeasures. For this purpose we used the radiographic cephalograms in 16 patients (13 females and 3 males) observed for more than five years after surgery. We measured the postoperative changes in the position of Pogonion and the inclination of maxillary and mandibular incisors. The results were as follows: 1. Pogonion was retruded 9.3 mm on the average by surgery. In the period from the removal of intermaxillary fixation to one year postoperatively Pogonion slipped forwardly and upwardly (average; anteriorly 1.0 mm, superiorly 0.8 mm). In the period from one year postoperatively to the time of the final observation Pogonion slipped to a downward direction. 2. No correlation was found between the amounts of vertical displacement by surgery and postoperative vertical changes of Pogonion. On the other hand, a high correlation was found between the amounts of mandibular setback and the forward displacement of Pogonion. 3. Mandibular incisors were tipped labially by preoperative orthodontic treatments, then maxillary and mandibular incisors moved lingually during intermaxillary fixation. After release of intermaxillary fixation, maxillary incisors moved labially in a small distance. After one year maxillary incisors were inclined slightly in the lingual direction. It is obvious that the position of the mandible and the incisor inclination tend to change even after one year. Because the postoperative changes were small in quantity, we supposed they were mostly caused by occlusal changes. Clinically, occlusal relationship in the incisor area, i.e., overjet and overbite, was maintained to be normal. It was considered that the changes of incisal inclination after release of intermaxillary fixation were of a compensatory or adaptational response to a positional change of the mandible.

[Effects of oral administration of various non-steroidal anti-inflammatory drugs on bone growth and bone wound healing in mice].

Morphometric and microdensitometric studies on normal bone growth and bone wound healing in growing ICR strain male mice were carried out as preliminary investigations. Then the effects of oral administration of non-steroidal anti-inflammatory drugs on bone growth and bone wound healing were studied. Femur length and width, and bone mineral contents in the femur as judged by microdensitometry of the soft x-ray films of the cortical portion of the femur showed time-dependent linear increase between 3 and 6 weeks of age. Wound holes were made in the parietal bones of mice with a dental bar at 4 weeks of age, and uncalcified area in the holes was determined by image analysis of soft x-ray films. The area decreased their size with the advance in age, especially a marked decrease was observed 2 weeks after the operation. From the above results, 4 week-old male mice were employed and 8 kinds of non-steroidal anti-inflammatory drugs (indomethacin, 1 mg/kg; mefenamic acid, 10 mg/kg; flufenamic acid, 10 mg/kg; diclofenac sodium, 1 mg/kg; ibuprofen, 10 mg/kg; naproxen, 10 mg/kg; phenylbutazone, 10 mg/kg and aspirin, 50 mg/kg) and dexamethasone (1 mg/kg), a steroidal anti-inflammatory drugs were administered orally every day for 9 or 11 days except Sunday, and 15 days after the initial administration, the length and width of the femur were measured. The bone mineral contents were determined by microdensitometry of the soft x-ray film of the femur. Hydroxyproline contents of the femur were also determined. Wound holes were made in the parietal bones at 4 week-old and administered the drugs from one day before the operation for 9 days and the uncalcified area in the wound hole was determined 2 weeks after the operation with an image analyzer. The results obtained were as follows: 1. A significant inhibition of the growth in length of the femur was observed by dexamethasone, diclofenac sodium and naproxen administration for 9 and 11 days. A significant inhibition of the growth in width was also observed by dexamethasone and diclofenac sodium administration for 9 days and by dexamethasone administration for 11 days.(ABSTRACT TRUNCATED AT 400 WORDS)

[Weekly CHOP chemotherapy in the treatment of intermediate-grade non-Hodgkin's lymphomas--cooperative group study by seven institutes].

Between 1985 and 1988, 49 previously untreated patients with intermediate-grade non-Hodgkin's lymphoma (LSG classification large cell 35 including 11 large cell immunoblastic by Working Formulation, medium-sized cell 7, mixed 7) were treated with the Weekly CHOP regimen (three successive weekly administration of cyclophosphamide, doxorubicin, vincristine and prednisolone) as a cooperative group study by seven institutes (Nagoya Lymphoma Study Group). Complete remission was achieved in 63.3% with Weekly CHOP alone and finally in 79.6% after the addition of radiotherapy and/or combination chemotherapies including etoposide, methotrexate, procarbazine, bleomycin. Patients with T cell phenotype, high grade PS and the presence of bulky mass had significantly lower rates of CR. After a median follow-up 36 months Kaplan-Meier estimates showed that overall survival was 60.4%, disease-free survival 51.4% and relapse-free survival 64.6%. The major toxicities were alopecia, leukopenia, infection, neuropathy and gastrointestinal symptoms. No treatment-related deaths were observed. Survival was adversely affected by high LDH level, poor PS, T cell phenotype, the presence of B symptoms and the bulky mass. But these characteristics gave no significant effects on relapse rate and relapse-free survival. Thus, Weekly CHOP is an effective treatment for intermediate-grade NHL.

New crystal form of recombinant murine interferon-beta.

Although we have reported (Matsuda, S., Kawano, G., Itoh, S., Mitsui, Y., and Iitaka, Y. (1986) J. Biol. Chem. 261, 16207-16209) that recombinant murine interferon-beta produced in Escherichia coli was crystallized in an orthorhombic space group C222(1) using polyethyleneglycol 8000 as precipitant, the crystals had an insufficient resolution and a marked tendency for orientational disorder around the c axis. We now report that another form of murine interferon-beta crystals with little disorder was obtained in the presence of dioxane using ammonium sulfate as precipitant. The new crystals belong to a hexagonal space group P6(1) or P6(5) with a = b = 71.4 A and c = 79.6 A having only one murine interferon-beta molecule in an asymmetric unit. The crystals are reasonably stable to x-rays and significantly diffract up to 2.2 A resolution when a synchrotron beam is used.

Mode of binding of E-64-c, a potent thiol protease inhibitor, to papain as determined by X-ray crystal analysis of the complex.

The three-dimensional structure of the E-64-c-papain complex has been determined by X-ray crystal analysis at 2.5 A resolution (conventional R = 26.9%). The structure determined indicates that: (i) the C2 atom of the oxirane ring of E-64-c is covalently bound by the S gamma atom of Cys-25 of papain; (ii) this covalent bond formation results in a configurational conversion of the oxirane C2 atom from the S- to the R-form; and (iii) extensive hydrogen bonding and hydrophobic interactions are responsible for the specific interaction of the E-64-c molecule with papain.

[MR imaging of the bone marrow using short TI IR. Part 1--Normal and pathological intensity distribution of the bone marrow].

Normal vertebral bone marrow intensity distribution and its alteration in various anemias were evaluated on short TI IR sequences. Material consists of 73 individuals, 48 normals and 25 anemic patients excluding neoplastic conditions. All normal and reactive hypercellular bone marrow revealed characteristic intensity distribution; marginal high intensity and central low intensity, corresponding well to normal distribution of red can yellow marrows and their physiological or reactive conversion between red and yellow marrows. Aplastic anemia did not reveal normal intensity distribution presumably due to autonomous condition.

[MR imaging of the bone marrow using short TI IR. Part 2--Normal and pathological intensity distribution of the bone marrow].

Bone marrow pathology of hematological neoplasms was evaluated using short TI IR sequences and T2 weighted STIR (IR 1500/100/40, IR 1500/100/80). Material consisted of 49 individuals including 10 leukemias, 5 myelofibroses and 30 multiple myelomas. On IR 1500/100/40, all cases revealed heterogenously or diffusely high intensity. On IR 1500/100/80, both of acute and chronic myelocytic leukemias revealed definite signal decrease, while chronic lymphocytic leukemia, myelofibrosis and multiple myeloma showed no obvious signal decrease.

Prediction of the conformation of ice-nucleation protein by conformational energy calculation.

The active conformation of an ice-nucleation protein, whose major portion consists of a long polypeptide segment of nearly repetitive octapeptides, is predicted by the analyses of conformational energy and the mechanism of crystal growth. The protein ideally has an exact octapeptide repetition and is assumed to have a helical conformation. The present study searched for low-energy helical conformations and each of the obtained low-energy conformations examined as to whether it has a surface structure that can promote crystal formation. Two conformations obtained were good candidates for an ice nucleus. Both were found to have on their surfaces an arrangement of hydrogen-bonding sites, which fits well with those of hydrogen bonds in hexagonal ice crystal. Further, one of the two conformations had a hexagonal conformational symmetry consistent with the hexagonal ice crystal structure. The other conformation had a pentagonal conformational symmetry that could enable the growth of an ice crystal--dendritic polycrystalline snow crystal--which grows on metastable cubic ice.

Kinetics of cis-dichlorodiammine platinum (II) during plasmapheresis.

The pharmacokinetics of cis-dichlorodiammineplatinum (II) (cisplatin) during double filtration plasmapheresis (PP) have been studied in ten patients who received the drug as a 30 min infusion. The patients receiving the drug exhibited biphase clearance of total platinum with a rapid initial phase (40 min) and a prolonged second phase (1-24 h). Free (ultrafiltrable) cisplatin was readily detectable and the clearance curve of free cisplatin was biphasic, with a short initial phase (1 h) and a long second phase (1-24 h). In conclusion, the loss of both total cisplatin and free cisplatin in serum were not found during plasmapheresis.
