Characterization of hemopoietic precursor cells in juvenile-type chronic myelocytic leukemia.

In order to study the pathogenesis of juvenile-type chronic myelocytic leukemia (CML), we examined the colony-forming capacity and colony composition in the bone marrow (BM) and peripheral blood (PB) of three children with juvenile-type CML. Large numbers of granulocytes and macrophage colonies were formed by BM and PB cells. Whole agar culture staining revealed that especially macrophage colonies increased in comparison with normal controls. After removal of carbonyl iron-laden cells with a magnet or deprivation of cells adherent to glass from BM cells, the number of macrophage colonies markedly reduced in comparison with the number of colonies formed by untreated BM cells, suggesting that some of the macrophage colony-forming cells (M-CFC) may have phagocytic and/or adherent activity. Radiation sensitivity and thymidine suicide rate of these M-CFC were not different from those of granulocyte colony-forming cells (G-CFC). The predominance of M-CFC in juvenile-type CML may be one of the reflections of fetal-type myelopoiesis since M-CFC are predominant in cord blood and PB in the neonatal period. Moreover, considerable numbers of erythroid-colony-forming units (CFU-E) were present in PB of all patients. It may be concluded that juvenile-type CML is a panmyelopathy with the predominance of M-CFC.

Proliferative state and radiosensitivity of human myeloma stem cells.

Human myeloma stem cells were detected by their capacity to form colonies in culture. Cells separated from aspirated marrow were cultured for 10 days in semi-solid methylcellulose with medium conditioned by T lymphocytes stimulated by phytohaemagglutinin (PHA-TCM). The colonies formed consisted mostly of lymphoplasmacytoid cells or plasma cells, and the immunoglobulins in the patients' myeloma cells were demonstrated also in the cytoplasm of the colony cells. The number of colonies were proportional to the number of cells plated and to the concentration of PHA-TCM. When the proportion of proliferating colony-forming units of multiple myeloma (CFU-MM) was studied using the (3H)-dT-suicide technique, the high-specific-activity (3H)-dT killed 21-45% of the CFU-MM in 7 myeloma patients. According to a single dose of Co-y-irradiation, the mean doses for impairment of regeneration (Do) were 1.00 and 1.63 Gy in 2 cases, the extrapolation numbers being 1.6 and 2.0.

Vindesine receptors in cells of a human leukaemia cell line.

To determine whether vindesine receptors are present in human leukaemic cells, K562 cells (established from chronic myelogenous leukaemia in blastic crisis) were incubated with 3H-vindesine. Binding of 3H-vindesine increased with incubation time and with increase in number of K562 cells. However, when excessive amounts of nonradioactive vindesine were added, the 3H-vindesine was displaced. Binding of 3H-vindesine was only inhibited by vinblastine, vincristine and vindesine. These results suggest that K562 cells have receptors for vindesine and that these receptors are common to vinca alkaloids. Scatchard analysis showed that the number of vindesine receptors differed according to the kind of cells tested. K562 and a T-cell leukaemia-derived cell line, MOLT-4, had more receptors than an acute promyelocytic leukaemia-derived cell line, HL-60, and normal blood lymphocytes. The degree of vindesine affinity to receptors did not differ markedly among the above-mentioned cells.

Giant renal artery aneurysm in a child.

A case of giant aneurysm arising from the right main renal artery of a 6-year-old boy with severe renin-dependent hypertension is presented. As simple aneurysmectomy was hardly performed because of tight adhesion to the surrounding tissues, the aneurysm was removed with the kidney. The blood pressure was normalized 10 days after surgery. The size of the aneurysm was 8.0 cm in diameter. This is the biggest one among the child cases reported so far.

Characteristics of granulocyte-macrophage colony formation in patients with chronic myelomonocytic leukemia.

3 patients with chronic myelomonocytic leukemia (CMMoL) were investigated using a culture technique in vitro for granulocyte and monocyte/macrophage precursors (GM-CFC). Time course studies revealed that the growth of colonies and clusters was delayed in CMMoL as compared with that in normal control bone marrow. In 1 of 3 cases of CMMoL, the number of GM colonies increased after removal of phagocytes. Chloroacetate esterase and nonspecific esterase activities in the colonies and clusters were examined by means of our improved cytochemical technique. When we classified colonies and clusters into granulocytic, monocyte/macrophagic, myelomonocytic and mixed type, the incidence of "myelomonocytic' colonies and clusters was significantly higher in two cases of CMMoL than in the normal controls. On the basis of these results, the characteristics of abnormal hemopoietic regulatory mechanisms were discussed.

[Experimental and clinical studies of cefmenoxime in the field of obstetrics and gynecology].

The study group was organized to evaluate the usefulness of cefmenoxime (CMX) injection, a new synthetic cephalosporin, for the treatment of infections in the field of obstetrics and gynecology. Fundamental and clinical studies were made by the society and the following results were obtained. 1. The peak distribution of CMX's MIC for E. coli, Klebsiella sp., Enterobacter sp., Bacteroides sp. and Peptococcus sp. isolated from obstetrical and gynecological infections with relatively high frequencies area 0.1, less than or equal to 0.05, 0.2, 3.13, 1.56 micrograms/ml, respectively, with an inoculation of 10(6) cells/ml. 2. When 1 g of CMX is administered by intravenous drip infusion for 1 hour, the maximum concentrations in various tissues of female genital organs were as follows: 14.2 and 13.2 micrograms/g in ovary and oviduct, respectively, at 1.20 hours after the start of administration, and 16.9 and 26.3 micrograms/g in corpus uteri and cervix uteri, respectively, after 1 hour. As for the transfer to the exudate in the pelvic dead cavity, the peak concentration was 15.6 micrograms/ml after 2.13 hours. 3. In the clinical studies, CMX was given to 258 cases with female genital organ infections and others. As for the clinical effects, with exclusion of 3 cases in which other antibiotics are concomitantly used, responses were excellent in 76 cases, good in 162 cases and poor in 17 cases, among 255 cases in total. The efficacy rate was 93.3%. The efficacy rates by diseases were 97.1% (68/70) for intrauterine infections, 88.8% (79/89) for intrapelvic infections, 98.4% (62/63) for adnexitis, and 100% (23/23) for infections of external genital organs. As for the clinical effects on causative bacteria, the efficacy rates were 100% (19/19) for single infections due to Gram-positive bacteria, 94.8% (55/58) for single infections due to Gram-negative bacteria, and 88.2% (15/17) for single infections due to anaerobic bacteria. And its efficacy rates were 89.6% (69/77) for mixed infection cases. Side effects were observed in 2 cases (0.8%); 1 case with eruption, and 1 case with diarrhea and vomiting. As for abnormal laboratory findings, lower white blood cell count was observed in 2 cases and elevation of the values regarding hepatic functions in 9 cases. All cases were returned to the normal after the completion of the administration. Cefmenoxime showed a satisfactory clinical efficacy and a potent bacteriological effect in treatment of the infections in the field of obstetrics and gynecology, and it has been concluded that cefmenoxime will be useful addition to the antibiotics for the therapy of these infections.

Hemopoietic precursor cells in erythroleukemia.

In order to study the changes in erythroid precursor cells in erythroleukemia, bone marrow cells from 4 patients were cultured for erythroid colony-forming units (CFU-e). The bone marrow from 3 of the patients showed an excess of ringed sideroblasts, and the incidence of CFU-e was very low. The one patient without ringed sideroblasts showed abundant erythroid colony formation. Erythroid colonies of more than eight erythroblasts consisted of normal-appearing erythroblast, while ringed sideroblasts were observed in scattered erythroblasts or in small clusters. The number of granulocyte macrophage colony-forming units (CFU-GM) was markedly lower than normal in all 4 cases. In 2 cases investigated, blast colonies were formed from bone marrow cells under the stimulation of phytohemagglutinin/leukocyte-conditioned medium. These results show that the differentiation of hemopoietic stem cells to erythroid as well as to myeloid cells is affected in erythroleukemia.

Hemopoietic stem cells in nude mice transplanted with colony-stimulating-factor-producing tumors.

When pieces of a human pulmonary carcinoma which was producing colony-stimulating factor (CSF) were transplanted into nude mice a marked granulocytosis of 10(6)/mm3 was observed in the nude mice 4 weeks after the transplantation. In order to elucidate the control mechanism of hemopoiesis, we studied the kinetics of the hemopoietic stem cells of these mice. The number of GM-CFC, CFUE, Meg-CFC and CFUS in the whole body increased to 6 times, 8 times, 6 times and 36 times as much respectively as those of the control mice. The increase in the number of these progenitors occurred mainly in the spleen. The sera of these mice contained only CSF. The cycling fraction of GM-CFC and CFUS determined by thymidine suicide technique was increased. From these findings, it could be considered that CSF produced in these mice acted directly or indirectly on CFUS inducing their differentiation into committed stem cells.

Hemopoietic colony-forming cells in Shwachman's syndrome.

Colony-forming capacities were studied in three Japanese children with Shwachman's syndrome (chronic neutropenia and exocrine pancreatic insufficiency). Bone marrow granulocyte-macrophage colony-forming cells assayed in a soft agar culture were markedly reduced in all three cases. The cytochemical examination of granulocyte-macrophage colonies by a new technique revealed that 90% of the colonies by a new technique revealed that 90% of the colonies consisted exclusively of granulocytes. Erythroid colony-forming cells assayed in a plasma clot culture were significantly reduced in two of the three cases. Bone marrow phagocytic cells did not suppress granulopoiesis in contrast to the cases of idiopathic aplastic anemia. Moreover, the patient serum did not inhibit granulopoiesis of normal bone marrow cells. These results have been discussed with the possibility of involving the hemopoietic stem cells and other additional factors.

[Assessment of follicular development and ovulation by ultrasound].

The relation between ultrasonographic signs of ovulation and serum estradiol (E2) rise and luteinizing hormone (LH) surge were studied. Forty-eight cycles were examined in Twenty-eight infertile patients. The correlation between the follicular diameter as measured by ultrasound and actual follicular diameter under direct visualization was also determined in 19 follicles of the 25 patients who had undergone laparotomy. In 34 of the 48 cycles, growing follicles and follicular rupture could be observed by ultrasound (72.3%). The mean diameter of the follicles measured the day before estimated ovulation was 22.4 +/- 2.75 mm(range 16-27mm). The ultrasonographic ovulation period (UOP) occurred within 24 hours from the day of maximal LH levels in 24 of the 34 cycles, and within 48 hours from the day of the E2 peak in 25 of the 34 cycles. The follicular diameter measured by ultrasound was greater than the actual diameter found during laparotomy, but there was a high degree of correlation (r = 0.900). Furthermore, the E2 and P levels in follicular fluid were high in large follicles and correlated well with the ultrasound assessments of follicular size. These results suggested that ultrasonic monitoring provides a reliable measure of follicular growth and prediction of ovulation, and offers a highly sophisticated scientific tool for the study of reproductive endocrinology.
