Immunohistochemical microquantification of fast-myosin in frozen histological sections of mammalian skeletal muscles.

Fast-myosin in frozen histological sections was quantified by an immunohistochemical micromethod based on the ELISA. Frozen tissue sections mounted on glass slides were used analogously to the antigen-precoated wells of ELISA plates. The intensity of immunoreactivity of frozen sections to an anti-fast-myosin monoclonal antibody was quantified directly from the color developed with the second antibody coupled with peroxidase using phenol-4-aminoantipyrine as a substrate. Fast-myosin levels in the masseter muscles of pigs, rats, and rabbits were 185 +/- 6, 223 +/- 9, and 178 +/- 12 mg/g of total protein, respectively, and those in the gastrocnemius muscles from cows, pigs, goats, rats, and rabbits were 172 +/- 12, 211 +/- 7, 177 +/- 9, 211 +/- 10, and 205 +/- 10 mg/g, respectively. In the masseter of cows and goats, fast-myosin was not detected. The results obtained by this immunohistochemical micromethod were in good agreement with those obtained by histomorphometrical and biochemical analyses. This immunohistochemical micromethod could be used to quantitatively evaluate the muscle contractile characteristics that determine meat quality.

[Combination therapy with arbekacin and fosfomycin against postoperative severe mixed-pneumonia of MRSA in primary lung cancer patients].

We experienced successful treatment of postoperative severe pneumonia of Methicillin-resistant Staphylococcus aureus (MRSA) with combination therapy of Arbekacin (ABK) and Fosfomycin (FOM) in three lung cancer patients. Case 1 was a advanced age of seventy-nine man who had had right upper lobectomy. Case 2 was a 61-year-old man who had had left lower lobectomy and extended bilateral mediastinal lymph-node dissection through the median sternotomy. And case 3 was a 59-year-old man who had suffered from pulmonary embolism after right pneumonectomy and partial resection of left atrium and superior vena cava. All cases were immuno-compromised patients and super-infected with Gram-negative rods, and Pseudomonas aeruginosa in case 1 and case 3. Clinical symptoms were improved after the start of administration of ABK and FOM inspite of ineffectiveness of prior treatment with other antibiotics. We added staggered chemotherapy of Sulbactam/Cefoperazone (SBT/CPZ) and Ceftazidime (CAZ) for case 1 and case 3 respectively. Thus, the combination therapy of ABK and FOM might be useful for severe pneumonia of MRSA in the immunocompromised patients, and the combined staggered chemotherapy of beta-lactum agents and above would be the first choice in the treatment for the case involving Pseudomonas aeruginosa.

[A case of diffuse malignant mesothelioma diagnosed by thoracoscopic biopsy].

We report a case of diffuse malignant mesothelioma discovered by thoracoscopic examination undertaken while formulating a preoperative diagnosis. A 61-year-old male complained of coughing and sputum production, and was admitted because his chest roentgenogram indicated an abnormal shadow. Chest CT scanning demonstrated right pleural effusion and multiple nodules sited on the diaphragm. Because aspiration biopsy of such nodules was difficult, we performed a thoracoscopic examination. The thoracoscopic findings demonstrated bloody pleural effusion in the right thoracic cavity and multiple nodules on the diaphragm, parietal and pulmonary pleura. They were white and 0.5 to 2 cm in size. By excisional biopsy, these were diagnosed as epithelial-type diffuse malignant mesothelioma. We therefore performed panpleuropneumonectomy. Even though we completely resected the canal of thoracoscope and drainage tube, microscopic findings showed invasion of mesothelioma cells into this thoracoscopic canal. Although we suggest that, in preoperative diagnosis of pleural tumor, thoracoscopy is efficient to perform a pathological examination and confirm the extension of tumor, in the actual operation, we believe that the thoracoscopic canal should be resected.

[Hyperbaric oxygen treatment for macular edema after retinal vein occlusion--fluorescein angiographic findings and visual prognosis].

It has been reported that hyperbaric oxygen treatment improves visual function in patients with chronic macular edema associated with retinal vein occlusion. This study was designed to investigate relationship between visual prognosis and characteristics of macular edema. From fluorescein angiography performed prior to the therapy, areas of capillary closure and fluorescein leakages were quantitatively evaluated within 1 disc diameter from the fovea. In 27 eyes for which good angiograms were available, 19 eyes were classified as having mild ischemia 1 (area of capillary closure < 25%) and 8 eyes were classified as having severe ischemia (closure area < or = 25%). Of the 27 eyes, 14 showed visual improvement after the therapy. While only one eyes (13%) had visual improvement in the severe ischemic group, 11 eyes (58%) showed visual improvement in the mild ischemic group. The degree of fluorescein leakage before the therapy had no effect on visual outcome. The degree of macular ischemia was found to be a more significant factor influencing visual prognosis after the treatment than the degree of macular edema.

[A case report of intralobar sequestration associated with lung aspergillus].

A 21-year-old woman with fever and dyspnea was admitted to our hospital. X-ray examination of this patient showed abscess forming in the right lower lobe of the lung. Aspergillus fumigatus was detected by percutaneous lung biopsy. She had repeated pneumonia since her childhood, so congenital lung disease was suspected. Aortic angiography showed the flow of an aberrant artery originated from the right inferior phrenic artery into the right lower lobe. Preoperative diagnosis of the patient was lung sequestration associated with aspergillosis. The operative procedure included right lower lobectomy with ligation of the aberrant artery. Postoperative diagnosis was intralobar sequestration of Pryce III type which was infected with aspergillus. Intralobar sequestration associated with aspergillosis is very rare, perhaps this is the third case in the literature of Japan.

[Evaluation of neck lymph node dissection and extended lymphadenectomy through a collar incision and median sternotomy for lung cancer].

Since 1983, 421 patients have been treated for lung cancer at this institute. Since 1988, neck lymph node dissection (11 cases) and new extended lymphadenectomy through a collar incision and median sternotomy (22 cases) have been conducted. Indications for this new radical operation are scalene, supraclavicular or highest mediastinal node involvement, or superior pulmonary sulcus carcinoma, in patients aged 70 or less without distant metastasis and NSCLC. No major complications and operative mortality were encountered in this study. Patients with scalene or supraclavicular node involvement showed poor prognosis. Postsurgical local recurrence was frequent. Whether resection in N3 disease should be conducted or not, remains a point of controversy. The authors consider that lymphadenectomy should be conducted more extensively. A significant better survival of N2 disease and satisfactory prognosis of patients without metastasis of cervical lymph nodes demonstrates the effectiveness of neck lymph node dissection in the present superradical operation for lung cancer.

[Activities of antipseudomonal agents against clinical isolates of Pseudomonas aeruginosa].

Using clinically isolated 114 strains of Pseudomonas aeruginosa that were collected from April to October 1994, activity of antipseudomonal agents against these organisms was determined using the method of liquid microdilution. In addition, antimicrobial activities of the agents were graded according to serological groups of organisms. The results of this study are summarized as follows. 1. Many strains of P. aeruginosa were isolated mainly from sputum, pus and urine. 2. Serological group G organisms of sputum origin, group I of pus and bile origin, and group E of urine origin were isolated most frequently. 3. The most powerful antipseudomonal agent was cefclidin. Its MIC50 and MIC90 were 0.78 and 6.25 micrograms/ml, respectively. The second most powerful agent was ciprofloxacin whose MIC50 and MIC90 were 0.39 and 12.5 micrograms/ml, respectively. 4. The proportions of resistant strains ranged from 0.9% for cefclidin to 40.4% for ofloxacin. The antipseudomonal agents to which 30% or more of strains were resistant were cefpirome, gentamicin and ofloxacin. 5. Cefclidin showed the most powerful activity against strains that were resistant to ceftazidime, imipenem, gentamicin and ofloxacin. Its MIC90 against all strains resistant to ceftazidime, gentamicin and ofloxacin was 6.25 micrograms/ml. The MIC90 of cefclidin and tobramycin against imipenem-resistant strains was 3.13 micrograms/ml. 6. Group E organisms were found among strains resistant to ceftazidime, gentamicin and ofloxacin at high rates, but no group E strains were found among imipenem-resistant organisms. 7. Agents with highest activities by serological group of organisms were cefclidin against group A, tobramycin and ciprofloxacin against group B, imipenem against group E, ciprofloxacin against group G, and cefclidin and ciprofloxacin against group I.(ABSTRACT TRUNCATED AT 250 WORDS)

[Anesthetic management of Nd-YAG laser surgery in the airway using a larengeal mask].

We used a laryngeal mask for Nd-YAG laser operations of airway stenosis due to tracheal granulation, broncheal granulation and subglottic tumor. Anesthesia was induced with inhalation of N2O-O2-sevoflurane in two cases and intravenous thiopental in one case. The laryngeal mask was inserted with intravenous injection of vecuronium. Anesthesia was maintained with N2O-O2-sevoflurane with intermittent positive pressure ventilation. Laryngeal mask airway was stable with respect to circulation and respiration during Nd-YAG laser irradiation. Ignition of a laryngeal mask caused by Nd-YAG laser is much less common compared to that of a tracheal tube with cuff. Therefore, we recommend to use laryngeal mask airway for Nd-YAG laser operation of airway stenosis because of much less danger of ignition and because it keeps good cardiovascular and respiratory conditions.

Infrequent somatic mutations of the p16 and p15 genes in human bladder cancer: p16 mutations occur only in low-grade and superficial bladder cancers.

A recently identified gene, p16, located on chromosome 9p21, has been shown to be deleted and/or mutated in various types of human cancers. To investigate structural alterations of p16 and a neighboring gene, p15, we examined human bladder cancers for mutations in the entire coding region of these genes using polymerase chain reaction and single-strand conformational polymorphism analysis. Of 50 samples obtained from patients with bladder cancer, 3 (6%), all low-grade and superficial tumors, were found to have p16 gene alterations. The alterations included 1 missense mutation and 2 single-base deletions. We found no p15 gene mutations in these 50 bladder cancers. Our results suggested that p16 gene mutations, although they occurred at low frequency, are involved in some low-grade and early stage bladder cancers.

Intracellular multiplication of Legionella pneumophila in Tetrahymena thermophila.

It has been shown that Legionella pneumophila proliferates intracellularly in more than ten species of protozoa, but the fate of the bacteria in Tetrahymena thermophila has not been reported. We investigated the multiplication of L. pneumophila Philadelphia-1 strain in micronucleated T. thermophila, and the effects of temperature and numbers of the bacteria ingested by the protozoa after in vitro feeding were studied. T. thermophila preyed actively upon the bacteria. After being ingested, the fate of the bacteria was affected by both temperature and the number of bacteria ingested. When the number of ingested bacteria was 30 per protozoon, the bacteria proliferated intracellularly at 35 degrees C. The bacteria, however, could not proliferate at 28 degrees C or 32 degrees C though they survived in the protozoa. When the ingested bacteria was 10 per protozoon, the bacteria were killed in the protozoa at all of the temperatures tested. Electron microscopic examination revealed that the protozoa ingesting the bacteria remarkably swelled and that protozoan food vacuoles which contained L. pneumophila were studded with ribosomes.

A morphometric study of spermatogenesis in the testes of mice of a senescence accelerated strain.

The morphometric parameters of spermatogenic cells in a mouse strain prone to accelerated senescence (SAM-P), a novel murine model of spontaneously promoted aging, were compared with those of a SAM resistant strain (SAM-R) after birth until 40 weeks (mean life span of SAM-P). A mixture of gonocytes and spermatogonia were present in the testis in 1-week-old mice, and no gonocytes were observed in 2-week-old mice. At 6 weeks of age, the absolute number of spermatogonia in SAM-P was 27% greater than that in SAM-R, whereas the cell number in 40-week-old SAM-P was 17% less than in SAM-R. Primary spermatocytes were first observed in 3-week-old animals, and the cell numbers in SAM-P at 3, 5 and 6 weeks were 78%, 31% and 25%, respectively, greater than in SAM-R, whereas the cell number in SAM-P at 40 weeks was 30% less than SAM-R. Round spermatids were first observed in all SAM-P at 4 weeks old, but 20% of SAM-R had no spermatids and the rest had only a few. At 5 and 6 weeks old, the absolute numbers of round spermatids in SAM-P were 38% and 41%, respectively, greater than in SAM-R, whereas the cell number in 40-week-old SAM-P was about 34% less than SAM-R. These results indicate that testicular maturation begins at an earlier age in SAM-P than SAM-R. Furthermore, at the age of 40 weeks signs of testicular deterioration are evident in SAM-P mice only.

Molecular cloning of the murine homologue of CD63/ME491 and detection of its strong expression in the kidney and activated macrophages.

The mouse homologue of CD63/ME491 (Mu-CD63) was molecularly cloned and analyzed. Mu-CD63 exhibited a strikingly high similarity to CD63/ME491 and the rat homologue. Northern blot analysis revealed that Mu-CD63 mRNA was expressed strongly in the kidney of adult mice, especially in the glomerulus fraction, implying the possibility that Mu-CD63 plays an important role in maintaining normal renal function. Activated macrophages and splenocytes exhibited strong expression of Mu-CD63 mRNA, whereas cultured thymocytes barely expressed the mRNA irrespective of cell activation. Taken together, the present results suggest that Mu-CD63 expression is associated with differentiation and/or development of certain cell types, but not necessarily with cell proliferation.

Soluble interleukin-6 receptors released from T cell or granulocyte/macrophage cell lines and human peripheral blood mononuclear cells are generated through an alternative splicing mechanism.

To detect transcripts encoding the interleukin-6 receptor (IL-6R) molecule lacking the transmembrane (TM) domain, in various cell lines and peripheral blood mononuclear cells (PBMC), we used the polymerase chain reaction (PCR) with primer pairs that flank the TM domain and which were selected to generate a 398-bp fragment. We detected 398-bp and 304-bp DNA molecules in the PCR products of the U1, J22HL60, MT-2, MT-4, U937 and HL60 cell lines and of PBMC isolated from several individuals. The sequencing analysis of both DNA molecules showed that a 94-bp region consisting of the TM domain of IL-6R was deleted in the 304-bp molecule. Moreover, we detected a soluble (s) IL-6R protein of 45 kDa in culture supernatants of the MT-2, MT-4 and U937 cell lines by radioimmunoprecipitation using specific antibodies against sIL-6R. Our results indicate that active deletion of the TM domain by alternative splicing of mRNA represents one mechanism for release of sIL-6R into the culture supernatants of cells, or into serum or urine.

The effect of sodium concentration on myocardial viability in donor heart preservation using a nondepolarizing solution.

This study examined the effect of different sodium concentrations in a nondepolarizing solution on myocardial viability and functional recovery of the canine donor heart. Isolated canine hearts were preserved for 6 h at 5 degrees C, followed by normothermic reperfusion for 2 h. Dogs were divided into two groups of nine dogs each: group 1 received a nondepolarizing solution with 70 mM Na+ and group 2 with 30 mM Na+. The myocardial Ca2+ concentration at the end of preservation was significantly higher in group 1 than in group 2 and increased after reperfusion in both groups without any intergroup difference. Myocardial concentrations of ATP, ADP, and total adenine nucleotide at the end of reperfusion were significantly higher in group 1 than in group 2. Myocardial cyclic adenosine monophosphate concentration was significantly higher in group 1 than in group 2 at the end of both preservation and reperfusion. The myocardial cyclic guanosine monophosphate concentration in group 1 increased and was higher than in group 2 at the end of preservation, but had returned to normal levels by the end of reperfusion. However, it remained unchanged through preservation and reperfusion in group 2. The left ventricular systolic and diastolic function, assessed by pressure-volume relationship, was better in group 1 than in group 2. Mitochondrial ultrastructural changes were similar. These results suggest that a nondepolarizing solution containing 70 mM Na+ provides better myocardial protection than a solution containing 30 mM Na+.

The effect of betamethasone on myocardial viability during cold cardioplegia in canine donor heart preservation.

This study examined the effect of betamethasone on myocardial viability of reperfused isolated canine hearts following a 6-h hypothermic cardioplegia. The dogs were divided into two groups: group I (n = 9) received nondepolarizing cardioplegia containing betamethasone 250 mg/l while group II (n = 7) was administered cardioplegia without betamethasone. The myocardial concentrations of calcium, ATP, ADP, total adenine nucleotide, cyclic adenosine monophosphate (cAMP) and cyclic guanosine monophosphate (cGMP) were identical in the two groups throughout the experiment. The coronary sinus plasma concentrations of MB fraction of creatine kinase (MB-CK), cAMP and cGMP after 2h of reperfusion were significantly lower in group I than in group II. The myocardial mitochondrial ultrastructure, as assessed by semiquantitative morphometry, was found to be significantly better preserved in group I than in group II at the end of both preservation and reperfusion. In addition, the left ventricular end-systolic pressure volume relation (ESPVR) showed a higher slope and lower intercept in group I than in group II. These results suggest that the addition of betamethasone to nondepolarizing cold cardioplegia enhances myocardial protection via membrane stabilization without affecting the adenine nucleotide metabolism.

Retroperitoneal tumor eleven years after initial treatment of testicular cancer.

Presented is a case report of retroperitoneal tumor eleven years after orchiectomy for testicular cancer. Complete remission was achieved with combined chemotherapy and retroperitoneal lymph node dissection. Histologic examination revealed that primary testicular tumor was pure choriocarcinoma and that the retroperitoneal tumor was likely embryonal cell carcinoma without foci of choriocarcinoma. It is difficult to determine whether the retroperitoneal tumor is a late recurrence of testicular cancer of a primary extragonadal germ cell tumor.
