Regional distribution of lead, zinc, iron and copper in suckling and adult rat brains.

The flameless atomic absorption method was applied to reveal regional differences in heavy metals between suckling and adult rat brains. The lead concentrations in the suckling brains were considerably different from those in the adult rats. The levels of lead concentration in the cerebrum and cerebellum of the normal suckling rats were 3-4 times higher than those of adult rats. Iron concentrations in the cerebrum of the suckling brains were approximately half those in the adult brains. Zinc and copper concentrations in medulla from the suckling rats were higher than those in the same region from the adult rats.

Role of endoscopic elimination of protein plugs in the treatment of chronic pancreatitis.

The purpose of this report is to evaluate the role of endoscopic elimination of protein plugs in the treatment of chronic pancreatitis (CP) and suspected CP. Endoscopic aspiration of pure pancreatic juice (PPJ) was performed on 69 patients with CP or suspected CP. PPJ was collected from within the main pancreatic duct by endoscopic retrograde catheterization of the papilla after a rapid intravenous injection of secretin and CCK-PZ. Following results were obtained. (1) Various numbers of protein plugs were obtained along with PPJ in 26 of the 69 patients. (2) Endoscopic elimination of protein plugs provided 17 of the 26 patients with dramatic relief from abdominal pain and back pain, indicting that the procedure was often useful, at least, for relieving pain in patients with protein plugs in the pancreatic duct system. (3) Follow-up studies suggested that the procedure could be an effective therapeutic tool in selected cases of CP or suspected CP in which no prominent stenotic lesions were noted in the major pancreatic duct system and abstinence from alcohol beverage was strictly observed. (4) In 43 patients with no protein plugs in the pancreatic juice, in contrast, transient or partial relief from abdominal pain was provided in only one patient, respectively.

[Moderate dose methotrexate with citrovorum factor rescue therapy in the treatment of head and neck cancer].

A clinical trial of moderate dose methotrexate (MTX)-CF rescue was conducted in 12 institutions. Thirty-seven patients with head and neck carcinoma entered this trial, of which 32 were evaluable. MTX was administered 350 mg/m2 (500 mg/body) by i.v. drip over 6 hours. Three hours after completion of MTX infusion, CF rescue was started. There was no complete response in 32 patients. Nine patients showed partial response with the response rate of 28%. The response rates were 21% for the group of patients treated previously, and 75% for the group untreated previously. MTX concentration in plasma was determined at 6, 24, 48 and 72 hours after the initiation of MTX infusion, and the assay results revealed a safe range. GI disturbances were seen at the rates of 11 to 38%. Bone marrow suppression was mild and no renal toxicity was observed. We concluded that the moderate dose MTX-CF rescue therapy was useful for head and neck carcinoma. As a next step, we are planning to conduct a clinical trial of high-dose MTX.

A study of the ecology of the house dust mite in dwelling houses.

The habitat of dust mites in dwelling houses in Japan was studied to determine the relation between mites in dust and mite allergy. Dust mites increased in the summer and decreased in the winter. Mite propagation was influenced by the temperature and relative humidity. Mites were found in higher concentrations in dust from concrete houses than that from wooden houses. Carpet dust contained numerous mites and tatami, sofas and bed mattresses were also habitats of mites. Mites existed universally in all samples of dust examined. Sensitization to mite allergen depends mainly on the atopic diathesis of patients and not on the mite count.

Assessment of postoperative hearing in 528 middle ear and mastoid surgery cases in Tokai University Hospital.

The postoperative hearing results in cases of tympanoplasty, radical mastoidectomy, modified radical mastoidectomy and tympanoplasty combined with modified radical mastoidectomy on 528 ears, performed in the Tokai University Hospital between 1975 and 1980 were assessed. The assessment was based on pure-tone audiograms taken 3 months to 5 years after the operation. Out of 528 cases 70.5% gained better postoperative hearing, 6% showed no change of hearing and 23.5% showed deterioration of hearing. Auditory success of those who attained social hearing, air-bone gap closure to within 20dB or gained hearing of 20 dB or more was 53.4%. The results in cholesteatoma cases were poorer than those of non-cholesteatoma cases. In 44% of the cases, ossiculoplasty was performed, and the most successful ossiculoplasty results were obtained when the malleus was repositioned to the head of the stapes. Fairly good results were achieved when an artificial prosthesis, such as a Teflon-piston, Gelfoam-wire or platinum-piston, was used. The least successful results were found when the incus was transpositioned. In the non-ossiculoplasty group, type I tympanoplasty produced 59.5% auditory success whereas 37.3% auditory success was achieved in radical mastoidectomy. Better auditory success rates were obtained when the bony bridge was preserved than when it was removed. The results were identical in cases where the mastoid cavity was obliterated or when it was kept open. A new technique, "scutumplasty", was used for reconstructing the scutum. Revision surgery was required in only 3.4% of the cases.

[Cystic meningioma associated with intratumoral hemorrhage in a child--report of a case and review of the literature].

The case reported here was a 6-year-old girl who complained of sudden headache and nausea followed by vomiting and visited our clinic. Physical examination revealed mental confusion, right abducent palsy and papilledema. Computed tomography demonstrated a round high-density area surrounded by a low-density zone indicating cystic nature and including high-density part. The lesion was located in the right parietal lobe attaching to the flax. Right frontoparietal craniotomy disclosed a brain tumor containing a large cyst in which bleeding from the tumor was confirmed. This mass lesion was totally resected out. Histo-pathological diagnosis was meningioma to contained cystic formation and hemorrhage from the tumor. Therefore, further discussion on the pathogenesis of cyst formation and bleeding from this tumor was presented.

Decrease of plasma vitamin A, albumin and zinc in cadmium-treated rats.

Male rats of the Wistar strain were injected subcutaneously with a total dose of 4.5 mg cadmium (Cd) per kg body weight. The first groups (Sc-1) was injected with 18 doses of 0.25 mg/kg, the second group (Sc-II) was injected with 9 doses of 0.50 mg/kg, and the third group (Sc-III) with 2 doses of 2.25 mg/kg. Another group was given water containing 75 ppm Cd for 68 days and 50 ppm for the ensuing 75 days. The plasma concentration of vitamin A (VA) was statistically decreased by the subcutaneous injections of Cd, but the liver concentration of VA was not altered. The fractionated injection of the total Cd apparently reduced the toxicity of Cd on VA metabolism. The decrease of plasma albumin was highly correlated with the decrease of plasma VA. The relative coefficient between them was calculated as 0.812 (n = 34, P less than 0.01) with all of the groups. the relative coefficient between plasma VA and zinc (Zn) was lower than that between plasma VA and albumin but was statistically significant (r = 0.697, P less than 0.01). These results may indicate the involvement of hepatic damage in Cd intoxications. The decrease of plasma VA, however, could not be explained by the Zn deficiency of the liver. To clarify the role of Zn in VA metabolism, the secretion mechanisms of them should be investigated in the liver.

Effects of toluene exposure on the rest-activity cycle of rats.

Rats were exposed to toluene at a concentration of 1000 ppm, 6 h/day, 6 times weekly, for 4 weeks. Functional disturbance of the circadian rhythm of spontaneous activity was found after repeated exposure to toluene, although single exposure under the same conditions did not influence the rhythm. The disturbance was characterized by a significant increase in distribution of activity in the "Light' (L) period, and a significant relationship was observed between the total number of days of exposure and the activity level in the L period. The toluene concentration in the blood immediately after the exposure was 6.8-7.2 microgram/ml after one and 18 days of exposure, respectively.
