[A novel phenotype (CD 4+, Leu 7+) in large granular lymphocyte leukemia: a case report].

We report a case of a 72-year-old man with large granular lymphocyte (LGL) leukemia. Immunophenotypical analysis of the abnormal cells showed the following results: CD 2+, CD 3+, CD 4+, CD 8-, CD 11+, CD 16-, Leu 7+. These cells had natural killer (NK) activity, responded to PHA and recombinant interleukin-2 (rIL-2), and showed neither helper nor suppressor function in B-cell differentiation. Molecular genetical analysis showed monoclonal rearrangement of T-cell receptor beta-chain gene, indicating they are of T-cell origin. These findings provide information on biological characteristics of normal CD 4+, Leu 7+ cells.

Malignant histiocytosis with rearrangement of the heavy chain gene and evidence of monocyte-macrophage lineage.

A unique case of malignant histiocytosis (MH) is reported. Its origin from the monocyte-macrophage system was indicated by expression of highly specific myeloid cell markers (My4, MCS2, and cytoplasmic lysozyme), diffuse activity of acid phosphatase and NaF-sensitive alpha-naphthyl acetate esterase, lack of immunologic markers specific for other cell lineages, and germ line configuration of the immunoglobulin light chain gene and the T-cell receptor beta-chain gene. Its neoplastic nature was suggested by the single rearranged band of the immunoglobulin heavy chain gene.

Reversed-phase ion-pair chromatography of straight- and branched-chain dicarboxylic acids in urine as their 2-nitrophenylhydrazides.

The derivatization of urinary dicarboxylic acids with 2-nitrophenylhydrazine hydrochloride produced corresponding monohydrazides, which were separated from monocarboxylic acid hydrazides by two step extraction with ethyl acetate at different pH values. Monohydrazides of 11 straight- and branched-chain dicarboxylic acids were eluted isocratically on reversed-phase ion-pair chromatography within 24 min by the combination of pH, the polarity of mobile phase, and the size of counter ion. The analytical results showed good recovery and reproducibility using 3,3-dimethyglutaric acid as an internal standard. The present method provides a notable HPLC method with precolumn derivatization for the analysis of urinary dicarboxylic acids.

Two major subgroups of human T-cell leukemia virus-1 in Japan.

T lymphocytes of patients with human T-cell leukemia virus type 1 (HTLV-1)-associated myelopathy (HAM) were cultured. After cultivating for several months, HAM-derived cell lines were tested for the presence of HTLV-1 proviral genome. We have found two major subgroups, the SacI type and the PstI type, of HTLV-1 by the restriction map analysis. They were almost equally distributed among HAM patients. We have also found two types of the provirus in DNA derived from fresh peripheral blood lymphocytes (PBL) or lymph node cells of adult T-cell leukemia/lymphoma (ATL) patients. The PstI type proviruses were predominant in ATL patients. It was concluded that two major subgroups of HTLV-1 exist in Japan and both types have an ability to cause either of two diseases, ATL or HAM.

A novel B cell line established from Ki-1-positive diffuse large cell lymphoma.

A novel cell line, designated KIS-1, was established from a patient with Ki-1-positive diffuse large cell lymphoma. Multiple phenotypic analysis of the KIS-1 cells was carried out with a total of 22 monoclonal antibodies defining hematopoietic cell subsets and lineages. The KIS-1 cells were positive for Ki-1, B4, HLA-DR, and 2D1 (common leucocyte) antigens, but were negative for the antigens reportedly specific for T cells, natural killer cells, granulocytes, monocytes, interdigitating reticulum cells and dendritic reticulum cells. The genomic analysis of the KIS-1 cells showed not only the rearrangement of JH and J kappa genes but also the probable rearrangement of C lambda genes. Moreover, the cells produced immunoglobulin lambda chains. Thus, KIS-1 was considered to be of B-cell lineage. The lymphoma-cell derivation of KIS-1 was based on the following facts. The cytochemical, immunologic, cytogenetic properties and the results of the molecular genomic analysis in the KIS-1 cells were essentially the same as those of the original tumor cells, and the KIS-1 cells were negative for Epstein-Barr virus-associated nuclear antigen. KIS-1 is the only known B-cell line derived from Ki-1-positive diffuse large cell lymphoma, and should be useful for defining the biological implications of Ki-1 antigen.

Immunogenotypes of lymphoid malignancies; the rearrangement of T cell receptor beta chain gene can occur before the gamma chain gene rearrangement.

Immunoglobulin (Ig) and T cell receptor (TcR) gene rearrangements were analyzed in 101 cases of lymphoid malignancies in association with a surface phenotype study. In leukemias/lymphomas with mature phenotype, there is a good correlation between phenotypes and genotypes. However, in leukemias/lymphomas with immature phenotype, we found many discordances between phenotypes and genotypes, suggesting the stochastic nature of hematopoietic cell differentiation at the early stage. As for TcR beta and gamma chains, the rearrangement of gamma chain gene is considered to occur slightly prior to that of beta chain gene. However, we observed a mature T cell malignancy, adult T-cell leukemia, with rearranged beta chain gene and germ line gamma chain gene, showing the possible existence of another pathway of T cell differentiation.

Phenotypic conversion of T lymphoblastic lymphoma to acute biphenotypic leukemia composed of lymphoblasts and myeloblasts. Molecular genetic evidence of the same clonal origin.

Acute biphenotypic leukemia composed of lymphoblasts and myeloblasts developed in a patient with T lymphoblastic lymphoma (T-LBL) who had an anterior mediastinal mass. A novel myeloid cell line, termed TK-1, has been established from his peripheral blood after the leukemic conversion. The identical rearranged pattern of T cell receptor gamma-chain gene was observed among the DNAs derived from lymph node cells in the lymphoma phase, the myeloid cell line TK-1, and the subclones with different karyotypes (TK-1B and TK-1D), which showed that myeloid cells had been derived from the T-LBL of the same patient. This finding demonstrates that phenotypic conversion occurs in the clonally propagating tumor cells and suggests that some hematopoietic cells retain the capacity to adopt either lineage.

Partial resection of atrial septal aneurysm with multiple fenestrations--a case report.

Aneurysms of the interatrial septum are uncommon lesions that are often accompanied by other cardiac anomalies or systemic thrombosis. The authors report 1 case of atrial septum aneurysm that was diagnosed by two-dimensional contrast echocardiography. At surgery a membrane-like, fenestrated aneurysmal protrusion to the right atrium without thrombosis was seen. The atrial septum was closed without using an artificial patch after some of the aneurysm with fenestrations was resected.

High-performance liquid chromatographic analysis of serum short-chain fatty acids by direct derivatization.

The application of direct derivatization in conjunction with high-performance liquid chromatography is described for the analysis of short-chain fatty acids in serum. The method is based on the reaction of these acids with acidic 2-nitrophenylhydrazine hydrochloride, without complicated isolation steps, which produces their non-volatile hydrazine derivatives. The hydrazides of fourteen saturated and unsaturated, straight and branched, short-chain fatty acids were separated from other acid hydrazides and interfering components by a simple solvent extraction, and were eluted isocratically on a reversed-phase C8 column within 24 min. UV detection demonstrated that the detection limits for the acids were 200-400 fmol per injection with linearity over the range from 400 fmol to 5 nmol per injection. Analytical recoveries ranged from 96.8% to 103.1% and coefficients of variation ranged from 0.9% to 3.8%. The present method is simple, accurate and adequate for the analysis of short-chain fatty acids in biological fluids and tissues of patients suffering from organic acidemias.

High-performance liquid chromatographic analysis of serum long-chain fatty acids by direct derivatization method.

A new visible-ultraviolet labelling method for the high-performance liquid chromatographic analysis in serum of individual free fatty acids, including polyunsaturated fatty acids, is described. Without commonly used isolation steps, fatty acids in serum were directly derivatized by treatment with acidic 2-nitrophenylhydrazine hydrochloride. The derivatized fatty acids were extracted into n-hexane and separated isocratically on a reversed-phase C8 column within 15 min. The detection limits ranged from 400 fmol to 1 pmol and from 100 to 200 fmol per injection with visible and ultraviolet detection, respectively. Visible detection had better selectivity, and free fatty acid levels were determined in sera obtained from healthy controls and patients with diabetes mellitus. In all the subjects studied, the precise quantitation could be performed with 25 microliters of serum. Analytical recoveries ranged from 98.3 to 103.4%. The intra- and inter-assay coefficients of variation were less than 2.7 and 3.5%, respectively. The present method is superior to the previously published methods for routine analyses: it is cheaper, the procedure is simpler, the analysis time is shorter and both resolution and sensitivity are better.

Increased arachidonic acid composition of phospholipids in colonic mucosa from patients with active ulcerative colitis.

The long chain fatty acid composition of phospholipids in colonic mucosa was determined by high performance liquid chromatography in nine patients with active ulcerative colitis and eight healthy controls. The arachidonic acid composition was 12.5 +/- 1.4 mol % (mean +/- 2 SEM) in the inflamed colonic mucosa from the patients with active ulcerative colitis and 6.8 +/- 1.2 mol % in the intact mucosa from healthy controls (p less than 0.001). In the inflamed colonic mucosa, oleic acid and palmitoleic acid were concomitantly decreased (p less than 0.001 and p less than 0.02, respectively), while docosahexaenoic acid was increased (p less than 0.05). Histopathological examination showed that there was a three fold increase in the cell density of inflammatory infiltrate in the lamina propria of the inflamed colonic mucosa (p less than 0.001). The cell density of inflammatory infiltrate correlated with the arachidonic acid composition of phospholipids in colonic mucosa (r = 0.89, p less than 0.005). These findings indicate that inflammation alters the long chain fatty acid composition of phospholipids in colonic mucosa. The observed increase in the arachidonic acid composition of phospholipids in inflamed colonic mucosa may contribute to the enhanced arachidonic acid metabolism in patients with active ulcerative colitis.

Lymphoma cell line (FL-18) and Epstein-Barr virus-carrying cell line (FL-18-EB) obtained from a patient with follicular lymphoma: monoclonal derivation and different properties.

A novel cell line, FL-18, was established from the pleural effusion of a patient with follicular small cleaved cell lymphoma. At the same time, an Epstein-Barr virus (EBV) nuclear antigen (EBNA)-positive cell line, FL-18-EB, was established from the EBV-infected culture of the same pleural effusion cells. Both cell lines had the same monoclonal surface immunoglobulin (IgG kappa), and they had the same karyotype as that of the fresh pleural effusion cells in which a reciprocal translocation between the long arm of chromosomes 14 and 18 [t(14;18)(q32;q21)] was detected. Gene rearrangement analysis of immunoglobulin heavy-chain gene (JH) and kappa light-chain gene (J kappa) showed the same rearranged configuration in the two cell lines; however, some morphological and phenotypic differences were found. The FL-18-EB cells, which were morphologically similar to common EBNA-positive lymphoblastoid cell lines of normal B cell origin at the initial phase of culture, were larger than the FL-18 cells and contained multinucleated giant cells. The FL-18 cells lacked cytoplasmic immunoglobulin and were positive for common acute lymphoblastic leukemia antigen (CALLA), whereas the FL-18-EB cells had cytoplasmic immunoglobulin and were negative for CALLA. Thus, the phenotype of FL-18-EB seems to be a result of a shift by EBV infection to a more mature stage in the B cell differentiation pathway than that of FL-18. The paired availability of EBV-free and EBV-infected cell lines of a neoplastic clone is unique and valuable in considering EBV infectibility of neoplastic B cells and resultant phenotypic changes.

The T-cell receptor gene rearrangements in T-lineage tumors without OKT-3,4,6,8 markers.

In the human system, discrete stages (I, II, and III) of intrathymic ontogeny have been defined on the basis of monoclonal antibody probes directed at unique T-lineage-specific surface glycoproteins. We have examined the relationship among T-cell receptor gene rearrangements and cell surface antigen expressions in T-cell malignancies. Twelve of 15 cases had the rearranged T-cell receptor beta chain gene, indicating that they represent cells already committed to the differentiated T-cell lineage at the gene level and are monoclonally proliferating regardless of the variable expression of surface antigens. We examined five cases of the earliest identifiable T-lineage cells (stage I) expressing WT-1 antigen without OKT-3, 4, 6, 8 antigens. Among them, two cases did not reveal the T-cell receptor beta chain gene rearrangements. In contrast, three cases demonstrated the T-cell receptor beta chain gene rearrangements even in stage I by the criteria of surface antigen expressions in contrast to the previous findings. Thus, we conclude that somatic rearrangement of the T-cell receptor gene of the beta chain occurs at the stage I level (early thymocyte) in the T-cell differentiation scheme. The phenotypically defined stage I T-cells consist of two populations with or without rearrangements of the T-cell receptor gene.

Relationship of human cord blood mononuclear cell lines, established by the HTLV-I integration, to expression of T-cell phenotypes and the T-cell receptor gene rearrangement.

The HTLV-I integrated cell lines originated from human cord blood mononuclear cells were examined for the T-cell receptor beta chain gene rearrangement in conjunction with immunological phenotype analysis. Five of seven cell lines contained the rearranged T-cell receptor beta chain gene. Interestingly, most of the rearranged lines failed to express the T cell differentiation antigens, OKT 4 and 8, though their positive expression of Leu 1 antigen indicated the commitment to the T-cell lineage. On the other hand, two cell lines retained the germ-line configuration of the beta chain gene, while phenotypical examination revealed that the two cell lines have distinct T-cell antigens. Non-rearranged cell lines have proved to be nearly monoclonal from HTLV-I integration pattern and/or surface marker clonality. Thus, the relationship between the immunological phenotypes and the T-cell receptor gene rearrangement observed in the T-cell lineage is not strict and often dissociates. A possible involvement of HTLV-I infection on the dissociation between phenotype and genotype is discussed from the viewpoint of phenotypical modulation.
