[A case of malignant fibrous histiocytoma occurring in the retroperitoneum with giant pyonephrosis].

A case of malignant fibrous histiocytoma (MFH) occurring in th retroperitoneum with giant pyonephrosis is reported. The patient was a 45-year-old male and his chief complaint was an abdominal mass. The abdominal fullness progressed so rapidly that he was admitted to our hospital. After examination, this case was diagnosed as a malignant tumor with left hydronephrosis, and an operation was performed on August 5, 1982. At operation, the left kidney contained about 11,000 ml of a pus-like fluid and in the retroperitoneum was found a hen-egg-sized solid tumor which was invading into the left kidney and the feeding vessels of the descending colon. So the tumor, left kidney and a part of the descending colon were resected en bloc. Pathological diagnosis was malignant fibrous histiocytoma. Chemotherapy (PPM regimen) and immunotherapy (OK-432) were administered after the operation, but multiple metastases appeared in the liver and bilateral lungs within 3 months. Then, the CY-VA-DIC regimen was followed. But, local recurrence was found in about 5 months, and the patient died on the 174 th day after the operation. Local recurrence and metastases in the liver, bilateral lungs, pleura and bones were confirmed at autopsy. Besides our case, a review of case reports of retroperitoneal MFH in Japan and comments are presented.

A rare variant of ureteric ectopia: opening in vagina and vesico-uretero-vaginal communication.

A rare variant of ureteric ectopia, i.e. an opening in the vagina and a vesico-uretero-vaginal communication, is herein reported. This type of congenital anomaly, found in 2 females, has not been reported before. One patient had a free, 2-way communication between the bladder and the vagina via the lower segment of the right ectopic ureter. The other was found to have a one-way communication from the vagina to the bladder through the ectopic distal ureter. Surgical intervention was undertaken in both cases. It is suggested that vaginography after distending the bladder with air is important in the detection of this bizarre anomaly.

Decreased suppressor T cell activity in patients with hepatic cirrhosis (HC).

Hypergammaglobulinaemia (HGG) is frequently found in patients with hepatic cirrhosis (HC). Using an assay system of in vitro PWM-stimulated immunoglobulin (Ig) production, the amounts of IgG, IgA, and IgM produced by peripheral blood lymphocytes (PBL) from 15 HBs Ag-negative patients with HC and from 16 age-matched healthy subjects were quantitated by radioimmunoassay. We found that PBL from patients with HC produced significantly greater amounts of IgG (P less than 0.05) but not IgA or IgM than did those from control subjects. This increased IgG production by PBL from patients with HC was attributed to enhanced T helper activity and not to enhanced B cell function. We also searched for defects in naturally occurring suppressor T cell activity which is sensitive to irradiation. Irradiation-induced enhancement for IgG production was significantly lower in patients with HC compared with age-matched control subjects (P less than 0.01). Similarly, we examined the effect of Con A-induced suppressor T cells on the in vitro PWM-stimulated IgG production by allogeneic PBL and observed the decrease of Con A-induced suppressor T cell activity in patients with HC (P = 0.01). We conclude, therefore, that the increased serum levels of Ig, particularly IgG in patients with HC may result from in part on the basis of depressed ability of naturally occurring suppressor T cells or Con A-induced suppressor T cells to suppress Ig production.

Dismembered pyelolithotomy: new procedure for removal of renal calculi.

A new operative technique for the removal of renal calculi, dismembered pyelolithotomy, is described. This operation was performed on 6 patients with staghorn renal calculi or multiple renal calculi with satisfactory results. This approach is effective in removing renal calculi without renal ischemic damage.

Signet-ring cell carcinoma involving the urinary bladder. Report of a case and review of 21 cases.

A case of signet-ring cell carcinoma involving the urinary bladder, which is a rare variant of adenocarcinoma, is presented. 21 cases have so far been reported in the occidental and Japanese journals. In 13 patients (62%) the carcinomas were of urachal origin and 8 (38%) of bladder origin. The prognosis of signet-ring cell carcinoma originating in bladder is poorer compared to that in urachus; 2-year survival of 40% for the bladder cancer group and 70% for the urachus. A radical cystectomy with excision of adjacent tissues, which might improve the prognosis in this fatal malignancy, should be considered.

Pathophysiologic analysis of peripheral blood lymphocytes from patients with primary immunodeficiency. I. Ig synthesis by peripheral blood lymphocytes stimulated with either pokeweed mitogen or Epstein-Barr virus in vitro.

In the present study 5 patients with common variable hypogammaglobulinemia (CVH) and 4 patients with selective IgA deficiency (IgA-D) were analyzed for the cellular defects responsible for impaired Ig synthesis with use of peripheral blood lymphocytes stimulated with either PWM or EBV in vitro. By the use of co-culture with PWM, all the patients examined had intrinsic B cell defects restricted to the synthesis of Ig class corresponding to the low or absent Ig class(es) in the sera. Two types of excessive suppressor T activity were found, which were abrogated by irradiation. One was isotype-nonspecific and the other was IgA-specific. Moreover, failure of IgA-specific helper T activity was demonstrated. The use of EBV as an agent that polyclonally activates B cells independently of T cells and monocytes should allow a clearer delineation of the level of the B cell defects. When co-cultured with EBV, B cells from 3 patients with CVH produced normal to subnormal quantities of IgM although they could produce no IgM upon co-culturing with normal T cells and PWM. B cells from 2 patients with CVH could produce IgM normally by stimulation with either PWM or EBV; however, there was no restoration to produce IgG or IgA in these patients. In addition, B cells from 2 patients with IgA-D produced not only IgG and IgM but also IgA almost normally at 4 days after in vitro stimulation with EBV.
