[A review of post-operative multiple organ failure in peripheral arterial diseases].

Two-hundred- and-fifty-one patients of peripheral arterial diseases underwent 281 operations for past 11 years in our institute, and multiple organ failure (MOF) occurred in 10 patients (4.0%). These patients were reviewed and were compared with in other diseases. Survival rate was 50% and all the patients with 4 or more organ disorders died. Incidence and survival rate were not significantly different from operative time, blood loss and blood transfusion. Significant difference was observed between elective operation (3.0%) and emergent operation (20.0%). Compared with other diseases, MOF occurred more rarely in peripheral arterial diseases than in aortic aneurysms, but occurred 1.4 times of digestive diseases. While the gastro-intestinal bleeding as the initial failure organ occurred more frequently in peripheral arterial diseases than in other diseases, heart and respiratory failures were rare. MOF occurred after peripheral arterial intervention as well as other abdominal and thoracic surgery. To avoid MOF it is important to avoid MOF that the emergent surgery should be kept out and that general status, especially the grade of diabetes mellitus should be evaluated sufficiently.

[Pharmacokinetics of high-dose cisplatin administration (relationship between pharmacokinetics and administration schedule)].

Three cases of urogenital malignancy were treated with 80-120/m2 of cisplatin by a 0.5-hour (rapid) or 4-hour (slow) infusion. Each patient received three courses of repeated chemotherapy. The kinetics of total platinum was measured up to 48 hours in the blood and urine. The blood peak concentrations were achieved 6.0 micrograms/ml by rapid infusion, 2.0 micrograms/ml by slow infusion. The time dependent urinary excretion rate against total administered dose was 40% in 60 min. by rapid infusion, 20% in 16 hours by slow infusion. The volume of distribution correlated with infusion time. Rapid infusion way of cisplatin could be better to treat the patient with bulky tumor mass or patients with uroepithelial cancer like CIS, because a higher blood and urine concentration can be obtained than by slow infusion.

[Clinical evaluation of Cernilton in benign prostatic hypertrophy].

Twenty patients with benign prostatic hypertrophy were treated with Cernilton, 6 tablets a day for an average of 13.2 weeks. Subjective effectiveness was observed in the improvement of sense of residual urine (92%), retardation (86%), night frequency (85%), strain on urination (56%), protraction (53%) and forceless urinary stream (53%). The overall subjective effectiveness was 80% of patients, and the overall objective effectiveness was 54% of patients. Night frequency, residual urine volume and tidal urine volume were improved significantly. The overall effectiveness was 80%. No side effects were observed.

[Treatment with K-18 (IgG-melphalan) in recurrent cases of hematopoietic malignancies].

K 18, an IgG-Melphalan conjugate was administered to 30 patients, who had recurrent hematopoietic malignancy. Ten out of the 30 patients received single doses of 1 to 20 enteric tablets containing 10 mg of K 18, as a phase I study. No side effects were observed. K 18 was administered every day to the remaining 20 patients in order to evaluate the side effects and therapeutic effects, as a phase II study. One patient attained partial remission. Although no remission effect was obtained in 14 of the 20 patients, antitumor effects such as a decrease in leukemia cells, were observed in 4 of 20 patients. As to side effects, neither recurrence of tumor nor cumulative toxicity were shown in one patient with NHL who received only K 18 for 14 months as maintenance therapy. Evaluation of antitumor effect was difficult in the case of the remaining 4 patients. In the 20 cases who entered the phase II study, a decrease in neutrophils was observed in 2 patients, a slight decrease in platelets in 3 patients and increased transaminase activity in one patient as side effects of K 18. In brief, compared with Melphalan, K 18 has between 1.3 and 2 times a more potent therapeutic effect, with extremely low side effects.

Differentiation-associated increase of cAMP-dependent type II protein kinase in a murine preadipose cell line (ST 13).

The activity of cAMP-dependent protein kinase and cAMP binding activity were studied during the differentiation of ST 13 murine preadipocytes into adipocytes. We found that both activities were marginally detectable in preadipose cells and increased remarkably when the cells were induced to differentiate, preceding by several days the morphological adipose conversion. The increased cAMP-dependent protein kinase was identified as type II enzyme by means of DEAE-Sephacel chromatography and by photoaffinity labeling with 8-azido[3H]cAMP. We further showed that the increase of protein kinase activity was specific to cell differentiation with the aid of modulators of the adipose conversion (insulin, fetal bovine serum, retinoic acid and 5-bromodeoxy-uridine). We propose that the increased expression of type II cAMP-dependent protein kinase would be a biochemical index of differentiation in ST 13 preadipocytes.

Molecular cloning and nucleotide sequences of cDNAs specific for rat liver ribosomal proteins S17 and L30.

cDNA clones coding for rat liver ribosomal proteins S17 and L30 have been isolated by positive hybridization-translation assay from a cDNA library prepared from 8-9S poly(A)+RNA from free polysomes of regenerating rat liver. The cDNA clone specific for S17 protein (pRS17-2) has a 466-bp insert with the poly(A) tail. The complete amino acid (aa) sequence of S17 protein was deduced from the nucleotide sequence of the cDNA. S17 protein consists of 134 aa residues with an Mr of 15 377. The N-terminal aa sequence of S17 protein determined by automatic Edman degradation is consistent with the sequence data. The aa sequence of S17 shows strong homology (76.9%) to that of yeast ribosomal protein 51 [Teem and Rosbash, Proc. Natl. Acad. Sci. USA 80 (1983) 4403-4407] in the two-thirds N-terminal region. The cDNA clone specific for L30 protein (pRL30) has a 394-bp insert. The aa sequence of L30 protein was deduced from the nucleotide sequence of the cDNA. The protein consists of 114 aa residues with an Mr of 12 652. When compared with the N-terminal aa sequence of rat liver L30 protein [Wool, Annu. Rev. Biochem. 48 (1979) 719-754], pRL30 was found not to contain the initiation codon and 5'-noncoding region. The cDNA showed twelve silent changes in the coding region, one point mutation and one base deletion in the 3'-noncoding region, compared with mouse genomic DNA for L30 protein [Wiedemann and Perry, Mol. Cell Biol. 4 (1984) 2518-2528].

Isolation of two syngeneic cell lines from a rat mammary carcinoma: growth factor production by neoplastic epithelial cells.

Two distinctly different clonal cell lines were isolated from a mammary tumor induced by ingestion of 7,12-dimethylbenz[a]anthracene (CAS: 57-97-6) in a SD rat. Cells of one of the clones, RMT-1 clone E4, showed typical epithelial characters; it was concluded that they were derived from neoplastic mammary epithelial cells. The other clone, M2, exhibited characters consistent with its derivation from the normal mammary myoepithelial component. The 2 cell lines had different proliferative responses to growth factors (GF) such as cholera toxin, dexamethasone, and epidermal GF. The epithelioid E4 cells were found to produce potent growth-promoting activity in culture medium that stimulated proliferation of myoepithelial M2 cells as well as that of stromal fibroblasts. The present work provides supporting evidence that the mechanism of "paracrine stimulation" is operative in the mammary tumorigenesis of the rat.

Casein and histone kinases of a rat ascites hepatoma as compared with those of rat liver.

Seryl/threonyl-protein kinases in cytosolic and particulate fractions from rat liver and AH-13, a rat ascites hepatoma, have been studied by chromatographing these fractions on DEAE-cellulose and assaying the eluates with casein, phosvitin, histone and protamine as substrates. Liver cytosolic fraction contains a group of well-characterized seryl/threonyl-protein kinases, namely, casein kinases I and II and histone kinases I and II. Liver particulate fraction, on the other hand, is almost totally devoid of casein kinase I and histone kinase I but contains an additional peak of casein kinase tentatively designated casein kinase III. In AH-13, cytosolic casein kinase I is markedly increased and particulate-associated casein kinases II and III are moderately increased as compared with liver. Moreover, it was found that in AH-13, the histone kinase I level is high in the particulate fraction but markedly decreased in the cytosolic fraction. It is suggested that particulate-associated histone kinase I may be of cytosolic origin.

The purification of an acid- and heat-labile transforming growth factor from an avian sarcoma virus-transformed rat cell line.

A new class of transforming growth factor (TGF), with chemical characteristics differing from previously reported TGFs, was isolated and purified from an avian sarcoma virus-transformed rat cell line, 77N1 . Purification steps were simple and consisted of ion-exchange chromatography on diethylaminoethyl (DEAE)-Sephacel, ammonium sulfate precipitation, Chromatofocusing, and DEAE-Sephadex A-25 chromatography. The purified TGF is a heat- and acid-labile protein with a molecular mass of 12,000 daltons and isoelectric point of 5.2-5.4. Because of the acid lability of this TGF, purification was carried out at neutral pH. The TGF induced DNA synthesis in growth-arrested BALB 3T3 cells and promoted anchorage-independent growth of nontransformed BALB 3T3 cells in soft agar; the latter activity is specific for the peptide growth factors, called TGFs, but it did not compete with epidermal growth factor (EGF) for binding to the EGF membrane receptors. The TGF activity was not potentiated by EGF. The purified preparation of the TGF stimulated BALB 3T3 cells to grow progressively in soft agar at a dose of 20 ng/ml.

Effects of retinoids on the proliferation of murine sarcoma virus-transformed cells in serum-free culture: potentiation of autocrine stimulation.

In a recent report, we described a growth stimulatory effect of vitamin A derivatives (retinoids) on murine sarcoma virus (MSV)-transformed cell lines when tested in a serum-free culture condition. These cell lines were presumed to proliferate via autostimulatory (autocrine) mechanism when serum was omitted from culture medium. In the present communication, we examined the effect of 6 synthetic derivatives on the serum-free proliferation of 2 MSV-transformed cell lines. We confirmed essentially the previous results and found that one of them (Ro 11-1430) was a very weak growth stimulator. Further evidence indicated that retinoid-mediated growth stimulator. was not the result of increased production of growth factors but the result of potentiation of autocrine stimulation.
