Serum bioactive and immunoreactive follicle-stimulating hormone in oligozoospermic and azoospermic men: application of a modified granulosa cell bioassay.

Serum follicle-stimulating hormone (FSH) levels measured by radioimmunoassay (RIA) usually correlate well with the rate of spermatogenesis. However, in certain cases this correlation does not exist. The purpose of this study was to establish a reliable bioassay of FSH for the andrological clinic. Follicle-stimulating hormone was measured by both standard RIA and bioassay in 98 men subgrouped into normospermic, oligospermic, and azoospermic. Bioactivity of FSH was determined using in vitro cultures of granulosa cells utilizing progesterone measurements for assessing FSH activity. Results of FSH levels obtained by both methods correlated well (r = 0.55, P less than 0.01) within themselves, and both correlated negatively and significantly with sperm concentration. The ratio between bioactivity and immunoreactivity of FSH did not correlate with sperm density. Thus, the decrease in sperm concentration and other sperm variables resulting from a germinal epithelial dysfunction was not mediated or associated with low biological activity of FSH. The application of this method can be of clinical value in cases where a discrepancy is found between serum RIA-FSH levels and sperm quality.

Complications in hyperthermia treatment of benign prostatic hyperplasia.

Of 435 patients treated with local hyperthermia to the prostate 27 had 29 complications, for an over-all complication rate of 6.6%. Urinary tract infections (7 cases), hematuria (6), and epididymitis (2) accounted for roughly half of the complications and could be attributed to the insertion of a catheter rather than the treatment itself but this remains to be proved. Most of the complications occurred within the first 3 treatment sessions. Despite the complication, 9 patients continued and completed the treatment course. Mean age of the study group was 73 +/- 8.3 years. Given the age of these patients, high incidence of accompanying diseases and low rate of complications, local hyperthermia should be considered a viable low risk treatment option in patients with benign prostatic hyperplasia.

Local microwave hyperthermia in the treatment of benign prostatic hypertrophy.

Local deep microwave hyperthermia has been introduced recently for the treatment of benign prostatic hypertrophy (BPH). We report the results of treatment in 114 patients divided into 2 groups. The first group included 83 patients with severe obstructive symptoms. Although some improvement was found in the scoring of both subjective and objective parameters, the number of patients benefiting was small, with only 28% showing improvement in both parameters. Approximately 50% reported subjective improvement but in only 25% was this significant. In 42% there was objective evidence of improvement. Prostatic volumes did not change as a result of treatment. The second group comprised 31 patients with indwelling catheters because of acute urinary retention. After treatment, 19 patients (61%) were able to dispense with the catheter. The difficulties of interpreting an open trial of treatment for BPH and assessing its impact on objective parameters and symptomatology are discussed. Our results should be regarded cautiously in view of these reservations.

Short-term effect of a new portable extracorporeal lithotripter on renal function.

Extracorporeal shock wave lithotripsy has become the treatment of choice for upper urinary tract stones. We evaluated the impact of a new lithotripter on the kidneys of 14 nonobstructed patients treated for renal stones, using several physiological parameters and renal ultrasonography. Glomerular filtration rates (GFR) were estimated from the renal scan. Complete disintegration of the stone was accomplished in all but one patient. Treatment did not alter the serum parameters or the renal ultrasonography. Also, no changes were found in the GFR on the follow-up renal scans in both kidneys. The implications of these findings are discussed.

Epididymo-orchitis: a retrospective study of 121 patients.

One hundred twenty-one patients with acute epididymitis or epididymo-orchitis were evaluated retrospectively according to their clinical symptoms, duration of symptoms, physical examination, and laboratory studies. The patients younger than 30 years of age usually showed less severity of symptoms than the patients older than 50 years of age. The latter often demonstrated evidence of outflow obstruction. Eighty-two percent of patients with demonstrated urographic abnormalities had lower tract abnormalities, mainly secondary to outflow obstruction. All of these patients were older than 50 years of age. An intravenous pyelogram is indicated only in patients over 50 years of age and in young adults with positive bacteriologic urine culture.

Changes in prostate-specific markers under chronic gonadotrophin-releasing hormone analogue treatment of state D prostatic cancer.

Nineteen patients with newly diagnosed advanced prostatic carcinoma were treated with daily inhalations of a gonadotrophin-releasing hormone (GnRH) analogue (buserelin acetate, Hoechst 766, Frankfurt, FRG), for up to 3 years. Successful long-term suppression of luteinizing hormone (LH) and testosterone was observed without any escape of testosterone. Ten patients were considered nonresponders, whereas nine were defined as responders (had either stabilized or regressed). Serum prostate-specific markers, prostatic acid phosphatase (PAP) and prostate-specific antigen (PSA), were monitored at fixed time intervals throughout treatment in these patients. Simultaneous determinations of both markers was made under treatment in 147 samples. The results suggest a high correlation between the disease state and the level of the markers (P less than 0.0001), as well as a very high correlation between serum concentrations of the markers themselves (correlation coefficient = 0.6). Thus, the authors could not point clearly towards a superiority of any marker when monitoring response to GnRH analogue treatment. Contrary to current belief, the pretreatment values of both markers were of no prognostic value with regard to response or survival.

Von Hippel-Lindau syndrome with unusual presentations in two brothers.

Two cases are reported of von Hippel-Lindau syndrome in two brothers. One had an asymptomatic adrenal pheochromocytoma (probably bilateral) and a synchronous metastatic hypernephroma which presented as a rather "innocent" renal colic. The second asymptomatic brother underwent urologic investigation which revealed multifocal tumors of the kidney. High clinical suspicion based on family history only is mandatory. The literature related to von Hippel-Lindau syndrome is reviewed.

Potential pitfalls in the obstructive renal scan in patients with double-pigtail ureteral catheters.

In 21 patients with a long-standing internal ureteral double-pigtail catheter we demonstrated the adverse effect of a full bladder on upper system drainage. A pseudo-obstructive pattern noted in these patients on routine scans was immediately rectified by mere bladder emptying and a normal pattern was demonstrated. We suggest the bladder be emptied in all patients with indwelling internal ureteral stents before renographic evaluation.

Primary hypomagnesemia with a probable double magnesium transport defect.

We describe a boy with a neonatally diagnosed primary nonfamilial hypomagnesemia. Oral supplementation of large quantities of magnesium salts was required to maintain low normal serum magnesium levels. Lately, a further increase in the oral supplementation had to be administered in order to avoid seizures. A thorough investigation was conducted. Both an intestinal and urinary magnesium wasting was noticed. The rarity of this simultaneous double transport defect merit its description.

The value of renal scan in acute renal colic--a clinical viewpoint.

The value of emergency room renal scan in acute renal colic was evaluated. Based on obstructive pattern on renal scan, 53 cases were admitted. The results indicate that only in 13 cases (25%) there was a need for further inpatient investigation followed by interventional treatment. In all 13 cases, either elevated fever, impaired renal function or intractable pain were present, in addition to an obstructive renal scan pattern. In the majority (75%) of cases, when there were no adjunctive ominous signs, hospital admission based only on an obstructed renal scan, showed need for conservative therapy only and was rather unjustified.

Use of gonadotropin-releasing hormone analogue in advanced prostatic cancer: 3 years clinical experience.

The relative efficacy of the recently introduced gonadotropin-releasing hormone (GnRH) analogues in advanced prostatic cancer, compared with surgical castration, is still debatable. High patient compliance, lack of side effects, and absence of the psychological impact of castration, seem to be the most prominent advantages cited. Our long-term follow-up in a group of 19 prostate cancer patients treated with GnRH analogue also indicated high patient compliance and mild side effects. However, we found a discrepancy between local prostatic regression and general clinical responsiveness, and a poorer therapeutic effect than that obtained with surgical castration. In this group of patients follow-up with either prostatic acid phosphatase or prostatic specific antigen seems promising. We found a high correlation between disease state and prostatic markers. We suggest that GnRH analogues are another addition to the armamentarium for treating advanced prostatic carcinoma, yet are limited in their therapeutic effectiveness.

The role of increased prolactin levels under GnRH analogue treatment in advanced prostatic carcinoma.

The role of prolactin in the progress of prostatic carcinoma still is obscure. It is known to modulate testosterone metabolism and may have a prognostic value in determining disease progression. Prolonged treatment with a GnRH analogue has been introduced recently as a therapeutic alternative to surgical castration in advanced prostatic carcinoma. It has been reported that prolactin levels remain unchanged or even decrease in GnRH analogue treatment. By contrast, in this series of 15 prostatic carcinoma patients treated with GnRH analogue, a significant rise was demonstrated in clinical responders as well as nonresponders. In the latter group, prolactin rise was more pronounced. These results suggest that frequent prolactin determinations might be of prognostic value in determining early disease progression under GnRH analogue treatment.

Epidemiological features of acute viral hepatitis among the Israeli Defence Forces, 1977-83.

High morbidity rates of non-B hepatitis in the Israeli Defence Forces has stimulated efforts towards its prevention. This paper analyses morbidity over a period of 7 years since a programme of pre-exposure prophylaxis with human normal immunoglobulin began. Among 1480 cases studied, about 5% were of hepatitis B. In 360 patients further investigated 11% of illnesses were caused by hepatitis non-A non-B viruses; the remainder by hepatitis A virus. Females comprised about 15% of all three groups. Ethnic origin, social class and education were similar in all those with hepatitis A and non-A non-B hepatitis. This was not so for hepatitis B. None of those with hepatitis B gave a history of former blood transfusions or illicit use of drugs. The importance of risk groups and mode of transmission of the various hepatitis viruses is discussed.
