Evaluation of root canal débridement by the endosonic ultrasonic synergistic system.

The endosonic ultrasonic synergistic system was compared to conventional hand-filing and irrigating techniques for its ability to débride the root canal system. Extracted human teeth were endodontically prepared by conventional and ultrasonic techniques. The roots were cross sectioned and evaluated microscopically at the 1, 3, and 5 mm. levels from the apex for cleanliness. The endosonically prepared canals were significantly cleaner at all levels.

Non-Hodgkin's malignant lymphomas: a cytologic classification based on statistical analysis and correlated with prognosis.

On the basis of cytologic studies of 144 patients with non-Hodgkin's malignant lymphomas (NHML), a cytologic classification was established that was composed of three groups (1, 2 and 3) containing nine classes (1A, 1B, 1C; 2A, 2B, 2C; 3A, 3B, 3C). Statistical analyses were carried out using 52 well-defined elementary cytologic characteristics. All the results given by the data-gathering procedures (hierarchical and dynamic clustering methods), by the descriptive analyses (multifactorial and canonical analysis) and by the stepwise discriminant analysis were in agreement with the proposed cytologic classification system. The most discriminating cytologic characteristics of the classes were the cell size, the nuclear-cytoplasmic ratio, the degree of cytoplasmic basophilia, the homogeneity of the cytoplasm and the number of mitoses. The use of these properties renders this classification system reproducible and applicable to clinical practice. A comparison was made between the cytologic classes and the NHML patients as grouped according to their clinical courses. Of the patients in classes 2A, 2B and and 2C, 94% showed acute leukemic characteristics. Of those contained in classes 3A, 3B and 3C, 78% showed poor prognosis or metastatic patterns. Of those contained in classes 1A, 1B and 1C, 76% showed a good prognosis pattern.

[Gene expression, metabolic regulation and aging].

The biological ageing comprises changes of of structure and function depending on time with the beginning of the autonomy of a living being up to death. On the one hand, the causes of this process are genetically fixed, on the other hand they depend on environmental factors and are not programmed. In the course of life the total genome and also the nucleic acid underlie influences, which must have consequences on the gene expression. In these cases the changed sensitivity of the genetic apparatus in comparison to hormones is impressionably changed. The decreased protein biosynthesis may be stimulated by increased amino acid concentrations and by SH-substances, Age-depending changes are also observed for enzymatically catalyzed reactions and in the regulation of chains of metabolism. At first the biologic ageing appears as a complex multiform process, which reaches the simplest integration level in the cellular metabolism.

[Reflexions on techniques of preoperative radiotherapy in the treatment of carcinoma of the bladder. Initial results and discussion of a therapeutic protocol based upon histo-prognosis (author's transl)].

Basing their indications on the histo-prognosis of Opperman and Bittard in malignant tumours of the bladder, the authors decided to irradiate before cystectomy those bladder tumours in which the histo-prognostic index indicated that it was a local disease with metastatic potential with a spontaneous life expectancy of 18 to 30 months. Radiotherapy was given spread over 23 or 27 fractions providing a total of 46 Gy. Four to 6 weeks later, cystoprostatectomy with trans-ileal cutaneous ureterostomy was performed, before chemotherapy for one year. Amongst 11 patients treated, in 6 cases the bladder was completely "sterilised" from a histological standpoint. However there was persistence of tumour in 5 bladders with an increase in the histo-prognostic index. Amongst these 11 patients, with a minimum follow-up of 18 months, there has been a pelvic recurrence in two cases and three developed fatal metastases. In the opinion of the authors, these preliminary results represent an encouragement to continue along the same lines.
