[Toxic hepatoses due to Gravistat].

With the help of 5 cases with toxic hepatosis by administration of Gravistat the problems concerning ovulation inhibitor and toxic hepatosis were explained. The clinical, paraclinical and histological findings may be pronounced differently. Anamnestically in prescribing ovulation inhibitors particularly a jaundice during pregnancy or a pronounced pruritus in the 3rd trimester of the pregnancy must be taken into consideration. Toxic liver damage may appear not only when the ovulation inhibitor is given the first time, but also after changing it. For this reasons idiopathic jaundice during pregnancy and condition after toxic hepatosis by ovulation inhibitor in every case must be regarded as contraindications for the application of oral contraceptives. In order to establish subclinical courses a control of transaminases in the first three cycles of using the remedies is to be demanded.

[Multiple eccrine spiradenomas. Apropos of 2 cases in 2 sisters].

Two cases of multiple eccrine spiradenoma were studied, representing respectively up to the present time the 7th and 8th registered cases in the world literature. These cases two are siblings, a fact that had never been observed before. The histogenesis of the tumour has been the subject of controversy. The presence of multiple lesions in each patient and the incidence in siblings are two important points in favour of the naevic character of eccrine spiradenoma. Among other therapeutic measures we wish to draw attention to the excellent result we obtained with small doses of radiotherapy (1,500 rads divided in ten applications). It should be kept in mind that remission of the disease may only be temporary.

[Concanavalin A-binding to cyclic reassembled microtubules from pig brain-tubulin (author's transl)].

Concanavalin A reacts to cyclic reassembled microtubules. The results were analysed morphometrically and topochemically. Concanavalin A can be observed mainly on the outer part of the microtubules and also partly in the lumen. After Concanavalin A reaction the diameters of microtubules are significantly higher than in the controls. It is shown that the saccharide component does not disappear during cyclic reassembly of microtubules. This compoenent is seen as a glycoprotein and as a part of at least one of their protein fractions or as a constitutive factor of microtubules.

[Studies on the effect of urapidil on the central nervous system and various organ functions in animal experiments (author's transl)].

Compared to the changes in spontaneous behaviour of mice and rats, 6-(3-[4-(o-methoxyphenyl)-1-piperazinyl]-propyl-amino)-1,3-dimethyluracil (urapidil, Ebrantil) in doses from 10 mg/kg p.o. has a central sedative action, for which it is typical that it can be interrupted by external stimuli after doses of up to approx. 100 mg/kg. By this action urapidil differs from other central-acting drugs, e.g. chlor-promazine, diazepam and clonidine. It can be assumed that urapidil has no central dopamine blocking qualities because it does neither influence the conditioned avoidance response nor the amphetamine-induced gnawing of the rat. Urapidil is also different from diazepam bacause it does not hinder spinal polysynaptic motor reflexes of the decerebrated cat. Up to 80 mg/kg p.o. of urapidil, there was no hypotensive effect in the rat to be seen and, therefore, it can be concluded that changes in spontaneous and reactive behaviour are not due to acute changes in cardiovascular functions. The antihistamine effect of urapidil in the isolated ileum of the guinea pig is approx. 15 times weaker than that of chlorpromazine, furthermore urapidil has no anticholinergic quality.

[Toxicologic study on the antihypertensive agent urapidil].

The acute toxicity of 6-(3-[4-(o-methoxyphenyl)-1-piperazinyl-propylamino)-1,3-dimethyluracil (urapidil, Ebrantil) was tested in rats and mice using oral and intravenous routes. The tolerance upon repeated oral administration was tested in subacute and chronic toxicity studies lasting 3 weeks, 3, 6 and 12 months in the rat, as well as studies lasting 4 weeks and 6 months in the dog. The effect of urapidil on the reproduction in mice, rats and rabbits was also investigated. Sedation is seen in mice and rats after a single administration of urapidil, at lethal doses, tremor and convulsions appear. The same symptoms were seen in experiments with repeated oral administration in the rat (stomach tube) or in the dog (tablets) as the criterium of tolerance. In feeding studies in the rat, the decreased body weight gain was the criterium of toxicity. The no-effect dose in the rat is 42 times the therapeutic dose in man. The no-effect dose in the dog lies between 8 and 21 times the average therapeutic dose. An inhibition of the estrus cycle in the rat proved to be species-specific. In reproductin studies, significant toxic effects were seen besides functional disturbances specific to the rat. Urapidil was not teratogenic. The development and reproductive capacity of the F1-generation was not affected.

[Experiences with the management of acute esophageal burns (author's transl)].

Corrosive burns of the esophagus are still relatively frequently encountered by the clinician. In cases of mild or moderately severe burns, accepted management involves the use of corticosteroids and antibiotics. However, controversy exists in such therapy for the severe burns. The present author's experience with these injuries over a 16 year period indicates that both steroids and antibiotics are necessary in all patients and that esophageal strictures can be prevented with corticosteroids without any side effects from the drug.

[Ketamine-droperidol-dualanaesthesia for surgical and diagnostic procedures in pediatric ophthalmology (author's transl)].

717 anaesthetics, consisting of a combination of droperidol and ketamine, were given to children aged between 2 months and 17 years for surgical and diagnostic procedures in ophthalmology. The administration of droperidol prior to ketamine resulted in (1) reduced doses of ketamine of 3.59 mg/kg body weight for a mean surgical duration of 18.11 min and (2) an absence of controversial psychomimetic side effects of ketamine. This procedure--called Dualanaesthesia--is simple and of low risk. Investigations were made concerning the cardiovascular and respiratory system, as well as the oculocardiac reflex, analgesic and antiemetic effects. Based on these positive experiences, this method is recommended for paediatric anesthesia.

Editorial: Chromosomes and human neoplasms. Achievements using new staining techniques.

Numerical and structural chromosome aberrations are frequently found in neoplastic cells. As demonstrated by the new chromosome banding techniques these aberrations are not random, but tend to show a specific occurrence. A model example is the leukemias where many cytogenetical investigations have been done to date. In leukemia chromosome analysis serves the following purposes: to identify a neoplastic process, to confirm and strengthen the hematological diagnosis, for the early diagnosis of transformation from a chronic leukemia into its blastic phase and for following up the clonal evolution of a leukemic cell line. In the discussion of chromosomes and neoplasms it must be mentioned that individuals demonstrating chromosomal instability and some trisomic patients show a greater tendency toward the development of a malignancy. Malignancy is primarily a cellular phenomenon caused by a disturbance in cellular regulation, whose fine events are not known. Therefore the exact role of the chromosomes in neoplastic processes cannot be stated. From experimental investigations it appears that the affected chromosomes carry cell growth regulating factors and also that a specific aberration is the result of the action of a specific agent.
