Implantable devices (implanted pump/implanted catheter and port) are recommended for continuous epidural application of opiates or local anaesthetics in order to reduce hygienic problems during long term epidural medication. No signs of contamination could be found during bacteriologic culture of residual volumes of 30 patients treated with epidural drug delivery systems and of samples collected during an in vitro investigation (incubation of filled external pump systems at body temperature/storage of syringes with a premixed solution of opiate and local anaesthetic for repeated epidural bolus application). As demonstrated in a case report prophylactic antibiotic coverage prior to implantation may be necessary in patients with a preexisting susceptibility to infection. The concentrations of morphine (radioimmuno-assay) and of bupivacaine (gas-solid chromatography) within the reservoir were stable during clinical therapy as well as during the in vitro experiments.

A case report is given of a man, who died at the age of 56. At post mortem examination the following was found: Postnecrotic cirrhosis of the liver, esophageal varicosis, ascites as well as multiple white nodes located in the serosal layer of the small intestine and the mesenterium. Histological examination showed fibromatosis. Differential diagnosis is discussed including other connective tissue tumors, Whipple's disease, peritoneal tuberculosis, Gardner syndrome, solitary fibromatosis, diffuse peritoneal fibromatosis and sclerosing peritonitis. Similarity of the findings reported with the disease entity of sclerosing mesenteritis is discussed as well as a possible causal relationship between liver cirrhosis and fibromatosis.
Hemotherapy performed in our clinic was investigated in patients operated on in the years 1973 and 1982. A total of 832 operations (638 gynaecological interventions and 194 cesarean sections) were used for evaluation. - Transfusion rates decreased from 72% (1973) to 13% (1982) due to systematic application of crystalloid and colloidal infusions to cases of blood loss of up to 20% of the total blood volume. The percentage of erythrocyte concentrate applied increased from 0.4% (1973) to 42% (1982), while it even reached 80% when related to the total blood transfusions of our clinic. To conclude, strict principles should be adopted in deciding for blood transfusion as well as in purposefully applying blood constituents lost. The advantageous development in the economy of surgery is outlined.

In patients with cancer pain caused by tumours in the upper thorax, shoulder or neck area the thoracic approach for peridural opiate analgesia is indicated. The thoracic puncture technique is more difficult and the possible complications are more serious than puncture in the lumbar area; it therefore belongs, as pain therapy generally, in the hands of an anaesthesiologist experienced in regional anaesthesia. In selecting the puncture level appropriate to the site of pain, it is necessary to take account of the fact that the vertebral interspace does not correspond with the spinal segment. Only thus can an optimal analgetic effect with a minimal dose b assured.
In invasively growing tumours of the urinary bladder specificity and sensitivity of the urine cytology are sufficiently high with 100 and 66%, respectively, it is, however, not successful. Endoscopy and histological ascertainment are now as ever prevailing (TUR). Cytology is indispensable to the diagnosis of the carcinoma in situ of the urinary bladder. In non-invasive urothelial carcinomas the classification of the WHO after Friedell stands the test. It is also shown a better correlation to the cytological findings which more frequently become positive with growing degree of malignity. High degree of malignity, multiple tumours and positive cytology are to be valuated as particular risk factors concerning frequent relapses and their appearance of invasively growing tumours. The domain of cytology is the control of the course with curative aim of patients who were operated on in order to sustain the organ. Relapses are frequently more dedifferentiated than the primary tumour so that the recidivation is often indicated cytologically. The endoscopic demonstration of the recidivation may still be absent at the beginning. Also for controls under local chemotherapy cytology stands the test, in which cases we may not sufficiently judge the value of this treatment method due to the small number of cases.
A sensitive, specific and rapid determination of 5-(4-chlorobenzoyl)-1,4-dimethyl-1H-pyrrole-2-acetic acid sodium salt dihydrate (zomepirac sodium, Zomax) in human plasma and urine by a continuous body fluid monitoring system based on high-performance liquid chromatography is described. Samples are directly injected into the apparatus which consists of commonly available HPLC-modules. Manual sample clean-up procedures as well as the addition of an internal standard are not needed. Using 50-microliter aliquots, the detection limit is lower than 0.05 microgram/ml and the calibration ranges from 0.5 to 100 micrograms/ml are linear for both, spiked samples and references. The recovery is 97% for plasma and 95% for urine samples at a sampling rate of about 15 samples/h.

A comparison was conducted in patients with differentiated thyroid carcinoma still under 131I therapy (n = 18) between 2 mCi-131I scans, posttherapeutic scans and radioimmunologically measurable serum thyroglobulin (TG). All 3 parameters were positive in only 9 cases; among the other 9 patients TG appeared to be the better indicator (ratio 5:1) of residual tumour tissue than the 2 mCi-131I scan. In a second group of patients who had completed 131I therapy the 2 mCi scan and TG were compared (n = 21). Both indices were negative in 15 cases, whereas TG was positive in 6 patients in spite of a negative scan. TG generally remained within the clearly positive range also during T4 therapy. TG was highest in metastatic disease and practically uninfluenced by T4, i.e. by the endogenous TSH level. TG antibodies were found in 15% of cases, generally with low titres only; this did not interfere with TG determination as tumour marker. In summary, TG measurement appears to be superior to 2 mCi-131I scans for follow-up of cases with differentiated thyroid carcinoma.
Glandular kallikrein can be temporarily acylated at the active centre by 4-amidinophenyl benzoates. The time course of reactivation depends on the nature of the acyl group introduced. Benzoyl kallikrein was used as a delivery system for kallikrein in vivo. In rabbits, bolus injection of the acylated enzyme caused a long-lasting drop in blood pressure due to sustained kinin liberation.
A method for the quantitative determination of free and esterified ergosterol in lipid fractions from biomasses grown on hydrocarbons as sole carbon source is described. Column chromatography was used for the fractionation of lipids. Esterified ergosterol was determined in the benzene fraction and free ergosterol in the methanol fraction by known methods.
In our model of hypodynamic endotoxic shock systemic pulmonary and metabolic alterations such as -hypotension, tachycardia, low output syndrome -pulmonary hypertension -pulmonary failure with arterial hypoxemia and - lactacidosis developed earlier than disturbances in oxygen supply of skeletal muscle. This points to the different responses of the organ systems in endotoxic shock.
